    Phenological and molecular studies on the introduced seaweed Dictyota cyanoloma (Dictyotales, Phaeophyceae) along the Mediterranean coast of the Iberian Peninsula

    Dictyota cyanoloma, a distinctive brown algal species characterized by a blue-iridescent margin, was recently reported as an introduced species in the Mediterranean Sea but little is known about its distribution dynamics, morphological plasticity and genetic structure. In this integrative study, we evaluate its past and present occurrence along the Mediterranean Iberian coast, assess the species' phenology in Palamos (Girona, Spain) and analyze the haplotype diversity by sequencing 49 individuals from nine sampling sites for different chloroplast and mitochondrial DNA regions. Although D. cyanoloma currently occurs along all the Mediterranean Iberian coasts (in 19 of 36 localities sampled, mostly in marinas and harbour environments), we were not able to find any herbarium material of this species (at BCN-Phyc and MA) predating the year 1987. In Palamos, D. cyanoloma is present all through the year, with a maximum development in winter and a minimum in summer. Fertile specimens are absent during summer (July and August). Sporophytes are dominant from January to June and gametophytes were found only in February, March and June. Information about the antheridia, which has never been described before, is provided. Two chloroplast and three mitochondrial haplotypes were observed, indicating that multiple introductions of D. cyanoloma occurred in the study area. Additionally, the genetic structure suggests that spread did not occur through simple advancing wave fronts but by several longdistance dispersal events. Further studies employing microsatellite markers could potentially offer a better resolution to unravel expansion and colonisation dynamics of D. cyanoloma in the Mediterranean Sea

    Optimizing statistical parametric mapping analysis of 18F-FDG PET in children.

    International audienceABSTRACT: BACKGROUND: Statistical parametric mapping (SPM) procedure is an objective tool to analyze 18F-fluoro-2-deoxy-d-glucose-positron-emission tomography (FDG-PET) images and a useful complement to visual analysis. However, SPM requires a comparison to control data set that cannot be obtained in healthy children for ethical reasons. Using adults as controls showed some limitations. The purpose of the present study was to generate and validate a group of pseudo-normal children as a control group for FDG-PET studies in pediatrics. METHODS: FDG-PET images of 47 children (mean +/- SD age 10.2 +/- 3.1 years) with refractory symptomatic (MRI-positive, n = 20) and cryptogenic (MRI-negative, n = 27) focal epilepsy planned for surgery were analyzed using visual and SPM analysis. Performances of SPM analysis were compared using two different control groups: (1) an adult control group consisting of healthy young adults (n = 25, 30.5 +/- 5.8 years, adult PET template) and (2) a pediatric pseudo-control group consisting of patients (n = 24, 10.6 +/- 3.1 years, children PET template) with refractory focal epilepsy; but negative MRI and PET are considered normal not only on visual analysis but also on SPM. RESULTS: Among the 47 children, visual analysis succeeded detecting at least one hypometabolic area in 87% of the cases (interobserver kappa = 0.81). Regarding SPM analysis, the best compromise between sensitivity and specificity was obtained with a threshold of p less than 0.001 as an extent of more than 40 voxels. There was a significant concordance to detect hypometabolic areas between both SPM analyses [kappa (K) = 0.59; p < 0.005] and between both SPM and visual analyses (K = 0.45; p < 0.005), in symptomatic (K = 0.74; p < 0.005) as in cryptogenic patients (K = 0.26; p < 0.01). The pediatric pseudo-control group dramatically improved specificity (97% vs. 89%; p < 0.0001) by increasing the positive predictive value (86% vs. 65%). Sensitivity remained acceptable although it was not better (79% vs. 87%, p = 0.039). The main impact was to reduce by 41% the number of hypometabolic cortical artifacts detected by SPM, especially in the younger epileptic patients, which is a key point in clinical practice. CONCLUSIONS: This age-matched pseudo-control group is a way to optimize SPM analysis of FDG-PET in children with epilepsy. It might also be considered for other brain pathologies in pediatrics in the future

    Preoperative chemoradiation with paclitaxel-carboplatin or with fluorouracil-oxaliplatin-folinic acid (FOLFOX) for resectable esophageal and junctional cancer: the PROTECT-1402, randomized phase 2 trial.

