    Caudal cervical vertebral morphological variation is not associated with clinical signs in Warmblood horses

    Background Variation in equine caudal cervical spine morphology at C6 and C7 has high prevalence in Warmblood horses and is suspected to be associated with pain in a large mixed-breed group of horses. At present no data exist on the relationship between radiographic phenotype and clinical presentation in Warmblood horses in a case-control study. Objectives To establish the frequency of radiographically visible morphologic variation in a large group of Warmblood horses with clinical signs and compare this with a group without clinical signs. We hypothesised that occurrence of morphologic variation in the case group would not differ from the control group, indicating there is no association between clinical signs and morphologic variation. Study design Retrospective case-control. Methods Radiographic presence or absence of morphologic variation of cervical vertebrae C6 and C7 was recorded in case (n = 245) and control horses (n = 132). Case and control groups were compared by univariable Pearson's Chi-square and multivariable logistic regression for measurement variables age, sex, breed, degenerative joint disease and morphologic variation at C6 and C7. Odds ratio and confidence intervals were obtained. A P <= 0.05 was considered statistically significant. Results Morphologic variation at C6 and C7 (n = 108/377 = 28.6%; Cases 58/245 = 23.7%; Control 50/132 = 38%) was less frequent in horses with clinical signs in univariable testing (OR 0.48, 95% CI 0.3-0.8, P = 0.001). Age, sex, breed and degenerative joint disease were not retained in the final multivariable logistic regression step whereas morphologic variation remained significantly less present in horses with clinical signs. Main limitations Possible demographic differences between equine clinics. Conclusions Morphologic variation in the caudal cervical spine was detected more frequently in horses without clinical signs. Therefore, radiographic presence of such variation does not necessarily implicate the presence of clinical signs

    Ecosystem Services and U.S. Stormwater Planning: An Approach for Improving Urban Stormwater Decisions

    Green stormwater infrastructure (GI) is gaining traction as a viable complement to traditional “gray” infrastructure in cities across the United States. As cities struggle with decisions to replace deteriorating stormwater infrastructure in the face of looming issues such as population growth and climate change, GI may offer a costeffective, efficient, and sustainable approach. However, decision makers confront challenges when integrating GI within city plans, including uncertainties around GI capacity and maintenance, resistance to collaboration across city governance, increasingly inflexible financing, accounting practices that do not incorporate the multiple values of GI, and difficulties in incorporating ecological infrastructure into stormwater management. This paper presents an ecosystem services framework for assessing the context-specific needs of decision makers, while considering the strengths and limitations of GI use in urban stormwater management. We describe multiple dimensions of the planning system, identify points of intervention, and illustrate two applications of our framework – Durham, North Carolina and Portland, Oregon (USA). In these case studies, we apply our ecosystem services framework to explicitly consider tradeoffs to assist planning professionals who are considering implementation of GI. We conclude by offering a research agenda that explores opportunities for further evaluations of GI design, implementation, and maintenance in cities

    Clinically Actionable Hypercholesterolemia and Hypertriglyceridemia in Children with Nonalcoholic Fatty Liver Disease

    OBJECTIVE: To determine the percentage of children with nonalcoholic fatty liver disease (NAFLD) in whom intervention for low-density lipoprotein cholesterol or triglycerides was indicated based on National Heart, Lung, and Blood Institute guidelines. STUDY DESIGN: This multicenter, longitudinal cohort study included children with NAFLD enrolled in the National Institute of Diabetes and Digestive and Kidney Diseases Nonalcoholic Steatohepatitis Clinical Research Network. Fasting lipid profiles were obtained at diagnosis. Standardized dietary recommendations were provided. After 1 year, lipid profiles were repeated and interpreted according to National Heart, Lung, and Blood Institute Expert Panel on Integrated Guidelines for Cardiovascular Health and Risk Reduction. Main outcomes were meeting criteria for clinically actionable dyslipidemia at baseline, and either achieving lipid goal at follow-up or meeting criteria for ongoing intervention. RESULTS: There were 585 participants, with a mean age of 12.8 years. The prevalence of children warranting intervention for low-density lipoprotein cholesterol at baseline was 14%. After 1 year of recommended dietary changes, 51% achieved goal low-density lipoprotein cholesterol, 27% qualified for enhanced dietary and lifestyle modifications, and 22% met criteria for pharmacologic intervention. Elevated triglycerides were more prevalent, with 51% meeting criteria for intervention. At 1 year, 25% achieved goal triglycerides with diet and lifestyle changes, 38% met criteria for advanced dietary modifications, and 37% qualified for antihyperlipidemic medications. CONCLUSIONS: More than one-half of children with NAFLD met intervention thresholds for dyslipidemia. Based on the burden of clinically relevant dyslipidemia, lipid screening in children with NAFLD is warranted. Clinicians caring for children with NAFLD should be familiar with lipid management

