Inverse-probability weighting and multiple imputation for evaluating selection bias in the estimation of childhood obesity prevalence using data from electronic health records
Background and objectives: Height and weight data from electronic health records are increasingly being used
to estimate the prevalence of childhood obesity. Here, we aim to assess the selection bias due to missing weight
and height data from electronic health records in children older than five.
Methods: Cohort study of 10,811 children born in Navarra (Spain) between 2002 and 2003, who were still living in
this region by December 2016. We examined the differences between measured and non-measured children older
than 5 years considering weight-associated variables (sex, rural or urban residence, family income and weight status
at 2–5 yrs). These variables were used to calculate stabilized weights for inverse-probability weighting and to conduct
multiple imputation for the missing data. We calculated complete data prevalence and adjusted prevalence considering
the missing data using inverse-probability weighting and multiple imputation for ages 6 to 14 and group ages 6 to 9 and
10 to 14.
Results: For 6–9 years, complete data, inverse-probability weighting and multiple imputation obesity age-adjusted
prevalence were 13.18% (95% CI: 12.54–13.85), 13.22% (95% CI: 12.57–13.89) and 13.02% (95% CI: 12.38–13.66) and
for 10–14 years 8.61% (95% CI: 8.06–9.18), 8.62% (95% CI: 8.06–9.20) and 8.24% (95% CI: 7.70–8.78), respectively.
Conclusions: Ages at which well-child visits are scheduled and for the 6 to 9 and 10 to 14 age groups, weight
status estimations are similar using complete data, multiple imputation and inverse-probability weighting. Readily
available electronic health record data may be a tool to monitor the weight status in children
Adherence to the Western, Prudent and Mediterranean dietary patterns and breast cancer risk: MCC-Spain study
Objective
To externally validate the previously identified effect on breast cancer risk of the Western, Prudent and Mediterranean dietary patterns.
Study design
MCC-Spain is a multicase-control study that collected epidemiological information on 1181 incident cases of female breast cancer and 1682 control cases from 10 Spanish provinces. Three dietary patterns derived in another Spanish case-control study were analysed in the MCC-Spain study. These patterns were termed Western (high intakes of fatty and sugary products and red and processed meat), Prudent (high intakes of low-fat dairy products, vegetables, fruits, whole grains and juices) and Mediterranean (high intake of fish, vegetables, legumes, boiled potatoes, fruits, olives, and vegetable oil, and a low intake of juices). Their association with breast cancer was assessed using logistic regression models with random province-specific intercepts considering an interaction with menopausal status. Risk according to tumour subtypes ? based on oestrogen (ER), progesterone (PR) and human epidermal growth factor 2 (HER2) receptors (ER+/PR+&HER2-; HER2+; ER-/PR-&HER2-) ? was evaluated with multinomial regression models.
Main outcome measures
Breast cancer and histological subtype.
Results
Our results confirm most of the associations found in the previous case-control study. A high adherence to the Western dietary pattern seems to increase breast cancer risk in both premenopausal women (OR4thvs.1stquartile(95%CI):1.68(1.02;2.79); OR1SD-increase(95%CI): 1.19(1.01;1.40)) and postmenopausal women (OR4thvs.1stquartile(95%CI):1.48(1.07;2.05); OR1SD-increase(95%CI): 1.14(1.01;1.28)). While high adherence to the Prudent pattern did not show any effect on breast cancer, the Mediterranean dietary pattern seemed to be protective, but only among postmenopausal women (OR4thvs.1stquartile (95%CI):0.72(95% CI 0.53;0.98); p-int = 0.075). There were no significant differences by tumour subtype.
