    Migration and mental health in Europe (the state of the mental health in Europe working group: appendix 1)

    BACKGROUND: This paper is a part of the work of the group that carried out the report "The state of the mental health in Europe" (European Commission, DG Health and Consumer Protection, 2004) and deals with the mental health issues related to the migration in Europe. METHODS: The paper tries to describe the social, demographical and political context of the emigration in Europe and tries to indicate the needs and (mental) health problems of immigrants. A review of the literature concerning mental health risk in immigrant is also carried out. The work also faces the problem of the health policy toward immigrants and the access to health care services in Europe. RESULTS: Migration during the 1990s has been high and characterised by new migrations. Some countries in Europe, that have been traditionally exporters of migrants have shifted to become importers. Migration has been a key force in the demographic changes of the European population. The policy of closed borders do not stop migration, but rather seems to set up a new underclass of so-called "illegals" who are suppressed and highly exploited. In 2000 there were also 392.200 asylum applications. The reviewed literature among mental health risk in some immigrant groups in Europe concerns: 1) highest rate of schizophrenia; suicide; alcohol and drug abuse; access of psychiatric facilities; risk of anxiety and depression; mental health of EU immigrants once they returned to their country; early EU immigrants in today disadvantaged countries; refugees and mental health Due to the different condition of migration concerning variables as: motivation to migrations (e.g. settler, refugees, gastarbeiters); distance for the host culture; ability to develop mediating structures; legal residential status it is impossible to consider "migrants" as a homogeneous group concerning the risk for mental illness. In this sense, psychosocial studies should be undertaken to identify those factors which may under given conditions, imply an increased risk of psychiatric disorders and influence seeking for psychiatric care. COMMENTS AND REMARKS: Despite in the migrants some vulnerable groups were identified with respect to health problems, in many European countries there are migrants who fall outside the existing health and social services, something which is particularly true for asylum seekers and undocumented immigrants. In order to address these deficiencies, it is necessary to provide with an adequate financing and a continuity of the grants for research into the multicultural health demand. Finally, there is to highlight the importance of adopting an integrated approach to mental health care that moves away from psychiatric care only

    Spatial variation of picoplankton communities along a cascade reservoir system in Patagonia, Argentina

    In this study we explored how picoplankton community structure and diversity varied along three cascade oligo-mesotrophic reservoirs of the Limay River (Patagonia, Argentina): Alicura, Piedra del Águila and Ramos Mexía. We analyzed the spatial changes, covering lotic and lentic stretches along a gradient of 262 km from Andes to steppe, and we also sampled the main affluent of the Limay River (Collon Cura). In all sampling sites the main limnological variables were measured, and the picoplankton abundance (autotrophic and heterotrophic) was analyzed by flow cytometry. The bacterial biodiversity was assessed using high throughput sequencing Illumina MiSeq. We expected an increase in the trophic state along this series of cascade reservoirs, which would determine spatial differences in the structure of the picoplankton communities. We also hypothesized that the lotic and lentic conditions along the system would influence the bacterial composition. The results showed a slight increase in trophic state together with an increase in overall picoplankton abundance downstream, towards Ramos Mexía Reservoir. Picocyanobacteria were represented by phycoerythrin-rich cells all along the system, in accordance to the pattern described for oligotrophic aquatic ecosystems. Multivariate analyses based on bacterial OTU composition and environmental variables showed a spatial ordination of sites following the trend of increasing trophic state downstream. Molecular analyses of bacterial OTU diversity also showed an increase in richness and a decrease in evenness at the lotic stretches, and the opposite pattern in the reservoirs, suggesting that water retention time may play a role in structuring the community composition.Fil: Bernal, María Carolina.     Holistic screening of collapsing honey bee colonies in Spain: a case study

    Background: Here we present a holistic screening of collapsing colonies from three professional apiaries in Spain. Colonies with typical honey bee depopulation symptoms were selected for multiple possible factors to reveal the causes of collapse. Results: Omnipresent were Nosema ceranae and Lake Sinai Virus. Moderate prevalences were found for Black Queen Cell Virus and trypanosomatids, whereas Deformed Wing Virus, Aphid Lethal Paralysis Virus strain Brookings and neogregarines were rarely detected. Other viruses, Nosema apis, Acarapis woodi and Varroa destructor were not detected. Palinologic study of pollen demonstrated that all colonies were foraging on wild vegetation. Consequently, the pesticide residue analysis was negative for neonicotinoids. The genetic analysis of trypanosomatids GAPDH gene, showed that there is a large genetic distance between Crithidia mellificae ATCC30254, an authenticated cell strain since 1974, and the rest of the presumed C. mellificae sequences obtained in our study or published. This means that the latter group corresponds to a highly differentiated taxon that should be renamed accordingly. Conclusion: The results of this study demonstrate that the drivers of colony collapse may differ between geographic regions with different environmental conditions, or with different beekeeping and agricultural practices. The role of other pathogens in colony collapse has to bee studied in future, especially trypanosomatids and neogregarines. Beside their pathological effect on honey bees, classification and taxonomy of these protozoan parasites should also be clarified

