    Slag Substitution as a Cementing Material in Concrete: Mechanical, Physical and Environmental Properties

    A circular economy is a current tenet that must be implemented in the field of construction. That would imply the study of the possibilities of the use of waste generated, for obtaining materials the used in construction as replacements for the raw material used. One of these possibilities is the substitution of the cement by slag, which contributes to the reduction of cement consumption, decreasing CO2 emissions, while solving a waste management problem. In the present paper, di erent types of concrete made by cement substitution with di erent type of slags have been studied in order to evaluate the properties of these materials. Cement is replaced by slag from di erent steel mills, both blast furnace and ladle furnace slag. The percentages of slag substitution by cement are 30%, 40% and 50% by weight. Mechanical, physical and environmental properties have been evaluated. Compressive and flexural strength have been analysed as the main mechanical properties. As far as physical properties go, density and porosity tests were be reported and analysed, and from an environmental point of view, a leachate study was performed. It has been found that some kinds of slag (blast furnace slag) are very suitable as substitutes for cement, providing properties above those of the reference concrete, while other types (ladle furnace slag) could be valid for non-structural applications, contributing in both cases to a circular econom

    Health of Spanish centenarians: A cross-sectional study based on electronic health records

    Background: With the number of centenarians increasing exponentially in Spain, a deeper knowledge of their socio-demographic, clinical, and healthcare use characteristics is important to better understand the health profile of the very elderly. Methods: We conducted a retrospective, cross-sectional observational study in the EpiChron Cohort (Aragon, Spain) aimed at analyzing the socio-demographic, clinical, drug use and healthcare use characteristics of 1680 centenarians during 2011-2015, using data from electronic health records and clinical-administrative databases. Results: Spanish centenarians (79.1% women) had 101.6 years on average. Approximately 80% of centenarians suffered from multimorbidity, with an average of 4.0 chronic conditions; 50% were exposed to polypharmacy, with an average of 4.8 medications; only 6% of centenarians were free of chronic diseases and only 7% were not on medication. Centenarians presented a cardio-cerebrovascular pattern in which hypertension, heart failure, cerebrovascular disease and dementia were the most frequent conditions. Primary care was the most frequently visited healthcare level (79% of them), followed by medical specialist consultations (23%), hospitalizations (13%), and emergency service use (9%). Conclusions: Multimorbidity is the rule rather than the exception in Spanish centenarians. Addressing medical care in the very elderly from a holistic geriatric view is critical in order to preserve their health, and avoid the negative effects of polypharmacy

    Metalloproteinase-dependent TLR2 ectodomain shedding is involved in soluble toll-like receptor 2 (sTLR2) production

    Toll-like receptor (TLR) 2, a type I membrane receptor that plays a key role in innate immunity, recognizes conserved molecules in pathogens, and triggering an inflammatory response. It has been associated with inflammatory and autoimmune diseases. Soluble TLR2 (sTLR2) variants have been identified in human body fluids, and the TLR2 ectodomain can negatively regulate TLR2 activation by behaving as a decoy receptor. sTLR2 generation does not involve alternative splicing mechanisms, indicating that this process might involve a post-translational modification of the full-length receptor; however, the specific mechanism has not been studied. Using CD14+ peripheral human monocytes and the THP-1 monocytic leukemia-derived cell line, we confirm that sTLR2 generation increases upon treatment with pro-inflammatory agents and requires a post-translational mechanism. We also find that the constitutive and ligand-induced release of sTLR2 is sensitive to pharmacological metalloproteinase activator and inhibitors leading us to conclude that metalloproteinase TLR2 shedding contributes to soluble receptor production. By expressing human TLR2 in ADAM10- or ADAM17-deficient MEF cells, we find both enzymes to be implicated in TLR2 ectodomain shedding. Moreover, using a deletion mutant of the TLR2 juxtamembrane region, we demonstrate that this domain is required for sTLR2 generation. Functional analysis suggests that sTLR2 generated by metalloproteinase activation inhibitsTLR2-induced cytokine production by this monocytic leukemia-derived cell line. The identification of the mechanisms involved in regulating the availability of soluble TLR2 ectodomain and cell surface receptors may contribute further research on TLR2-mediated processes in innate immunity and inflammatory disorders

