    Treatment of insomnia in myasthenia gravis-A prospective study on non-benzodiazepine hypnotics in the treatment of myasthenia gravis patients with insomnia

    Objectives: This study aimed to evaluate the efficacy and safety of non-benzodiazepine hypnotics in the treatment of myasthenia gravis (MG) patients with insomnia. Methods: This is a prospective longitudinal study. Outpatients who met the criteria for stable MG and insomnia diagnosis according to the International Classification of Sleep Disorders (third edition) were included in the study. They took a regular dose of non-benzodiazepine hypnotics (zolpidem 10 mg per night or zopiclone 7.5 mg per night) based on their own preferences. Patients received psychotherapy (including sleep health education) and were followed up for 4–5 weeks. Cases with lung diseases, respiratory disorders, or inappropriate use of hypnotic medications were excluded. The primary outcome is the difference in total Pittsburgh Sleep Quality Index (PSQI) score between baseline and the end of follow-up period. Secondary outcomes include the difference in Myasthenia Gravis Activities of Daily Living (MG-ADL) score, 7-item Generalized Anxiety Disorder Questionnaire (GAD-7), and the Patient Health Questionnaire-9 (PHQ-9) between baseline and the end of follow-up period and the safety of medication. Results: A total of 75 MG patients with insomnia were included in this study. After 4–5 weeks of treatment, the total PSQI score and MG-ADL score were lower than baseline (p < 0.01). No patients had an increased MG-ADL score. The incidence rate of adverse events was 16.0% (12 cases), including dizziness (6 cases, 8.0%), drowsiness (3 cases, 4.0%), fatigue (2 cases, 2.7%), and nausea (1 case, 1.3%), all of which were mild. No patients had new onset breathing disorders. Conclusion: Non-benzodiazepine hypnotics are safe and effective for stable MG patients who need insomnia treatment

    Validation of handheld fundus camera with mydriasis for retinal imaging of diabetic retinopathy screening in China: a prospective comparison study.

    OBJECTIVES: To investigate the clinical validity of using a handheld fundus camera to detect diabetic retinopathy (DR) in China. DESIGN AND SETTINGS: Prospective comparison study of the handheld fundus camera with a standard validated instrument in detection of DR in hospital and a community screening clinic in Guangdong Province, China. PARTICIPANTS: Participants aged 18 years and over with diabetes who were able to provide informed consent and agreed to attend the dilated eye examination with handheld tests and a standard desktop camera. PRIMARY AND SECONDARY OUTCOME MEASURES: Primary outcome was the proportion of those with referable DR (R2 and above) identified by the handheld fundus camera (the index test) compared with the standard camera. Secondary outcome was the comparison of proportion of gradable images obtained from each test. RESULTS: In this study, we examined 304 people (608 eyes) with each of the two cameras under mydriasis. The handheld camera detected 119 eyes (19.5%) with some level of DR, 81 (13.3%) of them were referable, while the standard camera detected 132 eyes (21.7%) with some level of DR and 83 (13.7%) were referable. It seems that the standard camera found more eyes with referable DR, although McNemar's test detected no significant difference between the two cameras.Of the 608 eyes with images obtained by desktop camera, 598 (98.4%) images were of sufficient quality for grading, 12 (1.9%) images were not gradable. By the handheld camera, 590 (97.0%) were gradable and 20 (3.2%) images were not gradable.The two cameras reached high agreement on diagnosis of retinopathy and maculopathy at all the levels of retinopathy. CONCLUSION: Although it could not take the place of standard desktop camera on clinic fundus examination, the handheld fundus camera showed promising role on preliminary DR screening at primary level in China.     Thiophene-Based Conjugated Acetylenic Polymers with Dual Active Sites for Efficient Co-Catalyst-Free Photoelectrochemical Water Reduction in Alkaline Medium

    Although being attractive materials for photoelectrochemical hydrogen evolution reaction (PEC HER) under neutral or acidic conditions, conjugated polymers still show poor PEC HER performance in alkaline medium due to the lack of water dissociation sites. Herein, we demonstrate that tailoring the polymer skeleton from poly(diethynylthieno[3,2-b]thiophene) (pDET) to poly(2,6-diethynylbenzo[1,2-b:4,5-b′]dithiophene (pBDT) and poly(diethynyldithieno[3,2-b:2′,3′-d]thiophene) (pDTT) in conjugated acetylenic polymers (CAPs) introduces highly efficient active sites for water dissociation. As a result, pDTT and pBDT, grown on Cu substrate, demonstrate benchmark photocurrent densities of 170 μA cm−2 and 120 μA cm−2 (at 0.3 V vs. RHE; pH 13), which are 4.2 and 3 times higher than that of pDET, respectively. Moreover, by combining DFT calculations and electrochemical operando resonance Raman spectroscopy, we propose that the electron-enriched Cβ of the outer thiophene rings of pDTT are the water dissociation active sites, while the −C≡C− bonds function as the active sites for hydrogen evolution. © 2021 The Authors.     Large Area Survey for z=7 Galaxies in SDF and GOODS-N: Implications for Galaxy Formation and Cosmic Reionization

