    Introducción: La artroplastia total de rodilla (PTR) es eficaz en el alivio del dolor y la función en los casos de artrosis de rodilla (AR) grave. Representa un problema de salud pública por su prevalencia. A pesar de los resultados clínicos obtenidos con la PTR, diversos estudios refieren que entre un 14-30% de pacientes se muestran insatisfechos con el resultado, al no alcanzar los beneficios esperados, lo que conlleva que otros desestimen el tratamiento quirúrgico. El modelo de cuidados de M. Mishel pertenece al paradigma de la transformación, parte de la teoría social crítica e incorpora principios de la teoría del caos para elaborar su teoría de la incertidumbre; la enfermedad crónica desencadena un desequilibrio y las personas que la experimentan pueden incorporar esta incertidumbre para adquirir un nuevo sentido de la vida

    Comparative study of preventive protocols in children at high cariogenic risk.

    The aim of this study was to compare the efficacy of two preventive protocols - fluoride gel (F) alone or combined with chlorhexidine varnishes (CHX) - on sialochemical, clinical and microbiological parameters in a group of children at high cariogenic risk. Two therapeutic-preventive protocols were applied in 73 children at high cariogenic risk (average age 6.2±1.4 years old) and clinical parameters (simplified oral hygiene index: OHIS; decayed, missing and filled teeth: dmf index; sugar intake and exposure to fluoride), as well as sialochemical parameters (salivary pH and flow, buffer capacity) and microbiological parameters (CFU/mg of dental biofilm of Streptococcus mutans group) were recorded and correlated before and after the protocols. Association was found between parameters that cause deficient control of dental biofilm: high values of OHIS index, CFU/mg dental biofilm, sugar intake and the d component of dmft index, and lower values of salivary flow rate and buffer capacity. After the protocols, a significant decrease was found in OHI-S and CFU/mg dental biofilm. No significant difference was found with children’s gender and age. The association observed between OHI-S and cariogenic bacteria emphasizes the importance of prevention, especially regarding the oral health of the most vulnerable children. The early inclusion of F associated with CHX in the initial step of preventive and therapeutic protocols would provide benefits regarding oral microbe control while children acquire new habits of oral hygiene.publishedVersio

    Essential oils from Argentinean native species reduce in vitro methane production

    The aim of this study was to evaluate the effect of Argentinean essential oils (EO) on methane production during in vitro fermentation compared to EO with proven effects as rumen fermentation modifiers. A complete randomized block design was used and the treatments included EO from Aloysia gratissima (50, 100, 150 and 300 mg/L), Eucalyptus globulus (5, 50, 150 and 300 mg/L), Lippia turbinata (30, 60, 120 and 240 mg/L), Mentha x piperita var. vulgaris (50, 100, 200 and 400 mg/L), Origanum vulgare ssp. hirtum cv. Compacto (0.5, 5, 50 and 250 mg/L) and cv. Mendocino (50, 150, 250 and 350 mg/L), Rosmarinus officinalis (100, 300, 500 and 700 mg/L), Schinus molle (75, 150, 300 and 600 mg/L), Tagetes minuta (5, 50, 125 and 250 mg/L), and Thymus vulgaris (5, 50, 150 and 300 mg/L). Two controls were included: control (not containing EO) and monensin (1.87 mg/L). Variables measured were: digestibility of neutral detergent fiber, gas and methane production. Compared to the control, monensin reduced methane production by 44%, but with a 15% reduction in fiber digestibility. Except for E. globulus and L. turbinata, each variable measured was significantly reduced with the higher level of inclusion compared to the control, representing that overall microbial activity was affected. L. turbinata (60.8% limonene) reduced methane by half compared to control, and by 35 to 85% when compared to monensin, without affecting digestibility of the fiber. Some essential oils from native Argentinean plants exhibited great potential to reduce enteric methane production without affecting digestibility, of which L. turbinata was the most promising alternative.El objetivo de este estudio fue evaluar el efecto de aceites esenciales (AE) de plantas nativas de Argentina en la producción de metano durante la fermentación in vitro, en comparación con AE con efectos comprobados como modificadores de la fermentación ruminal. Se realizó un diseño en bloque completos al azar y los AE y dosis evaluados fueron: Aloysia gratissima (0,5, 5, 50 y 250 mg/L) , Eucalyptus globulus (5, 50, 150 y 300 mg/L), Lippia turbinata (30, 60, 120 y 240 mg/L), Mentha x piperita var. vulgaris (50, 100, 200 y 400 mg/L), Origanum vulgare ssp. hirtum cv. Compacto (0,5, 5, 50 y 250 mg/L) y cv. Mendocino (50, 150, 250 y 350 mg/L), Rosmarinus officinalis (100, 300, 500 y 700 mg/L), Schinus molle (75, 150, 300 y 600 mg/L), Tagetes minuta (5, 50, 125 y 250 mg/L), y Thymus vulgaris (5, 50, 150 y 300 mg/L). Se incluyeron dos controles: control (sin AE) y monensina (1,87 mg/L). Las variables evaluadas fueron: digestibilidad in vitro de la fibra en detergente neutro, producción de gas y metano. En comparación al control, la monensina redujo la producción de metano en un 44%, pero con una reducción de un 15% en la digestibilidad de la fibra. Con excepción a E. globulus y L. turbinata, todas las variables fueron reducidas significativamente con el mayor nivel de inclusión comparado con el control, lo cual representa que la actividad microbiana se vio afectada. L. turbinata (60,8% limoneno) redujo la producción de metano a la mitad comparado al control, y entre un 35 y 85% en comparación a la monensina, sin que la digestibilidad de la fibra se vea afectada. Algunos aceites esenciales de plantas nativas de Argentina demostraron gran potencial para reducir la producción de metano entérico sin afectar la digestibilidad, de los cuales L. turbinata fue la alternativa más promisoria.EEA ManfrediFil: García, F.     Modelling phenolic and technological maturities of grapes by means of the multivariate relation between organoleptic and physicochemical properties

