    A valid and reliable scale to assess cultural sensibility in nursing

    Background: Cultural sensibility is an important concept linked to the achievement of cultural competence. Health professionals must first improve their cultural sensibility to become culturally competent and to be able to offer competent care to culturally diverse populations. Aim To develop and psychometrically test the Cultural Sensibility Scale for Nursing (CUSNUR), a cultural sensibility scale that can be used in nursing for the achievement of competencies needed to care for culturally diverse populations. Design and methods: The cross-sectional survey was conducted over two stages. The first stage involved the cross- cultural and discipline-specific adaptation of an existing scale addressing this concept in the field of law using the reverse translation method. Second, validation of the scale was carried out from October 2016–June 2017 by studying the psychometric properties of the questionnaire through an analysis of content acceptability and reliability and through exploratory factor analysis (EFA). Results: The questionnaire was designed to be clear, easy to understand, and of adequate length, and experts involved in content validation agreed that the scale meets these criteria. A total of 253 nursing students participated in the validation stage. Four factors were identified from the EFA: (1) patient and health professional behaviours, (2) self-assessments, (3) self-awareness, and (4) cultural influence. Two items were excluded. Factorial saturation is adequate for all factors (>0.30). The Cronbach alpha was measured as 0.75. Conclusions: This study presents the first version of the CUSNUR and demonstrates that the scale is valid and reliable

    Human mobility, presence and passive transportation of Aedes aegypti mosquito, and environmental characteristics are a group of factors which contribute to the success of dengue spread and establishment. To understand this process, we assess data from dengue national and municipal basins regarding population and demographics, transportation network, human mobility, and Ae. aegypti monitoring for the Brazilian state of Acre since the first recorded dengue case in the year 2000 to the year 2015. During this period, several changes in Acre?s transport infrastructure and urbanization have been started. To reconstruct the process of dengue introduction in Acre, we propose an analytic framework based on concepts used in malaria literature, namely vulnerability and receptivity, to inform risk assessments in dengue-free regions as well as network theory concepts for disease invasion and propagation. We calculate the probability of dengue importation to Acre from other Brazilian states, the evolution of dengue spread between Acrean municipalities and dengue establishment in the state. Our findings suggest that the landscape changes associated with human mobility have created favorable conditions for the establishment of dengue virus transmission in Acre. The revitalization of its major roads, as well as the increased accessibility by air to and within the state, have increased dengue vulnerability. Unplanned urbanization and population growth, as observed in Acre during the period of study, contribute to ideal conditions for Ae. aegypti mosquito establishment, increase the difficulty in mosquito control and consequently its local receptivity

    Duke Surgery Patient Safety: an open-source application for anonymous reporting of adverse and near-miss surgical events

    BACKGROUND: Studies have shown that 4% of hospitalized patients suffer from an adverse event caused by the medical treatment administered. Some institutions have created systems to encourage medical workers to report these adverse events. However, these systems often prove to be inadequate and/or ineffective for reviewing the data collected and improving the outcomes in patient safety. OBJECTIVE: To describe the Web-application Duke Surgery Patient Safety, designed for the anonymous reporting of adverse and near-miss events as well as scheduled reporting to surgeons and hospital administration. SOFTWARE ARCHITECTURE: DSPS was developed primarily using Java language running on a Tomcat server and with MySQL database as its backend. RESULTS: Formal and field usability tests were used to aid in development of DSPS. Extensive experience with DSPS at our institution indicate that DSPS is easy to learn and use, has good speed, provides needed functionality, and is well received by both adverse-event reporters and administrators. DISCUSSION: This is the first description of an open-source application for reporting patient safety, which allows the distribution of the application to other institutions in addition for its ability to adapt to the needs of different departments. DSPS provides a mechanism for anonymous reporting of adverse events and helps to administer Patient Safety initiatives. CONCLUSION: The modifiable framework of DSPS allows adherence to evolving national data standards. The open-source design of DSPS permits surgical departments with existing reporting mechanisms to integrate them with DSPS. The DSPS application is distributed under the GNU General Public License

