    La letteratura dal punto di vista degli scrittori

    La storia della critica letteraria è stata generalmente esaminata dal punto di vista degli studiosi di teoria. Manca, a tutt’oggi, una visione d’insieme che esamini in maniera sistematica la critica letteraria dal punto di vista interno, ovvero degli autori stessi. Muovendo da tali premesse, il volume propone un’articolata analisi dei «discorsi» sulla letteratura prodotti da scrittori di lingua inglese, dalla seconda metà del Cinquecento a oggi. Allo scopo di dare ordine a una materia ampia e frastagliata, sono state individuate tre principali tipologie discorsive, o forme testuali, attraverso cui gli autori hanno dato voce alle proprie idee sulla letteratura. A tali tipologie discorsive corrispondono le tre sezioni in cui è stata suddivisa la materia critica del volume. La prima sezione, «Saggi e paratesti», esamina le teorie letterarie esposte in forma di saggi, prefazioni, commenti da parte dell’autore. La seconda sezione, «Disseminazioni», analizza le idee sulla letteratura sparse all’interno di romanzi, drammi, poesie. La terza sezione, infine, «Maschere d’autore», si concentra sulle teorie letterarie la cui esposizione è affidata a un alter ego dello scrittore, ovvero a una maschera parzialmente autobiografica. Ne emerge un quadro ricco e composito all’interno del quale teoria e prassi letteraria si arricchiscono reciprocamente, fino a fondersi in una più complessa unità i cui confini appaiono labili, indefiniti, negoziabili

After summarising the design and implementation of WEAVE and its data systems, we present the organisation, science drivers and design of a five- to seven-year programme of eight individual surveys to: (i) study our Galaxy's origins by completing Gaia's phase-space information, providing metallicities to its limiting magnitude for ∼ 3 million stars and detailed abundances for ∼ 1.5 million brighter field and open-cluster stars; (ii) survey ∼ 0.4 million Galactic-plane OBA stars, young stellar objects and nearby gas to understand the evolution of young stars and their environments; (iii) perform an extensive spectral survey of white dwarfs; (iv) survey  ∼ 400 neutral-hydrogen-selected galaxies with the IFUs; (v) study properties and kinematics of stellar populations and ionised gas in z 1 million spectra of LOFAR-selected radio sources; (viii) trace structures using intergalactic/circumgalactic gas at z > 2.     Plunging ranula in children: case report and literature review

    Few cases of plunging ranulas (PRs) occur during childhood and the lesions are frequently misdiagnosed. Here, a PR in a child is reported along with a literature review. A seven-year-old female complaining of swelling in the midline neck, left-submandibular region, was evaluated. No oral cavity or major salivary glands abnormalities were detected. On palpation, a soft, painless, and fluid-containing mass was observed. The suspicion PR was performed by ultrasound. The diagnosis was confirmed with a histopathological examination. The lesion was removed with a cervical approach, without recurrence. PR is an uncommon condition in children under 10 years of age. Differential diagnosis depends on clinical examination and ultrasonography. A computed tomography-scan and magnetic resonance imaging can be performed if the diagnosis remains uncertain. In pediatrics, the key to success of the treatment may rely on the radical excision of the cyst and sublingual gland, via an intraoral or submandibular approach

    Recurrent intussusception caused by submucosal, heterotopic gastric mucosa in the terminal ileum

    Isolated, heterotopic gastric mucosa (HGM) can be a rare, pathologic leading point for intestinal intussusception in pediatric age. Affected children often experience recurrent episodes of intussusception, sometimes with delay in diagnosis and more than one surgery, because of difficulty in clear diagnosis and identification of HGM. Intraoperative findings of isolated HGM, as reported in literature, are visible or palpable lesions, protruding into the intestinal lumen. We describe the case of a 4 years old boy with recurrent intussusception, caused by a very small islet of ileal, isolated HGM, entirely developing in the submucosal layer and with a normal overlying mucosa. The difficulties in its diagnosis and treatment are described and the role of 99mTc pertechnetate scan and ultrasound are discussed.     Isolated colon ischemia with norovirus infection in preterm babies: a case series

    Introduction. Norovirus infection with necrotizing enterocolitis has so far been reported as a specific tropism of the small bowel in premature newborns. Case presentation. Three cases of premature newborns presenting with extensive isolated colonic ischemia due to norovirus infection are reported.Patient 1 was a Caucasian girl with a gestational age of 29+2 weeks. She had sudden onset of abdominal distension on the 30th day of life. Radiological signs of colonic pneumatosis were present 48 hours before perforation and stool analysis was positive for norovirus. On the 34th day, free air was detected on plain abdominal X-ray. At laparotomy, stenosis, necrosis and perforations involved the whole colon. The patient underwent ileostomy. A large colon resection and ileosigmoid anastomosis were done 3 months later.Patient 2 was a Caucasian boy with a gestational age of 28+3 weeks. On the 19th day, bloody stools with abdominal distension appeared. Stool analysis resulted positive for norovirus. A plain abdominal X-ray showed distended bowel loops. Antibiotic treatment was started. On the 32nd day due to the progressive deterioration of clinical conditions and the appearance of colic pneumatosis, a laparotomy was performed. Severe damage of the transverse colon and multiple areas of necrosis were found. Terminal ileostomy was performed. Six months later surgery consisted of mid-transverse colon resection as far as the splenic flexure, colocolic anastomosis and closure of ileostomy.Patient 3 was a Caucasian boy with a gestational age of 30 weeks. On the 44th day bloody-mucous stools appeared and stool analysis was positive for norovirus infection. Even with institution of antibiotic therapy clinical abdominal radiologic signs of colonic pneumatosis of the upper right quadrant were found. At the 70th day an explorative laparotomy showed dilated bowel loops and stenotic right colon and ileostomy was mandatory. Partial colectomy was later necessary and ileocolic anastomosis was performed. Conclusion: We hypothesize that norovirus infection may be responsible for severe, distinctive colonic lesions, even in premature newborn infants

    Cryptorchidism with short spermatic vessels: staged orchiopexy preserving spermatic vessels

    Purpose. Patients with cryptorchidism can have such short spermatic vessels that it is impossible to place the testicle in a satisfactory scrotal position using conventional orchiopexy. In these cases the most commonly used operation is 1 to 2-stage Fowler-Stephens orchiopexy. We present our surgical experience using staged inguinal orchiopexy without section of the spermatic vessels in patients with short spermatic vessels. Materials and Methods. We used 2-stage inguinal orchiopexy in 38 children with intra-abdominal testis or testis peeping through the internal ring and short spermatic vessels (7 bilateral). Spermatic vessels were not sectioned, but were lengthened through progressive traction of the spermatic cord wrapped in polytetrafluoroethylene pericardial membrane (Preclude®). In the first stage we mobilized the spermatic cord in the retroperitoneal space and then wrapped it in the polytetrafluoroethylene membrane. We subsequently attached the testis to the invaginated scrotal bottom. At 9 to 12 months we performed the second stage, which involved removing the polytetrafluoroethylene membrane. Results. From the first to the second stage we observed progressive descent of the testicle toward the scrotum. At 1 to 8-year followup after the second stage all 45 testicles were palpable in a satisfactory scrotal position with stable or increased testicular volume. Conclusions. This technique represents an alternative to Fowler-Stephens orchiopexy, which can be associated with a greater risk of testicular ischemia