    Tocilizumab in visual involvement of giant cell arteritis: a multicenter study of 471 patients

    Background: Visual involvement is the most feared complication of giant cell arteritis (GCA). Information on the efficacy of tocilizumab (TCZ) for this complication is scarce and controversial. Objective: We assessed a wide series of GCA treated with TCZ, to evaluate its role in the prevention of new visual complications and its efficacy when this manifestation was already present before the initiation of TCZ. Design: This is an observational multicenter study of patients with GCA treated with TCZ. Methods: Patients were divided into two subgroups according to the presence or absence of visual involvement before TCZ onset. Visual manifestations were classified into the following categories: transient visual loss (TVL), permanent visual loss (PVL), diplopia, and blurred vision. Results: Four hundred seventy-one GCA patients (mean age, 74 +/- 9 years) were treated with TCZ. Visual manifestations were observed in 122 cases (26%), of which 81 were present at TCZ onset: PVL (n = 60; unilateral/bilateral: 48/12), TVL (n = 17; unilateral/bilateral: 11/6), diplopia (n = 2), and blurred vision (n = 2). None of the patients without previous visual involvement or with TVL had new episodes after initiation of TCZ, while only 11 out of 60 (18%) patients with PVL experienced some improvement. The two patients with diplopia and one of the two patients with blurred vision improved. Conclusion: TCZ may have a protective effect against the development of visual complications or new episodes of TVL in GCA. However, once PVL was established, only a few patients improved

    Experiencias positivas de aprendizaje autónomo en el ámbito de las ciencias experimentales

    En este trabajo se presenta un conjunto de casos de estudio donde se han aplicado distintas metodologías que ayudan a la formación de conocimiento utilizando aprendizaje autónomo o semiautónomo. En todos los casos propuestos se ha podido percibir por ambas partes del proceso formativo el éxito en los resultados de aprendizaje. Entre los casos de estudio de éxito podemos mencionar: los trabajos cooperativos coordinados entre diversas asignaturas, el uso de “chuletas oficiales” en los exámenes o el aprendizaje por resolución de problemas. En cada caso se ha detallado el contexto de aplicación de la metodología docente, y en los casos que fuese necesario el equivalente en el grado de Biología y Ciencias del Mar. Además, se ha tenido en cuenta la perspectiva del alumnado, junto con la visión del profesorado, para dar mayor amplitud y proyección a la propuesta. Los resultados animan a continuar usando los casos de éxito y mejorar los mismos adaptando al contexto de la materia y mantenerse siempre abiertos a la incorporación de nuevas metodologías para un óptimo aprendizaje

    Complement component C4 structural variation and quantitative traits contribute to sex-biased vulnerability in systemic sclerosis

We investigated the association of C4 CN with systemic sclerosis (SSc) risk. Imputed total C4, C4A, C4B, and HERV-K CN were analyzed in 26,633 individuals and validated in an independent cohort. Our results showed that higher C4 CN confers protection to SSc, and deviations from CN parity of C4A and C4B augmented risk. The protection contributed per copy of C4A and C4B differed by sex. Stronger protection was afforded by C4A in men and by C4B in women. C4 CN correlated well with its gene expression and serum protein levels, and less C4 was detected for both in SSc patients. Conditioned analysis suggests that C4 genetics strongly contributes to the SSc association within the major histocompatibility complex locus and highlights classical alleles and amino acid variants of HLA-DRB1 and HLA-DPB1 as C4-independent signals

    Early mobilisation in critically ill COVID-19 patients: a subanalysis of the ESICM-initiated UNITE-COVID observational study

