    Power Training Improves the Sensorimotor Cortical Oscillations in Youth with Cerebral Palsy

    Background: Our magnetoencephalographic (MEG) brain imaging studies have shown that youth with cerebral palsy (CP) demonstrate altered sensorimotor beta (18-24Hz) cortical oscillations when controlling their leg motor actions and these anomalous cortical oscillations are linked with the extent of their mobility impairments. Current therapeutic trends for improving mobility have shifted from strength training to high-velocity power training, which has shown improvements in isokinetic strength, power production and mobility of youth with CP. However, no studies have assessed whether these clinically relevant improvements are linked with changes in the sensorimotor cortical oscillations. The objective of this study was to utilize MEG brain imaging to examine the potential changes in sensorimotor cortical oscillations following power training. Methods: Youth with CP (N=11; Age=15.9 ±1.1yrs; GMFCS I-III) and neurotypical controls (NT) (N=16; Age=14.6 ±0.8yrs) were recruited to participate in this study. The youth with CP underwent 24 high-velocity leg press power training sessions that were performed on a Total Gym® sled. Pre-Post bilateral leg press 1-repetition maximum (1RM) and peak power production were used to assess the muscular performance changes. The 1-minute walk was used to assess mobility changes. During MEG recordings, participants used their right leg to complete a goal-directed isometric target-matching task. Advanced beamforming methods were subsequently used to image the strength of the sensorimotor beta oscillatory power. The NTs only underwent the baseline MEG assessment. Results: Youth with CP increased their 1RM (Pre=158.3 ±24.7kg, Post=247.5 ±41.5kg, p\u3c0.01), and peak power production (Pre=509.9 ±64.7W, Post=677.1 ±113.3W, p=0.04). Participants with CP also improved their 1-minute walk (Pre=77.4 ±9.2m, Post=80.8 ±8.4m, p = 0.02). The beta sensorimotor cortical oscillations in the leg region were stronger in the youth with CP prior to training compared with the NTs (CP=-25.9±1.8%; NT=-17.2±3.6%, p=0.04). However, the youth with CP had a reduction in the strength of the beta oscillations after undergoing the power training (pre=-25.9 ±1.8%, post=-14.8 ±3.6%, p=0.02), and the strength of the oscillations was not significantly different from the NTs after training (p=0.68). Lastly, the peak power production after training was tightly linked with the strength of the post-therapy sensorimotor cortical oscillations (r=0.79, p=0.03). Conclusion: Power training appears to improve the neural generators that control the leg motor actions, and these neuroplastic changes partly contribute to improvements in the peak power production of youth with CP.     The development and pilot testing of the Self-management Programme of Activity, Coping and Education for Chronic Obstructive Pulmonary Disease (SPACE for COPD)

    Purpose: There is no independent standardized self-management approach available for chronic obstructive pulmonary disease (COPD). The aim of this project was to develop and test a novel self-management manual for individuals with COPD. Patients: Participants with a confirmed diagnosis of COPD were recruited from primary care. Methods: A novel self-management manual was developed with health care professionals and patients. Five focus groups were conducted with individuals with COPD (N = 24) during development to confirm and enhance the content of the prototype manual. The Self-management Programme of Activity, Coping and Education for Chronic Obstructive Pulmonary Disease (SPACE for COPD) manual was developed as the focus of a comprehensive self-management approach facilitated by health care professionals. Preference for delivery was initial face-to-face consultation with telephone follow-up. The SPACE for COPD manual was piloted with 37 participants in primary care. Outcome measures included the Self-Report Chronic Respiratory Questionnaire, Incremental Shuttle Walk Test, and Endurance Shuttle Walking Test (ESWT); measurements were taken at baseline and 6 weeks. Results: The pilot study observed statistically significant improvements for the dyspnea domain of the Self-Report Chronic Respiratory Questionnaire and ESWT. Dyspnea showed a mean change of 0.67 (95% confidence interval 0.23–1.11, P = 0.005). ESWT score increased by 302.25 seconds (95% confidence interval 161.47–443.03, P < 0.001). Conclusion: This article describes the development and delivery of a novel self-management approach for COPD. The program, incorporating the SPACE for COPD manual, appears to provoke important changes in exercise capacity and breathlessness for individuals with COPD managed in primary care