    Prevalence and distribution of (micro)albuminuria in toddlers

    Background. Microalbuminuria is common in the general adult population, with a prevalence of similar to 7%, and is an independent indicator of renal and cardiovascular risks. Whether albuminuria is acquired during life (as a result of hypertension/diabetes) or is congenital and already present at birth is unknown. We studied the prevalence of microalbuminuria in toddlers and compared the distribution of albuminuria with that of the general adult population. In addition, we looked for possible associations between microalbuminuria and antenatal, postnatal and maternal factors.Methods. The urinary albumin concentration (U-AC) was measured in 1352 children and the urinary albumin:creatinine ratio (U-ACR) in 1288 children from the Groningen Expert Center for Kids with Obesity (GECKO) Drenthe cohort (age range 20-40 months). Albuminuria distribution was compared with the albuminuria distribution in 40 854 participants of the general adult cohort of the Prevention of Renal and Vascular End stage Disease (PREVEND) study. Associations between albuminuria (expressed as U-AC and U-ACR) and antenatal, postnatal and maternal factors were tested with linear regression analysis.Results. The median U-AC in the GECKO study was 2.3 mg/L (5th-95th percentiles: 2.1-25.5) and in the PREVEND study it was 6.0 mg/L (2.3-28.6) (P distribution comparison 0.053). The prevalence of U-AC a parts per thousand yen 20 mg/L was 6.9% in the GECKO study and 7.8% in the PREVEND study (P = 0.195). The prevalence of U-ACR a parts per thousand yen 30 mg/g in the GECKO study was 23.4%. U-AC and U-ACR were lower in boys. U-AC was not associated with other determinants, but U-ACR was associated with age and gestational diabetes.Conclusions. The distribution of U-AC and the prevalence of U-AC &gt; 20 mg/L in toddlers and in the young general adult population are comparable. These findings suggest that microalbuminuria is a congenital condition that may predispose to a higher cardiovascular risk later in life.</p

    National implementation of an optimal standardised technique for right-sided colon cancer:protocol of an interventional sequential cohort study (Right study)

    Purpose: Minimally invasive right hemicolectomy (MIRH) is the cornerstone of treatment for patients with right-sided colon cancer. This operation has evolved during recent decades, with many innovations and improvements but this has also resulted in high variability of uptake with subsequent substantial variableness. The aim of this ongoing study is to identify current surgical variations, determine the most optimal and standardised MIRH and nationally train and implement that technique to improve short-term clinical and long-term oncological outcomes. Methods: The Right study is a national multicentre prospective interventional sequential cohort study. Firstly, current local practice was evaluated. Subsequently, a standardised surgical technique for right-sided colon cancer was determined using the Delphi consensus method, and this procedure was trained during hands-on courses. The standardised MIRH will be implemented with proctoring (implementation cohort), after which the performance will be monitored (consolidation cohort). Patients who will receive a minimally invasive (extended) right hemicolectomy for cT1-3N0-2M0 colon cancer will be included. The primary outcome is patient safety reflected in the 90-day overall complication rate according to the Clavien–Dindo classification. Secondary outcomes will include intraoperative complications, 90-day mortality rate, number of resected tumour-positive lymph nodes, completeness of mesocolic excision, surgical quality score, locoregional and distant recurrence and 5-year overall survival. A total number of 1095 patients (365 per cohort) will be included. Discussion: The Right study is designed to safely implement the best surgical practice concerning patients with right-sided colon cancer aiming to standardise and improve the surgical quality of MIRH at a national level.     The JCMT Gould Belt survey: Dense core clusters in Orion B