    Background Early mobilisation (EM) is an intervention that may improve the outcome of critically ill patients. There is limited data on EM in COVID-19 patients and its use during the first pandemic wave. Methods This is a pre-planned subanalysis of the ESICM UNITE-COVID, an international multicenter observational study involving critically ill COVID-19 patients in the ICU between February 15th and May 15th, 2020. We analysed variables associated with the initiation of EM (within 72 h of ICU admission) and explored the impact of EM on mortality, ICU and hospital length of stay, as well as discharge location. Statistical analyses were done using (generalised) linear mixed-effect models and ANOVAs. Results Mobilisation data from 4190 patients from 280 ICUs in 45 countries were analysed. 1114 (26.6%) of these patients received mobilisation within 72 h after ICU admission; 3076 (73.4%) did not. In our analysis of factors associated with EM, mechanical ventilation at admission (OR 0.29; 95% CI 0.25, 0.35; p = 0.001), higher age (OR 0.99; 95% CI 0.98, 1.00; p ≤ 0.001), pre-existing asthma (OR 0.84; 95% CI 0.73, 0.98; p = 0.028), and pre-existing kidney disease (OR 0.84; 95% CI 0.71, 0.99; p = 0.036) were negatively associated with the initiation of EM. EM was associated with a higher chance of being discharged home (OR 1.31; 95% CI 1.08, 1.58; p = 0.007) but was not associated with length of stay in ICU (adj. difference 0.91 days; 95% CI − 0.47, 1.37, p = 0.34) and hospital (adj. difference 1.4 days; 95% CI − 0.62, 2.35, p = 0.24) or mortality (OR 0.88; 95% CI 0.7, 1.09, p = 0.24) when adjusted for covariates. Conclusions Our findings demonstrate that a quarter of COVID-19 patients received EM. There was no association found between EM in COVID-19 patients' ICU and hospital length of stay or mortality. However, EM in COVID-19 patients was associated with increased odds of being discharged home rather than to a care facility.     Concrete Properties Comparison When Substituting a 25% Cement with Slag from Different Provenances

    Concrete consumption greatly exceeds the use of any other material in engineering. This is due to its good properties as a construction material and the availability of its components. Nevertheless, the present worldwide construction increases and the high-energy consumption for cement production means a high environmental impact. On the other hand, one of the main problems in the iron and steel industry is waste generation and byproducts that must be properly processed or reused to promote environmental sustainability. One of these byproducts is steel slag. The cement substitution with slag strategy achieves two goals: raw materials consumption reduction and waste management. In the present work, four different concrete mixtures are evaluated. The 25% cement substitution is carried out with different types of slag. Tests were made to evaluate the advantages and drawbacks of each mixture. Depending on the origin, characteristics, and treatment of the slag, the concrete properties changed. Certain mixtures provided proper concrete properties. Stainless steel slag produced a fluent mortar that reduced water consumption with a slight mechanical strength loss. Mixtures with ground granulated blast furnace slag properties are better than the reference concrete (without slag)

    Influence of the Fiber Alignment in the Ballistic Performance of Dyneema Non-Woven Fabrics

    The aim of this study was the determination of the deforming micromechanisms of needlepunched felts subjected to impact loads. A large experimental campaign has been carried out to analyze the influence of the fiber alignment in the ballistic performance. Ballistic limit curves of predeformed samples were compared. The fiber realignment was experimentally measure by means of 2D X-Ray diffraction. Higher specific absorption was observed for samples with a more isotropic mechanical response. A constitutive physicallybased model was developed within the context of the finite element method, which provided the constitutive response for a mesodomain including micromechanical aspects as fiber alignment, fiber sliding and pull-out. The macroscopic response has been validated with the experimental results, showing a very good agreement. The absorbed energy by the material during the impact was predicted and the fiber realignment evolution was also obtained
