    Resultados del tratamiento del absceso dentoalveolar y la celulitis facial odontógena

    Los procesos sépticos odontogénicos son diagnosticados y tratados con frecuencia en los servicios estomatológicos, ocasionando complicaciones por el manejo inadecuado. Se realizó un estudio observacional transversal y descriptivo para evaluar los resultados en el tratamiento del Absceso Dentoalveolar y la Celulitis Facial Odontógena en un período de noviembre del 2005 a mayo del 2006 en pacientes que acudieron a la Atención Primaria. La muestra estuvo constituida por 55 pacientes que acudieron a la Clínica Estomatológica “Enríque Núñez”, del municipio de Madruga, provincia La Habana, la información se obtuvo de las historias clínicas individuales; se confeccionó un modelo de recogida de datos, el procesamiento se realizó en un tabulador electrónico Micosoft Excel 2003. El Absceso Dentoalveolar fue diagnosticado con mayor frecuencia, las regiones más afectadas fueron la geniana, maseterina y nasal, la etiología más frecuente, la caries dental, en ambas patologías la extracción dentaria y el tratamiento canalicular en las primeras 24 horas son de elección, los pacientes tratados con este método evolucionaron satisfactoriamente y sin complicaciones, la penicilina fue el antimicrobiano más usado. Los ingresados en su hogar manifestaron conformidad con el tratamiento aplicado, la estadía diminuye considerablemente cuando se elimina el agente causa

    Introduction: Severe Acute Respiratory Syndrome (SARS-CoV-2) is the pathological entity responsible for the current pandemic that not only generates a distinctive respiratory pattern but has also been associated with multiple mechanisms of invasion of the central nervous system (CNS). Objective: To determine the affections, manifestations and neurological repercussions that SARS-CoV-2 infection can generate. Methods:  Review of the scientific literature of patients with SARS-CoV-2 infection, in whom the development of conditions, manifestations and neurological repercussions, selected outcome in the studies, presence of micro and macroscopic conditions of the CNS and peripheral (PNS) were evaluated. Results: 40 articles were included where the family and structure of SARS-CoV-2, pathophysiological mechanisms, neurological clinical manifestations, and possible repercussions at the central nervous system level were analyzed. Conclusions: SARS-CoV-2 is a pathological entity that is associated with different mechanisms of neurological intervention, through direct infection to the CNS, secondary to a parainfectious and postinfectious process, related to cytokine storm syndrome, endothelial damage, thrombotic disorders, in addition to secondary to hypoxia, hypoxemia and multiple organ failure.Introducción: El Síndrome Respiratorio Agudo Severo (SARS-CoV-2) es la entidad patológica responsable de la actual pandemia que no solo genera un cuadro respiratorio distintivo sino que también se ha asociado con múltiples mecanismos de invasión al sistema nervioso central (SNC). Objetivo: Determinar las afecciones, manifestaciones y repercusiones neurológicas que puede generar la infección por SARS-CoV-2. Métodos: Revisión de la literatura científica de pacientes con infección por SARS-CoV-2, en quienes se evaluó desarrollo de afecciones, manifestaciones y repercusiones neurológicas, desenlace seleccionado en los estudios, presencia de afecciones micro y macroscópicas del SNC y periférico (SNP). Resultados: Se estudiaron 40 artículos que analizaban la familia y estructura del SARS-CoV-2, mecanismos fisiopatológicos, manifestaciones clínicas neurológicas y las posibles repercusiones a nivel sistema nervioso central.     Socioeconomic Inequalities and Toothbrushing Frequency among Schoolchildren Aged 6 to 12 Years in a Multi-Site Study of Mexican Cities: A Cross-Sectional Study

    Periodic toothbrushing is the most common, effective, and reliable way to mechanically remove biofilm from oral tissues. The objective of the present study was to determine the association between toothbrushing frequency and socioeconomic position for schoolchildren between 6 and 12 years of age in four cities in Mexico. A cross-sectional study was conducted on 500 Mexican schoolchildren between 6 and 12 years of age from public schools in four Mexican cities. Questionnaires were administered to the parents/guardians of the schoolchildren to obtain the variables included in the study. The dependent variable was toothbrushing frequency, dichotomized as: 0 = less than twice a day and 1 = at least twice a day. The analysis was performed in Stata. The average age of the schoolchildren was 8.9 1.9 years; 50.4% were female. The prevalence of toothbrushing was 52.8% (at least twice a day) (95% CI = 48.457.1). In the multivariate model, the variables associated (p < 0.05) with toothbrushing frequency were older age of the schoolchild (OR = 1.14); younger age of the mother (OR = 0.93); being a girl (OR = 1.70); being enrolled in Seguro Popular (OR = 0.69); being in a household that was owned (OR = 2.43); and being a schoolchild who lived in a home that owned a car (OR = 1.31). The prevalence of toothbrushing at least twice a day was just over 50% in these Mexican children. We found demographic and socioeconomic variables to be associated with toothbrushing. Based on socioeconomic variables that were associated with toothbrushing frequency—such as health insurance, home ownership and the household owning a car—the results of the present study confirm the existence of health inequalities in toothbrushing frequency

    Vagal afferents contribute to sympathoexcitation-driven metabolic dysfunctions

    Multiple crosstalk between peripheral organs and the nervous system are required to maintain physiological and metabolic homeostasis. Using Vav3-deficient mice as a model for chronic sympathoexcitation-associated disorders, we report here that afferent fibers of the hepatic branch of the vagus nerve are needed for the development of the peripheral sympathoexcitation, tachycardia, tachypnea, insulin resistance, liver steatosis and adipose tissue thermogenesis present in those mice. This neuronal pathway contributes to proper activity of the rostral ventrolateral medulla, a sympathoregulatory brainstem center hyperactive in Vav3−/− mice. Vagal afferent inputs are also required for the development of additional pathophysiological conditions associated with deregulated rostral ventrolateral medulla activity.     Tooth brushing frequency in Mexican schoolchildren and associated sociodemographic, socioeconomic, and dental variables

