Factores de recidiva del carcinoma hepatocelular tras resección quirúrgica con intención curativa
El carcinoma hepatocel·lular representa del 70-80% dels tumors hepàtics primaris. És la sisena neoplàsia més freqüent i la tercera causa més freqüent per càncer. Els factors de risc més importants són l'hepatitis B i C. La resecció quirúrgica és la primera opció terapèutica. La causa de la mort dels pacients sotmesos a resecció hepàtica amb intenció curativa és la recidiva. Les taxes de recidiva global arriben al 70-100% als 5 anys. Identificar els factors de risc de recidiva és important per augmentar la supervivència, aplicant teràpies preventives o incloent en llista d'espera de trasplantament.El carcinoma hepatocelular representa del 70-80% de los tumores hepáticos primarios. Gaia Early Data Release 3: Summary of the contents and survey properties
Context. We present the early installment of the third Gaia data release, Gaia EDR3, consisting of astrometry and photometry for 1.8 billion sources brighter than magnitude 21, complemented with the list of radial velocities from Gaia DR2. Aims. A summary of the contents of Gaia EDR3 is presented, accompanied by a discussion on the differences with respect to Gaia DR2 and an overview of the main limitations which are present in the survey. Recommendations are made on the responsible use of Gaia EDR3 results. Methods. The raw data collected with the Gaia instruments during the first 34 months of the mission have been processed by the Gaia Data Processing and Analysis Consortium and turned into this early third data release, which represents a major advance with respect to Gaia DR2 in terms of astrometric and photometric precision, accuracy, and homogeneity. Results. Gaia EDR3 contains celestial positions and the apparent brightness in G for approximately 1.8 billion sources. For 1.5 billion of those sources, parallaxes, proper motions, and the (GBP − GRP) colour are also available. The passbands for G, GBP, and GRP are provided as part of the release. For ease of use, the 7 million radial velocities from Gaia DR2 are included in this release, after the removal of a small number of spurious values. New radial velocities will appear as part of Gaia DR3. Finally, Gaia EDR3 represents an updated materialisation of the celestial reference frame (CRF) in the optical, the Gaia-CRF3, which is based solely on extragalactic sources. The creation of the source list for Gaia EDR3 includes enhancements that make it more robust with respect to high proper motion stars, and the disturbing effects of spurious and partially resolved sources. The source list is largely the same as that for Gaia DR2, but it does feature new sources and there are some notable changes. The source list will not change for Gaia DR3. Conclusions. Gaia EDR3 represents a significant advance over Gaia DR2, with parallax precisions increased by 30 per cent, proper motion precisions increased by a factor of 2, and the systematic errors in the astrometry suppressed by 30-40% for the parallaxes and by a factor ~2.5 for the proper motions. The photometry also features increased precision, but above all much better homogeneity across colour, magnitude, and celestial position. Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)
Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters.
Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs).
Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001).

Spatiotemporal Characteristics of the Largest HIV-1 CRF02_AG Outbreak in Spain: Evidence for Onward Transmissions
Background and Aim: The circulating recombinant form 02_AG (CRF02_AG) is the predominant clade among the human immunodeficiency virus type-1 (HIV-1) non-Bs with a prevalence of 5.97% (95% Confidence Interval-CI: 5.41–6.57%) across Spain. Our aim was to estimate the levels of regional clustering for CRF02_AG and the spatiotemporal characteristics of the largest CRF02_AG subepidemic in Spain.Methods: We studied 396 CRF02_AG sequences obtained from HIV-1 diagnosed patients during 2000–2014 from 10 autonomous communities of Spain. Phylogenetic analysis was performed on the 391 CRF02_AG sequences along with all globally sampled CRF02_AG sequences (N = 3,302) as references. Phylodynamic and phylogeographic analysis was performed to the largest CRF02_AG monophyletic cluster by a Bayesian method in BEAST v1.8.0 and by reconstructing ancestral states using the criterion of parsimony in Mesquite v3.4, respectively.Results: The HIV-1 CRF02_AG prevalence differed across Spanish autonomous communities we sampled from (p < 0.001). Phylogenetic