    La Administración de Recursos Humanos se aplica en un contexto de organizaciones y personas. Administrar personas significa tener aptitud, capacidad para liderar, amplio espíritu de tolerancia, ser proactivo, comunicativo, responsable, con iniciativa y dotados de habilidades y conocimientos que ayudan a administrar al talento humano.Para las Alcaldia la gestión de la administración de recursos humanos esta dada mediante la correcta Aplicación de la Ley 502, de los Formatos y de los Manuales Internos, para este caso específico se toma la Alcaldía de El Crucero. Es por eso que en la Alcaldía de El Crucero, se ha ido rompiendo barreras, es decir más allá, antes existían un tabú direccional, en la área de recursos humanos, por lo que hoy en día, se ha permitido ir consolidando y cerrando la brecha relacional entre el servidor público y el departamento de recursos humanos llegando a una comunicación más amplia. El desarrollo de esta investigación se plantea bajo la perspectiva metodológica del modelo descriptivo y presenta la siguiente estructura: Capítulo I:Denominado Manuales Administrativos de una organización describe las generalidades de manuales, sus conceptos, importancia, clasificación, ventajas y desventajas, los aspectos claves de los manuales, su implementación y manejo de la información en las tareas asignadas en los colaboradores según área de trabajo. Capítulo II: Denominado Ley 502, Ley de Carrera Administrativa Municipal, en este acápite abordaremos los conceptos básicos de la ley y otros conceptos que tiene que ver con el manejo de los gobiernos municipales, así como las características, implementación, importancias y manejo de la ley en cada una de las dependencias según el uso de los diferentes instrumentos que se necesitan en una municipalidad, con el fin de cumplir sus obligaciones y responsabilidades profesionales que la ley otorga, al personal público. Capítulo III: denominado Manuales Internos de la Municipalidad El Crucero, en este punto, se pretende dar a conocer la importancia del uso y manejo de los instrumentos realizados en el departamento de recursos humanos, considerando que su procedimiento se cumpla y se utilice los formatos adecuados para tener personal capacitado que puedan dar un mejor servicio a la comunidad. Capítulo IV: Denominado Descripción de la Alcaldía del Poder Ciudadano El Crucero, con este inciso lo que se pretende es dar una breve descripción sobre las características del municipio y de la alcaldía, con el fin de conocer un poco de su geografía y aspectos culturales. Dentro de la conclusión fundamental hacemos mención, que hemos obtenido amplios conocimientos sobre toda la teoría existente en materia de Formatos y Manuales Internos de Personal, así mismo también identificamos la forma como se lleva a cabo ese proceso en la El Crucero el uso de los mismo e intentamos proponer que dicho procedimiento, sean apegándonos al contenido de la Ley 502, Ley de Carrera Administrativa Municipal, con el humilde deseo de aportar nuestro granito de arena a la consecución de los procesos de modernización que desde hace un tiempo viene implementando la Alcaldía de El Crucero

    Agentes y lazos sociales: la experiencia de volverse comunidad

    En la colonia Lomas de Polanco, al sur de la ciudad de Guadalajara, México, entre 1970 y 1990 se generó un movimiento ciudadano, enraizado en las Comunidades Eclesiales de Base y la educación popular, que emprendió una lucha exitosa para conseguir los servicios de infraestructura urbana ante las promesas incumplidas de autoridades y fraccionadores. La confluencia de actores, individuales y colectivos, de distintos contextos, contribuyeron a la reconstitución del tejido social y la creación de los lazos que dieron sentido de comunidad a los colonos y al desarrollo de su capacidad de agencia para la trasformación positiva de su entorno y de su vida. Esta experiencia se recupera en esta obra, a partir de una serie de entrevistas con los actores del movimiento, mediante un análisis retrospectivo y desde varias perspectivas teóricas.     Impact of interstitial lung disease on the survival of systemic sclerosis with pulmonary arterial hypertension

    To assess severity markers and outcomes of patients with systemic sclerosis (SSc) with or without pulmonary arterial hypertension (PAH-SSc/non-PAH-SSc), and the impact of interstitial lung disease (ILD) on PAH-SSc. Non-PAH-SSc patients from the Spanish SSc registry and PAH-SSc patients from the Spanish PAH registry were included. A total of 364 PAH-SSc and 1589 non-PAH-SSc patients were included. PAH-SSc patients had worse NYHA-functional class (NYHA-FC), worse forced vital capacity (FVC) (81.2 +/- 20.6% vs 93.6 +/- 20.6%, P < 0.001), worse tricuspid annular plane systolic excursion (TAPSE) (17.4 +/- 5.2 mm vs 19.9 +/- 6.7 mm, P < 0.001), higher incidence of pericardial effusion (30% vs 5.2%, P < 0.001) and similar prevalence of ILD (41.8% vs. 44.9%). In individuals with PAH-SSc, ILD was associated with worse hemodynamics and pulmonary function tests (PFT). Up-front combination therapy was used in 59.8% and 61.7% of patients with and without ILD, respectively. Five-year transplant-free survival rate was 41.1% in PAH-SSc patients and 93.9% in non-PAH-SSc patients (P < 0.001). Global survival of PAH-SSc patients was not affected by ILD regardless its severity. The multivariate survival analysis in PAH-SSc patients confirmed age at diagnosis, worse NYHA-FC, increased PVR, reduced DLCO, and lower management with up-front combination therapy as major risk factors. In conclusion, in PAH-SSc cohort risk of death was greatly increased by clinical, PFT, and hemodynamic factors, whereas it was decreased by up-front combination therapy. Concomitant ILD worsened hemodynamics and PFT in PAH-SSc but not survival regardless of FVC impairment

