    Investigating Different Levels of Bimanual Interaction With a Novel Motor Learning Task: A Behavioural and Transcranial Alternating Current Stimulation Study

    Many tasks require the skilled interaction of both hands, such as eating with knife and fork or keyboard typing. However, our understanding of the behavioural and neurophysiological mechanisms underpinning bimanual motor learning is still sparse. Here, we aimed to address this by first characterising learning-related changes of different levels of bimanual interaction and second investigating how beta tACS modulates these learning-related changes. To explore early bimanual motor learning, we designed a novel bimanual motor learning task. In the task, a force grip device held in each hand (controlling x- and y-axis separately) was used to move a cursor along a path of streets at different angles (0°, 22.5°, 45°, 67.5°, and 90°). Each street corresponded to specific force ratios between hands, which resulted in different levels of hand interaction, i.e., unimanual (Uni, i.e., 0°, 90°), bimanual with equal force (Bieq, 45°), and bimanual with unequal force (Biuneq 22.5°, 67.5°). In experiment 1, 40 healthy participants performed the task for 45 min with a minimum of 100 trials. We found that the novel task induced improvements in movement time and error, with no trade-off between movement time and error, and with distinct patterns for the three levels of bimanual interaction. In experiment 2, we performed a between-subjects, double-blind study in 54 healthy participants to explore the effect of phase synchrony between both sensorimotor cortices using tACS at the individual’s beta peak frequency. The individual’s beta peak frequency was quantified using electroencephalography. 20 min of 2 mA peak-to-peak amplitude tACS was applied during task performance (40 min). Participants either received in-phase (0° phase shift), out-of-phase (90° phase shift), or sham (3 s of stimulation) tACS. We replicated the behavioural results of experiment 1, however, beta tACS did not modulate motor learning. Overall, the novel bimanual motor task allows to characterise bimanual motor learning with different levels of bimanual interaction. This should pave the way for future neuroimaging studies to further investigate the underlying mechanism of bimanual motor learning

    Incidence, Risk Factors, and Outcomes of Intra-Abdominal Hypertension in Critically Ill Patients-A Prospective Multicenter Study (IROI Study)

    To identify the prevalence, risk factors, and outcomes of intra-abdominal hypertension in a mixed multicenter ICU population. Prospective observational study. Fifteen ICUs worldwide. Consecutive adult ICU patients with a bladder catheter. None. Four hundred ninety-one patients were included. Intra-abdominal pressure was measured a minimum of every 8 hours. Subjects with a mean intra-abdominal pressure equal to or greater than 12 mm Hg were defined as having intra-abdominal hypertension. Intra-abdominal hypertension was present in 34.0% of the patients on the day of ICU admission (159/467) and in 48.9% of the patients (240/491) during the observation period. The severity of intra-abdominal hypertension was as follows: grade I, 47.5%; grade II, 36.6%; grade III, 11.7%; and grade IV, 4.2%. The severity of intra-abdominal hypertension during the first 2 weeks of the ICU stay was identified as an independent predictor of 28-and 90-day mortality, whereas the presence of intra-abdominal hypertension on the day of ICU admission did not predict mortality. Body mass index, Acute Physiology and Chronic Health Evaluation II score greater than or equal to 18, presence of abdominal distension, absence of bowel sounds, and positive end-expiratory pressure greater than or equal to 7 cm H2O were independently associated with the development of intra-abdominal hypertension at any time during the observation period. In subjects without intra-abdominal hypertension on day 1, body mass index combined with daily positive fluid balance and positive end-expiratory pressure greater than or equal to 7 cm H2O (as documented on the day before intra-abdominal hypertension occurred) were-associated with the development of intraabdominal hypertension during the first week in the ICU. In our mixed ICU patient cohort, intra-abdominal hypertension occurred in almost half of all subjects and was twice as prevalent in mechanically ventilated patients as in spontaneously breathing patients. Presence and severity of intra-abdominal hypertension during the observation period significantly and independently increased 28-and 90-day mortality. Five admission day variables were independently associated with the presence or development of intra-abdominal hypertension.     COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    The ABC-proteins form a large family of proteins present in all the living organisms. These proteins use the energy of ATP hydrolysis to transport a wide range of substances across biological membranes. In humans, some of these proteins have significant implications in pathology. For example, dysfunction of CFTR causes cystic fibrosis, and the overproduction of MRP1 is often associated with multidrug resistance phenomena. The yeast Saccharomyces cerevisiae has a cluster of 6 proteins (Y! or1p, Ycf1p, Bpt1p, Ybt1p, Vmr1p, Nft1p) homologous to CFTR and MRP1. This family can thus serve as a model for the study of human proteins. The first part of this work was a biochemical study of the yeast protein Yor1p. We added a histidine tag to the YOR1 gene and placed this construction under the control of a promoter allowing an overproduction in yeast. We showed that the histidine tagged Yor1p was produced in a functional form and, subsequently, we developed a method allowing the solubilization and the purification of Yor1p in a single step by metal affinity chromatography. In a second part of this work, we produced in an isolated form in Escherichia coli the two NBD domains of Yor1p that are involved in ATP binding and hydrolysis and purified them to homogeneity. We studied ATP binding to these two domains, and were able to conclude that these domains are well structured. Thus, they can now be used for structural studies. Finally, we considered the role of Ycf1p in selenite detoxification. We observed that the selenite toxicity for yeast was considerably increased in the presence of thiols. The formation of reactive oxygen species is probably at the origin of this hypertoxicity.Les transporteurs ABC forment une vaste famille de protéines présentes dans tous les organismes vivants. Ces protéines utilisent l'énergie fournie par l'hydrolyse de l'ATP pour transporter à travers les membranes biologiques des substances très variées. Plusieurs protéines ABC sont importantes pour la santé humaine. Par exemple, le défaut fonctionnel de la protéine CFTR cause la mucoviscidose et la surproduction de protéine MRP1 est associée aux phénomènes de résistance aux traitements anti-tumoraux. La levure Saccharomyces cerevisiae possède une famille de protéines (Yor1p, Ycf1p, Bpt1p, Ybt1p, Vmr1p, Nft1p) apparentées à CFTR et MRP1. Cette famille peut servir de modèle à l'étude des protéines humaines. La première partie de ce travail de thèse a été consacrée à l'étude biochimique de la protéine de levure Yor1p. Nous avons fusionné YOR1 avec un fragment d'ADN codant un peptide de poly-histidine et avons placé cette construction sous contrôle d'un promoteur permettant une surproduction dans la levure. Nous avons alors montré que la protéine Yor1p poly-histidylée était produite sous forme fonctionnelle dans la levure, puis avons mis au point une méthode permettant de solubiliser puis de purifier cette protéine en une seule étape par chromatographie d'affinité sur une colonne greffée avec des ions métalliques. La deuxième partie de ce travail a consisté à produire sous forme isolée chez la bactérie Escherichia coli et à purifier à homogénéité les deux domaines de Yor1p impliqués dans la liaison et l'hydrolyse de l'ATP. Nous avons étudié la fixation de l'ATP sur ces deux domaines, ce qui nous a permis de conclure que ces domaines étaient bien structurés. Ils peuvent maintenant être utilisés pour des études structurales. Enfin, nous nous sommes intéressés au rôle la protéine Ycf1p dans la détoxication du sélénite. Nous avons observé que la toxicité du sélénite pour la levure était considérablement accrue par la présence de composés thiolés dans le milieu de culture. La formation de dérivés réactifs de l'oxygène est vraisemblablement à l'origine de cette hypertoxicité
