    An updated overview of clinical guidelines for chronic low back pain management in primary care

    Objectives: In the past decade many countries around the world have produced clinical practice guidelines to assist practitioners in providing a care that is aligned with the best evidence. The aim of this study was to present and compare the most established evidence-based recommendations for the management of chronic nonspecific low back pain in primary care derived from current high-quality international guidelines. Methods: Guidelines published or updated since 2002 were selected by searching PubMed, CINAHL, EMBASE, guidelines databases, and the World Wide Web. The methodological quality of the guidelines was assessed by three authors independently, using the Appraisal of Guidelines for Research and Evaluation (AGREE) Instrument. Guideline recommendations were synthesized into diagnostic and therapeutic approaches that were supported by strong, moderate or weak evidence. Results: Thirteen guidelines were included. In general, the quality was satisfactory. Guidelines had highest scores on clarity and presentation and scope and purpose domains, and lowest scores on applicability. There was a strong consensus among all the guidelines particularly regarding the use of diagnostic triage and the assessment of prognostic factors. Consistent therapeutic recommendations were information, exercise therapy, multidisciplinary treatment, and combined physical and psychological interventions. Conclusion: Compared to previous assessments, the average quality of the guidelines dealing with chronic low back pain has improved. Furthermore, all guidelines are increasingly aligning in providing therapeutic recommendations that are clearly differentiated from those formulated for acute pain. However, there is still a need for improving quality and generating new evidence for this particular condition. © 2011 Société française de rhumatologie

    Objectif. \u2013 Au cours de la derni\ue8re d\ue9cennie, de nombreux pays \ue0 travers le monde ont \ue9mis des recommandations pour aider les cliniciens dans leur pratique clinique concernant une prise en charge qui soit bas\ue9e sur des preuves. Le but de cette \ue9tude a \ue9t\ue9 de pr\ue9senter et de comparer les syst\ue8mes de recommandations internationales de qualit\ue9 et les mieux \ue9tablis sur des preuves concernant la prise en charge des lombalgies chroniques communes dans le cadre de m\ue9decine de premier recours. M\ue9thodes. \u2013 Les syst\ue8mes de recommandations publi\ue9s ou mis \ue0 jour depuis 2002 ont \ue9t\ue9 s\ue9lectionn\ue9s en recherchant sur les bases de donn\ue9es PubMed, CINAHL, EMBASE, sur les bases de donn\ue9es des recommandations et sur le web. La qualit\ue9 de la m\ue9thodologie des syst\ue8mes de recommandations a \ue9t\ue9 \ue9valu\ue9e ind\ue9pendamment par trois auteurs, \ue0 l\u2019aide de l\u2019outil Appraisal of Guidelines for Research and Evaluation (AGREE). Nous avons synth\ue9tis\ue9 les recommandations dans les domaines du diagnostic et du traitement, recommandations s\u2019appuyant sur des preuves fortes, moyennes ou faibles. R\ue9sultats. \u2013 Nous avons inclus 13 syst\ue8mes de recommandations. De mani\ue8re g\ue9n\ue9rale, leur qualit\ue9 \ue9tait satisfaisante. Les scores les plus \ue9lev\ue9s concernaient les domaines de la clart\ue9 et de la pr\ue9sentation, ainsi que dans celui du sujet et de son \ue9tendue ; les plus bas concernaient leur applicabilit\ue9. Il existe un fort consensus entre les recommandations, tout particuli\ue8rement concernant l\u2019arbre diagnostique d\ue9cisionnel et l\u2019\ue9valuation des facteurs pronostiques. Les recommandations th\ue9rapeutiques consensuelles concernaient l\u2019information au malade, les traitements physiques, la prise en charge multidisciplinaire et l\u2019association des traitements \ue0 vis\ue9e physique et psychologique. Conclusion. \u2013 En comparaison aux \ue9valuations pr\ue9c\ue9dentes, la qualit\ue9 moyenne des syst\ue8mes de recommandations ayant pour objet les lombalgies chroniques s\u2019est am\ue9lior\ue9e. De plus, les recommandations sont de plus en plus concordantes concernant le traitement des lombalgies chroniques, recommandations clairement distinctes de celles \ue9mises pour les lombalgies aigu\uebs. N\ue9anmoins, il reste n\ue9cessaire d\u2019am\ue9liorer encore la qualit\ue9 et d\u2019obtenir de nouvelles preuves dans cette pathologie

    Intestinal Hypoxia-inducible Factor-2α (HIF-2α) Is Critical for Efficient Erythropoiesis*

    Erythropoiesis is a coordinated process by which RBCs are produced. Erythropoietin, a kidney-derived hormone, and iron are critical for the production of oxygen-carrying mature RBCs. To meet the high demands of iron during erythropoiesis, small intestinal iron absorption is increased through an undefined mechanism. In this study, erythropoietic induction of iron absorption was further investigated. Hypoxia-inducible factor-2α (HIF-2α) signaling was activated in the small intestine during erythropoiesis. Genetic disruption of HIF-2α in the intestine abolished the increase in iron absorption genes as assessed by quantitative real-time reverse transcription-PCR and Western blot analyses. Moreover, the increase in serum iron following induction of erythropoiesis was entirely dependent on intestinal HIF-2α expression. Complete blood count analysis demonstrated that disruption of intestinal HIF-2α inhibited efficient erythropoiesis; mice disrupted for HIF-2α demonstrated lower hematocrit, RBCs, and Hb compared with wild-type mice. These data further cement the essential role of HIF-2α in regulating iron absorption and also demonstrate that hypoxia sensing in the intestine, as well as in the kidney, is essential for regulation of erythropoiesis by HIF-2α