Conclusion
Dietary recommendations based on a departure from the Western dietary pattern in favour of the Mediterranean diet could reduce breast cancer risk in the general population.The study was funded by Carlos III Institute of Health grants (PI12/00488, PI12/00265,

Assessment of the EarlyCDT-Lung test as an early biomarker of lung cancer in ever-smokers: A retrospective nested case-control study in two prospective cohorts
The EarlyCDT-Lung test is a blood-based autoantibody assay intended to identify high-risk individuals for low-dose computed tomography lung cancer screening. However, there is a paucity of evidence on the performance of the EarlyCDT-Lung test in ever-smokers. We conducted a nested case-control study within two prospective cohorts to evaluate the risk-discriminatory performance of the EarlyCDT-Lung test using prediagnostic blood samples from 154 future lung cancer cases and 154 matched controls. Cases were selected from those who had ever smoked and had a prediagnostic blood sample <3 years prior to diagnosis. Conditional logistic regression was used to estimate the association between EarlyCDT-Lung test results and lung cancer risk. Sensitivity and specificity of the EarlyCDT-Lung test were calculated in all subjects and subgroups based on age, smoking history, lung cancer stage, sample collection time before diagnosis and year of sample collection. The overall lung cancer odds ratios were 0.89 (95% CI: 0.34-2.30) for a moderate risk EarlyCDT-Lung test result and 1.09 (95% CI: 0.48-2.47) for a high-risk test result compared to no significant test result. The overall sensitivity was 8.4% (95% CI: 4.6-14) and overall specificity was 92% (95% CI: 87-96) when considering a high-risk result as positive. Stratified analysis indicated higher sensitivity (17%, 95% CI: 7.2-32.1) in subjects with blood drawn up to 1 year prior to diagnosis. In conclusion, our study does not support a role of the EarlyCDT-Lung test in identifying the high-risk subjects in ever-smokers for lung cancer screening in the EPIC and NSHDS cohorts
Occupational Heat Exposure and Breast Cancer Risk in the MCC-Spain Study
[EN] Background: Mechanisms linking occupational heat exposure with chronic diseases have been proposed. However, evidence on occupational heat exposure and cancer risk is limited. Methods: We evaluated occupational heat exposure and female breast cancer risk in a large Spanish case-control study. We enrolled 1,738 breast cancer cases and 1,910 frequency-matched population controls. A Spanish job-exposure matrix, MatEmEsp, was used to assign estimates of the proportion of workers exposed (P >= 25% for at least 1 year) and work time with heat stress (wet bulb globe temperature ISO 7243) for each occupation. We used three exposure indices: ever versus never exposed, lifetime cumulative exposure, and duration of exposure (years). We estimated ORs and 95% confidence intervals (CI), applying a lag period of 5 years and adjusting for potential confounders. Results: Ever occupational heat exposure was associated with a moderate but statistically significant higher risk of breast cancer (OR 1.22; 95% CI, 1.01-1.46), with significant trends across categories of lifetime cumulative exposure and duration (P-trend = 0.01 and 0.03, respectively). Stronger associations were found for hormone receptor-positive disease (OR ever exposure = 1.38; 95% Cl, 1.12-1.67). We found no confounding effects from multiple other common occupational exposures; however, results attenuated with adjustment for occupational detergent exposure. Effect of mistimed eating patterns on breast and prostate cancer risk (MCC-Spain Study)

We assessed whether timing of meals is associated with breast and prostate cancer risk taking into account lifestyle and chronotype, a characteristic correlating with preference for morning or evening activity. We conducted a population-based case-control study in Spain, 2008-2013. In this analysis we included 621 cases of prostate and 1,205 of breast cancer and 872 male and 1,321 female population controls who had never worked night shift. Subjects were interviewed on timing of meals, sleep and chronotype and completed a Food Frequency Questionaire. Adherence to the World Cancer Research Fund/American Institute of Cancer Research recommendations for cancer prevention was examined. Compared with subjects sleeping immediately after supper, those sleeping two or more hours after supper had a 20% reduction in cancer risk for breast and prostate cancer combined (adjusted Odds Ratio [OR] = 0.80, 95%CI 0.67-0.96) and in each cancer individually (prostate cancer OR = 0.74, 0.55-0.99; breast cancer OR = 0.84, 0.67-1.06). A similar protection was observed in subjects having supper before 9 pm compared with supper after 10 pm. The effect of longer supper-sleep interval was more pronounced among subjects adhering to cancer prevention recommendations (OR both cancers= 0.65, 0.44-0.97) and in morning types (OR both cancers = 0.66, 0.49-0.90). Adherence to diurnal eating patterns and specifically a long interval between last meal and sleep are associated with a lower cancer risk, stressing the importance of evaluating timing in studies on diet and cancer. What's new? Evidence shows that long-term disruption of endogenous circadian rhythms may be associated with cancer. The effects of mistimed sleeping and eating patterns that come with modern life are however less clear. This large Spanish population-based study examined whether meal timing and sleep patterns are associated with the two most common nightshift-related cancers. Adherence to a more diurnal eating pattern, and specifically an early supper and a long interval between last meal and sleep were associated with a lower breast and prostate cancer risk, stressing the importance of evaluating circadian rhythms in diet and cancer studies and revisiting recommendations for prevention
Dietary patterns related to biological mechanisms and survival after breast cancer diagnosis: results from a cohort study
BackgroundInflammatory, insulin and oestrogenic pathways have been linked to breast cancer (BC). We aimed to examine the relationship between pre-diagnostic dietary patterns related to these mechanisms and BC survival.MethodsThe diabetes risk reduction diet (DRRD), inflammatory score of diet (ISD) and oestrogen-related dietary pattern (ERDP) were calculated using dietary data from the European Prospective Investigation into Cancer and Nutrition (EPIC) study. Cox proportional hazards models were used to assess associations between dietary patterns and overall mortality and competing risk models for associations with BC-specific mortality.ResultsWe included 13,270 BC cases with a mean follow-up after diagnosis of 8.6 years, representing 2340 total deaths, including 1475 BC deaths. Higher adherence to the DRRD score was associated with lower overall mortality (HR1-SD 0.92; 95%CI 0.87-0.96). Greater adherence to pro-inflammatory diets was borderline associated with 6% higher mortality HR1-SD 1.06; 95%CI 1.00-1.12. No significant association with the oestrogen-related dietary pattern was observed. None of the dietary patterns were associated with BC-specific mortality.ConclusionsGreater adherence to an anti-diabetic and anti-inflammatory diet prior to diagnosis is associated with lower overall mortality among BC survivors. Long-term adherence to these dietary patterns could be a means to improve the prognosis of BC survivors
Characterization of the degree of food processing in the European Prospective Investigation into Cancer and Nutrition: Application of the Nova classification and validation using selected biomarkers of food processing
Background: Epidemiological studies have demonstrated an association
between the degree of food processing in our diet and the risk of various
chronic diseases. Much of this evidence is based on the international Nova
classification system, which classifies food into four groups based on the type
of processing: (1) Unprocessed and minimally processed foods, (2) Processed
culinary ingredients, (3) Processed foods, and (4) “Ultra-processed” foods
(UPF). The ability of the Nova classification to accurately characterise the
degree of food processing across consumption patterns in various European
populations has not been investigated so far. Therefore, we applied the Nova
coding to data from the European Prospective Investigation into Cancer and
Nutrition (EPIC) in order to characterize the degree of food processing in our
diet across European populations with diverse cultural and socio-economic
backgrounds and to validate this Nova classification through comparison with
objective biomarker measurements.