    BACKGROUND: Often curative treatment for locally advanced resectable esophageal or gastro-esophageal junctional cancer consists of concurrent neoadjuvant radiotherapy and chemotherapy followed by surgery. Currently, one of the most commonly used chemotherapy regimens in this setting is a combination of a fluoropyrimidin and of a platinum analogue. Due to the promising results of the recent CROSS trial, another regimen combining paclitaxel and carboplatin is also widely used by European and American centers. No clinical study has shown the superiority of one treatment over the other. The objective of this Phase II study is to clarify clinical practice by comparing these two chemotherapy treatments. Our aim is to evaluate, in operable esophageal and gastro-esophageal junctional cancer, the complete resection rate and severe postoperative morbidity rate associated with these two neoadjuvant chemotherapeutic regimens (carboplatin-paclitaxel or fluorouracil-oxaliplatin-folinic acid) when each is combined with the radiation regime utilized in the CROSS trial. METHODS/DESIGN: PROTECT is a prospective, randomized, multicenter, open arms, phase II trial. Eligible patients will have a histologically confirmed adenocarcinoma or squamous cell carcinoma and be treated with neoadjuvant radiochemotherapy followed by surgery for stage IIB or stage III resectable esophageal cancer. A total of 106 patients will be randomized to receive either 3 cycles of FOLFOX combined to concurrent radiotherapy (41.4 Grays) or carboplatin and paclitaxel with the same radiation regimen, using a 1:1 allocation ratio. DISCUSSION: This ongoing trial offers the unique opportunity to compare two standards of chemotherapy delivered with a common regimen of preoperative radiation, in the setting of operable locally advanced esophageal or gastro-esophageal junctional tumors.     Ocean-related options for climate change mitigation and adaptation: A machine learning-based evidence map protocol

    BackgroundOcean-related options (OROs) to mitigate and adapt to climate change are receiving increasing attention from practitioners, decision-makers, and researchers. In order to guide future ORO development and implementation, a catalogue of scientific evidence addressing outcomes related to different ORO types is critical. However, until now, such a synthesis has been hindered by the large size of the evidence base. Here, we detail a protocol using a machine learning-based approach to systematically map the extent and distribution of academic evidence relevant to the development, implementation, and outcomes of OROs.MethodTo produce this systematic map, literature searches will be conducted in English across two bibliographic databases using a string of search terms relating to the ocean, climate change, and OROs. A sample of articles from the resulting de-duplicated corpus will be manually screened at the title and abstract level for inclusion or exclusion against a set of predefined eligibility criteria in order to select all relevant literature on marine and coastal socio-ecological systems, the type of ORO and its outcomes. Descriptive metadata on the type and location of intervention, study methodology, and outcomes will be coded from the included articles in the sample. This sample of screening and coding decisions will be used to train a machine learning model that will be used to estimate these labels for all the remaining unseen publications. The results will be reported in a narrative synthesis summarising key trends, knowledge gaps, and knowledge clusters

    Study of Xenon Mobility in the Two Forms of MIL-53(Al) Using Solid-State NMR Spectroscopy

    The Al-based metal–organic framework (MOF) MIL-53­(Al) exhibits a structural transition between a large-pore (<i>lp</i>) form and a narrow-pore (<i>np</i>) one. Such change is induced by temperature, external pressure, or the adsorption of guest molecules. <sup>129</sup>Xe solid-state NMR experiments under static and magic-angle spinning (MAS) conditions have been used to study the <i>lp</i>–<i>np</i> transition in MIL-53­(Al) initially loaded with xenon gas under a pressure of 5 × 10<sup>4</sup> Pa (at room temperature). The conversion of the <i>lp</i> form into the <i>np</i> one when the temperature decreases from 327 to 237 K and the reopening of the pores below 230 K are then observed. Furthermore, <sup>1</sup>H → <sup>129</sup>Xe cross-polarization under MAS (CPMAS) experiments demonstrate the possibility to observe the <i>np</i> phase at <i>T</i> ≤ 230 K, while the <i>lp</i> one is unseen because the xenon residence time is too short for successful cross-polarization transfer. Moreover, even for the <i>np</i> phase at 199 K, the xenon atoms still exhibit significant motion on time scale faster than a few milliseconds. We prove the exchange of Xe atoms between the <i>lp</i> and <i>np</i> forms at room temperature with the two-dimensional (2D) <sup>129</sup>Xe EXchange SpectroscopY (EXSY) NMR method. Using <sup>129</sup>Xe selective inversion recovery (SIR) experiments, the rate for this exchange has been measured at 43 ± 6 s<sup>–1</sup>

    Incidence and predictive biomarkers of Clostridioides difficile infection in hospitalized patients receiving broad-spectrum antibiotics