analysis revealed that 52.7% of the CRF02_AG sequences formed 56 monophyletic clusters, with a range of 2–79 sequences. The CRF02_AG regional dispersal differed across Spain (p = 0.003), as suggested by monophyletic clustering. For the largest monophyletic cluster (subepidemic) (N = 79), 49.4% of the clustered sequences originated from Madrid, while most sequences (51.9%) had been obtained from men having sex with men (MSM). Molecular clock analysis suggested that the origin (tMRCA) of the CRF02_AG subepidemic was in 2002 (median estimate; 95% Highest Posterior Density-HPD interval: 1999–2004). Additionally, we found significant clustering within the CRF02_AG subepidemic according to the ethnic origin.Conclusion: CRF02_AG has been introduced as a result of multiple introductions in Spain, following regional dispersal in several cases. We showed that CRF02_AG transmissions were mostly due to regional dispersal in Spain. The hot-spot for the largest CRF02_AG regional subepidemic in Spain was in Madrid associated with MSM transmission risk group. The existence of subepidemics suggest that several spillovers occurred from Madrid to other areas. CRF02_AG sequences from Hispanics were clustered in a separate subclade suggesting no linkage between the local and Hispanic subepidemics
Resección hepática con intención curativa de pacientes con carcinoma hepatocelular. Valoración de los factores pronósticos de recidiva y supervivencia

El carcinoma hepatocelular (CHC) es un cáncer con elevada prevalencia que representa
del 70% al 80% de los tumores hepáticos primarios. Actualmente es la quinta neoplasia
más frecuente en hombres en el mundo (523.000 casos/año; 7,9% de todos los cánceres)
y la séptima más frecuente en mujeres (226.000 casos/año; 6,5% de todos los cánceres).
Es la segunda causa más frecuente de muerte por cáncer en el mundo. Está asociado
con ciertos factores riesgo, siendo los más importantes los virus de la hepatitis B y C.
La resección quirúrgica es la primera opción terapéutica en aquellos tumores que
aparecen sobre hígados no cirróticos (5% de los casos en occidente y 40% en Asia), en
los que se pueden realizar resecciones amplias con un riesgo bajo de complicaciones. Sin
embargo, en España, la mayoría de los CHC (más del 90%) aparecen sobre una
enfermedad hepática crónica, habitualmente en fase cirrótica. En este contexto, no es
posible realizar resecciones amplias debido al riesgo de insuficiencia hepática
postoperatoria, lo que limita la aplicabilidad de esta opción terapéutica. En aquellos
sujetos con cirrosis compensada es fundamental una correcta selección de los
potenciales candidatos a resección quirúrgica, con análisis del grado de reserva funcional.
Contenido de la investigación
Se han estudiado de forma retrospectiva a partir de una base de datos recogida de forma
prospectiva 55 variables en 68 casos intervenidos de hepatocarcinoma (14 parámetros
correspondientes al paciente y su hepatopatía, 8 variables de la cirugía, 8 variables del
postoperatorio, 14 variables correspondientes al tumor hepático y 11 variables del
seguimiento).
Los objetivos del trabajo fueron los siguientes:
1. Analizar la morbi-mortalidad de los pacientes sometidos a una resección hepática
por un CHC.
2. Determinar si existen factores predictivos de dicha morbimortalidad.
3. Analizar la tasa de recidiva tumoral global, la recidiva precoz y la tardía de los
pacientes intervenidos.
4. Determinar la supervivencia actuarial y la supervivencia libre de enfermedad en
los pacientes sometidos a una resección hepática por un CHC.
5. Determinar los factores pronósticos que influyen en la recidiva, la supervivencia
actuarial y la supervivencia libre de enfermedad, clasificados según los factores:
a. dependientes del estado previo del paciente
b. dependientes de su hepatopatía de base
c. dependientes de la cirugía hepática
d. dependientes de las características del tumor
e. dependientes del seguimiento postoperatorio
6. Analizar la evolución de los parámetros operatorios y peroperatorios en distintos
periodos de tiempo de nuestra serie.
7. Comparar nuestros resultados con los resultados estándares aceptados en la
literatura actual.
Conclusión
En base a los resultados obtenidos tras el análisis de la morbi-mortalidad, la recidiva
tumoral (global, precoz y tardía), la supervivencia actuarial y la supervivencia libre de
enfermedad, podemos afirmar que los resultados de nuestra serie, se adecuan a los
datos publicados y aceptados en la literatura en el tratamiento quirúrgico mediante
resección con intención curativa de los pacientes con carcinoma hepatocelular.
