    O ultrassom transcraniano é útil para o diagnóstico da doença de Parkinson na prática clínica?

    A ultrassonografia transcraniana (UTC) é um exame complementar para diagnóstico de doença de Parkinson (DP). Objetivo Avaliar as características da UTC em pacientes com DP e seus diagnósticos diferenciais e estabelecer a precisão desse exame para o diagnóstico clínico de DP após seguimento. Métodos Avaliou-se retrospectivamente 85 pacientes com suspeita clínica inicial de DP, parkinsonismo atípico (PA) ou tremor essencial (TE), todos submetidos a UTC. Um consenso de dois especialistas determinou o diagnóstico clínico final após revisar os registros médicos das consultas de seguimento. A precisão do UTC foi calculada usando métodos estatísticos Bayesianos. Resultados O achado de hiperecogenicidade da substância negra (> 20 mm2) mostrou alta sensibilidade (93,4%) e especificidade (86,6%). A razão de verossimilhança positiva mostra 6.93 vezes mais chances de diagnosticar DP do que uma condição alternativa, se o achado estiver presente. Conclusões Este estudo demonstra a utilidade prática do UTC na diferenciação de DP de condições clínicas similares quando o diagnóstico clínico é inicialmente pouco claro.Transcranial sonography (TCS) is an emerging ancillary examination for diagnosing Parkinson’s disease (PD). Objective To evaluate TCS features in patients with PD and its mimics, and establish their accuracy in predicting the final clinical diagnosis after follow-up. Methods We retrospectively studied 85 patients with an initial clinical suspicion of PD, atypical parkinsonism or essential tremor, all of whom underwent TCS. Two specialists reviewed the follow-up clinical visit records and determined the final clinical diagnosis. The accuracy analysis of the TCS was determined using Bayesian statistical methods. Results The finding of substantia nigra hyperechogenicity (> 20 mm2) showed high sensitivity (93.4%) and specificity (86.6%). The positive likelihood ratio showed 6.93-fold greater odds for diagnosing PD than an alternative condition when this finding was present. Conclusions This study revealed the practical usefulness of TCS in differentiating PD from its prevalent mimics when the clinical diagnosis was initially unclear

    Human visceral leishmaniasis and relationship with vector and canine control measures

    OBJECTIVE: Estimate the coverage of control measures of visceral leishmaniasis and relate them with the occurrence of human visceral leishmaniasis in endemic urban area. METHODS: Cases of human and canine visceral leishmaniasis were considered as study population and evaluated by a serological survey conducted in Araçatuba, state São Paulo, from 2007 to 2015. The cases of human visceral leishmaniasis were geocoded by the address of the patients and the canine disease by the address of the dogs’ owners. The coverage of serological survey, euthanasia, and insecticide spraying was calculated, as well as the canine seroprevalence and the incidence rates of human visceral leishmaniasis. The relationship between human visceral leishmaniasis and control measures was evaluated, as well as the seroprevalence by comparing maps and by linear regression. The relationship between the canine and the human disease was also evaluated by the Ripley’s K function. RESULTS: The incidence rates of human visceral leishmaniasis showed a period of decline (2007 to 2009) and a period of stability (2010 to 2015), a behavior similar to that of canine seroprevalence. In general, the coverage of control measures was low, and the non-association with the incidence of human visceral leishmaniasis can be a result of the period analyzed and of the small number of analyzed units (sectors of the Superintendence for the Control of Endemic Diseases). The distribution of human cases showed spatial dependence with the distribution of seropositive dogs from 2007 to 2009. CONCLUSIONS: This study reaffirmed the relationship between the occurrence of the disease in humans and dogs, it verified a decrease in the rates of visceral leishmaniasis in Araçatuba over time, even at low coverage of control activities. However, further studies are needed to determine if factors beyond monitoring and control measures are involved in the reduction of incidences.OBJETIVO: Estimar a cobertura das atividades de controle da leishmaniose visceral e relacioná-las com a ocorrência de leishmaniose visceral em humanos em área urbana endêmica. MÉTODOS: Foram considerados como população de estudo os casos de leishmaniose visceral em humanos e em cães avaliados por inquérito sorológico censitário realizado em Araçatuba, SP, de 2007 a 2015. Os casos de leishmaniose visceral em humanos foram geocodificados pelo endereço de residência dos pacientes e, os cães, pelo endereço de residências dos respectivos tutores. Foram calculadas as coberturas do inquérito sorológico, da eutanásia e de borrifação de inseticida, as soroprevalências caninas e as taxas de incidência de leishmaniose visceral em humanos. A relação entre a leishmaniose visceral em humanos e as medidas de controle, bem como a soroprevalência foram avaliadas por comparação de mapas e por meio de regressão linear. A relação entre a doença canina e a humana também foi avaliada por meio da função K de Ripley. RESULTADOS: As taxas de incidência de leishmaniose visceral em humanos apresentaram um período de declínio (2007 a 2009) e um período de estabilidade (2010 a 2015), comportamento semelhante ao das soroprevalências caninas. Em geral, a cobertura das medidas de controle foi baixa e a não associação com a incidência de leishmaniose visceral em humanos pode ser consequência do período analisado e do número pequeno de unidades analisadas (setores da Superintendência de Controle de Endemias). A distribuição dos casos humanos apresentou dependência espacial com a distribuição dos cães soropositivos de 2007 a 2009. CONCLUSÕES: Este trabalho reafirmou a relação entre a ocorrência da doença no homem e no cão, verificou a diminuição das taxas de leishmaniose visceral em humanos e em cães em Araçatuba ao longo do tempo, mesmo em baixa cobertura das atividades de controle. Entretanto, novos estudos são necessários para averiguar se fatores além das atividades de vigilância e controle estariam envolvidos na diminuição das incidências

