    Translational studies in growth plate research: the effect of glucocorticoids and growth factors on the growth plate

    Glucocorticoids (GCs) regulate many physiological systems in the body and have potent immunosuppressive and anti-inflammatory properties. In children prolonged administration causes a reduction in growth, which is in part due to the direct effects they have on the growth plate. This effect is dependent on the dose, duration and type of GCs used.This thesis consists of four major types of studies each utilising different models of growth and chondrocyte biology, which in combination strengthens the understanding of the effects of GC and growth factors on the growing skeleton. The initial in vivo study showed that in children treated with Dexamethasone (Dex) or Prednisolone (Pred) for Acute Lymphoblastic Leukaemia, the effects of Dex on body composition were more apparent in that it was up to 18 times more potent at reducing short term linear growth than Pred.The ATDC5 chondrocyte cell line was fully characterised, which allowed a unique opportunity to study GC effects on a homogeneous population of chondrocytes at the chondrogenesis and terminal differentiation phases. The GCs caused a reduction in cell number, cell proliferation and proteoglycan content whilst stimulating chondrocyte differentiation. These effects were dose dependent and only observed during the chondrogenesis phase when the cells are rapidly dividing. Furthermore these negative effects could be partially reversed with the use of a GC receptor antagonist and completely reversed with IGF-I.These observations were further translated into increasingly physiological models of bone growth. Foetal mouse metatarsal organ explants, where the three dimensional structure and cell connections of the growing bone remain intact, again demonstrated that Dex and IGF-I had opposite effects on bone growth. The length of the metatarsals at day 10 from harvesting day length in the control, Dex and IGF-I bones was 50% ± 3, 42% ± 2, xv (p<0.05) 99.3% ± 5(p<0.05) respectively. In contrast to Dex the effects of IGF-I were immediate. Most importantly, it was demonstrated for the first time in vitro that IGF-I increased the size of the hypertrophic zone, as occurs in vivo, and this accounted for most of the increase in metatarsal lengthPrenatal administration of Dex caused a reduction in birth weight and length and this difference was greater in the female mice. The growth restriction was associated with elevated IGF-I and IGFBP-2 levels raising the possibility of a state of IGF-I insensitivity, which may explain subsequent growth failure.In conclusion, this thesis translated the clinical observation that Dex is more potent than Pred at inhibiting linear growth and these effects are dependent on the dose and duration of the GC exposure as well as the chondrocyte phenotype. These negative effects of GC can be reversed by IGF-I administration

    Society for Endocrinology UK guidance on the initial evaluation of a suspected difference or disorder of sex development (DSD) (Revised 2021)

    It is paramount that any child or adolescent with a suspected difference or disorder of sex development (DSD) is assessed by an experienced clinician with adequate knowledge about the range of conditions associated with DSD and is discussed with the regional DSD service. In most cases, the paediatric endocrinologist within this service acts as the first point of contact but involvement of the regional multidisciplinary service will also ensure prompt access to specialist psychology and nursing care. The underlying pathophysiology of DSD and the process of delineating this should be discussed with the parents and affected young person with all diagnostic tests undertaken in a timely fashion. Finally, for rare conditions such as these, it is imperative that clinical experience is shared through national and international clinical and research collaborations

    Using referral rates for genetic testing to determine the incidence of a rare disease: The minimal incidence of congenital hyperinsulinism in the UK is 1 in 28,389

Available from PLOS via the DOI in this record. Congenital hyperinsulinism (CHI) is a significant cause of hypoglycaemia in neonates and infants with the potential for permanent neurologic injury. Accurate calculations of the incidence of rare diseases such as CHI are important as they inform health care planning and can aid interpretation of genetic testing results when assessing the frequency of variants in large-scale, unselected sequencing databases. Whilst minimal incidence rates have been calculated for four European countries, the incidence of CHI in the UK is not known. In this study we have used referral rates to a central laboratory for genetic testing and annual birth rates from census data to calculate the minimal incidence of CHI within the UK from 2007 to 2016. CHI was diagnosed in 278 individuals based on inappropriately detectable insulin and/or C-peptide measurements at the time of hypoglycaemia which persisted beyond 6 months of age. From these data, we have calculated a minimum incidence of 1 in 28,389 live births for CHI in the UK. This is comparable to estimates from other outbred populations and provides an accurate estimate that will aid both health care provision and interpretation of genetic results, which will help advance our understanding of CHI.Wellcome Trus

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy

We aimed to estimate the relative and absolute effects of antiplatelet therapy on recurrent intracerebral haemorrhage and whether this risk might exceed any reduction of occlusive vascular events. Methods: The REstart or STop Antithrombotics Randomised Trial (RESTART) was a prospective, randomised, open-label, blinded endpoint, parallel-group trial at 122 hospitals in the UK. We recruited adults (≥18 years) who were taking antithrombotic (antiplatelet or anticoagulant) therapy for the prevention of occlusive vascular disease when they developed intracerebral haemorrhage, discontinued antithrombotic therapy, and survived for 24 h. Computerised randomisation incorporating minimisation allocated participants (1:1) to start or avoid antiplatelet therapy. We followed participants for the primary outcome (recurrent symptomatic intracerebral haemorrhage) for up to 5 years. We analysed data from all randomised participants using Cox proportional hazards regression, adjusted for minimisation covariates. This trial is registered with ISRCTN (number ISRCTN71907627). Findings: Between May 22, 2013, and May 31, 2018, 537 participants were recruited a median of 76 days (IQR 29–146) after intracerebral haemorrhage onset: 268 were assigned to start and 269 (one withdrew) to avoid antiplatelet therapy. Participants were followed for a median of 2·0 years (IQR [1·0– 3·0]; completeness 99·3%). 12 (4%) of 268 participants allocated to antiplatelet therapy had recurrence of intracerebral haemorrhage compared with 23 (9%) of 268 participants allocated to avoid antiplatelet therapy (adjusted hazard ratio 0·51 [95% CI 0·25–1·03]; p=0·060). 18 (7%) participants allocated to antiplatelet therapy experienced major haemorrhagic events compared with 25 (9%) participants allocated to avoid antiplatelet therapy (0·71 [0·39–1·30]; p=0·27), and 39 [15%] participants allocated to antiplatelet therapy had major occlusive vascular events compared with 38 [14%] allocated to avoid antiplatelet therapy (1·02 [0·65–1·60]; p=0·92).     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Background: To assess relationship between previouscaesarean scar and subsequent development of placentapraevia and its morbid adherence.Methods: This analytical study was conducted inthe Department of Gynaecology and Obstetrics, MilitaryHospital, Rawalpindi from 1st July to 31st December 2006.122 pregnant women with history of previous caesareansections beyond 32 weeks of gestation whether booked orunbooked, irrespective to number of caesarean scars withor without bleeding per vagina were included in thisstudy. Cases with previous myomectomy, uterine repair,placental abruption, bleeding per vagina due to localcauses were excluded. Patients were divided into fourgroups. Group A with previous one caesarean section,group B with previous, two caesarean sections, Group Cprevious three caesarean sections and, Group D withprevious four caesarean sections.Results: Group A included 69 patients and 07 (10%)were found to have placenta praevia. In Group B 32patients were studied and placenta praevia was diagnosedin 5 (15.6%) of them. Similarly in Group C 12 patients werestudied and 4 (33.3%) had placenta praevia. The study ofGroup D showed placenta praevia in 3 out of 9 (33.3%)patients. There was 2 case of placenta accreta in group A, Cand D and one in group B.Conclusion: The percentage of placenta praevia andmorbidly adherent placenta rises with increasing numberof caesarean sections