El estadio tumoral y el gradiente de presión portal, se ha relacionado de forma directa
con un mayor riesgo de recidiva global. La presencia de invasión vascular por el
tumor ha sido un factor de mal pronostico respecto a la recidiva tumoral precoz. Por
el contrario, la presencia de hepatitis C ha sido un factor de buen pronostico sobre la
incidencia de recidiva tumoral precoz. La morbilidad en el postoperatorio y el estadio
tumoral han influido en una mayor tasa de recidiva tumoral tardía. Los pacientes que
tengan tumores con invasión de la cápsula tumoral e invasión vascular tienen un
peor pronóstico respecto a la supervivencia y la supervivencia libre de enfermedad
no descrita anteriormente. La supervivencia libre de enfermedad sólo se ha visto
influenciada de forma negativa por la presencia de invasión de la cápsula tumoral.[cat] Introducció

sent els més importants els virus de l’hepatitis B i C.
La resecció quirúrgica és la primera opció terapèutica en aquells tumors que apareixen a
fetges no cirròtics (5% dels casos a occident i 40% dels casos a Àsia), on es poden
realitzar grans reseccions amb un vaig riscs de complicacions. No obstant això, a
Espanya, la majoria dels CHC (més del 90%) apareixen damunt d’una malaltia hepàtica
crònica, normalment en fase cirròtica. En aquest context, no és possible fer reseccions
grans degut al risc d’insuficiència hepàtica postquirúrgica, fet que limita aquesta opció
terapèutica. En aquells malalts amb cirrosis compensada és fonamental una correcta

Conclusió
D’acord amb els resultats obtinguts després de l’anàlisi de la morbi-mortalitat, la
recidiva tumoral (global, precoç i tardana), la supervivència actuarial i la
supervivència lliure de malaltia, podem dir que els resultats de la nostra sèrie,
s’adeqüen a les dades publicades i acceptades a la literatura en el tractament
quirúrgic mitjançant resecció amb intenció curativa dels malalts amb carcinoma
hepatocel·lular.
L’estadi tumoral i el gradient de pressió portal, s’ha relacionat de forma directa amb
un major risc de recidiva global. La presència d’invasió vascular pel tumor ha sigut un
factor de mal pronòstic respecte a la recidiva tumoral precoç. El contrari ha passat,
amb la presència d’hepatitis C que ha sigut un factor de bon pronòstic damunt la
incidència de recidiva tumoral precoç. La morbilitat en el postoperatori i l’estadi

Hepatocellular carcinoma (HCC) is a high prevalence cancer that accounts for 70% to
80% of primary liver tumors. At present it is the most frequent fifth neoplasia in men in the
world (523. 000 cases / year; 7,9 % of all the cancers) and the seventh most frequent in
women (226. 000 cases / year; 6,5 % of all the cancers). It is the most frequent second
cause of death for cancer in the world. Risk is associated with certain factors, being the
most important the viruses of the B and C hepatitis.
Surgical resection is the first treatment option in non-cirrhotic livers (5% of cases in the
West and 40% in Asia), in that wide resections can be performed with a low complications
risk. However, in Spain, most HCCs (more than 90%) arise on chronic liver disease,
usually in the cirrhotic phase. In this context, it is not possible to perform extensive
resections due to the risk of postoperative liver failure, and can’t do this therapeutic option.
In subjects with compensated cirrhosis, a correct selection of the potential candidates is
mandatory to perform a surgical resection, with the previous analysis of the degree of
functional reserve.
Content of research
Fifty-five variables in 68 cases of hepatocellular carcinoma (14 parameters corresponding
to the patient and his liver disease, 8 variables of the surgery, 8 variables of the
postoperative period, and 14 variables corresponding to the liver tumor and 11 follow-up
variables) were analyzed retrospectively from a database prospectively collected.
The objectives of the work were the following:
1. To analyse the morbidity and mortality of patients undergoing liver resection for HCC.
2. To determine if there are factors predictive of such morbidity and mortality.
3. To analyse the overall tumor recurrence rate, the early and late recurrence of the
patients undergoing surgery.
4. To determine overall survival and disease-free survival in patients undergoing CHC
liver resection.
