    Cerebrospinal Fluid Diagnostics of Alzheimer’s Disease in Patients with Idiopathic Normal Pressure Hydrocephalus

    Background: Alzheimer’s disease (AD) is the most common cause of dementia worldwide and a frequent comorbidity in idiopathic normal pressure hydrocephalus (iNPH). The presence of AD pathology is associated with worse outcomes after a shunt procedure in iNPH. Preoperative diagnosis of AD is challenging in patients with iNPH, which involves reduced concentrations of the cerebrospinal fluid (CSF) AD biomarkers.// Objective: Our aim was to estimate the effect size of iNPH as a factor in CSF levels of AD biomarkers and to test if correction could be used to improve diagnostic value.// Methods: Our cohort included 222 iNPH patients with data in the Kuopio NPH registry and brain biopsy and CSF samples available. We divided the patients into groups according to AD pathology per brain biopsy. For control cohorts, we had CSF samples from cognitively healthy individuals (n = 33) and patients with diagnosed AD and no iNPH (n = 39).// *-31pt// Results: Levels of all investigated biomarkers differed significantly between groups, with the exception of t-Tau levels between healthy individuals and iNPH patients with AD pathology. Applying a correction factor for each biomarker (0.842*AÎČ₁ ₋ ₄₂, 0.779*t-Tau, and 0.610*P-Tau₁₈₁) for the effect of iNPH yielded a sensitivity of 2.4% and specificity of 100%. The ratio of P-Tau₁₈₁ to AÎČ₁ ₋ ₄₂ was moderately effective in aiding recognition of AD pathology in iNPH patients (sensitivity 0.79, specificity 0.76, area under the curve 0.824). Conclusion: Correcting for iNPH as a factor failed to improve diagnostic effectiveness, but the P-Tau₁₈₁ /AÎČ₁ ₋ ₄₂ ratio showed some utility in the diagnosis of AD in iNPH patients

    Concordance of Alzheimer's Disease-Related Biomarkers Between Intraventricular and Lumbar Cerebrospinal Fluid in Idiopathic Normal Pressure Hydrocephalus

    BACKGROUND: Alzheimer's disease cerebrospinal fluid (CSF) biomarkers amyloid-ÎČ 1-42 (AÎČ42), total tau (T-tau), and phosphorylated tau 181 (P-tau181) are widely used. However, concentration gradient of these biomarkers between intraventricular (V-CSF) and lumbar CSF (L-CSF) has been demonstrated in idiopathic normal pressure hydrocephalus (iNPH), potentially affecting clinical utility. OBJECTIVE: Here we aim to provide conversion factors for clinical and research use between V-CSF and L-CSF. METHODS: Altogether 138 iNPH patients participated. L-CSF samples were obtained prior to shunt surgery. Intraoperative V-CSF samples were obtained from 97 patients. Post-operative follow-up L- and V-CSF (shunt reservoir) samples of 41 patients were obtained 1-73 months after surgery and then after 3, 6, and 18 months. CSF concentrations of AÎČ42, T-tau, and P-tau181 were analyzed using commercial ELISA assays. RESULTS: Preoperative L-CSF AÎČ42, T-tau, and P-tau181 correlated to intraoperative V-CSF (ρ= 0.34-0.55, p < 0.001). Strong correlations were seen between postoperative L- and V-CSF for all biomarkers in every follow-up sampling point (ρs AÎČ 42: 0.77-0.88, T-tau: 0.91-0.94, P-tau181: 0.94-0.96, p < 0.0001). Regression equations were determined for intraoperative V- and preoperative L-CSF (AÎČ 42: V-CSF = 185+0.34*L-CSF, T-tau: Ln(V-CSF) = 3.11+0.49*Ln(L-CSF), P-tau181: V-CSF = 8.2+0.51*L-CSF), and for postoperative V- and L-CSF (AÎČ 42: V-CSF = 86.7+0.75*L-CSF, T-tau: V-CSF = 86.9+0.62*L-CSF, P-tau181: V-CSF = 2.6+0.74*L-CSF). CONCLUSION: AÎČ 42, T-tau, and P-tau181 correlate linearly in-between V- and L-CSF, even stronger after CSF shunt surgery. Equations presented here, provide a novel tool to use V-CSF for diagnostic and prognostic entities relying on the L-CSF concentrations and can be applicable to clinical use when L-CSF samples are not available or less invasively obtained shunt reservoir samples should be interpreted

