    Papillary thyroid carcinoma tall cell variant shares accumulation of mitochondria, mitochondrial DNA mutations, and loss of oxidative phosphorylation complex I integrity with oncocytic tumors

Pathogenic homoplasmic/highly heteroplasmic somatic mitochondrial DNA (mtDNA) mutations, usually affecting oxidative phosphorylation (OXPHOS) complex I subunits, are hallmarks of oncocytic cells. To clarify the relationship between PTC-TCV and oncocytic thyroid tumors, 17 PTC-TCV and 16 PTC non-TCV controls (cPTC) were subjected to: (1) transmission electron microscopy (TEM) to assess mitochondria accumulation, (2) next-generation sequencing to analyze mtDNA and nuclear genes frequently mutated in thyroid carcinoma, and (3) immunohistochemistry (IHC) for prohibitin and complex I subunit NDUFS4 to evaluate OXPHOS integrity. TEM showed replacement of cytoplasm by mitochondria in PTC-TCV but not in cPTC cells. All 17 PTC-TCV had at least one mtDNA mutation, totaling 21 mutations; 3/16 cPTC (19%) had mtDNA mutations (p < 0.001). PTC-TCV mtDNA mutations were homoplasmic/highly heteroplasmic, 16/21 (76%) mapping within mtDNA-encoded complex I subunits. MtDNA mutations in cPTC were homoplasmic in 2 cases and at low heteroplasmy in the third case, 2/3 mapping to mtDNA-encoded complex I subunits; 2/3 cPTC with mtDNA mutations had small tall cell subpopulations. PTC-TCV showed strong prohibitin expression and cPTC low-level expression, consistent with massive and limited mitochondrial content, respectively. All 17 PTC-TCV showed NDUFS4 loss (partial or complete) and 3 of 16 cPTC (19%) had (partial) NDUFS4 loss, consistent with lack of complex I integrity in PTC-TCV (p < 0.001). IHC loss of NDUFS4 was associated with mtDNA mutations (p < 0.001). Four BRAF V600E mutated PTCs had loss of NDUSF4 expression limited to neoplastic cell subpopulations with tall cell features, indicating that PTCs first acquire BRAF V600E and then mtDNA mutations. Similar to oncocytic thyroid tumors, PTC-TCV is characterized by mtDNA mutations, massive accumulation of mitochondria, and loss of OXPHOS integrity.     Adjuvant Radiotherapy in Patients with Squamous Cell Carcinoma of the Oral Cavity or Oropharynx and Solitary Ipsilateral Lymph Node Metastasis (pN1) : A Prospective Multicentric Cohort Study

    (1) Background: Evaluation of impact of adjuvant radiation therapy (RT) in patients with oral squamous cell carcinoma of the oral cavity/oropharynx (OSCC) of up to 4 cm (pT1/pT2) and solitary ipsilateral lymph node metastasis (pN1). A non-irradiated group with clinical follow-up was chosen for control, and survival and quality of life (QL) were compared; (2) Methods: This prospective multicentric comprehensive cohort study included patients with resected OSCC (pT1/pT2, pN1, and cM0) who were allocated into adjuvant radiation therapy (RT) or observation. The primary endpoint was overall survival. Secondary endpoints were progression-free survival and QL after surgery; (3) Results: Out of 27 centers, 209 patients were enrolled with a median follow-up of 3.4 years. An amount of 137 patients were in the observation arm, and 72 received adjuvant irradiation. Overall survival did not differ between groups (hazard ratio (HR) 0.98 [0.55–1.73], p = 0.94). There were fewer neck metastases (HR 0.34 [0.15–0.77]; p = 0.01), as well as fewer local recurrences (HR 0.41 [0.19–0.89]; p = 0.02) under adjuvant RT. For QL, irradiated patients showed higher values for the symptom scale pain after 0.5, two, and three years (all p < 0.05). After six months and three years, irradiated patients reported higher symptom burdens (impaired swallowing, speech, as well as teeth-related problems (all p < 0.05)). Patients in the RT group had significantly more problems with mouth opening after six months, one, and two years (p < 0.05); (4) Conclusions: Adjuvant RT in patients with early SCC of the oral cavity and oropharynx does not seem to influence overall survival, but it positively affects progression-free survival. However, irradiated patients report a significantly decreased QL up to three years after therapy compared to the observation group

    Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∌99% of the euchromatic genome and is accurate to an error rate of ∌1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead

    « ExtrĂȘme » ?