    The Bom Santo Cave (Lisbon, Portugal): catchment, diet, and patterns of mobility of a Middle Neolithic population

    The study of the Bom Santo Cave (central Portugal), a Neolithic cemetery, indicates a complex social, palaeoeconomic, and population scenario. With isotope, aDNA, and provenance, analyses of raw materials coupled with stylistic variability of material culture items and palaeogeographical data, light is shed on the territory and social organization of a population dated to 3800-3400 cal BC, i.e. the Middle Neolithic. Results indicate an itinerant farming, segmentary society, where exogamic practices were the norm. Its lifeway may be that of the earliest megalithic builders of the region, but further research is needed to correctly evaluate the degree of this community's participation in such a phenomenon

Our aim was to identify independent predictors of success in CTO PCI in order to create an accurate score. Methods: In a single-center observational registry of CTO PCI, demographic and clinical data and anatomical characteristics of coronary lesions were recorded. Linear and logistic regression analysis were used to identify predictors of success. A score to predict success was created and its accuracy was measured by receiver operating curve analysis. Results: A total of 377 interventions were performed (334 patients, age 68±11 years, 75% male). The success rate was 65% per patient and 60% per procedure. Predictors of success in univariate analysis were absence of active smoking (OR 2.02, 95% CI 1.243-3.29; p=0.005), presence of tapered stump (OR 5.2, 95% CI 2.7-10.2; p8 with high probability (95%). Conclusion: In our sample only anatomical characteristics were predictors of success. The creation of a score to predict success, with good accuracy, may enable selection of cases that can be treated by any operator, those in which a dedicated operator will be desirable, and those with an extremely low probability of success, which should be considered individually for conservative management, surgical revascularization or PCI by a team experienced in CTO.info:eu-repo/semantics/publishedVersio

    Participantes de ensayos clínicos en oncología : perfil y aspectos implicados en sus decisiones

