    It is paradoxically reductive to summarize the bachelardian imaginary of the four fundamental elements. Not only the elements themselves are declined in a variety of materials, but also the bachelardian dynamic is open to the size of a cosmos that can not be reduced to the logic of a quadrangulation. In this context, for the philosopher, the vegetal world also seems to be an inexhaustible source of references and rêveries, which constitute a main pole of meditation. Naturally, references to the plant world appear dispersed in the work of the philosopher: the “aerial tree”, the root, the vine, the water lilies. They are not less multiplied and subtly recurring. Thus Bachelard talks about “notre végétalisme profond” (our deep vegetality). It is necessary to consider the meaning of this enigmatic formula. And this is even more important since the philosopher has always kept himself to the edge of the animal world, to which, after all, he rarely refers to, and in front of which he repeatedly expresses a kind of surprising reluctance. It may be possible that it is not adapted to what, for Bachelard, is more essential while he proceed with is work to a “philosophie du repos” (philosophy of rest). We can also make a disturbing hypothesis: is animality, full of the “Will to Live” that Schopenhauer placed at the heart of his system? Is it ontologically close to that troubled humanity the philosopher feels a very ambiguous sympathy for? Bachelardian vegetality would it be the opposite of what we might call problematic humanism

    El bajo peso al nacer, es un importante problema de salud pública que afecta la salud del recién nacido e impacta la morbimortalidad, la educación, productividad, la familia, la transmisión intergeneracional de la pobreza y la desigualdad. Esta investigación, establece la relación entre la geohelmintiasis materna y el bajo peso al nacer en los recién nacidos de una Clínica de Gineco-obstétrica de la ciudad de Cartagena. Se realizó un estudio transversal con análisis de casos y controles. Se estudiaron 836 recién nacidos seleccionados por muestreo aleatorio sistemático, evaluando la presencia de geohelmintiasis y factores sociodemográficos, antecedentes personales, maternos y factores relacionados con el recién nacido, encontrando una prevalencia de BPN de 7,2% y geohelmintiasis materna de 2,4%. La geohelmintiasis materna mostro asociación con el bajo peso al nacer en esta población. Los factores de riesgo que estadísticamente se asociaron a bajo peso en este grupo de pacientes, fueron el control prenatal, periodo intergenésico menor a 2 años y parto a término como un factor protector

    Puesta en valor de la gastronomía tradicional de Valle de Bravo

    Tesis de MaestríaEl presente documento muestra los elementos que constituyen la puesta en valor de una gastronomía y describe los aportes y saberes culinarios como parte de la identidad y recurso patrimonial de la cocina tradicional de Valle de Bravo. Se aborda desde la perspectiva de la puesta en valor, es decir, se muestra de forma especificada las fases que constituyen el proceso de la puesta en valor. La fase uno del proceso de la puesta en valor denominada el fomento gastronómico, abarca desde el modelo teórico de intervención, en el que se muestra la conceptualización de la gastronomía tradicional y la puesta en valor, la metodología utilizada para la investigación, así como los instrumentos utilizados para la investigación de campo. Y finalmente, las fases del proceso de la puesta en valor. Posteriormente se da paso a la segunda fase, el manejo gastronómico. Se inicia con el apartado de los resultados, en el cual se recopila una conceptualización de Valle de Bravo describiendo su localización, características, historia y antecedentes prehispánicos y coloniales, y una breve diacrónica de los sucesos más relevantes a lo largo de la historia del municipio, hasta permitirnos visibilizar un primer acercamiento al turismo en Valle de Bravo. Una vez comprendida la concientización y conceptualización de la puesta en valor, en la cual se realiza una clasificación y constitución de los elementos culinarios para su recuperación e interpretación categorizados en entradas y sopas, platos fuertes, salsas, bebidas y postres. Consecutivamente, y como resultado de la fase tres la vinculación con la comunidad, por medio de la cual se realizó una recuperación de la gastronomía para resaltar su originalidad y se establecieron las medidas de difusión para fomentar su perdurabilidad a través de talleres de cocina vallesana con la comunidad estudiantil del Tecnológico de Estudios Superiores de Valle de Bravo. Y en la fase cuatro visualización y construcción de vínculos, se realizó una divulgación y transferencia de la información documentada por medio de la creación de un recetario, así como el desarrollo de un evento temático de “cocina Vallesana” Y finalmente, se realiza una discusión y conclusión de los alcances obtenidos, respecto a las metas y objetivos planteados