    En inventant la droite et la gauche comme forme de positionnement politique, la RĂ©volution française voit poindre, aux confins d’un Ă©chiquier ainsi redĂ©fini dĂšs la fin de 1789, des formes de radicalitĂ© qui ont contribuĂ© Ă  rendre le dĂ©bat plus tendu. En mĂȘme temps que la politique naĂźt une forme d’« extrĂ©misme » Ă  droite comme Ă  gauche, qui se caractĂ©rise par une grande attention thĂ©orique tout autant que par des pratiques de rupture avec le dĂ©bat supposĂ© plus calme des AssemblĂ©es. Les actes de ce colloque se proposent d’interroger la notion d’« extrĂȘme » depuis son apparition dans le dĂ©bat rĂ©volutionnaire jusqu’à ses plus rĂ©centes manifestations, en France ou en Italie par exemple, oĂč une gauche radicale a pu se manifester et continue d’exister sous diffĂ©rentes formes. Ce sont de multiples facettes de cette rĂ©alitĂ© mouvante qui sont ici proposĂ©es et Ă©tudiĂ©es. Qu’en est-il d’une dĂ©finition prĂ©cise de l’« extrĂȘme » ? Est-ce une posture idĂ©ologique revendiquĂ©e dans une AssemblĂ©e reprĂ©sentative ? Ou bien, de par sa radicalitĂ©, ne peut-elle se dĂ©clarer que dans un espace autre, celui de la militance, de la sociĂ©tĂ© civile et de l’action sociale ? Est-elle une rĂ©alitĂ© homogĂšne ou bien Ă  son tour traversĂ©e de nuances, voire d’oppositions qui la divisent ? Ne serait-elle pas plutĂŽt une rĂ©alitĂ© et un mot imposĂ©s par ses dĂ©tracteurs inquiets ou agressifs, sous la forme d’un stigmate visant Ă  dĂ©crĂ©dibiliser depuis deux cents ans ceux dont la politique est dite d’« extrĂȘme gauche » ? « ExtrĂȘme » devient alors un stigmate qui permet de ranger sous un mĂȘme mot le terroriste, le partageux, la pĂ©troleuse, ou le militant syndical trop actif. Qu’en est-il des acteurs de cette radicalitĂ© de gauche ? Qu’en est-il des systĂšmes de reprĂ©sentations qui, depuis plus de deux cents ans, tentent de la marginaliser au nom d’une politique du bon sens, au nom d’un centre modĂ©rĂ© rejetant Ă  sa pĂ©riphĂ©rie celles et ceux qui sont considĂ©rĂ©s comme des dangers pour l’ordre public et social ? Il s’agit ici d’ouvrir des pistes pour inscrire dans l’histoire du politique cette rĂ©alitĂ© de la gauche « extrĂȘme », ces combats d’images, ces luttes de mots, autour d’une pensĂ©e et d’une pratique de gauche dont on essaie de savoir si elle est « extrĂȘme » parce que dĂ©calĂ©e par des discours dominants faisant de la modĂ©ration l’objet de l’ordre politique

    Physiological Arousal in Autism and Fragile X Syndrome: Group Comparisons and Links With Pragmatic Language

    This study tested the hypothesis that pragmatic (i.e., social) language impairment is linked to arousal dysregulation in autism spectrum disorder (ASD) and fragile X syndrome (FXS). Forty boys with ASD, 39 with FXS, and 28 with typical development (TD), aged 4–15 years, participated. Boys with FXS were hyperaroused compared to boys with TD but did not differ from boys with ASD. Dampened vagal tone predicted pragmatic impairment in ASD, and associations emerged between cardiac activity and receptive/expressive vocabulary across groups. Findings support autonomic dysfunction as a mechanism underlying pragmatic impairment in ASD and suggest that biophysiological profiles are shared in ASD and FXS, which has implications for understanding the role of fragile X mental retardation-1 (FMR1, the FXS gene) in the pathophysiology of ASD