    Background: Tooth brushing (with fluoridated toothpaste) is the most cost-effective intervention in dentistry and it is widely recommended to preserve good oral health. We aimed to determine the frequency of tooth brushing and the variables associated with this practice in schoolchildren living in southeast Mexico. Material and methods: A cross-sectional study was carried out in 1644 schoolchildren, 6 to 13 years old. Questionnaires with socio-demographic, socioeconomic, and dental variables were administered to mothers/guardians of children. The dependent variable was the frequency of tooth brushing, which was categorized as 0 = tooth brushing less than once a day and 1 = tooth brushing at least once a day. A logistic regression model was used to evaluate the final results. Results: Mean age was 9.06±2.02 years and 49.1% were girls. The prevalence of tooth brushing at least once a day was 49.8%. In the multivariate model, characteristics related to tooth brushing frequency (p<0.05) were: older age (OR=1.11), being female (OR=1.64), having a larger family (OR=0.87), having had a visit to a dentist during the year preceding the study (OR=1.37), and having had fluoride applications by a professional (OR=1.39). Conclusions: The results suggested that different variables (demographic, socioeconomic and dental) are associated with the frequency of tooth brushing. Family size (proxy variable for socioeconomic status) may indicate certain oral health inequalities in this population

    The spread of KPC-3 Klebsiella pneumoniae in hospitals in Bogotá over a three-year period (2008-2010)

    Introducción. Uno de los principales problemas de salud pública a nivel mundial son las infecciones producidas por enterobacterias resistentes a los carbapenémicos, entre las cuales Klebsiella pneumoniae es uno de los patógenos que con mayor frecuencia causa infecciones en el ámbito hospitalario. Objetivo. El objetivo del estudio fue describir la diseminación de aislamientos clínicos de K. pneumoniae productores de la enzima KPC-3 recuperados en hospitales de Bogotá. Materiales y métodos. Se analizaron 82 aislamientos de K. pneumoniae resistentes a antibióticos carbapenémicos recuperados entre el 2008 y el 2010 en 10 hospitales, a los cuales se les realizaron pruebas de detección fenotípica de enzimas por difusión de disco y microdilución, y de detección genotípica por PCR. La determinación de perfiles de sensibilidad frente a 13 antimicrobianos se realizó por métodos automatizados y manuales. La relación genética de los aislamientos se obtuvo por la técnica de PFGE. Resultados. Este estudio presenta el panorama del comportamiento de las enterobacterias resistentes a los carbapenémicos diseminadas en el curso de tres años en 10 hospitales de la ciudad, con características de resistencia a múltiples familias de antibióticos y pertenecientes a varios grupos de clones, cada uno con diferentes subtipos. Conclusiones. La diseminación de aislamientos clínicos de K. pneumoniae productores de enzima KPC-3 en Bogotá plantea la necesidad de fortalecer las acciones de vigilancia epidemiológica frente a este tipo de microorganismos y el desarrollo prioritario de actividades específicas de prevención y control de infecciones.224-231Introduction: One of the major worldwide public health problems today are the infections caused by carbapenem-resistant Enterobacteriaceae (CRE), among which carbapenem-resistant Klebsiella pneumoniae (CRKP), constitutes one of the most common pathogens causing nosocomial infection. Objective: This study was aimed at describing the dissemination of KPC-3 enzyme-producing Klebsiella pneumoniae in clinical isolates from hospitals in Bogotá. Materials and methods: Eighty-two CRKP isolates collected from 10 hospitals in Bogotá from 2008-2010 were analysed; disk diffusion and microdilution were used for phenotypic detection of enzymes and PCR for genotyping. Automated and manual methods were used for determining profiles for antimicrobial susceptibility testing (AST) with 13 agents. PFGE was used for obtaining the isolates’ genetic relationship. Results: This study gives an overview of CRKP patterns in 10 hospitals in Bogota which were found to present resistance to multiple antibiotic families. The CRKPs were grouped in different clones, each having different subtypes, and were spread in the 10 hospitals over the three-year period (2008-2010). Conclusions: The dissemination of KPC-3-producing Klebsiella pneumoniae nosocomial isolates in Bogota highlights the need for strengthening epidemiological surveillance against this type of microorganism and the development of specific priority activities for preventing and controlling such infection

    VAV2 signaling promotes regenerative proliferation in both cutaneous and head and neck squamous cell carcinoma

    Regenerative proliferation capacity and poor differentiation are histological features usually linked to poor prognosis in head and neck squamous cell carcinoma (hnSCC). However, the pathways that regulate them remain ill-characterized. Here, we show that those traits can be triggered by the RHO GTPase activator VAV2 in keratinocytes present in the skin and oral mucosa. VAV2 is also required to maintain those traits in hnSCC patient-derived cells. This function, which is both catalysis- and RHO GTPase-dependent, is mediated by c-Myc- and YAP/TAZ-dependent transcriptomal programs associated with regenerative proliferation and cell undifferentiation, respectively. High levels of VAV2 transcripts and VAV2-regulated gene signatures are both associated with poor hnSCC patient prognosis. These results unveil a druggable pathway linked to the malignancy of specific SCC subtypes. The Rho signalling pathway is frequently activated in squamous carcinomas. Here, the authors find that the Rho GEF VAV2 is over expressed in both cutaneous and head and neck squamous cell carcinomas and that at the molecular level VAV2 promotes a pro-tumorigenic stem cell-like signalling programme
