Rapid diagnostic tests duo as alternative to conventional serological assays for conclusive Chagas disease diagnosis
Chagas disease is caused by the parasite Trypanosoma cruzi. It
affects several million people, mainly in Latin America, and
severe cardiac and/or digestive complications occur in ~30% of
the chronically infected patients. Disease acute stage is mostly
asymptomatic and infection goes undiagnosed. In the chronic
phase direct parasite detection is hampered due to its concealed
presence and diagnosis is achieved by serological methods, like
ELISA or indirect hemagglutination assays. Agreement in at least
two tests must be obtained due to parasite wide antigenic
variability. These techniques require equipped labs and trained
personnel and are not available in distant regions. As a result,
many infected people often remain undiagnosed until it is too
late, as the two available chemotherapies show diminished
efficacy in the advanced chronic stage. Easy-to-use rapid
diagnostic tests have been developed to be implemented in remote
areas as an alternative to conventional tests. They do not need
electricity, nor cold chain, they can return results within an
hour and some even work with whole blood as sample, like Chagas
Stat-Pak (ChemBio Inc.) and Chagas Detect Plus (InBIOS Inc.).
Nonetheless, in order to qualify a rapidly diagnosed positive
patient for treatment, conventional serological confirmation is
obligatory, which might risk its start. In this study two rapid
tests based on distinct antigen sets were used in parallel as a
way to obtain a fast and conclusive Chagas disease diagnosis
using whole blood samples. Chagas Stat-Pak and Chagas Detect
Plus were validated by comparison with three conventional tests
yielding 100% sensitivity and 99.3% specificity over 342
patients seeking Chagas disease diagnosis in a reference centre
in Sucre (Bolivia). Combined used of RDTs in distant regions
could substitute laborious conventional serology, allowing
immediate treatment and favouring better adhesion to it
Implante de cemento óseo como alternativa para la reconstrucción de piso orbitario: reporte de caso
Antecedentes: El manejo de las fracturas orbitarias es uno de los retos del trauma facial. La gama de materiales para su reconstrucción es amplia y se mantiene en crecimiento constante, no así el consenso para su uso ni la literatura que lo sustenta.
Objetivo: Presentar el uso y diseno˜ del implante óseo preformado como alternativa para la reconstrucción de las fracturas de piso de órbita en edad pediátrica.
Caso clínico: Varón de 7 anos, ˜ quien sufre trauma contuso en hemicara derecha. Se integra diagnóstico clínico y tomográfico de fractura de piso de órbita derecha, de tipo estallamiento, pura, con atrapamiento de músculo recto inferior y ptosis palpebral derecha postraumática. Como tratamiento se realiza con éxito reconstrucción quirúrgica a los 7 días, con colocación de implante de cemento óseo preconstruido. A las 8 semanas posquirúrgicas, el paciente presenta ptosis leve residual, sin limitación a los movimientos ni diplopía.
Conclusiones: El uso de cemento óseo se puede considerar adecuado para la reconstrucción de dichas fracturas, como una alternativa más dentro de los biomateriales que pueden ser utilizados por el oftalmólogo. Consideramos que nuestro optimismo por los resultados obtenidos en este reporte de caso nos obliga a aumentar el número de pacientes para recabar mayores evidencias y mayor seguimiento a largo plazo.
ABSTRAC
Background: The management of orbitary fractures is one of the most challenging in facial trauma; the variety of reconstruction materials for its treatment is broad and is constantly improving, but despite this there is no consensus for its use or literature that sustains it.
Objective: To present the use and design of a preformed bone implant as an alternative for the reconstruction of orbital floor fractures in the pediatric age group.
Clinical case: A 7-year old male who suffered a right hemifacial contusion trauma with clinical and tomographic diagnosis of right pure blowout type orbital floor fracture with inferior rectus muscle entrapment and right post-traumatic palpebral ptosis. Successful surgical reconstruction was performed 7 days later with a pre-constructed bone cement implant. Eight weeks after surgery the patient presented with mild residual palpebral ptosis, no ocular movement limitations and no diplopia.
Conclusions: The use of a bone cement implant can be considered appropriate for the reconstruction of these fractures, as another alternative to be used by the ophthalmologist among the variety of all the other materials used for this purpose. We consider that our optimism based on the results obtained in this case obligates us to increase the number of patients treated in order to gather more evidence and do larger follow u
Prevalence of smoking and incidence of initiation in the Latin American adult population: the PLATINO study
Background: the PLATINO project was launched in 2002 in order to study the prevalence of chronic obstructive pulmonary disease (COPD) in Latin America. Because smoking is the main risk factor for COPD, detailed data on it were obtained. the aim of this paper was to evaluate the prevalence of smoking and incidence of initiation among middle-aged and older adults (40 years or older). Special emphasis was given to the association between smoking and schooling.Methods: PLATINO is a multicenter study comprising five cross-sectional population-based surveys of approximately 1,000 individuals per site in São Paulo (Brazil), Santiago (Chile), Mexico City (Mexico), Montevideo (Uruguay) and Caracas (Venezuela). the outcome variable was smoking status (never, former or current). Current smokers were those who reported to smoke within the previous 30 days. Former smokers were those who reported to quit smoking more than 30 days before the survey. Using information on year of birth and age of smoking onset and quitting, a retrospective cohort analysis was carried out. Smoking prevalence at each period was defined as the number of subjects who started to smoke during the period plus those who were already smokers at the beginning of the period, divided by the total number of subjects. Incidence of smoking initiation was calculated as the number of subjects who started to smoke during the period divided by the number of non-smokers at its beginning. the independent variables included were sex, age and schooling.Results: Non-response rates ranged from 11.1% to 26.8%. the prevalence of smoking ranged from 23.9% (95% CI 21.3; 26.6) in São Paulo to 38.5% (95% CI 35.7; 41.2) in Santiago. Males and middle-aged adults were more likely to smoke in all sites. After adjustment for age, schooling was not associated with smoking. Using retrospective cohort analysis, it was possible to detect that the highest prevalence of smoking is found between 20-29 years, while the highest incidence is found between 10-19 years. Persistence of COVID-19 Symptoms after Recovery in Mexican Population
The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is responsible for the
coronavirus disease (COVID-19), a highly contagious infectious disease that has caused many deaths
worldwide. Despite global efforts, it continues to cause great losses, and leaving multiple unknowns
that we must resolve in order to face the pandemic more effectively. One of the questions that has
arisen recently is what happens, after recovering from COVID-19. For this reason, the objective of
this study is to identify the risk of presenting persistent symptoms in recovered from COVID-19.
