Midwives' competence : is it affected by working in a rural location?
Introduction: Rising health care costs and the need to consolidate expertise in tertiary services have led to the centralisation of services. In the UK, the result has been that many rural maternity units have become midwife-led. A key consideration is that midwives have the skills to competently and confidently provide maternity services in rural areas, which may be geographically isolated and where the midwife may only see a small number of pregnant women each year. Our objective was to compare the views of midwives in rural and urban settings, regarding their competence and confidence with respect to âcompetenciesâ identified as being those which all professionals should have in order to provide effective and safe care for low-risk women. Method: This was a comparative questionnaire survey involving a stratified sample of remote and rural maternity units and an ad hoc comparison group of three urban maternity units in Scotland. Questionnaires were sent to 82 midwives working in remote and rural areas and 107 midwives working in urban hospitals with midwife-led units. Results: The response rate from midwives in rural settings was considerably higher (85%) than from midwives in the urban areas (60%). Although the proportion of midwives who reported that they were competent was broadly similar in the two groups, there were some significant differences regarding specific competencies. Midwives in the rural group were more likely to report competence for breech delivery (p = 0.001), while more urban midwives reported competence in skills such as intravenous fluid replacement (p <0.001) and initial and discharge examination of the newborn (p <0.001). Both groups reported facing barriers to continuing professional development; however, more of the rural group had attended an educational event within the last month (p <0.001). Lack of time was a greater barrier for urban midwives (p = 0.02), whereas distance to training was greater for rural midwives (p = 0.009). Lack of motivation or interest was significantly higher in urban units (p = 0.006). Conclusion: It is often assumed that midwives in rural areas where there are fewer deliveries, will be less competent and confident in their practice. Tick size and price diffusion
A tick size is the smallest increment of a security price. It is clear that
at the shortest time scale on which individual orders are placed the tick size
has a major role which affects where limit orders can be placed, the bid-ask
spread, etc. This is the realm of market microstructure and there is a vast
literature on the role of tick size on market microstructure. However, tick
size can also affect price properties at longer time scales, and relatively
less is known about the effect of tick size on the statistical properties of
prices. The present paper is divided in two parts. In the first we review the
effect of tick size change on the market microstructure and the diffusion
properties of prices. The second part presents original results obtained by
investigating the tick size changes occurring at the New York Stock Exchange
(NYSE). We show that tick size change has three effects on price diffusion.
First, as already shown in the literature, tick size affects price return
distribution at an aggregate time scale. Second, reducing the tick size
typically leads to an increase of volatility clustering. We give a possible
mechanistic explanation for this effect, but clearly more investigation is
needed to understand the origin of this relation. Third, we explicitly show
that the ability of the subordination hypothesis in explaining fat tails of
returns and volatility clustering is strongly dependent on tick size. While for
large tick sizes the subordination hypothesis has significant explanatory
power, for small tick sizes we show that subordination is not the main driver