    Microbiological quality of human milk from a Brazilian milk bank

    OBJECTIVE: The objectives of the present study were to determine the prevalence of potentially pathogenic microorganisms that indicate the hygienic and sanitary conditions of human milk samples collected at a Human Milk Bank. METHODS: Three hundred and thirty eight (338) samples of human milk collected from a milk bank in a maternity in the municipality of Goiânia, in the state of Goias, Brazil were submitted to microbiological analysis. The latter were plated on McConkey agar according to the type of bacteria. Among the total number of samples collected, 194 consisted of raw milk and the remaining 144 were pasteurized milk. RESULTS: The presence of Staphylococcus spp., Streptococcus spp., yeasts and molds, and Enterobacteriaceae was verified in the raw milk samples. Staphylococcus aureus were isolated in 10 (5.2%) samples, Staphylococcus epidermidis in 28 (14.4%) samples, Streptococcus spp. in three (1.6%) samples, yeasts and molds in 43 (22.2%) and Enterobacteriaceae in 49 (25.3%) samples. In a hundred and forty four (144) samples which underwent thermal treatment Staphylococcus aureus was detected in five (3.5%) samples, Staphylococcus epidermidis in 15 (10.4%), Staphylococcus lugdenensis in two (1.4%), Streptococcus spp. in four (2.8%), yeasts and molds in 37 (25.7%), and Enterobacteriaceae in nine (6.3%). CONCLUSIONS: Analysis indicated a high degree of contamination in raw human milk, and as for the pasteurized milk, despite elimination of the great majority of potentially pathogenic microorganisms, the percentage of yeasts and molds was higher than in raw milk, demonstrating that a lower degree of initial contamination would be necessary for pasteurization to be an efficient means of microbiological control.OBJETIVO: Determinar a prevalência de microrganismos indicadores e potencialmente patogênicos que indicam as condições higiênico-sanitárias das amostras de leite humano ordenhado coletadas em banco de leite. MÉTODOS: Foram realizadas análises microbiológicas de 338 amostras de leite humano ordenhado, sendo 194 de leite cru e 144, pasteurizado, coletadas em banco de leite humano de um hospital materno infantil de Goiânia, GO. As análises microbiológicas foram realizadas com semeadura em ágar Mc Conkey, de acordo com o tipo de bactéria. RESULTADOS: No leite cru, verificou-se a presença de Staphylococcus spp. Streptococcus spp., bolores e leveduras e Enterobacteriaceae. Observou-se que Staphylococcus aureus esteve presente em 10 (5,2%) amostras, Staphylococcus epidermidis em 28 (14,4%), Streptococcus spp. em três (1,6%), bolores e leveduras em 43 (22,2%) e Enterobacteriaceae em 49 (25,3%). Das 144 amostras de leite humano ordenhado pasteurizado, detectaram-se Staphylococcus aureus em cinco (3,5%), Staphylococcus epidermidis em 15 (10,4%), Staphylococcus lugdenensis em duas (1,4%), Streptococcus spp. em quatro (2,8%), bolores e leveduras em 37 (25,7%) e Enterobacteriaceae em nove (6,3%). CONCLUSÕES: Os resultados mostraram um alto grau de contaminação no leite cru. No leite pasteurizado, apesar da eliminação da grande maioria de microrganismos potencialmente patogênicos, a percentagem de bolores e leveduras excedeu a de leite cru, mostrando a necessidade de obtenção de um leite com carga microbiana inicial mais baixa para que a pasteurização seja eficiente no controle microbiológico

    Genomic diversity, linkage disequilibrium and selection signatures in European local pig breeds assessed with a high density SNP chip

