    Introdução à música contemporânea portuguesa de viola d’arco : um estudo de caso

    Mestrado em Ensino de MúsicaEsta dissertação baseia-se no estudo da introdução da música contemporânea portuguesa de viola d’arco num grupo de alunos de viola d’arco da Casa Pia de Lisboa, através de composições encomendadas a nove compositores portugueses. Incide também na análise das atitudes em relação à música contemporânea portuguesa por parte de professores de viola d’arco portugueses. Procurou-se ampliar o repertório da viola d’arco através da encomenda de obras didáticas, e o repertório expressivo dos alunos envolvidos. Verificou-se que a introdução tardia da música contemporânea no ensino da música e a desatualização dos programas podem condicionar a atitude dos músicos em relação à mesma, sendo importante uma mudança de paradigma. Parecem não existir dificuldades técnicas na abordagem a estas obras por parte dos alunos. Houve também uma aceitação por parte dos professores em introduzir estes conceitos, tendo existido igualmente uma abertura por parte dos compositores contemporâneos envolvidos, em criar obras didáticas para viola d’arco.This dissertation is based on the study of the introduction of contemporary Portuguese viola music for a group of viola students at the Casa Pia in Lisbon through commissioned works from nine Portuguese composers. Its scope is also focused upon the attitudes of Portuguese viola teachers regarding contemporary Portuguese music, seeking to expand the repertoire of the viola through the commission of didactic works, and the expressive repertoire of the students involved in the study. It was possible to verify that the late introduction of contemporary music in music education and the out of date contents of the viola programs may condition the musicians attitude towards it, making it important the occurrence of a change in paradigm. Apparently there were no technical difficulties on the approach to these works by the students. There was an acceptance in the introduction of these concepts by the teachers, and also an opening from the contemporary composers involved in creating didactic works for viola

    This paper reflects an attempt to rethink the process of analysis of energy efficiency initiatives using soft systems methodology (SSM) as a problem structuring tool. The aim of the work is to provide public and private initiative promoters or evaluators with a structured support for a more informed decision regarding the implementation of energy efficiency measures. The SSM approach contributed with the identification of all market players and their relations, as well as the insight into the deficiencies of current methodologies.     Structuring an MCDA model using SSM: A case study in energy efficiency

    This work presents the use of a problem structuring method, Soft Systems Methodology (SSM), to structure a Multi-Criteria Decision Analysis (MCDA) model, aimed at appraising energy efficiency initiatives. SSM was useful to help defining clearly the decision problem context and the main actors involved, as well as to unveil the relevant objectives for each stakeholder. Keeney’s Value Focused Thinking approach was then used to refine and structure the list of objectives according to the perspective of the main evaluators identified.     Development of nevirapine resistance in children exposed to the prevention of mother-to-child HIV-1 transmission programme in Maputo, Mozambique

    Background: Single-dose nevirapine (sd-NVP) has been the main option for prevention of mother-to-child transmission (PMTCT) of HIV-1 in low-resource settings. However, sd-NVP can induce the selection of HIV-1 resistant mutations in mothers and infants. In Mozambique, there are limited data regarding the profile of NVP resistance associated mutations (RAM) in the context of PMTCT. Objectives: To assess the prevalence and the factors associated with NVP RAM among children born to HIV-1 infected mothers enrolled in the PMTCT programme adopted in Mozambique. Methods: One hundred and fifty seven children aged 6 to 48 weeks were sequentially included (July 2011 to March 2012) at four centres in Maputo. Genotyping of RAM was performed in samples with HIV-1 RNA≥ 100 copies/μL (Viroseq). Sequencing was performed with ABI 3100 (Applied Biosystems). Logistic regression modelling was undertaken to identify the factors associated with NVP RAM. Results: Seventy-nine children had their samples genotyped. Their median age was 7.0 (3–12) months and 92.4% received prophylaxis with sd-NVP at birth plus daily NVP. 35.4% of mothers received antiretrovirals (ARVs) for PMTCT. ARV RAM were detected in 43 (54.4%) of the children. 45.6% of these children had at least one NVP RAM. The most common mutations associated with NVP resistance were K103N (n = 16) and Y181C (n = 15). NVP RAM was significantly associated with mother exposure to PMTCT (crude odds ratio [OR] 30.3, 95% CI 4.93–186.34) and with mother’s CD4 count < 350 cells/mm3 (crude OR 3.08, 95% CI 1.02–9.32). In the multivariable analysis the mother’s exposure to PMTCT was the only variable significantly associated with NVP RAM (adjusted OR 48.65, 95% CI 9.33–253.66). Conclusions: We found a high prevalence of NVP RAM among children who were exposed to the drug regimen for PMTCT in Mozambique. Publisher Copyright: ©Objectives To estimate the prevalence of medical care-seeking among adults with low back pain (LBP) and to characterise and compare use of diagnostic procedures and medical management between primary and secondary care. Design Cross-sectional study. Setting Data from the EpiReumaPt, a nationwide population-based study conducted in Portugal including a representative sample of non-institutionalised adults (n=10 661) stratified by administrative territorial units was analysed. Participants Individuals who self-reported history of LBP within the previous 12 months (n=6434) and sought medical care for this problem in the same period (n=2618). Outcome measures Patients' self-reported diagnostic workup and management procedures performed by medical care for LBP collected through a structured questionnaire. Medical care procedures were stratified by level of care. Results The prevalence of medical care-seeking for LBP was 38.0% (95% CI 35.9% to 40.1%). Primary care in isolation (45.3%) was the most sought level of care. Emergency departments (25.9%) and orthopaedics (19.4%) were the most sought secondary medical specialties. Several pathoanatomical diagnoses were used, supported by laboratory or imaging tests (91.1%). Disc herniation (20.4%) and osteoarthritis (19.7%) were the most frequent diagnoses, and X-ray (63.7%) was the most frequent diagnostic procedure self-reported by individuals. Most (75.1%) reported being treated for LBP: 80.4% with oral medication and 49.9% with injectables. The mean duration of pharmacological treatment was 104.24 (SD, 266.80) days. The use of pathoanatomical diagnoses, laboratory or imaging tests, and pharmacological treatments were generally more frequent for secondary care (p<0.05). Approximately one-quarter of individuals (24.5%) reported seeking care from additional healthcare providers, physiotherapists (66.9%) were the most frequent. Conclusions Medical care for LBP is frequent and associated with high levels of pathoanatomical diagnoses, imaging and laboratory tests and pharmacological therapy in both primary and secondary care settings. Funding and delivery actions should be prioritised to assure appropriate care for LBP.publishersversionpublishe