Methods: After grouping foods in the EPIC dataset according to the Nova
classification, a total of 476,768 participants in the EPIC cohort (71.5% women;
mean age 51 [standard deviation (SD) 9.93]; median age 52 [percentile (p)25–
p75: 58–66] years) were included in the cross-sectional analysis that
characterised consumption patterns based on the Nova classification. The
consumption of food products classified as different Nova categories were
compared to relevant circulating biomarkers denoting food processing,
measured in various subsamples (N between 417 and 9,460) within the EPIC
cohort via (partial) correlation analyses (unadjusted and adjusted by sex,
age, BMI and country). These biomarkers included an industrial transfatty
acid (ITFA) isomer (elaidic acid; exogenous fatty acid generated during
oil hydrogenation and heating) and urinary 4-methyl syringol sulfate (an
indicator for the consumption of smoked food and a component of liquid
smoke used in UPF).
Results: Contributions of UPF intake to the overall diet in % grams/day varied
across countries from 7% (France) to 23% (Norway) and their contributions to
overall % energy intake from 16% (Spain and Italy) to >45% (in the UK and
Norway). Differences were also found between sociodemographic groups;
participants in the highest fourth of UPF consumption tended to be younger,
taller, less educated, current smokers, more physically active, have a higher
reported intake of energy and lower reported intake of alcohol. The UPF
pattern as defined based on the Nova classification (group 4;% kcal/day) was
positively associated with blood levels of industrial elaidic acid (r = 0.54) and
4-methyl syringol sulfate (r = 0.43). Associations for the other 3 Nova groups
with these food processing biomarkers were either inverse or non-significant
(e.g., for unprocessed and minimally processed foods these correlations were
–0.07 and –0.37 for elaidic acid and 4-methyl syringol sulfate, respectively).
Conclusion: These results, based on a large pan-European cohort,
demonstrate sociodemographic and geographical differences in the
consumption of UPF. Furthermore, these results suggest that the Nova
classification can accurately capture consumption of UPF, reflected by
stronger correlations with circulating levels of industrial elaidic acid and a
syringol metabolite compared to diets high in minimally processed foods

Associations between dietary inflammatory scores and biomarkers of inflammation in the European Prospective Investigation into Cancer and Nutrition (EPIC) cohort
Background: Since the first version of the dietary inflammatory index (DII & REG;) developed in the past decade, several other versions have been developed. However, to date no study has attempted to compare these versions with respect to their associations with biomarkers of inflammation. Objective: We aimed to investigate the relationship between four dietary inflammatory scores [DII, two energy-adjusted derivatives (E-DII and E-DIIr), and the Inflammatory Score of the Diet (ISD)], and circulating levels of several inflammatory markers and adipokines. Methods: This study included 17 637 participants from the European Prospective Investigation into Cancer and Nutrition (EPIC) cohort with at least one marker of inflammation measured in blood. Associations between the four scores and C-reactive protein (CRP), interleukin (IL)6, IL10, IL1RA, tumor necrosis factor-a (TNFa), soluble tumor necrosis factor receptor-1 (sTNFR1), sTNFR2, leptin, soluble leptin receptor (sLeptin R), adiponectin, and High Molecular Weight (HMW) adiponectin were evaluated using multivariable linear regressions adjusted for potential confounders. Results: Positive associations were observed between the four dietary inflammatory scores and levels of CRP, IL6, sTNFR1, sTNFR2 and leptin. However, only the DII and the ISD were positively associated with IL1RA levels and only the DII and the E-DIIr were positively associated with TNFa levels. The proportion of variance of each biomarker explained by the scores was lower than 2%, which was equivalent to the Conclusions: Our results suggest that the four dietary inflammatory scores were associated with some biomarkers of inflammation and could be used to assess the inflammatory potential of diet in European adults but are not sufficient to capture the inflammatory status of an individual. These findings can help to better understand the inflammatory potential of diet, but they need to be replicated in studies with repeated dietary measurements.