    Genetic characterization of local breeds is essential to preserve their genomic variability, to advance conservation policies and to contribute to their promotion and sustainability. Genomic diversity of twenty European local pig breeds and a small sample of Spanish wild pigs was assessed using high density SNP chips. A total of 992 DNA samples were analyzed with the GeneSeek Genomic Profiler (GGP) 70 K HD porcine genotyping chip. Genotype data was employed to compute genetic diversity, population differentiation and structure, genetic distances, linkage disequilibrium and effective population size. Our results point out several breeds, such as Turopolje, Apulo Calabrese, Casertana, Mora Romagnola and Lithuanian indigenous wattle, having the lowest genetic diversity, supported by low heterozygosity and very small effective population size, demonstrating the need of enhanced conservation strategies. Principal components analysis showed the clustering of the individuals of the same breed, with few breeds being clearly isolated from the rest. Several breeds were partially overlapped, suggesting genetic closeness, which was particularly marked in the case of Iberian and Alentejana breeds. Spanish wild boar was also narrowly related to other western populations, in agreement with recurrent admixture between wild and domestic animals. We also searched across the genome for loci under diversifying selection based on F-S(T) outlier tests. Candidate genes that may underlie differences in adaptation to specific environments and productive systems and phenotypic traits were detected in potentially selected genomic regions

    Autoinhibition of TBCB regulates EB1-mediated microtubule dynamics

    Tubulin cofactors (TBCs) participate in the folding, dimerization, and dissociation pathways of the tubulin dimer. Among them, TBCB and TBCE are two CAP-Gly domain-containing proteins that interact and dissociate the tubulin dimer. Here we show how TBCB localizes at spindle and midzone microtubules during mitosis. Furthermore, the motif DEI/M-COO– present in TBCB, which is similar to the EEY/F-COO– element characteristic of EB proteins, CLIP-170, and α-tubulin, is required for TBCE–TBCB heterodimer formation and thus for tubulin dimer dissociation. This motif is responsible for TBCB autoinhibition, and our analysis suggests that TBCB is a monomer in solution. Mutants of TBCB lacking this motif are derepressed and induce microtubule depolymerization through an interaction with EB1 associated to microtubule tips. TBCB is also able to bind to the chaperonin complex CCT containing α-tubulin, suggesting that it could escort tubulin to facilitate its folding and dimerization, recycling or degradation

    Associações de Foraminíferos em Resposta a Variações Ambientais da Laguna de Aveiro -- Portugal