5. To determine the prognostic factors that influence relapse, overall survival and
disease-free survival, classified by factors:
a. patient's previous state
b. their liver disease
c. hepatic surgery
d. tumor characteristics
e. postoperative follow-up
6. To analyze the evolution of the operative and perioperative parameters in different
periods of time.
7. Compare our results with the standard results accepted in the current literature.
Conclusion
After the analysis of morbidity and mortality, tumor recurrence (global, early and late),
overall survival and disease-free survival, we can asses that the results of our series are
adjusted to the data published in the surgical treatment by resection with curative intention
of patients with hepatocellular carcinoma.
The tumor stage and the portal pressure gradient have been directly related to an
increased risk of global recurrence. The presence of vascular invasion by the tumor has
been a factor of poor prognosis regarding the early tumor recurrence. In contrast, the
presence of C hepatitis has been a good prognostic factor on the incidence of early tumor
recurrence. Postoperative morbidity and tumor stage have influenced a higher rate of late
tumor recurrence. Patients who have tumors with invasion of the tumor capsule and
vascular invasion have a poorer prognosis regarding survival and disease-free survival not
previously described. Disease-free survival has only been negatively influenced by the
presence of invasion of the tumor capsule

Análisis de la supervivencia de los pacientes sometidos a resección curativa de carcinoma hepatocelular. Experiencia del servicio de Cirugía General y del Aparato Digestivo del Hospital Universitario Son Espases
The hepatocellular carcinoma is a high prevalence cancer that represents 70% to 80% of primary liver tumors. Surgical resection
is the first therapeutic option, but most appear on a chronic liver disease, usually in the cirrhotic phase, which limits its application
only in very selected cases. In the Balearic Community, the reference center for patients affected by hepatocellular carcinoma is the
University Hospital Son Espases. Therefore, we consider the analysis of our experience and determine overall survival, and disease-free
survival in patients undergoing hepatic resection for hepatocarcinoma, and determine the prognostic factors that influence
them. To this end, we performed a retrospective study with 68 patients and 55 variables from a database collected prospectively.
The study began in May 2002 and finished in December 2015. The overall survival at 1, 3 and 5 years is 91.3%, 69.5% and 46.9%
respectively, and disease-free survival decreases to 1, 3 and 5 years to 89.9%, 63.5% and 35.8%, respectively.
Patients with tumors with capsule invasion and vascular invasion have a worse prognosis regarding survival and disease-free survival.
Disease-free survival has only been negatively influenced by the presence of invasion of the tumor capsule.El carcinoma hepatocelular es un cáncer de alta prevalencia que representa del 70% al 80% de los tumores hepáticos primarios.
La resección quirúrgica es la primera opción terapéutica, pero la gran mayoría aparecen sobre una enfermedad hepática crónica,
habitualmente en fase cirrótica, que limita la cirugía a casos muy seleccionados. En la Comunidad Balear el centro de referencia
de los pacientes afectos de carcinoma hepatocelular es el Hospital Universitario Son Espases. Por ello, nos planteamos el análisis
de nuestra experiencia, determinar la supervivencia actuarial, la supervivencia libre de enfermedad en los pacientes sometidos a
una resección hepática por un hepatocarcinoma, así como determinar los factores pronósticos que influyen en ellas. Se han estudiado
de forma retrospectiva a partir de una base de datos recogida de forma prospectiva 55 variables en 68 casos intervenidos.
El estudio se inició en mayo de 2002, finalizando la recogida de datos de nuevos casos en diciembre de 2015. La supervivencia
actuarial a 1, 3 y 5 años es del 91,3%, 69,5% y 46,9% respectivamente, y la supervivencia libre de enfermedad desciende hasta
el 89,9%, 63,5% y 35,8% a 1, 3 y 5 años respectivamente.
Los pacientes que tengan tumores con invasión de la cápsula tumoral e invasión vascular tienen un peor pronóstico respecto a la
supervivencia y la supervivencia libre de enfermedad. La supervivencia libre de enfermedad sólo se ha visto influenciada de forma

Uso de la fluorescencia con verde de indocianina en la isquemia mesentérica aguda
Introduction: Acute mesenteric ischemia has a high morbidity and mortality and constitutes an intraoperative challenge in the management of ischemic areas. In this context, we analyze the use of indocyanine green fluorescence to assess intestinal vascularization through 3 clinical cases.