    Time Trends of Cerebrospinal Fluid Biomarkers of Neurodegeneration in Idiopathic Normal Pressure Hydrocephalus

    Background: Longitudinal changes in cerebrospinal fluid (CSF) biomarkers are seldom studied. Furthermore, data on biomarker gradient between lumbar (L-) and ventricular (V-) compartments seems to be discordant. Objective: To examine alteration of CSF biomarkers reflecting Alzheimer's disease (AD)-related amyloid-beta (A beta) aggregation, tau pathology, neurodegeneration, and early synaptic degeneration by CSF shunt surgery in idiopathic normal pressure hydrocephalus (iNPH) in relation to AD-related changes in brain biopsy. In addition, biomarker levels in L- and V-CSF were compared. Methods: L-CSF was collected prior to shunt placement and, together with V-CSF, 3-73 months after surgery. Thereafter, additional CSF sampling took place at 3, 6, and 18 months after the baseline sample from 26 iNPH patients with confirmed A beta plaques in frontal cortical brain biopsy and 13 iNPH patients without A beta pathology. CSF Amyloid-beta(42) (A beta(42)), total tau (T-tau), phosphorylated tau (P-tau(181)), neurofilament light (NFL), and neurogranin (NRGN) were analyzed with customized ELISAs. Results: All biomarkers but A beta(42) increased notably by 140-810% in L-CSF after CSF diversion and then stabilized. A beta(42) instead showed divergent longitudinal decrease between A beta-positive and -negative patients in L-CSF, and thereafter increase in A beta-negative iNPH patients in both L- and V-CSF. All five biomarkers correlated highly between V-CSF and L-CSF (A beta(42) R = 0.87, T-tau R = 0.83, P-tau R = 0.92, NFL R = 0.94, NRGN R = 0.9; all p < 0.0001) but were systematically lower in V-CSF (A beta(42) 14 %, T-tau 22%, P-tau 20%, NFL 32%, NRGN 19%). With APOE genotype-grouping, only A beta(42) showed higher concentration in non-carriers of allele epsilon 4. Conclusion: Longitudinal follow up shows that after an initial post-surgery increase, T-tau, P-tau, and NRGN are stable in iNPH patients regardless of brain biopsy A beta pathology, while NFL normalized toward its pre-shunt levels. A beta(42) as biomarker seems to be the least affected by the surgical procedure or shunt and may be the best predictor of AD risk in iNPH patients. All biomarker concentrations were lower in V-than L-CSF yet showing strong correlations.Peer reviewe

    Genome-wide meta-analysis for Alzheimer's disease cerebrospinal fluid biomarkers

    Amyloid-beta 42 (A beta 42) and phosphorylated tau (pTau) levels in cerebrospinal fluid (CSF) reflect core features of the pathogenesis of Alzheimer's disease (AD) more directly than clinical diagnosis. Initiated by the European Alzheimer & Dementia Biobank (EADB), the largest collaborative effort on genetics underlying CSF biomarkers was established, including 31 cohorts with a total of 13,116 individuals (discovery n = 8074; replication n = 5042 individuals). Besides the APOE locus, novel associations with two other well-established AD risk loci were observed; CR1 was shown a locus for A beta 42 and BIN1 for pTau. GMNC and C16orf95 were further identified as loci for pTau, of which the latter is novel. Clustering methods exploring the influence of all known AD risk loci on the CSF protein levels, revealed 4 biological categories suggesting multiple A beta 42 and pTau related biological pathways involved in the etiology of AD. In functional follow-up analyses, GMNC and C16orf95 both associated with lateral ventricular volume, implying an overlap in genetic etiology for tau levels and brain ventricular volume.Peer reviewe

We first perform a large genetic association study by merging all available case-control datasets and by-proxy study results (discovery n = 409,435 and validation size n = 58,190). Here, we add six variants associated with Alzheimer's disease risk (near APP, CHRNE, PRKD3/NDUFAF7, PLCG2 and two exonic variants in the SHARPIN gene). Assessment of the polygenic risk score and stratifying by APOE reveal a 4 to 5.5 years difference in median age at onset of Alzheimer's disease patients in APOE ɛ4 carriers. Because of this study, the underlying mechanisms of APP can be studied to refine the amyloid cascade and the polygenic risk score provides a tool to select individuals at high risk of Alzheimer's disease