    Estudo sobre os participantes de ensaios clínicos na área de oncologia, discutindo o paradoxo entre progresso científico e iniquidade social. Buscou-se conhecer quem são essas pessoas e analisar aspectos envolvidos nas suas decisões, com base em entrevistas e documentos. Houve maior participação feminina. Os participantes tendem a ter poucos anos de estudo formal e baixa renda. A maioria é aposentada e do lar e não tem assistência à saúde privada. Suas decisões giraram em torno da busca pela cura ou melhora, e pela garantia de acesso regular a cuidados integrais de saúde e medicamentos. A assinatura do termo de consentimento livre e esclarecido não garantiu a expressão da autonomia, pois informações essenciais como os objetivos, riscos e cuidados pós-estudo são praticamente desconhecidas. Os participantes da pesquisa tendem a não compreender os objetivos da investigação, ou superestimam os benefícios médicos diretos de sua participação, sem consciência dos riscos envolvidos e do que significa uma pesquisa. Os resultados deveriam incitar ao exercício e ao diálogo mais críticos entre os diferentes atores e instituições envolvidos na área da pesquisa com seres humanos, objetivando promover uma ciência consciente e responsável, que impeça que pessoas sejam colocadas em situação de desigualdade, vulnerabilidade e sofrimento moral.This is a study about the participants of clinical trials in the field of oncology that discusses the paradox between scientific advances and social inequality. We sought to get to know these people and to analyze the aspects involved in their decisions based on interviews and documents. Female participation was more expressive. The participants tend to have low schooling and low income. Most of them are retirees and housewives, and do not get assistance from the private health system. Their decisions revolved around the search for cure or recovery, as well as for the assurance of regular access to comprehensive health care and medicines. Signing the free and informed consent form did not guarantee the expression of autonomy, because crucial information, such as the goals, risks and post-study care are virtually unknown. The research participants tend to not understand the goals of the investigation, or they overestimate the direct medical benefits of their participation, and they tend to be unaware of the risks involved and of what does a research mean. The results should stimulate more critical practices and dialogues among the different players and institutions involved in the field of the research with human subjects, with the goal of promoting a science that is conscious and responsible, and that prevents people from being put in situations of inequality, vulnerability and moral suffering.Estudio sobre los participantes de ensayos clínicos en el área de oncología, discutiendo la paradoja entre avance científico y desigualdad social. Se buscó conocer quiénes son esas personas y analizar los aspectos involucrados en sus decisiones, con base en entrevistas y documentos. Hubo mayor participación femenina. La tendencia es que los participantes tienen pocos años de estudio formal y bajos ingresos. La mayoría es jubilada y ama de casa, y no tiene asistencia médica privada. Sus decisiones giraron en torno a la búsqueda de la cura o una mejora, y el acceso regular garantizado a la atención médica integral y a los medicamentos. La firma del formulario de consentimiento voluntario e informado no constituyó una expresión de autonomía, ya que información esencial como objetivos, riesgos y cuidados posteriores al estudio es prácticamente desconocida. Los participantes de la investigación tienden a no comprender los objetivos de la investigación, o sobreestiman los beneficios médicos directos de su participación, sin ser conscientes de los riesgos implicados y lo que significa una investigación. Los resultados deberían incitar al ejercicio y al diálogo más críticos entre los diferentes actores e instituciones involucrados en el área de la investigación con seres humanos, a fin de promover un entendimiento consciente y responsable, que impida que las personas sean colocadas en situación de desigualdad, vulnerabilidad y sufrimiento moral