    A hydrogen-bonded assembly of cucurbit[6]uril and [MoO2Cl2(H2O)2] with catalytic efficacy for the one-pot conversion of olefins to alkoxy products

    The reaction of the macrocyclic cavitand cucurbit[6]uril (CB[6]) and the diaqua complex [MoO2Cl2(H2O)2] in hydrochloric acid solution gave a water insoluble supramolecular compound with the general composition 2[MoO2Cl2(H2O)2]·CB[6]·xH2O·yHCl·z(CH3COCH3) (2). Single crystal X-ray diffraction (XRD) analysis revealed the presence of barrel-shape supramolecular entities, {CB[6]·10(H2O)}, aligned in layers which are shifted relative to adjacent layers to form a brick-like pattern. The CB[6]/water hydrogen-bonded entities further engage in intermolecular interactions with water, HCl and [MoO2Cl2(H2O)2] molecules to form a three-dimensional (3D) framework. Compound 2 was characterised by thermogravimetric analysis (TGA), IR and Raman vibrational spectroscopy, and 13C{1H} CP MAS NMR. The reference complex [MoO2Cl2(H2O)2]·(diglyme)2 (1) and compound 2 were studied for the oxidative catalytic conversion of olefins (cis-cyclooctene, cyclohexene and styrene) with aqueous H2O2 as oxidant. Using alcohols as solvents, 2 was employed in a one-pot two-stage strategy for converting olefins to alkoxy products, which involves oxidation (with H2O2) and acid chemistry. Mechanistic studies were carried out using different intermediates as substrates, and the type of solvent and substrate scope were investigated. The results demonstrated the ability of the CB[6]/MoVI supramolecular adduct to function as an acid-oxidation multifunctional catalyst, and its recovery and reuse via relatively simple procedures.We acknowledge the support of CICECO – Aveiro Institute of Materials [FCT (Fundação para a Ciência e a Tecnologia) Ref.     Background: swallowing disorders often occur in Stroke. The Eating Assessment Tool-10 allows early detection of dysphagia and response to treatment. Objective: To evaluate the metric properties of the Eating Assessment Tool-10 in the person with Stroke that presents swallowing disorders. Method: Systematic Review of Literature based on the recommendations of the Joanna Brigs Institute for the PICo strategy, what are the metric properties of the Eating Assessment Tool-10 in people with Stroke that present a swallowing disorder? The research was carried out using the electronic database platforms: EBSCO, Host, Google Scolar, Cochrane Library Collection, Scielo and Proquest, having identified, selected, evaluated the methodological quality and included the articles that take into account the PRISMA recommendations. Results: Six studies were included that fulfilled the eligibility and methodological quality criteria. Studies on the reproducibility, validity and responsiveness of the Eating Assessment Tool-10 were found. However, this last propertyneeds to be studied in future investigations. The limitation identified was the applicability of the scale in people with cognitive alterations. Conclusions: The Eating Assessment Tool-10 is a reliable, valid and responsive tool for people with stroke that present swallowing disorders.Introducción: Con el Accidente Vascular Cerebral, surgen con frecuencia alteraciones de la deglución. El Eating Assessment Tool-10 es un instrumento que permite detectar precozmente la disfagia y la respuesta al tratamiento. Método: Revisión Sistemática de la Literatura basada en las recomendaciones del Joanna Brigs Institute para la estrategia PICo, cuáles las propiedades métricas del Eating Assessment Tool-10 en personas con Accidente VascularCerebral con alteración de la deglución. La investigación se realizó utilizando plataformas de bases de datos electrónicas EBSCO, Host, Google Escolar, Cochrane Lybrary Collection, Scielo y Proquest., Habiendo identificado, seleccionados, evaluados en la calidad metodológica e incluidos los artículos teniendo en cuenta las recomendaciones PRISMA. Resultados: Se incluyeron seis estudios que cumplían los criterios de elegibilidad y de calidad metodológica. Se han encontrado estudios sobre la reproducibilidad, validez y responsividad del Eating Assessment Tool-10. Sin embargo,esta última propiedad necesita ser estudiada en futuras investigaciones. La limitación identificada fue en la aplicabilidad de la escala en personas con alteraciones cognitivas.