    Effectiveness, retrievability, and safety of celect vs. ALN inferior vena cava filters: a randomized prospective multicenter controlled study

    The purpose of this study is to compare IVC Celect and ALN filters in regard to their efficacy, retrievability, and one-year follow-up after retrieval. Materials and Methods: This is a prospective randomized study, conducted in three centers between April 2020 and May 2021. A total of 115 participants were randomized, of which 15 participants were excluded for various reasons. Each group of 50 participants was finally assigned a type of filter (Celect n = 50 and ALN = 50). Tilt angles at placement and retrieval, rates of overall filter retrieval, complications, complex retrieval, and clinical follow-up at 12 months were compared. Results: One hundred participants (59 men and 41 women) were included. The mean age was 62.4 ± 13.3, with no significant differences between both groups (p 0.503). The mean of dwelling time was 44.7 ± 93 days. (p 0.520) Filter retrieval was successful in all participants (100%). The main complication in CT prior to removal was tilt >15° (31%) and filter tip embedment (16%). No significant differences were observed in the type of retrieval (standard or complex) between both filters (p 0.24); however, fluoroscopy time was significantly longer in the complex removal of the Celect filter: 29.1 ± 12 min vs. 17.5 ± 10.3 (p 0.005). After one year, no recurrent pulmonary embolism was observed in both types of filters. Conclusions: The ALN filter had a significantly lower rate of tip and filter tip embedding, with no differences in complexity or removal success rate. There were significant differences in the fluoroscopy time consumed for standard and complex retrievals between both filters, with less time for the ALN filter

    La imagen y la narrativa como herramientas para el abordaje psicosocial en escenarios de violencia. Departamento de Cundinamarca, Colombia y Ciudad de París, Francia

    El presente documento recoge las diferentes experiencias y conocimientos adquiridos a lo largo del diplomado: Acompañamiento psicosocial en escenarios de violencia, resaltando diferentes herramientas de intervención como lo son el enfoque narrativo y la foto intervención. Por un lado, se presenta el caso de Carlos Arturo, un joven de catorce años, víctima del conflicto armado quien sufrió secuelas psicosociales por desplazamiento forzado y afectaciones físicas. A partir de este caso se plantean tres acciones de intervención y se estudia la pregunta como herramienta de análisis. Además, se reflexiona acerca del caso de Peñas Coloradas, una comunidad que es desalojada de su tierra por dedicarse al cultivo de coca, ya que no tenían otros recursos para resurgir laboralmente y son condenados de guerrilleros. Después de analizar este caso se plantean tres estrategias intervención además de la reflexión de los impactos psicosociales que sufre esta población. Finalmente, se presenta un estudio analítico y reflexivo de la experiencia de la foto voz realizada en la ciudad de Bogotá, Colombia y París, Francia, en diferentes escenarios que tienen como común denominador una situación de violencia, estos lugares también se caracterizan por haber transformado ese dolor en historias de esperanza.This document gathers different experiences and knowledge we acquired throughout the course: Psychosocial accompaniment in scenarios of violence, highlighting the intervention tools such as the Narrative Approach and the Photo Voice. On the first hand, it presents the case of Carlos Arturo, a 14-year-old victim of the armed conflict, who suffered psychosocial sequels due to forced migration and physical affectations. Based on this case, three actions are proposed, as well as the question is studied as a tool for analysis from the Narrative Approach. In addition, the case of Peñas Coloradas was also discussed. This community was evicted from their land for cultivating coca, since they had no other resources to recover their labor and were condemned as guerrillas (as a terrorist group). After analyzing this case, three strategies for intervention are proposed, as well the psychosocial impacts suffered by this population were analyzed. Finally, we present an analytical and reflective comment of the experience of the Photo Voice that each of us had in different scenarios of Bogota, Colombia and one scenario in Paris, France. All these events have as common denominator: a situation of violence, but they have managed to transform that pain to overcome the trauma and create new stories

    A web-based educational therapy intervention associated with physical exercise to promote health in fibromyalgia in Brazil: the Amigos De Fibro (Fibro Friends) study protocol.

