Twelve Tips for Just in Time Teaching of Communication Skills for Difficult Conversations in the Clinical Setting
The ability to communicate well with patients and other members of the healthcare team is a vital skill for physicians to have, but one that is often not emphasized in medical education. Learners of all levels can obtain and develop good communication skills regardless of their natural ability in this area, and the clinical setting represents an underutilized resource to accomplish this task. With this in mind, we have reviewed the growing body of literature on the subject and organized our findings into twelve tips to help educators capitalize on these missed opportunities. While our emphasis is helping learners with difficult discussions, these tips can be easily adapted to any other clinical encounter requiring clear communication. Teaching effective communication skills in the clinical setting requires some extra time, but the steps outlined should not take more than a few minutes to complete. Taking the time to develop these skills in our learners will make a significant difference not only their lives but also their patients and their families

How Food & Fitness Community Partnerships Successfully Engaged Youth
Authentic youth engagement was a central component of the Food & Fitness (F&F) Initiative, a 9-year community-based intervention, whose goal was to ensure that all children have equitable access to healthy food and built environments that promote safe physical activity. The youth engagement component focused on strategies and structures that would support a model framework for youth involved in F&F community partnerships. These strategies empowered youth by providing the leadership and technical skills needed in collaborative efforts to sustain change in communities with inequities, where structural racism and inequities result in poor health outcomes for children. This article describes the models that the diverse urban and rural communities across the United States employed to successfully engage youth in the vision and work of F&F and discusses overall lessons learned, challenges, and best practices/recommendations for effectively engaging youth in community-determined change
What Do Program Directors Value in Personal Statements? A Qualitative Analysis
Background: All applicants to accredited training programs must write a personal statement as part of the application process. This may provoke anxiety on the part of the applicant and can result in an impersonal product that does not enhance his or her application. Little has been written about what program directors are seeking in personal statements. Objective: To gain a better understanding of how pulmonary and critical care fellowship program directors view and interpret these essays and to help applicants create more effective personal statements and make the writing process less stressful. Methods: We surveyed the membership of the Association of Pulmonary and Critical Care Medicine Program Directors in 2018. Quantitative data were collected regarding the importance of the personal statement in the candidate selection process. Qualitative data exploring the characteristics of personal statements, what the personal statement reveals about applicants, and advice for writing them were also collected. Comparative analysis was used for coding and analysis of qualitative data. Results: Surveys were completed by 114 out of 344 possible respondents (33%). More than half of the respondents believed that the personal statement is at least moderately important when deciding to offer an interview, and 40% believed it is at least moderately important when deciding rank order. A qualitative analysis revealed consistent themes: communication skills, provision of information not found elsewhere, applicant characteristics, and things to avoid. Conclusion: The respondents view the personal statement as moderately important in the application process. They value succinct, quality writing that reveals personal details not noted elsewhere. The information presented may help reduce anxiety associated with writing the personal statement and result in making the personal statement a more meaningful part of the application
The Far-Ultraviolet "Continuum" in Protoplanetary Disk Systems II: CO Fourth Positive Emission and Absorption
We exploit the high sensitivity and moderate spectral resolution of the
-Cosmic Origins Spectrograph to detect far-ultraviolet spectral features
of carbon monoxide (CO) present in the inner regions of protoplanetary disks
for the first time. We present spectra of the classical T Tauri stars HN Tau,
RECX-11, and V4046 Sgr, representative of a range of CO radiative processes. HN
Tau shows CO bands in absorption against the accretion continuum. We measure a
CO column density and rotational excitation temperature of N(CO) = 2 +/- 1
10 cm and T_rot(CO) 500 +/- 200 K for the absorbing gas.
We also detect CO A-X band emission in RECX-11 and V4046 Sgr, excited by
ultraviolet line photons, predominantly HI LyA. All three objects show emission
from CO bands at 1560 \AA, which may be excited by a combination
of UV photons and collisions with non-thermal electrons. In previous
observations these emission processes were not accounted for due to blending
with emission from the accretion shock, collisionally excited H, and
photo-excited H2; all of which appeared as a "continuum" whose components could
not be separated. The CO emission spectrum is strongly dependent upon the shape
of the incident stellar LyA emission profile. We find CO parameters in the
range: N(CO) 10 cm, T_{rot}(CO) > 300 K for the LyA-pumped
emission. We combine these results with recent work on photo- and
collisionally-excited H emission, concluding that the observations of
ultraviolet-emitting CO and H2 are consistent with a common spatial origin. We
suggest that the CO/H2 ratio in the inner disk is ~1, a transition between the
much lower interstellar value and the higher value observed in solar system
comets today, a result that will require future observational and theoretical

ASASSN-14ko is a Periodic Nuclear Transient in ESO 253-G003
We present the discovery that ASASSN-14ko is a periodically flaring AGN at
the center of the galaxy ESO 253-G003. At the time of its discovery by the
All-Sky Automated Survey for Supernovae (ASAS-SN), it was classified as a
supernova close to the nucleus. The subsequent six years of V- and g-band
ASAS-SN observations reveal that ASASSN-14ko has nuclear flares occurring at
regular intervals. The seventeen observed outbursts show evidence of a
decreasing period over time, with a mean period of days
and a period derivative of . The most recent
outburst in May 2020, which took place as predicted, exhibited spectroscopic
changes during the rise and a had a UV bright, blackbody spectral energy
distribution similar to tidal disruption events (TDEs). The X-ray flux
decreased by a factor of 4 at the beginning of the outburst and then returned
to its quiescent flux after ~8 days. TESS observed an outburst during Sectors
4-6, revealing a rise time of days in the optical and a decline
that is best fit with an exponential model. We discuss several possible
scenarios to explain ASASSN-14ko's periodic outbursts, but currently favor a
repeated partial TDE. The next outbursts should peak in the optical on UT