    Macroinvertebrate community responses to river impoundment at multiple spatial scales

    River impoundment by the construction of dams potentially modifies a wide range of abiotic and biotic factors in lotic ecosystems and is considered one of the most significant anthropogenic impacts on rivers globally. The past two decades have witnessed a growing body of research centred on quantifying the effects of river impoundment, with a focus on mitigating and managing the effects of individual large dams. This study presents a novel multi-scale comparison of paired downstream and control sites associated with multiple water supply reservoirs (n = 80) using a spatially extensive multi-year dataset. Macroinvertebrate community structure and indices were analysed in direct association with spatial (e.g. region) and temporal variables (e.g. season) to identify consistent patterns in ecological responses to impoundment. Macroinvertebrate communities at monitoring sites downstream of water supply reservoirs differed significantly from those at control sites at larger spatial scales, both in terms of community structure and taxa richness. The effect was most significant at the regional scale, while biogeographical factors appeared to be important drivers of community differences at the national scale. Water supply reservoirs dampened natural seasonal patterns in community structure at sites downstream of impoundments. Generally, taxonomic richness was higher and %EPT richness lower at downstream sites. Biomonitoring indices used for river management purposes were able to detect community differences, demonstrating their sensitivity to river regulation activities. The results presented improve our understanding of the spatially extensive and long-term effects of water supply reservoirs on instream communities and provide a basis for the future implementation of mitigation measures on impounded rivers and heavily modified waterbodies

    A mixed-method investigation of patient monitoring and enhanced feedback in routine practice: Barriers and facilitators

    Objective: To investigate the barriers and facilitators of an effective implementation of an outcome monitoring and feedback system in a UK National Health Service psychological therapy service. Method: An outcome monitoring system was introduced in two services. Enhanced feedback was given to therapists after session 4. Qualitative and quantitative methods were used, including questionnaires for therapists and patients. Thematic analysis was carried out on written and verbal feedback from therapists. Analysis of patient outcomes for 202 episodes of therapy was compared with benchmark data of 136 episodes of therapy for which feedback was not given to therapists. Results: Themes influencing the feasibility and acceptability of the feedback system were the extent to which therapists integrated the measures and feedback into the therapy, availability of administrative support, information technology, and complexity of the service. There were low levels of therapist actions resulting from the feedback, including discussing the feedback in supervision and with patients. Conclusions: The findings support the feasibility and acceptability of setting up a routine system in a complex service, but a number of challenges and barriers have to be overcome and therapist differences are apparent. More research on implementation and effectiveness is needed in diverse clinical settings

    Capturing health and eating status through a nutritional perception screening questionnaire (NPSQ9) in a randomised internet-based personalised nutrition intervention : the Food4Me study

    BACKGROUND: National guidelines emphasize healthy eating to promote wellbeing and prevention of non-communicable diseases. The perceived healthiness of food is determined by many factors affecting food intake. A positive perception of healthy eating has been shown to be associated with greater diet quality. Internet-based methodologies allow contact with large populations. Our present study aims to design and evaluate a short nutritional perception questionnaire, to be used as a screening tool for assessing nutritional status, and to predict an optimal level of personalisation in nutritional advice delivered via the Internet. METHODS: Data from all participants who were screened and then enrolled into the Food4Me proof-of-principle study (n = 2369) were used to determine the optimal items for inclusion in a novel screening tool, the Nutritional Perception Screening Questionnaire-9 (NPSQ9). Exploratory and confirmatory factor analyses were performed on anthropometric and biochemical data and on dietary indices acquired from participants who had completed the Food4Me dietary intervention (n = 1153). Baseline and intervention data were analysed using linear regression and linear mixed regression, respectively. RESULTS: A final model with 9 NPSQ items was validated against the dietary intervention data. NPSQ9 scores were inversely associated with BMI (β = -0.181, p < 0.001) and waist circumference (Β = -0.155, p < 0.001), and positively associated with total carotenoids (β = 0.198, p < 0.001), omega-3 fatty acid index (β = 0.155, p < 0.001), Healthy Eating Index (HEI) (β = 0.299, p < 0.001) and Mediterranean Diet Score (MDS) (β = 0. 279, p < 0.001). Findings from the longitudinal intervention study showed a greater reduction in BMI and improved dietary indices among participants with lower NPSQ9 scores. CONCLUSIONS: Healthy eating perceptions and dietary habits captured by the NPSQ9 score, based on nine questionnaire items, were associated with reduced body weight and improved diet quality. Likewise, participants with a lower score achieved greater health improvements than those with higher scores, in response to personalised advice, suggesting that NPSQ9 may be used for early evaluation of nutritional status and to tailor nutritional advice.     Diatoms as indicators of the effects of river impoundment at multiple spatial scales