    Trial enrichment using gut microbiota derived biomarkers by high-risk individuals can improve the feasibility of randomized controlled trials for prevention of Clostridioides difficile infection (CDI). Here, we report in a prospective observational cohort study the incidence of CDI and assess potential clinical characteristics and biomarkers to predict CDI in 1,007 patients ≥ 50 years receiving newly initiated antibiotic treatment with penicillins plus a beta- lactamase inhibitor, 3rd/4th generation cephalosporins, carbapenems, fluoroquinolones or clindamycin from 34 European hospitals. The estimated 90-day cumulative incidences of a first CDI episode is 1.9% (95% CI 1.1-3.0). Carbapenem treatment (Hazard Ratio (95% CI): 5.3 (1.7-16.6)), toxigenic C. difficile rectal carriage (10.3 (3.2-33.1)), high intestinal abundance of Enterococcus spp. relative to Ruminococcus spp. (5.4 (2.1-18.7)), and low Shannon alpha diversity index as determined by 16 S rRNA gene profiling (9.7 (3.2-29.7)), but not nor- malized urinary 3-indoxyl sulfate levels, predicts an increased CDI risk

    Materialist and Post-Materialist Concerns and the Wish for a Strong Leader in 27 Countries

    There is evidence that democracies are under threat around the world while the quest for strong leaders is increasing. Although the causes of these developments are complex and multifaceted, here we focus on one factor: the extent to which citizens express materialist and post-materialist concerns. We explore whether objective higher levels of democracy are differentially associated with materialist and post-materialist concerns and, in turn, whether this is related to the wish for a strong leader. Testing this hypothesis across 27 countries (N = 5,741) demonstrated a direct negative effect of democracies' development on the wish for a strong leader. Further, multi-level mediation analysis showed that the relation between the Democracy Index and the wish for a strong leader was mediated by materialist concerns. This pattern of results suggests that lower levels of democracy are associated with enhanced concerns about basic needs and this is linked to greater support for strong leaders.Peer reviewe

    A whole-genome sequence and transcriptome perspective on HER2-positive breast cancers

    HER2-positive breast cancer has long proven to be a clinically distinct class of breast cancers for which several targeted therapies are now available. However, resistance to the treatment associated with specific gene expressions or mutations has been observed, revealing the underlying diversity of these cancers. Therefore, understanding the full extent of the HER2-positive disease heterogeneity still remains challenging. Here we carry out an in-depth genomic characterization of 64 HER2-positive breast tumour genomes that exhibit four subgroups, based on the expression data, with distinctive genomic features in terms of somatic mutations, copy-number changes or structural variations. The results suggest that, despite being clinically defined by a specific gene amplification, HER2-positive tumours melt into the whole luminal-basal breast cancer spectrum rather than standing apart. The results also lead to a refined ERBB2 amplicon of 106 kb and show that several cases of amplifications are compatible with a breakage-fusion-bridge mechanism

    Minimally invasive versus open distal pancreatectomy: an individual patient data meta-analysis of two randomized controlled trials

    Background: Minimally invasive distal pancreatectomy (MIDP) has been suggested to reduce postoperative outcomes as compared to open distal pancreatectomy (ODP). Recently, the first randomized controlled trials (RCTs) comparing MIDP to ODP were published. This individual patient data meta-analysis compared outcomes after MIDP versus ODP combining data from both RCTs. Methods: A systematic literature search was performed to identify RCTs on MIDP vs. ODP, and individual patient data were harmonized. Primary endpoint was the rate of major (Clavien-Dindo ≥ III) complications. Sensitivity analyses were performed in high-risk subgroups. Results: A total of 166 patients from the LEOPARD and LAPOP RCTs were included. The rate of major complications was 21% after MIDP vs. 35% after ODP (adjusted odds ratio 0.54; p = 0.148). MIDP significantly reduced length of hospital stay (6 vs. 8 days, p = 0.036), and delayed gastric emptying (4% vs. 16%, p = 0.049), as compared to ODP. A trend towards higher rates of postoperative pancreatic fistula was observed after MIDP (36% vs. 28%, p = 0.067). Outcomes were comparable in high-risk subgroups. Conclusion: This individual patient data meta-analysis showed that MIDP, when performed by trained surgeons, may be regarded as the preferred approach for distal pancreatectomy. Outcomes are improved after MIDP as compared to ODP, without obvious downsides in high-risk subgroups