    Health education is one of the main items to enable health promotion to patients with fibromyalgia. The objective of the study "Amigos de Fibro (Fibro Friends)" is to evaluate the impact of an educational intervention associated with physical exercise based on the web in promoting health and quality of life of patients with fibromyalgia in Brazil. A study with a randomized controlled trial approach will be carried out. The sample will consist of 24 participants, divided into two groups, with 12 individuals each. The experimental group will participate in meetings with lectures, debates, conversation rounds and exercises by a multidisciplinary team. Physical exercises will also be performed in an online environment. On the other hand, the control group will receive an e-book of education and self-care. Primary outcomes will be quality of life. The secondary outcomes will be sociodemographic and health profile, pain intensity, sleep quality, self-care agency, usage and costs of health and social care services, viability of the program and program participation. In addition, a qualitative evaluation process will be carried out with the participants. After the intervention, the data of both groups will be collected again, as well as after 3, 6, and 12 months to verify the effect and the maintenance of the intervention. The results will provide data for studies to consider the use of this tool in the future by professionals working in the field of rheumatology.     Diabetes autorreferido e fatores associados na população adulta brasileira : Pesquisa Nacional de Saúde, 2019

    This study aims to analyze the prevalence of self-reported diabetes and its associated factors in the Brazilian adult population. It is a cross-sectional study using the 2019 National Health Survey. Prevalence and crude prevalence ratios (PRc) and adjusted prevalence ratios (PRa) of self-reported diabetes were estimated, with confidence intervals (95% CI), using Poisson regression. In the 82,349 adults, the prevalence of self-reported diabetes was 7.7%. Positively associated factors were: advanced age with greater association after 60 years (PRa 24.87; 95%CI 15.78-39.18); living in the Northeast (PRa 1.16; 95%CI 1.04-1.29), Southeast (PRa 1.27; 95% CI 1.14-1.43), South (PRa 1.18; 95%CI 1, 05- 1.34), and Midwest (PRa 1.21; 95%CI 1.06- 1.38); being a former smoker (PRa 1.17; 95%CI 1.09-1.27); self-assessment of regular health (PRa 2.41; 95%CI 2.21-2.64), bad/very bad (PRa 3.45; 95%CI 3.06-3.88); having heart disease (PRa 1.81; 95%CI 1.64-2.00), hypertension (PRa 2.84; 95%CI 2.60-3.69), high cholesterol (PRa 2.22; 95%CI 2.05-2.41), overweight (PRa 1.49; 95%CI 1.36-1.64), and obesity (PRa 2.25; 95%CI 2.05- 2.47). It could be concluded that diabetes in Brazilian adults is associated with sociodemographic factors, aging, lifestyle, and morbidities. These results can guide public policies for the prevention and control of disease in Brazil.O estudo analisa a prevalência de diabetes autorreferido e fatores associados na população adulta brasileira. Estudo transversal usando a Pesquisa Nacional de Saúde 2019. Estimaram-se as prevalências e razões de prevalência brutas (RPb) e ajustadas (RPa) de diabetes autorreferido, com intervalos de confiança (IC95%), empregando-se regressão de Poisson. Nos 82.349 adultos, a prevalência de diabetes autorreferido foi de 7,7%. Associaram-se positivamente: idade avançada, sendo maior após 60 anos (RPa 24,87; IC95%: 15,78-39,18); residir nas regiões Nordeste (RPa 1,16; IC95%: 1,04-1,29), Sudeste (RPa 1,27; IC95%: 1,14-1,43), Sul (RPa 1,18; IC95%: 1,05- 1,34) e Centro-Oeste (RPa 1,21; IC95%: 1,06- 1,38), ser ex-fumante (RPa 1,17; IC95%: 1,09- 1,27), autoavaliação de saúde regular (RPa 2,41; IC95%: 2,21-2,64), ruim/muito ruim (RPa 3,45; IC95%: 3,06-3,88), ter doença cardíaca (RPa 1,81; IC95%: 1,64-2,00), hipertensão (RPa 2,84; IC95%: 2,60-3,69), colesterol elevado (RPa 2,22; IC95%: 2,05-2,41), sobrepeso (RPa 1,49; IC95%: 1,36-1,64) e obesidade (RPa 2,25; IC95%: 2,05- 2,47). Conclui-se que o diabetes nos adultos brasileiros se associa a fatores sociodemográficos, envelhecimento, estilos de vida e morbidades. Esses resultados podem orientar políticas públicas para prevenção e controle da doença no Brasil