    We present results of our large-area survey for z'-band dropout galaxies at z=7 in a 1568 arcmin^2 sky area covering the SDF and GOODS-N fields. Combining our ultra-deep Subaru/Suprime-Cam z'- and y-band (lambda_eff=1um) images with legacy data of Subaru and HST, we have identified 22 bright z-dropout galaxies down to y=26, one of which has a spectroscopic redshift of z=6.96 determined from Lya emission. The z=7 luminosity function (LF) yields the best-fit Schechter parameters of phi*=0.69 +2.62/-0.55 x10^(-3) Mpc^(-3), Muv*=-20.10 +/-0.76 mag, and alpha=-1.72 +/-0.65, and indicates a decrease from z=6 at a >95% confidence level. This decrease is beyond the cosmic variance in our two fields, which is estimated to be a factor of <~2. We have found that the cosmic star formation rate density drops from the peak at z=2-3 to z=7 roughly by a factor of ~10 but not larger than ~100. A comparison with the reionization models suggests either that the Universe could not be totally ionized by only galaxies at z=7, or more likely that properties of galaxies at z=7 are different from those at low redshifts having, e.g., a larger escape fraction (>~0.2), and/or a flatter IMF. Our SDF z-dropout galaxies appear to form 60-Mpc long filamentary structures, and the z=6.96 galaxy with Lya emission is located at the center of an overdense region consisting of four UV bright dropout candidates, which might suggest an existence of a well-developed ionized bubble at z=7.Comment: 20 pages; ApJ in press, measurements improved with HST/WFC3 data point

    AbstractKonjugierte Polymere sind attraktive Materialien für die photoelektrochemische Wasserstoffentwicklungsreaktion (PEC‐HER) unter neutralen oder sauren Bedingungen. Jedoch zeigen sie im alkalischen Medium immer noch eine geringe PEC‐HER‐Leistung aufgrund des Fehlens von Wasserdissoziations‐Zentren. Hier zeigen wir, dass durch Anpassung des konjugierten acetylenischen Polymers (CAPs) von Poly(diethinylthieno[3,2‐b]thiophen) (pDET) zu Poly(2,6‐diethinylbenzo[1,2‐b:4,5‐b′]dithiophen (pBDT) und Poly(diethinyldithieno[3,2‐b:2′,3′‐d]thiophen) (pDTT) hocheffiziente aktive Wasserdissoziations‐Zentren in CAPs eingeführt werden können. Infolgedessen zeigen auf Cu‐Substrat gewachsene pDTT und pBDT Benchmark‐Photostromdichten von 170 μA cm−2 und 120 μA cm−2 (bei 0,3 V vs. RHE; pH 13), die 4,2‐ bzw. 3‐mal höher sind als die von pDET. DFT‐Rechnungen und elektrochemische Operando‐Resonanz‐Raman‐Spektroskopie zeigen, dass die elektronenangereicherten Cβ der äußeren Thiophenringe von pDTT die aktiven Zentren für die Wasserdissoziation sind, während die ‐C≡C‐Bindungen als aktive Zentren für die Wasserstoffentwicklung fungieren

    Nearly Periodic Fluctuations in the Long Term X-ray Light Curves of the Blazars AO 0235+164 and 1ES 2321+419

    We have performed a structure function analysis of the Rossi X-ray Timing Explorer All Sky Monitor data to search for variability in 24 blazars using data trains that each exceed 12 years. Although 20 of them show nominal periods though this technique, the great majority of these `periods' are clearly related to yearly variations arising from the instrument.Nonetheless, an apparently real periodic component of about 17 days was detected for the blazar AO 0235+164 and it was confirmed by discrete correlation function and periodogram analyses. For 1ES 2321+419 a component of variability with a near periodicity of about 420 days was detected by all of these methods. We discuss several possible explanations for these nearly periodic components and conclude that they most likely arise from the intersections of a shock propagating down a relativistic jet that possesses a helical structure.Comment: 24 pages, 4 figures, 2 table
