Importance of clinical and morphological correlations in diagnosing langerhans cell histiocytosis
Langerhans cell histiocytosis (LCH) is a clonal histiocytic disorder. The variable
clinical manifestations from isolated bone lesion to multisystem disease can cause
difficulties and delay in diagnosis. We report a 2 years and 8 months-old girl who
presented with a 2 weeks history of persistent fever and weight loss associated with
progressive abdominal distension. Physical examination revealed pallor, bilateral
proptosis, seaborrheic dermatitis over the scalp and hepatosplenomegaly. Skull
X-ray demonstrated multiple lytic lesions at the base and the skull vault. Bone
marrow morphology showed numerous abnormal Langerhans cells (LCs) and
foamy macrophages. The trephine immunohistochemistry (IHC) stains for CD1a,
S-100 and CD68 were inconclusive. The diagnosis of multisystem Langerhans
cell histiocytosis (MS-LCH) in this patient was based on the clinical presentation,
radiological and morphological analysis. She subsequently received chemotherapy
and currently she is on maintenance therapy with a good clinical response. LCH is
a rare disease and although the IHC was inconclusive, the correlation of clinical,
radiological and morphological data are essential for the diagnosis
First detection of a VHE gamma-ray spectral maximum from a Cosmic source: H.E.S.S. discovery of the Vela X nebula
The Vela supernova remnant (SNR) is a complex region containing a number of
sources of non-thermal radiation. The inner section of this SNR, within 2
degrees of the pulsar PSR B0833-45, has been observed by the H.E.S.S. gamma-ray
atmospheric Cherenkov detector in 2004 and 2005. A strong signal is seen from
an extended region to the south of the pulsar, within an integration region of
radius 0.8 deg. around the position (RA = 08h 35m 00s, dec = -45 deg. 36'
J2000.0). The excess coincides with a region of hard X-ray emission seen by the
ROSAT and ASCA satellites. The observed energy spectrum of the source between
550 GeV and 65 TeV is well fit by a power law function with photon index = 1.45
+/- 0.09(stat) +/- 0.2(sys) and an exponential cutoff at an energy of 13.8 +/-
2.3(stat) +/- 4.1(sys) TeV. The integral flux above 1 TeV is (1.28 +/- 0.17
(stat) +/- 0.38(sys)) x 10^{-11} cm^{-2} s^{-1}. This result is the first clear
measurement of a peak in the spectral energy distribution from a VHE gamma-ray
source, likely related to inverse Compton emission. A fit of an Inverse Compton
model to the H.E.S.S. spectral energy distribution gives a total energy in
non-thermal electrons of ~2 x 10^{45} erg between 5 TeV and 100 TeV, assuming a
distance of 290 parsec to the pulsar. The best fit electron power law index is

3.9 day orbital modulation in the TeV gamma-ray flux and spectrum from the X-ray binary LS 5039
New observations of LS 5039, a High Mass X-ray Binary comprising a massive
star and compact object, were carried out with the High Energy Stereoscopic
System of Cherenkov Telescopes (H.E.S.S.) in 2005 at very high energy (VHE)
gamma-ray energies. These observations reveal that its flux and energy spectrum
are modulated with the 3.9 day orbital period of the binary system. This is the
first time in gamma-ray astronomy that orbital modulation has been observed,
and periodicity clearly established using ground-based gamma-ray detectors. The
VHE gamma-ray emission is largely confined to half of the orbit, peaking around
the inferior conjunction epoch of the compact object. For this epoch, there is
also a hardening of the energy spectrum in the energy range between 0.2 TeV and
a few TeV. The flux vs. orbital phase profile provides the first clear
indication of gamma-ray absorption via pair production within an astrophysical
source, a process which is expected to occur if the gamma-ray production site
is situated within ~1 AU of the compact object. Moreover the production region
size must be not significantly greater than the binary separation (~0.15 AU).