    Varespladib and cardiovascular events in patients with an acute coronary syndrome: the VISTA-16 randomized clinical trial

    IMPORTANCE: Secretory phospholipase A2(sPLA2) generates bioactive phospholipid products implicated in atherosclerosis. The sPLA2inhibitor varespladib has favorable effects on lipid and inflammatory markers; however, its effect on cardiovascular outcomes is unknown. OBJECTIVE: To determine the effects of sPLA2inhibition with varespladib on cardiovascular outcomes. DESIGN, SETTING, AND PARTICIPANTS: A double-blind, randomized, multicenter trial at 362 academic and community hospitals in Europe, Australia, New Zealand, India, and North America of 5145 patients randomized within 96 hours of presentation of an acute coronary syndrome (ACS) to either varespladib (n = 2572) or placebo (n = 2573) with enrollment between June 1, 2010, and March 7, 2012 (study termination on March 9, 2012). INTERVENTIONS: Participants were randomized to receive varespladib (500 mg) or placebo daily for 16 weeks, in addition to atorvastatin and other established therapies. MAIN OUTCOMES AND MEASURES: The primary efficacy measurewas a composite of cardiovascular mortality, nonfatal myocardial infarction (MI), nonfatal stroke, or unstable angina with evidence of ischemia requiring hospitalization at 16 weeks. Six-month survival status was also evaluated. RESULTS: At a prespecified interim analysis, including 212 primary end point events, the independent data and safety monitoring board recommended termination of the trial for futility and possible harm. The primary end point occurred in 136 patients (6.1%) treated with varespladib compared with 109 patients (5.1%) treated with placebo (hazard ratio [HR], 1.25; 95%CI, 0.97-1.61; log-rank P = .08). Varespladib was associated with a greater risk of MI (78 [3.4%] vs 47 [2.2%]; HR, 1.66; 95%CI, 1.16-2.39; log-rank P = .005). The composite secondary end point of cardiovascular mortality, MI, and stroke was observed in 107 patients (4.6%) in the varespladib group and 79 patients (3.8%) in the placebo group (HR, 1.36; 95% CI, 1.02-1.82; P = .04). CONCLUSIONS AND RELEVANCE: In patients with recent ACS, varespladib did not reduce the risk of recurrent cardiovascular events and significantly increased the risk of MI. The sPLA2inhibition with varespladib may be harmful and is not a useful strategy to reduce adverse cardiovascular outcomes after ACS. TRIAL REGISTRATION: clinicaltrials.gov Identifier: NCT01130246.     La tradición oral del caribe colombiano como estrategia pedagógica para fortalecer la comprensión lectora

    At a global level, the problems of reading comprehension have been constant, since students present problems to reach the inferential and critical level, being easier to cover the literal level. The development and strengthening of reading comprehension in students has become a challenge for current education, since, it is intended that at the end of school education they will obtain the necessary tools to deal with any type of text, however, the little interest and reading habits requires establishing strategies and sources of motivation to awaken the love of reading, therefore, the present research is designed with the objective of developing a pedagogical strategy mediated by the oral tradition of the Colombian Caribbean to strengthen reading comprehension in the 6th students of the Jorge Robledo Ortíz District Educational Institution of Barranquilla. A qualitative research was developed, with a methodological approach Research-Action - Participation, with a sample of 12 students of grade 6. The results allowed to demonstrate significant advances in the strengthening of reading comprehension, pointing to the inferential and critical levels.A nivel mundial los problemas sobre comprensión lectora han sido constantes, ya que, los estudiantes presentan problemas para alcanzar el nivel inferencial y crítico, siendo más fácil abarcar el nivel literal. El desarrollo y fortalecimiento de la comprensión lectora en los estudiantes se ha convertido en un desafío para la educación actual, ya que, se pretende que al finalizar la formación escolar obtengan las herramientas necesarias para enfrentarse a cualquier tipo de texto, no obstante, el poco interés y hábitos de lectura requiere establecer estrategias y fuentes de motivación para despertar el amor por la lectura, por ello, se diseña la presente investigación con el objetivo de desarrollar una estrategia pedagógica mediada por la tradición oral del caribe Colombiano para fortalecer la compresión lectora en los estudiantes de 6° de la Institución Educativa Distrital Jorge Robledo Ortíz de Barranquilla.     Cause of Death and Predictors of All-Cause Mortality in Anticoagulated Patients With Nonvalvular Atrial Fibrillation : Data From ROCKET AF

