    Chorioamnionitis and Five-Year Neurodevelopmental Outcome in Preterm Infants

    Background: Chorioamnionitis, a risk factor for preterm delivery, has been suggested to be associated with suboptimal neurological development in premature infants. Objective: To evaluate the association between chorioamnionitis and neurodevelopment in preterm infants at 5 years of age. Methods Very low birth weight and very low gestational age infants (n = 197) were recruited. Placental samples (n = 117) were evaluated for histological chorioamnionitis. Fetal histological chorioamnionitis was analyzed as a subgroup. The diagnosis of clinical chorioamnionitis was derived from medical records. Neurodevelopmental impairments were evaluated at 2 years of age, and cognitive development (n = 188) and neuropsychological performance (n = 193) were evaluated at 5 years of age. Results: There were no associations between histological or clinical chorioamnionitis and neurodevelopmental impairments at 2 years of age. Clinical chorioamnionitis and fetal histological chorioamnionitis were not associated with cognitive development or neuropsychological performance, but histological chorioamnionitis was associated with poorer cognitive outcome (regression coefficient = -7.22, 95% CI: -14.31 to -0.13) and weaker memory and learning functions (regression coefficient = -1.29, 95% CI: -2.40 to -0.18) at 5 years of age. Conclusion: Our study findings do not support clinical chorioamnionitis having a major independent role in the pathogenesis of neurodevelopmental problems in very preterm infants. Histological chorioamnionitis was associated with slightly less optimal performance at 5 years of age, but further studies are needed to verify the clinical significance of these findings.     The Network Structure of Childhood Psychopathology in International Adoptees

    International adoptees are at an increased risk of emotional and behavioral problems, especially those who are adopted at an older age. We took a new approach in our study of the network structure and predictability of emotional and behavioral problems in internationally adopted children in Finland. Our sample was from the on-going adoption study and comprised 778 internationally adopted children (387 boys and 391 girls, mean age 10.5 (SD 3.4) years). Networks were estimated using Gaussian graphical models and lasso regularization for all the children, and separately for those who were adopted at different ages. The results showed that anxiety/depressive symptoms, social problems, and aggressiveness were the most central symptom domains. Somatic symptoms were the least central and had the weakest effect on the other domains. Similarly, aggressiveness, social problems, and attention problems were high in terms of predictability (73-65%), whereas internalizing problems were relatively low (28-56%). There were clear but local age-group differences in network structure, symptom centrality, and predictability. According to our findings, network models provide important additional information about the centrality and predictability of specific symptom domains, and thus may facilitate targeted interventions among international adoptees.Peer reviewe

    Anticorpos neutralizantes contra poliovírus em soros de recém-nascidos antes e após imunização em massa da população brasileira de zero a cinco anos de idade. São Paulo, Brasil (1980)

    Foram colhidas amostras de sangue de 178 recém-nascidos (RN) em berçårios de hospital localizado no Município de São Paulo. Noventa crianças foram puncionadas antes do primeiro "Dia Nacional de Vacinação Contra a Poliomielite" e as outras 88, após o segundo "Dia Nacional de Vacinação Contra a Poliomielite", realizados em 1980. Nessas campanhas foram imunizadas as crianças com idade de zero a cinco anos, em todo o Brasil. No presente trabalho pesquisou-se os títulos de anticorpos neutralizantes contra poliovírus nos dois grupos de recém-nascidos. Após a imunização em massa verificou-se que a taxa de recém-nascidos triplo suscetíveis decresceu de 8,9% para 4,5%, enquanto que o aumento observado do triplo imunes foi de 38,9% para 52,3%; essas diferenças mostraram-se estatisticamente significantes ao nível de 5,0%. A proporção de recém-nascidos, com títulos de anticorpos neutralizantes contra poliovírus iguais ou maiores do que 8, aumentou após as campanhas de imunização, quando passaram de 68,9% para 81,8%, de 73,3% para 83,0% e de 57,8% para 70,5%, respectivamente, para os sorotipos 1, 2 e 3. Essas diferenças mostraram se estatisticamente significantes, ao nível de 5,0%, em relação aos poliovírus 1 e 3

    This Opinion of the EFSA Panel on Dietetic Products, Nutrition, and Allergies (NDA) deals with the setting of Dietary Reference Values (DRVs) for fats. A lower bound of the reference intake range for total fat of 20 energy % (E%) and an upper bound of 35 E% are proposed. Fat intake in infants can gradually be reduced from 40 E% in the 6-12 month period to 35-40 E% in the 2nd and 3rd year of life. For specific fatty acids the following is proposed: saturated fatty acid (SFA) and trans fatty acid intake should be as low as possible; not to set any DRV for cis-monounsaturated fatty acids; not to formulate a DRV for the intake of total cis-polyunsaturated fatty acids (PUFA); not to set specific values for the n-3/n-6 ratio; to set an Adequate Intake (AI) of 4 E% for linolenic acid; not to set any DRV for arachidonic acid; not to set an UL for total or any of the n-6 PUFA; to set an AI for alpha-linilenic acid (ALA) of 0.5 E%; not to set an UL for ALA; to set an AI of 250 mg for eicosapentaenoic acid (EPA) plus docosahexaenoic acid (DHA) for adults; to set an AI of 100 mg DHA for infants (>6 months) and young children <24 months; to increase by 100-200 mg preformed DHA in addition to the AI for adults as an adequate supply of n-3 long chain PUFA during pregnancy and lactation; not to set any DRV for conjugated linoleic acid. For cholesterol it was decided not to propose a reference value beside the limitation on the intake of SF