Strong Carbon Features and a Red Early Color in the Underluminous Type Ia SN 2022xkq
We present optical, infrared, ultraviolet, and radio observations of SN
2022xkq, an underluminous fast-declining type Ia supernova (SN Ia) in NGC 1784
( Mpc), from to 180 days after explosion. The
high-cadence observations of SN 2022xkq, a photometrically transitional and
spectroscopically 91bg-like SN Ia, cover the first days and weeks following
explosion which are critical to distinguishing between explosion scenarios. The
early light curve of SN 2022xkq has a red early color and exhibits a flux
excess which is more prominent in redder bands; this is the first time such a
feature has been seen in a transitional/91bg-like SN Ia. We also present 92
optical and 19 near-infrared (NIR) spectra, beginning 0.4 days after explosion
in the optical and 2.6 days after explosion in the NIR. SN 2022xkq exhibits a
long-lived C I 1.0693 m feature which persists until 5 days post-maximum.
We also detect C II 6580 in the pre-maximum optical spectra. These
lines are evidence for unburnt carbon that is difficult to reconcile with the
double detonation of a sub-Chandrasekhar mass white dwarf. No existing
explosion model can fully explain the photometric and spectroscopic dataset of
SN 2022xkq, but the considerable breadth of the observations is ideal for

Association Between Advanced Maternal Age and Maternal and Neonatal Morbidity: A Cross-Sectional Study on a Spanish Population
Background and objective: Over recent decades, a progressive increase in the maternal age at childbirth has been observed in developed countries, posing a health risk for both women and infants. The aim of this study was to analyze the association between advanced maternal age (AMA) and maternal and neonatal morbidity.
Material and methods: A cross-sectional study of 3,315 births was conducted in the north of Spain in 2014. We compared childbirth between women aged 35 years or older, with a reference group of women aged between 24 and 27 years. AMA was categorized based on ordinal ranking into 35-38 years, 39-42 years, and >42 years to estimate a dose-response pattern (the older the age, the greater the risk). As an association measure, crude and adjusted Odds Ratios (OR) were estimated by non-conditional logistic regression and 95% Confidence Intervals (95%CI) were calculated.
Results: Repeated abortions were more common among women of AMA in comparison to pregnant women aged 24-27 years (reference group): adjusted OR = 2.68; 95%CI (1.52-4.73). A higher prevalence of gestational diabetes was also observed among women of AMA, reaching statistical significance when restricted to first time mothers: adjusted OR = 8.55; 95%CI (1.12-65.43). In addition, the possibility of an instrumental delivery was multiplied by 1.6 and the possibility of a cesarean by 1.5 among women of AMA, with these results reaching statistical significance, and observing a dose-response pattern. Lastly, there were associations between preeclampsia, preterm birth (<37 weeks) and low birthweight, however without reaching statistical significance.
Conclusion: Our results support the association between AMA and suffering repeated abortions. Likewise, being of AMA was associated with a greater risk of suffering from gestational diabetes, especially among primiparous women, as well as being associated with both instrumental deliveries and cesareans among both primiparous and multiparous women
Implementation of the evidence for the improvement of nursing care to the critical patient's family: a participatory action research
Background: There are many descriptive studies regarding the needs of the family, as well as those regarding nursing care aimed directly at family members. However, there is no widespread application of such evidence in clinical practice. There has also been no analysis made of the evolution of patterns of knowing during the act of improving clinical practice. Therefore, the purpose of the study is to understand the change process aimed at improving care to critical patient's families, and to explore the evolution of patterns of knowing that nurses use in this process. Methods: Qualitative study with a Participatory Action Research method, in accordance with the Kemmis and McTaggart model. In this model, nurses can observe their practice, reflect upon it and compare it with scientific evidence, as well as define, deploy and evaluate improvement strategies adapted to the context. Simultaneously, the process of empowerment derived from the Participatory Action Research allows for the identification of patterns of knowing and their development over time. The research will take place in the Intensive Care Units of a tertiary hospital. The participants will be nurses who are part of the regular workforce of these units, with more than five years of experience in critical patients, and who are motivated to consider and critique their practice. Data collection will take place through participant observation, multi-level discussion group meetings and documentary analysis. A content analysis will be carried out, following a process of codification and categorisation, with the help of Nvivo10. The approval date and the beginning of the funding were December 2012 and 2013, respectively. Discussion: The definition, introduction and evaluation of care strategies for family members will allow for their real and immediate implementation in practice. The study of the patterns of knowing in the Participatory Action Research will be part of the theoretical and practical feedback process of a professional discipline. Also, the identification of the construction and evolution of knowledge will provide decision elements to managers and academics when choosing strategies for increased quality