    The ripeness of grapes at the harvest time is one of the most important parameters for obtaining high quality red wines. Traditionally the decision of harvesting is to be taken only after analysing sugar concentration, titratable acidity and pH of the grape juice (technological maturity). However, these parameters only provide information about the pulp ripeness and overlook the real degree of skins and seeds maturities (phenolic maturity). Both maturities, technological and phenolic, are not simultaneously reached, on the contrary they tend to separate depending on several factors: grape variety, cultivar, adverse weather conditions, soil, water availability and cultural practices. Besides, this divergence is increasing as a consequence of the climate change (larger quantities of CO2, less rain, and higher temperatures). 247 samples collected in vineyards representative of the qualified designation of origin Rioja from 2007 to 2011 have been analysed. Samples contain the four grape varieties usual in the elaboration of Rioja wines (‘tempranillo’, ‘garnacha’, ‘mazuelo’ and ‘graciano’). The present study is the first systematic investigation on the maturity of grapes that includes the organoleptic evaluation of the degree of grapes maturity (sugars/acidity maturity, aromatic maturity of the pulp, aromatic maturity of the skins and tannins maturity) together with the values of the physicochemical parameters (probable alcohol degree, total acidity, pH, malic acid, K, total index polyphenolics, anthocyans, absorbances at 420, 520 and 620 nm, colour index and tartaric acid) determined over the same samples. A varimax rotation of the latent variables of a PLS model between the physicochemical variables and the mean of four sensory variables allows identifying both maturities.     Low penetrance in facioscapulohumeral muscular dystrophy type 1 with large pathological D4Z4 alleles: a cross-sectional multicenter study.

    BackgroundFacioscapulohumeral muscular dystrophy type 1(FSHD1) is an autosomal dominant disorder associated with the contraction of D4Z4 less than 11 repeat units (RUs) on chromosome 4q35. Penetrance in the range of the largest alleles is poorly known. Our objective was to study the penetrance of FSHD1 in patients carrying alleles ranging between 6 to10 RUs and to evaluate the influence of sex, age, and several environmental factors on clinical expression of the disease. Methods A cross-sectional multicenter study was conducted in six French and one Swiss neuromuscular centers. 65 FSHD1 affected patients carrying a 4qA allele of 6¿10 RUs were identified as index cases (IC) and their 119 at-risk relatives were included. The age of onset was recorded for IC only. Medical history, neurological examination and manual muscle testing were performed for each subject. Genetic testing determined the allele size (number of RUs) and the 4qA/4qB allelic variant. The clinical status of relatives was established blindly to their genetic testing results. The main outcome was the penetrance defined as the ratio between the number of clinically affected carriers and the total number of carriers. Results Among the relatives, 59 carried the D4Z4 contraction. At the clinical level, 34 relatives carriers were clinically affected and 25 unaffected. Therefore, the calculated penetrance was 57% in the range of 6¿10 RUs. Penetrance was estimated at 62% in the range of 6¿8 RUs, and at 47% in the range of 9¿10 RUs. Moreover, penetrance was lower in women than men. There was no effect of drugs, anesthesia, surgery or traumatisms on the penetrance. Conclusions Penetrance of FSHD1 is low for largest alleles in the range of 9¿10 RUs, and lower in women than men. This is of crucial importance for genetic counseling and clinical management of patients and families