Methods and Results-In the Rivaroxaban Once Daily Oral Direct Factor Xa Inhibition Compared with Vitamin K Antagonism for Prevention of Stroke and Embolism Trial in Atrial Fibrillation (ROCKET AF) study, patients with nonvalvular atrial fibrillation were randomized to rivaroxaban or dose-adjusted warfarin. Cox proportional hazards regression with backward elimination identified factors at randomization that were independently associated with all-cause mortality in the 14 171 participants in the intention-to-treat population. The median age was 73 years, and the mean CHADS(2) score was 3.5. Over 1.9 years of median follow-up, 1214 (8.6%) patients died. Kaplan-Meier mortality rates were 4.2% at 1 year and 8.9% at 2 years. The majority of classified deaths (1081) were cardiovascular (72%), whereas only 6% were nonhemorrhagic stroke or systemic embolism. No significant difference in all-cause mortality was observed between the rivaroxaban and warfarin arms (P=0.15).     Why Are Outcomes Different for Registry Patients Enrolled Prospectively and Retrospectively? Insights from the Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF).

    Background: Retrospective and prospective observational studies are designed to reflect real-world evidence on clinical practice, but can yield conflicting results. The GARFIELD-AF Registry includes both methods of enrolment and allows analysis of differences in patient characteristics and outcomes that may result. Methods and Results: Patients with atrial fibrillation (AF) and ≥1 risk factor for stroke at diagnosis of AF were recruited either retrospectively (n = 5069) or prospectively (n = 5501) from 19 countries and then followed prospectively. The retrospectively enrolled cohort comprised patients with established AF (for a least 6, and up to 24 months before enrolment), who were identified retrospectively (and baseline and partial follow-up data were collected from the emedical records) and then followed prospectively between 0-18 months (such that the total time of follow-up was 24 months; data collection Dec-2009 and Oct-2010). In the prospectively enrolled cohort, patients with newly diagnosed AF (≤6 weeks after diagnosis) were recruited between Mar-2010 and Oct-2011 and were followed for 24 months after enrolment. Differences between the cohorts were observed in clinical characteristics, including type of AF, stroke prevention strategies, and event rates. More patients in the retrospectively identified cohort received vitamin K antagonists (62.1% vs. 53.2%) and fewer received non-vitamin K oral anticoagulants (1.8% vs . 4.2%). All-cause mortality rates per 100 person-years during the prospective follow-up (starting the first study visit up to 1 year) were significantly lower in the retrospective than prospectively identified cohort (3.04 [95% CI 2.51 to 3.67] vs . 4.05 [95% CI 3.53 to 4.63]; p = 0.016). Conclusions: Interpretations of data from registries that aim to evaluate the characteristics and outcomes of patients with AF must take account of differences in registry design and the impact of recall bias and survivorship bias that is incurred with retrospective enrolment.     Moodle y Jclic como herramientas de apoyo pedagógico para el aprendizaje de números fraccionarios es un estudio orientado al fortalecimiento del aprendizaje de este conjunto numérico con los estudiantes de grado quinto de la I. E Sagrada Corazón de Jesús del Municipio de Buenavista – Córdoba. La problemática identificada subyace en escasa comprensión conceptual y operativa de los estudiantes frente al tema en mención. Hecho que afecta el desempeño académico de los mismo y socava la calidad educativa de la institución. Metodológicamente, esta investigación se enmarca en el enfoque multimodal o mixto con diseño cuasi experimental cuya prueba de hipótesis se sustenta en la correlación del valor promedio de los puntajes obtenidos en la aplicación de cuestionario pretest y otro postest sobre una muestra no probabilística de 12 estudiantes elegidos de manera intencional según sus posibilidades para participar. Por otra parte, la descripción y análisis del comportamiento social de partícipes de ejercicio investigativo están sujeto a la observación y a la reflexión.