Notably, these constraints are also considerably smaller than the collimated
outflows or jets (extending out to ~1000 AU) observed in LS 5039. The spectral
hardening could arise from variations with phase in the maximum electron

Rumours, sects and rallies : the ethnic politics of recent Hmong Millenarian movements in Vietnam’s highlands
Contrary to modernist assumptions, millenarianism has not died out but continues to influence the politics of many marginalised groups in upland Southeast Asia, including the Hmong. This article summarises and analyses post-World War II Hmong millenarian activity in Vietnam, focusing on three case studies from the 1980s onwards, within the political backdrop of ongoing government suspicions of ethnic separatism and foreign interference. Far from being isolated or peripheral, Hmong millenarian rumours and movements interact with overseas diasporas, human rights agencies and international religious networks to influence state responses, sometimes in unexpected ways
Duffy blood group gene polymorphisms among malaria vivax patients in four areas of the Brazilian Amazon region
<p>Abstract</p> <p>Background</p> <p>Duffy blood group polymorphisms are important in areas where <it>Plasmodium vivax </it>predominates, because this molecule acts as a receptor for this protozoan. In the present study, Duffy blood group genotyping in <it>P. vivax </it>malaria patients from four different Brazilian endemic areas is reported, exploring significant associations between blood group variants and susceptibility or resistance to malaria.</p> <p>Methods</p> <p>The <it>P. vivax </it>identification was determined by non-genotypic and genotypic screening tests. The Duffy blood group was genotyped by PCR/RFLP in 330 blood donors and 312 malaria patients from four Brazilian Amazon areas. In order to assess the variables significance and to obtain independence among the proportions, the Fisher's exact test was used.</p> <p>Results</p> <p>The data show a high frequency of the <it>FYA/FYB </it>genotype, followed by <it>FYB/FYB, FYA/FYA</it>, <it>FYA/FYB-33 </it>and <it>FYB/FYB-33</it>. Low frequencies were detected for the <it>FYA/FY</it><sup><it>X</it></sup>, <it>FYB/FY</it><sup><it>X</it></sup>, <it>FYX/FY</it><sup><it>X </it></sup>and <it>FYB-33/FYB-33 </it>genotypes. Negative Duffy genotype (<it>FYB-33/FYB-33</it>) was found in both groups: individuals infected and non-infected (blood donors). No individual carried the <it>FY</it><sup><it>X</it></sup><it>/FYB-33 </it>genotype. Some of the Duffy genotypes frequencies showed significant differences between donors and malaria patients.</p> <p>Conclusion</p> <p>The obtained data suggest that individuals with the <it>FYA/FYB </it>genotype have higher susceptibility to malaria. The presence of the <it>FYB-33 </it>allele may be a selective advantage in the population, reducing the rate of infection by <it>P. vivax </it>in this region. Additional efforts may contribute to better elucidate the physiopathologic differences in this parasite/host relationship in regions endemic for <it>P. vivax </it>malaria, in particular the Brazilian Amazon region.</p
Predicting dementia from primary care records: a systematic review and meta-analysis
Introduction
Possible dementia is usually identified in primary care by general practitioners (GPs) who refer to specialists for diagnosis. Only two-thirds of dementia cases are currently recorded in primary care, so increasing the proportion of cases diagnosed is a strategic priority for the UK and internationally. Clinical entities in the primary care record may indicate risk of developing dementia, and could be combined in a predictive model to help find patients who are missing a diagnosis. We conducted a meta-analysis to identify clinical entities with potential for use in such a predictive model for dementia in primary care.
Methods and Findings
We conducted a systematic search in PubMed, Web of Science and primary care database bibliographies. We included cohort or case-control studies which used routinely collected primary care data, to measure the association between any clinical entity and dementia. Meta-analyses were performed to pool odds ratios. A sensitivity analysis assessed the impact of non-independence of cases between studies.
From a sift of 3836 papers, 20 studies, all European, were eligible for inclusion, comprising >1 million patients. 75 clinical entities were assessed as risk factors for all cause dementia, Alzheimer’s (AD) and Vascular dementia (VaD). Data included were unexpectedly heterogeneous, and assumptions were made about definitions of clinical entities and timing as these were not all well described. Meta-analysis showed that neuropsychiatric symptoms including depression, anxiety, and seizures, cognitive symptoms, and history of stroke, were positively associated with dementia. Cardiovascular risk factors such as hypertension, heart disease, dyslipidaemia and diabetes were positively associated with VaD and negatively with AD. Sensitivity analyses showed similar results.
Conclusions
These findings are of potential value in guiding feature selection for a risk prediction tool for dementia in primary care. Limitations include findings being UK-focussed. Further predictive entities ascertainable from primary care data, such as changes in consulting patterns, were absent from the literature and should be explored in future studies
Children reading to dogs: a systematic review of the literature
Background
Despite growing interest in the value of human-animal interactions (HAI) to human mental and physical health the quality of the evidence on which postulated benefits from animals to human psychological health are based is often unclear. To date there exist no systematic reviews on the effects of HAI in educational settings specifically focussing on the perceived benefits to children of reading to dogs. With rising popularity and implementation of these programmes in schools, it is essential that the evidence base exploring the pedagogic value of these initiatives is well documented.