    Explicar cómo y en qué condiciones fueron construidos algunos de los instrumentos científicos más emblemáticos del mundo moderno sigue siendo un desafío para los historiadores de la ciencia y de la tecnología. Este principio afecta también a los instrumentos náuticos. Así, el objetivo del presente artículo es considerar el “cómo” y el “en qué condiciones”, en un sentido amplio, analizando uno de estos artefactos: las cartas náuticas construidas durante la expansión marítima europea. Originalidad: La originalidad del texto no reside únicamente en llevar a cabo una reconstrucción de este instrumento en Portugal y España que aporte nuevos datos sobre su proceso de fabricación y sus fabricantes, sino también en arrojar luz sobre antiguos debates historiográficos que subyacen a la génesis de la ciencia moderna, tales como la difícil colaboración entre hombres prácticos y hombres teóricos; la aportación de comunidades de artesanos al mundo del conocimiento; el papel que los saberes prácticos tuvieron en el establecimiento de nuevas formas de estandarización en ciencia; la adopción de una metodología empírica para el dominio del mundo o la construcción de grandes imperios globales cimentados sobre la base del trabajo manual. Metodología: El análisis de estos debates requiere una aproximación metodológica que atienda a las categorías y los conceptos principales puestos en circulación tanto por la historia como por la filosofía de la ciencia más reciente. En definitiva, se trata de atender a las condiciones que permitieron el establecimiento de acuerdos (y también de controversias) epistemológicos entre diferentes comunidades de conocimiento tomando un instrumento náutico como “mediador”. Conclusiones: El artículo concluye que la consideración de las culturas empíricas del conocimiento y sus realizaciones materiales son una condición sine qua non para entender la génesis de la modernidad científica europea y la construcción de un mundo globalExplaining how and under what conditions some of the most emblematic scientific instruments of the modern world were built remains a challenge for historians of science and technology. This principle also affects nautical instruments. Thus, the objective of this article is to consider the “how” and the “under what conditions”, in a broad sense, by examining one of these devices: the nautical charts developed during the European maritime expansion. Originality: The originality of this article lies not only in the reconstruction of this instrument in Portugal and Spain, providing new information about its development process and its manufacturers, but also in shedding light on old historiographical debates that underlie the genesis of modern science, such as the difficult collaboration between practical men and theoretical men; the contribution of artisan communities to the world of knowledge; the role that practical knowledge played in the establishment of new forms of standardization in science; the adoption of an empirical methodology for achieving mastery of the world or the construction of large global empires founded upon manual labor. Methodology: Analyzing these debates requires a methodological approach that addresses the main categories and concepts put into circulation by the most recent history and philosophy of science. In short, unveiling the conditions that enabled the establishment of epistemological agreements (and also disputes) between different knowledge communities by taking a nautical instrument as “mediator.” Conclusions: The article concludes that examining empirical knowledge cultures and their material achievements are a sine qua non condition to understand the genesis of European scientific modernity and the construction of a global worldExplicar como e em que condições foram construídos alguns dos instrumentos científicos mais emblemáticos do mundo moderno continua sendo um desafio para os historiadores da ciência e da tecnologia. Esse princípio afeta também os instrumentos náuticos. Assim, o objetivo do presente artigo é considerar o “como” e o “em que condições”, em um sentido amplo, e analisar um desses artefatos: as cartas náuticas construídas durante a expansão marítima europeia.     Impact of Biological Agents on Postsurgical Complications in Inflammatory Bowel Disease : A Multicentre Study of Geteccu

    Background: The impact of biologics on the risk of postoperative complications (PC) in inflammatory bowel disease (IBD) is still an ongoing debate. This lack of evidence is more relevant for ustekinumab and vedolizumab. Aims: To evaluate the impact of biologics on the risk of PC. Methods: A retrospective study was performed in 37 centres. Patients treated with biologics within 12 weeks before surgery were considered "exposed". The impact of the exposure on the risk of 30-day PC and the risk of infections was assessed by logistic regression and propensity score-matched analysis. Results: A total of 1535 surgeries were performed on 1370 patients. Of them, 711 surgeries were conducted in the exposed cohort (584 anti-TNF, 58 vedolizumab and 69 ustekinumab). In the multivariate analysis, male gender (OR: 1.5; 95% CI: 1.2-2.0), urgent surgery (OR: 1.6; 95% CI: 1.2-2.2), laparotomy approach (OR: 1.5; 95% CI: 1.1-1.9) and severe anaemia (OR: 1.8; 95% CI: 1.3-2.6) had higher risk of PC, while academic hospitals had significantly lower risk. Exposure to biologics (either anti-TNF, vedolizumab or ustekinumab) did not increase the risk of PC (OR: 1.2; 95% CI: 0.97-1.58), although it could be a risk factor for postoperative infections (OR 1.5; 95% CI: 1.03-2.27). Conclusions: Preoperative administration of biologics does not seem to be a risk factor for overall PC, although it may be so for postoperative infections

    Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2

    The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