Case presentation: we present 3 clinical cases operated for acute mesenteric ischemia. Evaluation of intestinal viability is performed under infrared light after intravenous infusion of 25 mg of indocyanine green. Case 1 is a 42-year-old male with multiple antecedents of severe vascular disease, presenting with a massive acute mesenteric ischemia involving multiple intestinal segments. Fluorescence allows two adjusted bowel resections with double intestinal anastomosis. Case 2 is a 74-year-old woman with a history of non-anticoagulated atrial fibrillation who is decided to perform an urgent surgery when an esophageal, gastric and portal system pneumatosis is observed, as indirect signs of ischemia, in urgent CT. During the surgical act there is an ischemia of the terminal ileum and right colon without clear signs of involvement at the esophageal-gastric level. The assessment after administration of ICG discriminates the clear ischemic involvement from terminal ileum to ascending colon and patched in the transverse and left colon, without esophageal or gastric involvement. Case 3 is a 49-year-old woman with aortoiliac and visceral Takayasu disease and revascularization surgery of the celiac trunk. Given the increase in abdominal pain, a new CT scan demonstrates colonic pneumatosis. Urgent laparotomy shows necrosis at the level of the left colon and hypoperfusion of the cecum. The administration of ICG finds a lack of uptake of the entire colon. A subtotal colectomy with ileostomy and mucous fistula is performed.
Conclusions: The fluorescence with ICG provides a better visualization of the intestinal vascularization in the AMI, and allows to determine the limits of the affected tissue to perform adjusted resections.Introducción: La isquemia mesentérica aguda presenta una elevada morbi-mortalidad y constituye un reto intraoperatorio en el manejo de las áreas isquémicas. En este contexto, analizamos el uso de la fluorescencia con verde de indocianina para valorar la vascularización intestinal a través de 3 casos clínicos.
Presentación de los casos: presentamos 3 casos clínicos intervenidos por isquemia mesentérica aguda. Se realiza valoración de la viabilidad intestinal bajo luz infrarroja tras la infusión endovenosa de 25 mg de verde de indocianina. El caso 1 es un varón de 42 años con múltiples antecedentes de enfermedad vascular grave, que presenta una isquemia mesentérica aguda masiva con afecación parchaeada de múltiples segmentos intestinales. La fluorescencia permite la realización de dos resecciones intestinales ajustadas con doble anastomosis intestinal. El caso 2 es una mujer de 74 años con antecedentes de fibrilación auricular no anticoagulada a la que se le decide realizar una cirugía urgente al objetivarse una neumatosis esofágica, gástrica y en sistema porta, como signos indirectos de isquemia, en la TAC urgente. Durante el acto quirúrgico se halla una isquemia de ileon terminal y colon derecho sin claros signos de afectación a nivel esófago-gástrico. La valoración tras administración de ICG discrimina la clara afectación isquémica desde ileon terminal a colon ascendente y parcheada en colon transverso e izquierdo, sin afectación esofágica ni gástrica. El caso 3 es una mujer de 49 años con enfermedad de Takayasu aortoilíaca y visceral y cirugía de revascularización del tronco celíaco. Ante el aumento del dolor abdominal, una nueva TAC evidencia neumatosis colónica. La laparotomía urgente muestra necrosis a nivel del colon izquierdo e hipoperfusión del ciego. La administración de ICG halla una falta de captación de todo el colon. Se realiza una colectomía subtotal con ileostomía y fístula mucosa.
Conclusiones: La fluorescencia con ICG aporta una mejor visualización de la vascularización intestinal en la IMA, y permite determinar los límites del tejido afecto para realizar resecciones ajustadas.

Factores de recidiva del carcinoma hepatocelular tras resección quirúrgica con intención curativa
El carcinoma hepatocel·lular representa del 70-80% dels tumors hepàtics primaris. És la sisena neoplàsia més freqüent i la tercera causa més freqüent per càncer. Els factors de risc més importants són l'hepatitis B i C. La resecció quirúrgica és la primera opció terapèutica. La causa de la mort dels pacients sotmesos a resecció hepàtica amb intenció curativa és la recidiva. Les taxes de recidiva global arriben al 70-100% als 5 anys. Identificar els factors de risc de recidiva és important per augmentar la supervivència, aplicant teràpies preventives o incloent en llista d'espera de trasplantament.El carcinoma hepatocelular representa del 70-80% de los tumores hepáticos primarios.