Methods
Using PRISMA guidelines we systematically investigated the literature reporting the pedagogic effects of reading to dogs. Because research in this area is in the early stages of scientific enquiry we adopted broad inclusion criteria, accepting all reports which discussed measurable effects related to the topic that were written in English. Multiple online databases were searched during January-March 2015; grey literature searches were also conducted. The search results which met the inclusion criteria were evaluated, and discussed, in relation to the Oxford Centre for Evidence Based Medicine levels of evidence; 27 papers were classified as Level 5, 13 as Level 4, 7 as Level 2c and 1 as Level 2b.
Conclusion
The evidence suggests that reading to a dog may have a beneficial effect on a number of behavioural processes which contribute to a positive effect on the environment in which reading is practiced, leading to improved reading performance. However, the evidence base on which these inferences are made is of low quality. There is a clear need for the use of higher quality research methodologies and the inclusion of appropriate controls in order to draw causal inferences on whether or how reading to dogs may benefit children’s reading practices. Doubly Uniparental Inheritance of Mitochondria As a Model System for Studying Germ Line Formation
BACKGROUND: Doubly Uniparental Inheritance (DUI) of mitochondria occurs when both mothers and fathers are capable of transmitting mitochondria to their offspring, in contrast to the typical Strictly Maternal Inheritance (SMI). DUI was found in some bivalve molluscs, in which two mitochondrial genomes are inherited, one through eggs, the other through sperm. During male embryo development, spermatozoon mitochondria aggregate in proximity of the first cleavage furrow and end up in the primordial germ cells, while they are dispersed in female embryos. METHODOLOGY/PRINCIPAL FINDINGS: We used MitoTracker, microtubule staining and transmission electron microscopy to examine the mechanisms of this unusual distribution of sperm mitochondria in the DUI species Ruditapes philippinarum. Our results suggest that in male embryos the midbody deriving from the mitotic spindle of the first division concurs in positioning the aggregate of sperm mitochondria. Furthermore, an immunocytochemical analysis showed that the germ line determinant Vasa segregates close to the first cleavage furrow. CONCLUSIONS/SIGNIFICANCE: In DUI male embryos, spermatozoon mitochondria aggregate in a stable area on the animal-vegetal axis: in organisms with spiral segmentation this zone is not involved in cleavage, so the aggregation is maintained. Moreover, sperm mitochondria reach the same embryonic area in which also germ plasm is transferred. In 2-blastomere embryos, the segregation of sperm mitochondria in the same region with Vasa suggests their contribution in male germ line formation. In DUI male embryos, M-type mitochondria must be recognized by egg factors to be actively transferred in the germ line, where they become dominant replacing the Balbiani body mitochondria. The typical features of germ line assembly point to a common biological mechanism shared by DUI and SMI organisms. Although the molecular dynamics of the segregation of sperm mitochondria in DUI species are unknown, they could be a variation of the mechanism regulating the mitochondrial bottleneck in all metazoans
Discovery of Very High Energy Gamma-Ray Emission from the BL Lac Object H2356-309 with the H.E.S.S. Cherenkov Telescopes
The extreme synchrotron BL Lac object H2356-309, located at a redshift of z =
0.165, was observed from June to December 2004 with a total exposure of approx.
40 h live-time with the H.E.S.S. (High Energy Stereoscopic System) array of
atmospheric-Cherenkov telescopes (ACTs). Analysis of this data set yields, for
the first time, a strong excess of 453 gamma-rays (10 standard deviations above
background) from H2356-309, corresponding to an observed integral flux above
200 GeV of I(>200GeV) = (4.1+-0.5) 10^12 cm^-2.s^-1 (statistical error only).
The differential energy spectrum of the source between 200 GeV and 1.3 TeV is
well-described by a power law with a normalisation (at 1 TeV) of N_0 = (3.00 +-
0.80_stat +- 0.31_sys) 10^-13 cm^-2.s^-1.TeV^-1 and a photon index of Gamma =
3.09 +- 0.24_stat +- 0.10_sys. H2356-309 is one of the most distant BL Lac
objects detected at very-high-energy gamma-rays so far. Results from
simultaneous observations from ROTSE-III (optical), RXTE (X-rays) and NRT
(radio) are also included and used together with the H.E.S.S. data to constrain
a single-zone homogeneous synchrotron self-Compton (SSC) model. This model
provides an adequate fit to the H.E.S.S. data when using a reasonable set of
