    Playing I Learn Digital Classroom: Innovating the Teaching of English Communication Skills

    The use of resources for digital education provides a favorable environment for students to practice and acquire a second language quickly and effectively. The objective of this educational study was to demonstrate that the implementation of digital educational resources improves English communicative skills in basic education students, Ecuador, 2023. A quasi-experimental methodology with a quantitative approach was used, using 80 students as the study population. The pretest results indicated slightly superior performance in the experimental group in all evaluated categories, highlighting a 10% increase in the Listening category. Subsequently, a 'Program for the use of digital educational resources' was introduced to enhance communicative skills in English. The results of the subsequent phase revealed that the majority of the students in the sixth grade 'A' had Listening (97.50%) and Reading (95.0%) skills at intermediate levels. However, a significant need for improvement in Writing skills was identified, since more than 50% of the students were at the low level. In response to these findings, a didactic guide focused on the development of English writing skills was developed, supporting the continuous need to adjust and perfect educational approaches to meet the changing demands of language learning

    El presente trabajo es el resultado del diplomado “Acompañamiento Psicosocial en Escenarios de Violencia” por medio del cual desde el enfoque narrativo se realiza un análisis y evaluación tomando como base dos relatos (Ana Ligia y Cañas Coloradas) los cuales están caracterizados por ser escenarios de violencia donde se evidencia el desplazamiento forzoso a causa del conflicto armado en Colombia siendo este uno de los impactos psicosociales que deja la guerra más significativo en la vida de las personas ya que desencadena una variedad de consecuencias psicológicas y emocionales en sus víctimas, por esta razón se proponen preguntas de tres tipos diferentes, acciones de apoyo y estrategias psicosociales para obtener un acercamiento psicosocial ético con las victimas enfocado en fortalecer los recursos para que logren afrontar estos episodios de violencia. Por otra parte, se realiza una exposición de imágenes y narrativas dadas por la realidad y la subjetividad por medio de la experiencia de la foto voz el cual se usa como instrumento para lograr una identificación de variables psicosociales pero que también aporta en la construcción de memoria y la transformación psicosocial; Esta experiencia fue realizada en cinco contextos diferentes que tiene algo en común y es que son escenarios de violencia en los cuales de identifican problemáticas como el consumo de alcohol y sustancias psicoactivas, homicidios, minas anti personas, abusos sexuales, secuestros, reclutamientos forzados, desplazamiento, violencia intrafamiliar, pobreza, prostitución, muertes, corrupción, atracos y desapariciones.The present work is the result of the diploma "Psychosocial Accompaniment in Violence Scenarios" through which, from the narrative approach, an analysis and evaluation is carried out based on two stories (Ana Ligia and Cañas Coloradas) which are characterized as violence scenarios where the forced displacement due to the armed conflict in Colombia is evidenced, this being one of the psychosocial impacts that the war leaves the most significant in people's lives since it triggers a variety of psychological and emotional consequences in its victims, for this reason it is They propose questions of three different types, support actions and psychosocial strategies to obtain an ethical psychosocial approach with the victims focused on strengthening the resources so that they can face these episodes of violence.     The clinical and molecular cardiometabolic fingerprint of an exploratory psoriatic arthritis cohort is associated with the disease activity and differentially modulated by methotrexate and apremilast

    Objectives: (1) To evaluate clinical and molecular cardiovascular disease (CVD) signs and their relationship with psoriatic arthritis (PsA) features and (2) to identify a clinical patient profile susceptible to benefit from methotrexate (MTX) and/or apremilast regarding CVD risk. Methods: This cross-sectional study included 100 patients with PsA and 100 age-matched healthy donors. In addition, an exploratory cohort of 45 biologically naïve patients treated for 6 months with apremilast, MTX or combined therapy according to routine clinical practice was recruited. Extensive clinical and metabolic profiles were obtained. Ninety-nine surrogate CVD-related molecules were analysed in plasma and peripheral blood mononuclear cells (PBMCs). Hard cluster analysis was performed to identify the clinical and molecular phenotypes. Mechanistic studies were performed on adipocytes. Results: Cardiometabolic comorbidities were associated with disease activity and long-term inflammatory status. Thirty-five CVD-related proteins were altered in the plasma and PBMCs of PsA patients and were associated with the key clinical features of the disease. Plasma levels of some of the CVD-related molecules might distinguish insulin-resistant patients (MMP-3, CD163, FABP-4), high disease activity (GAL-3 and FABP-4) and poor therapy outcomes (CD-163, LTBR and CNTN-1). Hard cluster analysis identified two phenotypes of patients according to the rates of cardiometabolic comorbidities with distinctive clinical and molecular responses to each treatment. Conclusions: (1) Novel CVD-related proteins associated with clinical features could be emerging therapeutic targets in the context of PsA and (2) the pleiotropic action of apremilast could make it an excellent choice for the management of PsA patients with high CVD risk, targeting metabolic alterations and CVD-related molecules

    Risk Factors and Predictive Score for Bacteremic Biliary Tract Infections Due to Enterococcus faecalis and Enterococcus faecium: a Multicenter Cohort Study from the PROBAC Project

    Biliary-tract bloodstream infections (BT-BSI) caused by Enterococcus faecalis and E. faecium are associated with inappropriate empirical treatment and worse outcomes compared to other etiologies. The objective of this study was to investigate the risk factors for enterococcal BT-BSI. Patients with BT-BSI from the PROBAC cohort, including consecutive patients with BSI in 26 Spanish hospitals between October 2016 and March 2017, were selected; episodes caused by E. faecalis or E. faecium and other causes were compared. Independent predictors for enterococci were identified by logistic regression, and a predictive score was developed. Eight hundred fifty episodes of BT-BSI were included; 73 (8.5%) were due to target Enterococcus spp. (48 [66%] were E. faecium and 25 [34%] E. faecalis). By multivariate analysis, the variables independently associated with Enterococcus spp. were (OR; 95% confidence interval): cholangiocarcinoma (4.48;1.32 to 15.25), hospital acquisition (3.58;2.11 to 6.07), use of carbapenems in the previous month (3.35;1.45 to 7.78), biliary prosthesis (2.19;1.24 to 3.90), and moderate or severe chronic kidney disease (1.55;1.07 to 2.26). The AUC of the model was 0.74 [95% CI0.67 to 0.80]. A score was developed, with 7, 6, 5, 4, and 2 points for these variables, respectively, with a negative predictive value of 95% for a score # 6. A model, including cholangiocarcinoma, biliary prosthesis, hospital acquisition, previous carbapenems, and chronic kidney disease showed moderate prediction ability for enterococcal BT-BSI. Although the score will need to be validated, this information may be useful for deciding empirical therapy in biliary tract infections when bacteremia is suspected. IMPORTANCE Biliary tract infections are frequent, and a significant cause of morbidity and mortality. Bacteremia is common in these infections, particularly in the elderly and patients with cancer. Inappropriate empirical treatment has been associated with increased risk of mortality in bacteremic cholangitis, and the probability of receiving inactive empirical treatment is higher in episodes caused by enterococci. This is because many of the antimicrobial agents recommended in guidelines for biliary tract infections lack activity against these organisms. To the best of our knowledge, this is the first study analyzing the predictive factors for enterococcal BT-BSI and deriving a predictive score.8 página