    River impoundment constitutes one of the most important anthropogenic impacts on the World’s rivers. An increasing number of studies have tried to quantify the effects of river impoundment on riverine ecosystems over the past two decades, often focusing on the effects of individual large reservoirs. This study is one of the first to use a large-scale, multi-year diatom dataset from a routine biomonitoring network to analyse sample sites downstream of a large number of water supply reservoirs (n = 77) and to compare them with paired unregulated control sites. We analysed benthic diatom assemblage structure and a set of derived indices, including ecological guilds, in tandem with multiple spatio-temporal variables to disclose patterns of ecological responses to reservoirs beyond the site-specific scale. Diatom assemblage structure at sites downstream of water supply reservoirs was significantly different to control sites, with the effect being most evident at the regional scale. We found that regional influences were important drivers of differences in assemblage structure at the national scale, although this effect was weaker at downstream sites, indicating the homogenising effect of river impoundment on diatom assemblages. Sites downstream of reservoirs typically exhibited a higher taxonomic richness, with the strongest increases found within the motile guild. In addition, Trophic Diatom Index (TDI) values were typically higher at downstream sites. Water quality gradients appeared to be an important driver of diatom assemblages, but the influence of other abiotic factors could not be ruled out and should be investigated further. Our results demonstrate the value of diatom assemblage data from national-scale biomonitoring networks to detect the effects of water supply reservoirs on instream communities at large spatial scales. This information may assist water resource managers with the future implementation of mitigation measures such as setting environmental flow targets

    Modeling the observed tropospheric BrO background: Importance of multiphase chemistry and implications for ozone, OH, and mercury

    Aircraft and satellite observations indicate the presence of ppt (ppt ≡ pmol/mol) levels of BrO in the free troposphere with important implications for the tropospheric budgets of ozone, OH, and mercury. We can reproduce these observations with the GEOS-Chem global tropospheric chemistry model by including a broader consideration of multiphase halogen (Br–Cl) chemistry than has been done in the past. Important reactions for regenerating BrO from its non-radical reservoirs include HOBr+Br−/Cl− in both aerosols and clouds, and oxidation of Br− by ClNO3 and ozone. Most tropospheric BrO in the model is in the free troposphere, consistent with observations, and originates mainly from the photolysis and oxidation of ocean-emitted CHBr3. Stratospheric input is also important in the upper troposphere. Including production of gas phase inorganic bromine from debromination of acidified sea salt aerosol increases free tropospheric Bry by about 30 %. We find HOBr to be the dominant gas-phase reservoir of inorganic bromine. Halogen (Br-Cl) radical chemistry as implemented here in GEOS-Chem drives 14 % and 11 % decreases in the global burdens of tropospheric ozone and OH, respectively, a 16 % increase in the atmospheric lifetime of methane, and an atmospheric lifetime of 6 months for elemental mercury. The dominant mechanism for the Br-Cl driven tropospheric ozone decrease is oxidation of NOx by formation and hydrolysis of BrNO3 and ClNO3