This case-control study was conducted in one state of Mexico. Initially the data were obtained
from the participants, through a questionnaire about symptoms that they had at the moment of the
interview. Initially were captured the collected data, to make a dataset. After the pre-processed using
the R project tool to eliminate outliers or missing data. Obtained finally a total of 219 participants,
141 recovered and 78 controls. It was used confidence level of 90% and a margin of error of 7%.
From results it was obtained that all symptoms have an associated risk in those recovered. The relative
risk of the selected symptoms in the recovered patients goes from 3 to 22 times, being infinite for the
case of dyspnea, due to the fact that there is no control that presents this symptom at the moment of
the interview, followed by the nausea and the anosmia with a RR of 8.5. Therefore, public health
strategies must be rethought, to treat or rehabilitate, avoiding chronic problems in patients recovered
from COVID-19
Guía clínica de la enfermedad de Pompe de inicio tardío
Hasta 2006, la enfermedad de Pompe o glucogenosis tipo II era una enfermedad incurable y con tratamiento meramente paliativo. El desarrollo de la terapia de sustitución con la enzima α-glucosidasa recombinante humana ha constituido el primer tratamiento específico para esta enfermedad. El objetivo de esta guía es servir de referencia en el manejo de la variedad de inicio tardío de la enfermedad de Pompe, es decir, la que aparece después del primer año de
vida. En la guía, un grupo de expertos españoles hace recomendaciones específicas en cuanto a diagnóstico, seguimiento y tratamiento de esta enfermedad. En cuanto al diagnóstico, el método de la muestra en sangre seca es imprescindible como primer paso para el diagnóstico de la enfermedad de Pompe, y el diagnóstico de confirmación de la enfermedad de Pompe debe realizarse mediante un estudio de la actividad enzimática en muestra líquida en linfocitos aislados o mediante
el análisis mutacional del gen de la alfa-glucosidasa. En cuanto al tratamiento de la enfermedad con terapia de sustitución enzimática, los expertos afirman que es eficaz en la mejoría o estabilización de la función motora y pulmonar, y debe iniciarse cuando aparezcan los síntomas atribuibles a la enfermedad de Pompe.

Controlo químico de infestantes
Uma planta é considerada infestante quando nasce espontaneamente num local e momento indesejados, podendo interferir negativamente com a cultura instalada.
As infestantes competem com as culturas para o espaço, a luz, água e nutrientes, podendo atrasar e prejudicar as operações de colheita, depreciar o produto final e assegurarem a reinfestação nas culturas seguintes.
Dado o modo de propagação diferenciado das diversas espécies de infestantes, com as anuais a propagarem-se por semente e as perenes ou vivazes a assegurarem a sua propagação através de órgãos vegetativos (rizomas, bolbos, tubérculos, etc.), assim, também o seu controlo quer químico, quer mecânico terá que ser diferenciado, ou seja, para controlar infestantes anuais será suficiente destruir a sua parte aérea, enquanto para controlar infestantes perenes teremos que destruir os seus órgãos reprodutivos.
O controlo de infestantes poderá ser químico, através da utilização de herbicidas, ou mecânico pela utilização de alfaias agrícolas, tais como a charrua de aivecas, a charrua de discos, a grade de discos, o escarificador e a fresa. Quando a técnica utilizada na instalação das culturas é a sementeira directa, o controlo das infestantes terá que ser obrigatoriamente químico, enquanto se o recurso à mobilização do solo for a técnica mais utilizada (sistema de mobilização tradicional ou sistema de mobilização reduzida), o controlo das infestantes tanto poderá ser químico como mecânico.

Intestinal parasites and genotyping of Giardia duodenalis in children: first report of genotype B in isolates from human clinical samples in Mexico
First report on dung beetles in intra-Amazonian savannahs in Roraima, Brazil
This is the first study to address the dung beetle (Coleoptera: Scarabaeidae: Scarabaeinae) diversity in intra-Amazonian savannahs in the state of Roraima, Brazil. Our aim was to survey the dung beetle fauna associated with these savannahs (regionally called 'lavrado'), since little is known about the dung beetles from this environment. We conducted three field samples using pitfall traps baited with human dung in savannah areas near the city of Boa Vista during the rainy seasons of 1996, 1997, and 2008. We collected 383 individuals from ten species, wherein six have no previous record in intra-Amazonian savannahs. The most abundant species were Ontherus appendiculatus (Mannerheim, 1829), Canthidium aff. humerale (Germar, 1813), Dichotomius nisus (Olivier, 1789), and Pseudocanthon aff. xanthurus (Blanchard, 1846). We believe that knowing the dung beetles diversity associated with the intra-Amazonian savannahs is ideal for understanding the occurrence and distribution of these organisms in a highly threatened environment, it thus being the first step towards conservation strategy development