    Introduction: Range of motion (ROM) is defined as the angular displacement of a joint. This displacement is necessary for performing functional activities and varies from individual to individual depending on age, gender, physical activity, presence or absence of dysfunction, and degree of muscle strength. WMD assessment is used in physical assessment to identify joint limitations and enable practitioners to quantitatively monitor the effectiveness of the interventions applied. Objective: To analyze hip range of motion associated with body mass index in the elderly. Materials and Method: The study was an experimental cross-sectional research with quantitative approach. Sixty elderly (30 Senior Citizenship Center - CCI and 30 Outdoor Academies - AAL) aged 60 to 70 years old, of both sexes, were evaluated. Results and discussion: The study showed a prevalence of female elderly, who practiced activities independently and who had at least one type of injury. At the ICC there was the presence of a Physical Education professional, which was not found in AAL. The most common diseases between the two groups were Hypertension, Diabetes and Osteoarthritis. In both again, BMI was high and ROM was low, and when correlated with ROM was inversely proportional. Conclusion: It is concluded that the prevalence of overweight elderly, injured and physical activity practitioners without an individualized exercise program can contribute to low ROM.Introdução: A amplitude de movimento (ADM) é definida como o deslocamento angular de uma articulação. Esse deslocamento é necessário para a realização de atividades funcionais e varia de indivíduo para indivíduo dependendo da idade, sexo, prática de atividade física, presença ou ausência de disfunção e o grau de força muscular. A avaliação da ADM é utilizada na avaliação física para identificar limitações articulares e permitir aos profissionais o acompanhamento de modo quantitativo a eficácia das intervenções aplicadas. Objetivo: analisar a amplitude de movimento de quadril associada ao índice de massa corporal em idosos. Matérias e Método:  O estudo tratou-se de uma pesquisa experimental, transversal com abordagem quantitativa. Foram avaliados 60 idosos (30 Centro de Convivência dos Idosos - CCI e 30 das Academias ao Ar Livre - AAL) com idade entre 60 a 70 anos, de ambos os sexos. Resultados e discussão: O estudo apresentou prevalência de idosos do sexo feminino, que praticavam atividades de forma independente e que possuíam pelo menos um tipo de lesão. No CCI havia a presença de um profissional de Educação Física, o que não foi constatado na AAL. As doenças mais comuns entre os dois grupos foram Hipertensão, Diabetes e Osteoartrites. Em ambos novamente, o IMC apresentou-se elevado e a ADM baixa, e quando correlacionado com a ADM mostrou-se inversamente proporcional. Conclusão: Conclui-se que a prevalência de idosos com sobrepeso, lesionados e praticantes de atividade física sem um programa de exercícios individualizados podem contribuir para a ADM baixa.       &nbsp

    MAMMALS IN PORTUGAL : A data set of terrestrial, volant, and marine mammal occurrences in P ortugal

    Mammals are threatened worldwide, with 26% of all species being includedin the IUCN threatened categories. This overall pattern is primarily associatedwith habitat loss or degradation, and human persecution for terrestrial mam-mals, and pollution, open net fishing, climate change, and prey depletion formarine mammals. Mammals play a key role in maintaining ecosystems func-tionality and resilience, and therefore information on their distribution is cru-cial to delineate and support conservation actions. MAMMALS INPORTUGAL is a publicly available data set compiling unpublishedgeoreferenced occurrence records of 92 terrestrial, volant, and marine mam-mals in mainland Portugal and archipelagos of the Azores and Madeira thatincludes 105,026 data entries between 1873 and 2021 (72% of the data occur-ring in 2000 and 2021). The methods used to collect the data were: live obser-vations/captures (43%), sign surveys (35%), camera trapping (16%),bioacoustics surveys (4%) and radiotracking, and inquiries that represent lessthan 1% of the records. The data set includes 13 types of records: (1) burrowsjsoil moundsjtunnel, (2) capture, (3) colony, (4) dead animaljhairjskullsjjaws, (5) genetic confirmation, (6) inquiries, (7) observation of live animal (8),observation in shelters, (9) photo trappingjvideo, (10) predators dietjpelletsjpine cones/nuts, (11) scatjtrackjditch, (12) telemetry and (13) vocalizationjecholocation. The spatial uncertainty of most records ranges between 0 and100 m (76%). Rodentia (n=31,573) has the highest number of records followedby Chiroptera (n=18,857), Carnivora (n=18,594), Lagomorpha (n=17,496),Cetartiodactyla (n=11,568) and Eulipotyphla (n=7008). The data setincludes records of species classified by the IUCN as threatened(e.g.,Oryctolagus cuniculus[n=12,159],Monachus monachus[n=1,512],andLynx pardinus[n=197]). We believe that this data set may stimulate thepublication of other European countries data sets that would certainly contrib-ute to ecology and conservation-related research, and therefore assisting onthe development of more accurate and tailored conservation managementstrategies for each species.