    The James Clerk Maxwell Telescope Gould Belt Legacy Survey obtained SCUBA-2 observations of dense cores within three sub-regions of OrionB: LDN1622, NGC2023/2024, and NGC2068/2071, all of which contain clusters of cores. We present an analysis of the clustering properties of these cores, including the two-point correlation function and Cartwright’s Q parameter. We identify individual clusters of dense cores across all three regions using a minimal spanning tree technique, and find that in each cluster, the most massive cores tend to be centrally located. We also apply the independent M–Σ technique and find a strong correlation between core mass and the local surface density of cores. These two lines of evidence jointly suggest that some amount of mass segregation in clusters has happened already at the dense core stage

    A novel biomarker-based proxy for the spring phytoplankton bloom in Arctic and sub-arctic settings – HBI T25

The spring phytoplankton bloom is a characteristic feature of mid-high latitudes in modern times, but can be challenging to identify in palaeo records. In the current study, we investigated the absolute and relative distributions of two diatom-derived tri-unsaturated highly branched isoprenoid (HBI) lipids, at least one of which has previously been suggested to be a possible proxy for the productive region of the marginal ice zone (MIZ) in the Polar Regions. Based on a comparison of their distributions in surface sediments from the Barents Sea and neighbouring regions with a range of oceanographic parameters, we identify, via principal component analysis, a strong association between the relative proportion of the two HBIs and satellite-derived spring chlorophyll a (chl a) concentration. Further, based on agglomerative hierarchical clustering, we identify two clusters of HBI biomarker ratios and spring chl a together with a potential threshold biomarker ratio (termed HBI TR25) for the spring phytoplankton bloom. A modified version of HBI TR25 (i.e. HBI T25) provides a potentially more straightforward binary measure of the spring phytoplankton bloom. Analysis of HBI TR25 and HBI T25 values in a series of short (spanning recent centuries) and long (Holocene) sediment cores from the region provides an initial evaluation of the applicability of this novel proxy in the palaeo record. Outcomes are mainly consistent with the findings from the surface sediments and with other proxy-based reconstructions, including estimates of past sea ice cover, which is well-known to influence primary production in the region. Indeed, we suggest that the new HBI T25 phytoplankton bloom proxy may also represent an important new tool for characterising the MIZ in palaeo records, especially when used alongside well-established sea ice proxies, such as IP25 and PIP25. Despite the largely empirical nature of the study, we also provide a possible explanation for the observed biomarker ratio-chl a relationship. Thus, a previous laboratory investigation showed that the distributions of the same two HBIs analysed herein in their likely source (viz. Rhizosolenia setigera) was strongly influenced by culture temperature and growth rate. Confirmation of the generality of our findings and of the causal relationship between HBI T25 and the spring phytoplankton bloom will, however, require further laboratory- and field-based studies in the futur

    The JCMT Gould Belt Survey: a quantitative comparison between SCUBA-2 data reduction methods

    Performing ground-based submillimetre observations is a difficult task as the measurements are subject to absorption and emission from water vapour in the Earth's atmosphere and time variation in weather and instrument stability. Removing these features and other artefacts from the data is a vital process which affects the characteristics of the recovered astronomical structure we seek to study. In this paper, we explore two data reduction methods for data taken with the Submillimetre Common-User Bolometer Array-2 (SCUBA-2) at the James Clerk Maxwell Telescope (JCMT). The JCMT Legacy Reduction 1 (JCMT LR1) and The Gould Belt Legacy Survey Legacy Release 1 (GBS LR1) reduction both use the same software (starlink) but differ in their choice of data reduction parameters. We find that the JCMT LR1 reduction is suitable for determining whether or not compact emission is present in a given region and the GBS LR1 reduction is tuned in a robust way to uncover more extended emission, which better serves more in-depth physical analyses of star-forming regions. Using the GBS LR1 method, we find that compact sources are recovered well, even at a peak brightness of only three times the noise, whereas the reconstruction of larger objects requires much care when drawing boundaries around the expected astronomical signal in the data reduction process. Incorrect boundaries can lead to false structure identification or it can cause structure to be missed. In the JCMT LR1 reduction, the extent of the true structure of objects larger than a point source is never fully recovered
