    Inter-Agency Cooperation in Combating Terrorism in Nigeria: Enhancing Existing Instruments and Frameworks in the Security Services

    In recent times, terrorism has become one of the most dangerous threats topeace and order nationally and globally. It is so pervasive that no country onearth can claim immunity from its violent acts of bombing, shooting, armedrobbery, kidnapping, hostage-taking and bank robbery. This paper examinesthis global human phenomenon through the analysis of its various definitions, its nature and consequences. Based on the analysis, the paper proffers a ten-point policy recommendation for enhancing the existing instruments and frameworks in the security services that would foster interagency cooperation in combating terrorism in Nigeria

    Analgesic and Anti-inflammatory Activities of the Stem Bark of Yellow Flamboyant (Peltophorum pterocarpum)

    Peltophorum pterocarpum (family; Fabaceae) is an evergreen perennial crop grown in tropical gardens. Different parts of this tree are being used for the management of many diseases. This study investigated the acute toxicity, analgesic and anti-inflammatory potentials of n-hexane extract of P. pterocarpum stem bark. Acute oral toxicity of n-hexane extract of P. pterocarpum stem bark was investigated using standard method. Analgesic activity was investigated by using acetic acid-induced writhing model, using indomethacin as a reference drug. Antiinflammatory activity of n-hexane extract of P. pterocarpum stem bark was investigated by using formalin-induced paw licking model, using aspirin as standard drug. In the acute toxicity study, mortality was observed at 500 and 2500 mg/kg body weight. In the acetic acid induced writhing test, the n-hexane extract of P. pterocarpum (100 and 200 mg/kg body weight) showed a significant reduction in the number of writhing with 55.5 % and 60 % of inhibition respectively. In formalin-induced rat paw oedema test for acute inflammation, the n-hexane extract of P. pterocarpum in 50, 100 and 200 mg/kg body weight showed 26.00 %, 27.89 % and 32.27 % inhibition of oedema respectively after 4hours, which is comparable to that of standard drug-aspirin (33.59 %). These results validated that the extract of P. pterocarpum possesses significant analgesic and anti-inflammatory properties.Keywords: Peltophorum pterocarpum, acute toxicity, analgesic activity, anti-inflammatory activit

    Relationship between the Clinical Frailty Scale and short-term mortality in patients ≥ 80 years old acutely admitted to the ICU: a prospective cohort study.

    BACKGROUND: The Clinical Frailty Scale (CFS) is frequently used to measure frailty in critically ill adults. There is wide variation in the approach to analysing the relationship between the CFS score and mortality after admission to the ICU. This study aimed to evaluate the influence of modelling approach on the association between the CFS score and short-term mortality and quantify the prognostic value of frailty in this context. METHODS: We analysed data from two multicentre prospective cohort studies which enrolled intensive care unit patients ≥ 80 years old in 26 countries. The primary outcome was mortality within 30-days from admission to the ICU. Logistic regression models for both ICU and 30-day mortality included the CFS score as either a categorical, continuous or dichotomous variable and were adjusted for patient's age, sex, reason for admission to the ICU, and admission Sequential Organ Failure Assessment score. RESULTS: The median age in the sample of 7487 consecutive patients was 84 years (IQR 81-87). The highest fraction of new prognostic information from frailty in the context of 30-day mortality was observed when the CFS score was treated as either a categorical variable using all original levels of frailty or a nonlinear continuous variable and was equal to 9% using these modelling approaches (p < 0.001). The relationship between the CFS score and mortality was nonlinear (p < 0.01). CONCLUSION: Knowledge about a patient's frailty status adds a substantial amount of new prognostic information at the moment of admission to the ICU. Arbitrary simplification of the CFS score into fewer groups than originally intended leads to a loss of information and should be avoided. Trial registration NCT03134807 (VIP1), NCT03370692 (VIP2)

    An Unusual Case of Marble Intraocular Foreign Body

    This report presents a case of marble intraocular foreign body that developed toxic complications during surgery. The patient is a 25 years old male who presented to the University of Benin Teaching Hospital with a history of trauma to the right eye while cutting marble. He was examined, had an ocular ultrasound scan and subsequently had an extracapsular cataract extraction. His visual acuity in the right eye was light perception. There was an entry point on the cornea, the lens was opaque, there was vitreous haemorrhage and the intraocular foreign body was localized in the posterior part of the posterior segment by ultrasound scan. He had extracapsular cataract extraction. During anterior capsulotomy, the cornea suddenly and rapidly became cloudy with a brownish tinge and the corneal epithelium started desquamating.Marble on its own may not be toxic but the other chemicals including cement, used in the processing of the marble were responsible for this delayed toxicity.     Hysterosalpingogram a common radiological examination is routinely used in the investigation of infertility and sub-fertility in women. The close proximity of the uterus to adjacent pelvic structures results in various types of pelvic fistulas after pelvic disease, pelvic radiation therapy, trauma or pelvic surgery. The case of a 34 years old Para 0+1 woman with a diagnosis of secondary infertility and booked for hysterosalpingogram as part of the routine work-up is presented. There was a positive history of chronic pelvic inflammatory disease, myomectomy and appendicectomy. The result showed evidence of opacification of the right fallopian tube with contrast outlining the adjacent large bowels.Asymptomatic pelvic fistulas maybe a silent cause of infertility, and association can only be stated as all other factors of infertility could not be convincingly ruled out (Afr J Reprod Health 2010; 14[1]:139- 143).L’on se sert systématiquement de l’ hystérosalpinogramme, un examen radiologique commun, pour étudier la stérilité et la sous–stérilité chez la femme. Le fait que l’utérus est très proche des structures pelviennes avoisinantes mène à de divers types des fistules pelviennes suite à une maladie pelvienne, une radiothérapie, le traumatisme ou une chirurgie pelvienne. Le cas d’une nullipare âgée de 34 ans avec un diagnostic de la stérilité secondaire et pour qui une place a été réservée pour l’hystérosalpinogramme comme faisant partie du bilan de routine, est présenté dans cette étude. Il y avait une histoire positive de la salpingite aigüe, la myomectomie et l’appendicectomie. Le résultat a montré une évidence de l’opacification de la troupe utérine droite y compris le contraste qui trace les grande lignes de gros intestins adjacents. Des fistules pelviennes asymptomatiques peuvent être une cause silencieuse de la stérilité et l’on ne peut que citer l’association puisque les autres facteurs de la stérilité ne pouvaient pas être facilement exclus (Afr J Reprod Health 2010; 14[1]:139-143)