    This study is based on the characterization of the sedimentary environment in selected sites, located in Aveiro canals and salt-pans, in the eastern-central and northern part of Aveiro Lagoon. Results of physicochemical parameters measured in water and sediment, as well as grain size, geochemical and microfaunal (benthic foraminifera) data are analyzed. The determinants factors of spatial and seasonal variability of living and dead foraminiferal assemblages were analyzed in two sampling periods, late summer and late winter, in order to study the response of these organisms to the impact caused by pollution and sazonal variation of physicochemical parameters. Biotic and abiotic results are analyzed to discriminate sites with different degree of environmental stress. The conditions of salinity, temperature, pH and Eh in the studied sites vary spatially and seasonally. The sediment in these locations is generally poorly oxygenated, suboxic or even anoxic a few millimeters below the surface. Some of the sites are affected by chemicals derived from industrial effluents and urban contaminants. The most polluted areas by heavy metals such as As, Cr, Cu and Zn are Porto de Salreu, Largo do Laranjo and some locals in the city of Aveiro. Foraminiferal assemblages in the studied sites integrate common lagoonal and estuarine euryhaline and eurythermic species, most of which tolerant to the oxygen reduction. The size of the living foraminiferal communities is reduced and their structure affected by increasing concentrations of heavy metals such as As, Cr, Cu and Zn, as well as by the high variability of environmental parameters such as low Eh, related to depressed levels of oxygen in the sediment, leading to the death of many species. The results also indicate a greater tolerance of some opportunistic species to the Pb enrichment, since the environment offer them abundance and quality of food.Este estudo baseia-se na caracterização do ambiente sedimentar de locais selecionados em canais e salinas da cidade de Aveiro, na zona central leste e na zona norte da Ria de Aveiro. Nessa caracterização foram utilizados os resultados de parâmetros físico-químicos medidos na água e nos sedimentos, assim como dados granulométricos, geoquímicos e microfaunais (foraminíferos bentônicos). Os fatores condicionantes da variabilidade espacial e sazonal das associações viva e morta de foraminíferos foram avaliados em dois períodos de amostragem: no final do verão e no final do inverno, tendo em vista a análise da resposta destes organismos ao impacto causado pela poluição e variação sazonal dos parâmetros físico-químicos. Os resultados bióticos e abióticos foram analisados para discriminar locais com diferente grau de estresseambiental. As condições de salinidade, temperatura, pH e Eh nos locais estudados variam espacial e sazonalmente. O sedimento nesses locais é em geral mal oxigenado, subóxico ou mesmo anóxico em alguns milímetros abaixo da superfície. Alguns dos locais encontram-se afetados por contaminantes químicos derivados de efluentes industriais e urbanos. As zonas mais poluídas por metais pesados como o As, Cr, Cu e Zn incluem o Porto de Salreu, Largo do Laranjo e alguns locais da Cidade de Aveiro. As associações de foraminíferos, nos locais estudados, integram espécies euritérmicas e eurihalinas comuns em ambientes lagunares e estuarinos, sendo majoritariamente tolerantes à redução de oxigênio. A dimensão das comunidades vivas de foraminíferos foi reduzida e a sua estrutura afetada pelo aumento das concentrações de metais pesados como As, Cr, Cu e Zn, assim como, pela variabilidade dos parâmetros ambientais, nomeadamente o Eh, relacionado com as condições de oxigenação do sedimento. Os dados indicam que as condições adversas podem ser causa de morte destes organismos. Sugerem também uma maior tolerância destes organismos ao enriquecimento por Pb, desde que o meio lhes proporcione abundância de alimento

    Pervasive gaps in Amazonian ecological research

    Biodiversity loss is one of the main challenges of our time and attempts to address it require a clear understanding of how ecological communities respond to environmental change across time and space. While the increasing availability of global databases on ecological communities has advanced our knowledge of biodiversity sensitivity to environmental changes, vast areas of the tropics remain understudied. In the American tropics, Amazonia stands out as the world's most diverse rainforest and the primary source of Neotropical biodiversity, but it remains among the least known forests in America and is often underrepresented in biodiversity databases. To worsen this situation, human-induced modifications may eliminate pieces of the Amazon's biodiversity puzzle before we can use them to understand how ecological communities are responding. To increase generalization and applicability of biodiversity knowledge, it is thus crucial to reduce biases in ecological research, particularly in regions projected to face the most pronounced environmental changes. We integrate ecological community metadata of 7,694 sampling sites for multiple organism groups in a machine learning model framework to map the research probability across the Brazilian Amazonia, while identifying the region's vulnerability to environmental change. 15%–18% of the most neglected areas in ecological research are expected to experience severe climate or land use changes by 2050. This means that unless we take immediate action, we will not be able to establish their current status, much less monitor how it is changing and what is being lost

    Long-term modification of cortical synapses improves sensory perception

    Synapses and receptive fields of the cerebral cortex are plastic. However, changes to specific inputs must be coordinated within neural networks to ensure that excitability and feature selectivity are appropriately configured for perception of the sensory environment. Long-lasting enhancements and decrements to rat primary auditory cortical excitatory synaptic strength were induced by pairing acoustic stimuli with activation of the nucleus basalis neuromodulatory system. Here we report that these synaptic modifications were approximately balanced across individual receptive fields, conserving mean excitation while reducing overall response variability. Decreased response variability should increase detection and recognition of near-threshold or previously imperceptible stimuli, as we found in behaving animals. Thus, modification of cortical inputs leads to wide-scale synaptic changes, which are related to improved sensory perception and enhanced behavioral performance
