    Clinical assessment of memory is important for the diagnostic understanding, management and rehabilitation of individuals with significant brain dysfunction. The present study investigated ways in which disorganised thinking or impaired information processing contribute to memory problems in survivors of traumatic brain injury (TBI). Memory performance on the Rey Auditory-Verbal Learning Test was assessed for 141 TBI patients and 59 controls and the relative effectiveness of different strategies was evaluated. Results showed a significant main effect between group scores with controls demonstrating progressively superior performance across trials. The range of learning strategies observed across both groups were condensed into three sub-categories. Subjects who employed no strategies at all performed less well than those who used passive strategies, who in turn performed less well than subjects who adopted active strategies. This latter group obtained the highest memory scores in their respective TBI or control groups. However, a temporal effect was evident in that unlike controls who maintained their best performance from initial trials to delayed recall, TBI subjects showed a marked decline in long term memory recall. Taken together, these findings suggest that the ability to initiate, maintain and transfer learning strategies depends on intact meta-cognitive processes such as executive functioning and metamemory, whereby the individual actively employs effective learning strategies. This is an effortful and elaborative process that demands vigilance and planning. When such functions are compromised, as commonly occurs in TBI patients, subsequent learning abilities may be progressively constrained. Where some residual learning ability is indicated, retraining programmes should focus on the development of self-monitoring and other metacognitive skills before instruction in mnemonic techniques

    Collisional quantum thermometry

    We introduce a general framework for thermometry based on collisional models, where ancillas probe the temperature of the environment through an intermediary system. This allows for the generation of correlated ancillas even if they are initially independent. Using tools from parameter estimation theory, we show through a minimal qubit model that individual ancillas can already outperform the thermal Cramer-Rao bound. In addition, due to the steady-state nature of our model, when measured collectively the ancillas always exhibit superlinear scalings of the Fisher information. This means that even collective measurements on pairs of ancillas will already lead to an advantage. As we find in our qubit model, such a feature may be particularly valuable for weak system-ancilla interactions. Our approach sets forth the notion of metrology in a sequential interactions setting, and may inspire further advances in quantum thermometry

    Building capacity for development and implementation of clinical practice guidelines

    Robust, reliable and transparent methodologies are necessary to ensure that clinical practice guidelines (CPGs) meet international criteria. In South Africa (SA) and other low- and middle-income countries, upskilling and training of individuals in the processes of CPG development is needed. Since de novo CPG development is time-consuming and expensive, new emerging CPG-development approaches (adopting, contextualising, adapting and updating existing good-quality CPGs) are potentially more appropriate for our context. These emerging CPG-development methods are either not included or sparsely covered in existing training opportunities. The SA Guidelines Excellence (SAGE) team has responded innovatively to the need for CPG training in SA. We have revised an existing SA course and developed an online, open-access CPG-development toolkit. This Guideline Toolkit is a comprehensive guideline resource designed to assist individuals who are interested in knowing how to develop CPGs. Findings from the SAGE project can now be implemented with this innovative CPG training programme. This level of CPG capacity development has the potential to influence CPG knowledge, development, practices and uptake by clinicians, managers, academics and policy-makers around the country

    Testing the stability of behavioural coping style across stress contexts in the Trinidadian guppy

