    Dilute-and-shoot coupled to mixed mode liquid chromatography-tandem mass spectrometry for the analysis of persistent and mobile organic compounds in human urine

    In this work, a comprehensive method for the simultaneous determination of 33 diverse persistent and mobile organic compounds (PMOCs) in human urine was developed by dilute-and-shoot (DS) followed by mixed-mode liquid chromatography coupled with tandem mass spectrometry (MMLC-MS/MS). In the sample preparation step, DS was chosen since it allowed the quantification of all targets in comparison to lyophilization. For the chromatographic separation, Acclaim Trinity P1 and P2 trimodal columns provided greater capacity for retaining PMOCs than reverse phase and hydrophilic interaction liquid chromatography. Therefore, DS was validated at 5 and 50 ng/mL in urine with both mixed mode columns at pH = 3 and 7. Regarding figures of merit, linear calibration curves (r2 > 0.999) built between instrumental quantification limits (mostly below 5 ng/mL) and 500 ng/mL were achieved. Despite only 60% of the targets were recovered at 5 ng/mL because of the dilution, all PMOCs were quantified at 50 ng/mL. Using surrogate correction, apparent recoveries in the 70–130% range were obtained for 91% of the targets. To analyse human urine samples, the Acclaim Trinity P1 column at pH = 3 and 7 was selected as a consensus between analytical coverage (i.e. 94% of the targets) and chromatographic runs. In a pooled urine sample, industrial chemicals (acrylamide and bisphenol S), biocides and their metabolites (2-methyl-4-isothiazolin-3-one, dimethyl phosphate, 6-chloropyridine-3-carboxylic acid, and ammonium glufosinate) and an artificial sweetener (aspartame) were determined at ng/mL levels.     Millennial land use explains modern high‐elevation vegetation in the submediterranean mountains of Southern Europe

    Aim Pinewood decline and scrubland expansion are major features of Late Holocene vegetation history in the Cantabrian Range. However, the drivers of this remarkable vegetation shift remain to be investigated. Here, we aim at disentangling the role of past land use and climate in shaping the high-elevation Cantabrian landscape during the past two millennia. Location Cantabrian Range (northern Iberia). Taxa Pinus sylvestris, Betula, Ericaceae, Juniperus, Poaceae. Methods We conducted high-resolution multiproxy palaeoecological analyses (pollen, plant macrofossils, microscopic charcoal and dung fungi) on lake sediments from Lago del Ausente to reconstruct vegetation, fire occurrence and grazing through time. The chronology is based on 14C (terrestrial plant macrofossils) and 210Pb dating, and Bayesian age-depth modelling (‘rbacon’). We carried out cross-correlation analysis to quantify vegetation responses to fire. Results Between 250 and 900 CE, the vegetation above 1700 m a.s.l. consisted of subalpine scrubland and scattered P. sylvestris trees/stands. Pinewoods with Betula were widespread at slightly lower elevation. This vegetation was resilient to moderate fire disturbance associated with limited pastoral activities. In contrast, enhanced fire occurrence alongside heavier pastoralism led to the demise of pinewoods and their replacement with Betula stands, subalpine scrublands, and meadows between 900 and 1100 CE. Later, the subalpine scrubland-birch tree line did not respond to Little Ice Age cooling. However, further intensification of transhumant herding between 1300 and 1860 CE (‘La Mesta’) triggered birch decline and the establishment of the modern treeless landscape. Main conclusions The extant high-elevation vegetation of the Cantabrian Range is largely the legacy of intensive land use starting more than one millennium ago. Recurrent and severe fires to promote pasturelands led to the regional extirpation of the previously widespread Pinus sylvestris. Future management should aim at preserving the valuable cultural open landscape of mountain scrubland and meadows and also at restoring patches of ancient pine-birch woodlands

    Guideline for dialysate quality of Spanish Society of Nephrology (second edition, 2015)

    A Best Practice Guideline about Dialysis fluid purity was developed under the leadership of the Spanish Society of Nephrology in 2004. The second edition revised Guideline considered new evidences and International Standard. The Guideline has established recommen- dations for standards for preparing dialysate: water, concentrates and hemodialysis proportioning systems. This Guideline is based on the ISO 13959, European Pharmacopoeia, the Real Farmacopea Española, the AAMI Standards and Recommended Practices, Euro- pean Best Practice Guidelines for Haemodialysis, literature reviews, according to their level of evidence, and the opinion of the expert Spanish group. Two levels of quality of water were defined: purified water and high purified water (ultra pure) and for dialysate: ultra pure dialysate. Regular use of ultra pure dialysate is recom- mended for all type of hemodialysis to prevent and delay the occurrence of complications: inflammation, malnutrition, anaemia and amiloidosis. Water, concentrates and dialysate quality requirements are defined as maximum allowa- ble contaminant levels: chemicals (4.1.2), conductivity, microbial and endotoxins (4.1.1): Monitoring frequency, maintenance and corrective actions were specified. Methods of sampling and analysis were described in appendix (anexos). For microbiological monitoring, R2A medium is recommended, incubated during 7-14 days at a temperature of 17-23 °C. The dialysate quality assurance process involves all dialysis staff members and requires strict protocols. The physician in charge of hemodialysis has the ultimate responsibility for dialysate quality

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    En este trabajo queremos hacer unas reflexiones generales sobre la vida religiosa en América Latina, que ayuden a comprender su situación actual, lo legitimo y necesario del cambio que se está operando, y a orientar cristianamente ese cambio. Al hablar de vida religiosa nos referimos preferentemente a aquellas órdenes y congregaciones que no se dedican puramente a la vida contemplativa —las cuales además son una minoría entre nosotros— sino a las que ejercitan un apostolado. Al considerar la vida religiosa nos reducimos a reflexionar sobre lo que son sus características fundamentales, tanto porque provienen de la historia reciente de la vida religiosa, como porque parecen desprenderse de la realidad cristiana de toda vida religiosa. No pretendemos por lo tanto analizar en detalle los innumerables y complejos problemas que presentan cada uno de los puntos que vamos a tratar, sino ofrecer un modelo global de vida religiosa para nosotros