    C-reactive protein cut-off for early tocilizumab and dexamethasone prescription in hospitalized patients with COVID-19

    Dexamethasone and tocilizumab have been associated with reduction in mortality, however, the beneficial effect is not for all patients and the impact on viral replication is not well defined. We hypostatized that C-reactive protein (CRP) could help in the identification of patients requiring anti-inflammatory therapy. Patients admitted for > 48 h in our hospital for a confirmed or suspected infection by SARS-CoV-2 from February 2020 to February 2021 were retrospectively evaluated. The primary outcome was mortality at 30 days. Demographics and the most relevant variables related with the outcome were included. CRP was stratified by percentiles. Univariate and multivariate analysis were performed. A total of 3218 patients were included with a median (IQR) age of 66 (74-78) years and 58.9% were males. The rate of intensive care unit admission was 24.4% and the 30-day mortality rate was 11.8%. Within the first 5 days from admission, 1018 (31.7%) patients received dexamethasone and 549 tocilizumab (17.1%). The crude analysis showed a mortality reduction in patients receiving dexamethasone when CRP was > 13.75 mg/dL and > 3.5 mg/dL for those receiving tocilizumab. Multivariate analysis identified the interaction of CRP > 13.75 mg/dL with dexamethasone (OR 0.57; CI 95% 0.37-0.89, P = 0014) and CRP > 3.5 mg/dL with tocilizumab (0.65; CI95%:0.44-0.95, P = 0.029) as independent predictors of mortality. Our results suggest that dexamethasone and tocilizumab are associated with a reduction in mortality when prescribed to patients with a certain inflammatory activity assessed by C-reactive protein

    Universidad Autónoma del Estado de MéxicoEl mundo de las profesiones está cambiando rápidamente en las última tres décadas, no existe profesión alguna que permanezca estática ante el impacto del desarrollo tecnológico. En el caso de la profesión del diseño se ha trastocado su concepción, fundamento y acción ante el advenimiento de la era digital. El desarrollo tecnológico de la era digital ha provocado una disrupción en la industria y en los servicios, estamos ante la cuarta revolución industrial y de servicios (4.0 RIS), que demanda un nuevo diseñador con cualidades renovadas preparado para el manejo de nuevas tecnologías y de nuevos materiales con ideas innovadoras y creativas que responda a una visión sustentable, aplicable y funcional de los objetos, productos, sistemas y procesos. Frente a la emergencia de ecosistemas industriales y de servicios emergentes la profesión de diseño se coloca como la profesión del futuro. Esto exige conocer la realidad actual de ésta profesión para pensar en el futuro mediato; con este espíritu, nace la obra que el lector tiene en sus manos: La profesión del diseño, expresiones y experiencias. Estamos ciertos que hoy existe una convivencia de paradigmas de las revoluciones industriales, diseños basados en tecnologías manuales, mecánicas, automáticas y digitales. Cada tecnología trae consigo su marco interpretativo teórico, contextual e histórico, riqueza que se muestra, en esta dirección, en las páginas de este libro. En este libro participan diseñadores industriales y gráficos –y uno que otro invitado no diseñador- que nos comparten de forma apasionada sus expresiones y experiencias que van desde la preocupación con miras al futuro del 2030 hasta propuesta de una formación creativa para el diseñador a través del desarrollo del Jugendgarten. También, se presenta la dimensión simbólica del diseño arquitectónico a través de sus formas geométricas y sus rituales sagrados. No se olvidan las dimensiones del desarrollo humano y de las expresiones posmodernas del diseño; así como, de las miradas antropológica, sociocultural, de identidad y de género. Sin duda alguna, se muestra tres momentos históricos del diseño: artesanal, industrial y digital.La profesión del diseño, expresiones y experiencias, es un libro orgánico con la participación de especialistas en diseño que comparten sus puntos de vista y sus experiencias. Brinda un profundo análisis de la situación que guarda el diseño en los contextos social, industrial, empresarial y académico. Una de las conclusiones relevantes es que la profesión del diseño es una profesión del futuro porque la acción de diseñar es inherente a la transformación del mundo en la cuarta revolución industrial resultado de la evolución científica y tecnológica. Sus coordinadores, estudiosos de la temática, gozan del prestigio en la comunidad científica, son integrantes del Sistema Nacional de Investigadores, la Dra.