Within-populations, individuals can vary in stress response, a multivariate phenomenon comprising neuroendocrine, physiological and behavioural traits. 2. Verbal models of individual stress ‘coping style’ have proposed that the behavioural component of this variation can be described as a single axis, with each individual’s coping style being consistent across time and stress contexts. 3. Focusing on this behavioural component of stress response, and combining repeated measures of multiple traits with a novel multivariate modelling framework, we test for the existence of coping style variation and assess its stability across contexts in the Trinidadian guppy (Poecilia reticulata). 4. Specifically, we test the following hypotheses: (i) there exists repeatable among-individual behavioural (co)variation (‘personality’) within a mild stress context consistent with a risk-averse—risk-prone continuum of behavioural coping style, (ii) there is population-level plasticity in behaviour as a function of stressor severity, (iii) there is among-individual variation in plasticity (i.e., IxE), and (iv) the presence of IxE reduces cross-context stability of behavioural coping style. 5. We found significant repeatable among-individual behavioural (co)variation in the mild stress context (open field trial), represented as an I matrix. However, I was not readily described by a simple risk-averse—risk-prone continuum as posited by the original coping style model. We also found strong evidence for population-level changes in mean behaviour with increasing stressor severity (simulated avian and piscine predation risks). 6. Single-trait analyses did show the presence of individual-by-environment interactions (IxE), as among-individual cross-context correlations were significantly less than +1. However, multi-trait analysis revealed the consequences of this plasticity variation were minimal. Specifically, we found little evidence for changes in the structure of I between mild and moderate stress contexts overall, and only minor changes between the two moderate contexts (avian versus piscine predator). 7. We show that a multivariate approach to assessing changes in among individual (co)variance across contexts can prevent the over-interpretation of statistically significant, but small, individual-by-environment effects. While behavioural flexibility enables populations (and individuals) to respond rapidly to changes in the environment, multivariate personality structure can be conserved strongly across such contexts.Funding was provided by a grant from the Biotechnology and Biological Sciences Research Council (BBSRC, grant BB/L022656/1).     High flow rate injection and related hydromechanical interaction are the most important factors in reservoir development of Enhanced Geothermal Systems (EGS). GeoLaB, a new generic geothermal underground research laboratory (URL), is proposed for controlled high flow rate experiments (CHFE) to address limited comprehension of coupled processes connected to EGS reservoir flow conditions. As analogue for typical EGS development, CHFE require specific hydromechanical conditions including a connected fracture network in crystalline basement rock, sufficient hydraulic fracture transmissivities, a strike-slip to normal faulting tectonic regime, controllable hydraulic boundary conditions, and hydrothermal alteration fracture fillings that improve conditions for hydromechanical interaction. With the aim to identify most appropriate areas for future site selection, four criteria have been established based on the EGS reference site of Soultz. Two URLs in crystalline basement worldwide approximate the requirements of a new generic GeoLaB and may be used for accompanying experimentation. Besides favourable geological, hydraulic, and stress conditions, the vicinity to long-term EGS production favours the southern Black Forest as potential region for GeoLaB. Therefore, an exemplary site assessment has been carried out at “Wilhelminenstollen” in the southern Black Forest (Germany). New remote sensing, hydrochemical, and geophysical analyses as well as reactivation potential, and stress modelling were added to complement existing geological and hydrogeological information. At this site, reactivation potential analysis reveals two local maxima prone for shear reactivation as strike-slip faults. The highest lineament density is observed for the N110°E strike direction that is associated with both slip and dilation tendency maxima. Clay minerals occur in fractures and the matrix. Local, partly water-bearing fractures, when partly filled with ore minerals, were connected to veins in the tunnel using shallow geophysical methods. Hydrochemical data reveal infiltration of the tunnel water from at least 500 m above the tunnel. The results suggest a crystalline basement with a fracture network that is regionally connected and water-conducting. Hydraulic conductivity in the southern Black Forest granite is estimated to amount to about 4.5·10−8 m s−1 at 500 m depth. The hydraulic boundary conditions exclude unknown drainage. Analyses of the influence of topography on orientation and magnitude of the maximum stress indicate a minimum overburden of about 500 m for regional reactivation to be valid. In conclusion, the southern Black Forest and in particular “Wilhelminenstollen” offers favourable condition for CHFE. Final decision on the GeoLaB site is to be drawn from forthcoming exploration wells

    Correlation of high temperature X-ray photoemission spectral features and conductivity of epitaxially strained (La0.8Sr0.2)0.95Ni0.2Fe0.8O3/SrTiO3(110)

    Reversible and irreversible discontinuities at around 573 K and 823 K in the electric conductivity of a strained 175 nm thin film of (La0.8Sr0.2)0.95Ni0.2Fe0.8O3-{\delta} grown by pulsed laser deposition on SrTiO3 (110) are reflected by valence band changes as monitored in photoemission and oxygen K-edge x-ray absorption spectra. The irreversible jump at 823 K is attributed to depletion of doped electron holes and reduction of Fe4+ to Fe3+, as evidenced by oxygen and iron core level soft x-ray spectroscopy, and possibly of a chemical origin, whereas the reversible jump at 573 K possibly originates from structural changes

It is important to consider priority conditions requiring rehabilitation, so it can be realistically positioned and costed in national health financing systems like South Africa (SA)‘s proposed National Health Insurance (NHI). This paper describes temporal trends of top-ranked conditions on years lived with disability (YLDs) rates in SA, for which physical rehabilitation can ameliorate associated disability. Methods: This study is a systematic synthesis of publicly available Global Burden of Disease (GBD) 2017 estimates. The top 11 conditions contributing most to YLDs and for which evidence-based rehabilitation interventions exist were identified. Age-standardized rates per 100,000 and YLDs counts were extracted from 1990 to 2017. Significance of changes in temporal trends was determined using Mann-Kendall trend tests. Best-fit rates of yearly changes were calculated per condition, using GBD estimates (2012–2017), and extrapolated (by imposing the bestfit regression line onto results for each subsequent predicted year) as forecasts (2018–2022). Results: Trends for YLDs counts per condition year (1990–2017) and forecasted values (2018–2022) showed an overall steady increase for all conditions, except HIV and respiratory conditions. YLDs counts almost doubled from 1990 to 2017, with a 17% predicted increase from 2017 to 2022. The proportionate contribution to YLDs counts reduced over time for all conditions, except HIV. Although age-standardized YLDs rates appear relatively stable over the analyzed periods for all conditions (except HIV, respiratory conditions and type 2 diabetes), trend changes in YLDs rates over 28 years were significant for all conditions, except neonatal (p = 0.855), hearing loss (p = 0.100) and musculoskeletal conditions (p = 0.300). Significant trend decreases were apparent for 4/9 conditions, implying that another 5/9 conditions showed trend increases over 28 years. Predicted all-age prevalence in 2022 suggests relatively large increases for cardiovascular disease and heart failure, and burns, while relative decreases are predicted for fractures and dislocations, stroke, and musculoskeletal conditions. Conclusion: Rehabilitation needs in SA are potentially massive and unmet, highlighting the need for innovative and context-specific rehabilitation that considers current local needs and projected changes.