We demonstrate that the two diseases share the same protective association at the HLA locus. HLA-specific fine-mapping showed that hierarchical protective effects of HLA-DRB1*04 subtypes best accounted for the association, strongest with HLA-DRB1*04:04 and HLA-DRB1*04:07, and intermediary with HLA-DRB1*04:01 and HLA-DRB1*04:03. The same signal was associated with decreased neurofibrillary tangles in postmortem brains and was associated with reduced tau levels in cerebrospinal fluid and to a lower extent with increased AÎČ42. Protective HLA-DRB1*04 subtypes strongly bound the aggregation-prone tau PHF6 sequence, however only when acetylated at a lysine (K311), a common posttranslational modification central to tau aggregation. An HLA-DRB1*04-mediated adaptive immune response decreases PD and AD risks, potentially by acting against tau, offering the possibility of therapeutic avenues.This work was supported by the Michael J.     I denna undersökning studeras ett reservdelsflöde inom eftermarknad ur ett organisatorisktperspektiv. Reservdelen glidskenan pÄ Atlas Copcos borriggar fungerar som fallstudieobjekt. Glidskenan Àr en tunn stÄlprofil, som mÀter mellan tvÄ och Ätta meter och skyddar matarbalkarna pÄ ovan- och underjordsborriggar mot slitage. VÄr uppdragsgivare Atlas Copcos Shared Distributionscenter (i fortsÀttningen DC) upplever problem i flödet av glidskenor. Glidskenan Àr skrymmande och svÄr att hantera, förpacka och transportera och flödet flyter inte effektivt. De upplevda problemen har varit kÀnda i över 12 Är och förbÀttringsansatser har runnit ut i sanden. Genom kartlÀggning av hur flödet flyter idag och hur organisationen runt flödet agerar har tre teman presenterats och analyserats. Dessa tre teman förklarar de bakomliggande organisatoriska aspekterna som pÄverkar glidskenans flöde och varför de upplevda problemen inte blir ÄtgÀrdade. Studien lyfter fram vikten av samarbete mellan de olika organisatoriska funktionerna och individerna i organisationen. Om samarbetet dem emellan fungerar kan synergieffekter erhÄllas, men ett bristande samarbete kan ocksÄ hÀmma organisationens potential. Det första temat behandlar att kunskap finns inom organisationen, men att den inte sprids mellan medarbetarna. Med utgÄngspunkt i studiens andra tema konstaterar vi att medarbetarnas olika syn pÄ glidskenan utgör en barriÀr för kunskapsspridning. Olika syn hÀmmar Àven medarbetarnas engagemang och intresse för att hitta förbÀttringar inom glidskenans flöde. I studiens tredje tema, avstÄnd, ser vi att organisationens fysiska men framförallt mentala avstÄnd hindrar organisationsmedlemmarna att kÀnna en vi-samhet. Utan denna vi-samhet finns inga incitament till samarbete för att effektivisera glidskenans flöde.In this research a spare part flow within aftermarket has been studied through an organizational perspective. The slide-bar is a spare part in Atlas Copco Rock Drills assortment and forms a case study for our research. The slide bar is a thin steel profile which measures between two and eight centimetres. They are placed on the feeders of surface- and underground drilling rigs as a protection from wear and tear. Atlas Copco Shared Distribution Center (DC) has given us a case as a background for our research. The slide bars are bulky to handle, wrap and transport and for DC the flow appears to be ineffective and expensive. DC are experiencing problems within the material flow of slide bars and is puzzled why no improvement has been done, although the problems has been known to DC for over twelve years. Projects to improve the flow has been initiated but gone down the drains

    The aim of this study was to examine the relationship between gender and self reported symptoms (physiological and psychological) and sense of coherence (SOC) and Job Demand-Control-Social support (JDCS). The sample consisted of 360 respondents (nfemale= 261, nmale= 99), in the ages between 19-53 (M=25,57, SD=5,77), all students from VĂ€xjö University. A quantitative work method was used in the study; data was collected using questionnaires that consisted of self-reported questions, based on three parts: A modified symptom checklist, SOC 13 and a modified JDCS questionnaire. The result of our study showed that both SOC and JDCS had an effect on self-reported symptoms but there were no interaction between SOC and JDCS. No significant gender differences were found regarding the level of SOC. Female students reported symptoms in a significant higher extent than male students. There were no gender differences regarding JDCS. Our results are discussed in relation to previous studies. Our conclusion is that it is important to perform further research on students’ health and work environment