The effect of the SAFE or SORRY? programme on patient safety knowledge of nurses in hospitals and nursing homes: a cluster randomised trial
Background: patients in hospitals and nursing homes are at risk for the development of often preventable adverse events. Guidelines for the prevention of many types of adverse events are available, however compliance with these guidelines appears to be lacking. As a result many patients do not receive appropriate care. We developed a patient safety program that allows organisations to implement multiple guidelines simultaneously and therefore facilitates guideline use to improve patient safety. This program was developed for three frequently occurring nursing care related adverse events: pressure ulcers, urinary tract infections and falls. For the implementation of this program we developed educational activities for nurses as a main implementation strategy.Objectives: the aim of this study is to describe the effect of interactive and tailored education on the knowledge levels of nurses.Design: a cluster randomised trial was conducted between September 2006 and July 2008.Settings: ten hospital wards and ten nursing home wards participated in this study. Prior to baseline, randomisation of the wards to an intervention or control group was stratified for centre and type of ward.Participants: all nurses from participating wards.Methods: a knowledge test measured nursesâ knowledge on the prevention of pressure ulcers, urinary tract infections and falls, during baseline en follow-up. The results were analysed for hospitals and nursing homes separately.Results: after correction for baseline, the mean difference between the intervention and the control group on hospital nursesâ knowledge on the prevention of the three adverse events was 0.19 points on a zero to ten scale (95% CI: ?0.03 to 0.42), in favour of the intervention group. There was a statistically significant effect on knowledge of pressure ulcers, with an improved mean mark of 0.45 points (95% CI: 0.10â0.81). For the other two topics there was no statistically significant effect. Nursing home nursesâ knowledge did neither improve (0 points, CI: ?0.35 to 0.35) overall, nor for the separate subjects.Conclusion: the educational intervention improved hospital nursesâ knowledge on the prevention of pressure ulcers only. Graviton Mass from Close White Dwarf Binaries Detectable with LISA
The arrival times of gravitational waves and optical light from orbiting
binaries provide a mechanism to understand the propagation speed of gravity
when compared to that of light or electromagnetic radiation. This is achieved
with a measurement of any offset between optically derived orbital phase
related to that derived from gravitational wave data, at a specified location
of one binary component with respect to the other. Using a sample of close
white dwarf binaries (CWDBs) detectable with the Laser Interferometer Space
Antenna (LISA) and optical light curve data related to binary eclipses from
meter-class telescopes for the same sample, we determine the accuracy to which
orbital phase differences can be extracted. We consider an application of these
measurements involving a variation to the speed of gravity, when compared to
the speed of light, due to a massive graviton. For a subsample of 400
CWDBs with high signal-to-noise gravitational wave and optical data with
magnitudes brighter than 25, the combined upper limit on the graviton mass is
at the level of eV. This limit is two orders of
magnitude better than the present limit derived by Yukawa-correction arguments

Carbon nanotubes for coherent spintronic devices
Carbon nanotubes bridge the molecular and crystalline quantum worlds, and
their extraordinary electronic, mechanical and optical properties have
attracted enormous attention from a broad scientific community. We review the
basic principles of fabricating spin-electronic devices based on individual,
electrically-gated carbon nanotubes, and present experimental efforts to
understand their electronic and nuclear spin degrees of freedom, which in the

CSF1R inhibitor JNJ-40346527 attenuates microglial proliferation and neurodegeneration in P301S mice
Neuroinflammation and microglial activation are significant processes in Alzheimer's disease pathology. Recent genome-wide association studies have highlighted multiple immune-related genes in association with Alzheimer's disease, and experimental data have demonstrated microglial proliferation as a significant component of the neuropathology. In this study, we tested the efficacy of the selective CSF1R inhibitor JNJ-40346527 (JNJ-527) in the P301S mouse tauopathy model. We first demonstrated the anti-proliferative effects of JNJ-527 on microglia in the ME7 prion model, and its impact on the inflammatory profile, and provided potential CNS biomarkers for clinical investigation with the compound, including pharmacokinetic/pharmacodynamics and efficacy assessment by TSPO autoradiography and CSF proteomics. Then, we showed for the first time that blockade of microglial proliferation and modification of microglial phenotype leads to an attenuation of tau-induced neurodegeneration and results in functional improvement in P301S mice. Overall, this work strongly supports the potential for inhibition of CSF1R as a target for the treatment of Alzheimer's disease and other tau-mediated neurodegenerative diseases
Inflammatory biomarkers in Alzheimer's disease plasma
Introduction: Plasma biomarkers for Alzheimer's disease (AD) diagnosis/stratification are a \u201cHoly Grail\u201d of AD research and intensively sought; however, there are no well-established plasma markers. Methods: A hypothesis-led plasma biomarker search was conducted in the context of international multicenter studies. The discovery phase measured 53 inflammatory proteins in elderly control (CTL; 259), mild cognitive impairment (MCI; 199), and AD (262) subjects from AddNeuroMed. Results: Ten analytes showed significant intergroup differences. Logistic regression identified five (FB, FH, sCR1, MCP-1, eotaxin-1) that, age/APO\u3b54 adjusted, optimally differentiated AD and CTL (AUC: 0.79), and three (sCR1, MCP-1, eotaxin-1) that optimally differentiated AD and MCI (AUC: 0.74). These models replicated in an independent cohort (EMIF; AUC 0.81 and 0.67). Two analytes (FB, FH) plus age predicted MCI progression to AD (AUC: 0.71). Discussion: Plasma markers of inflammation and complement dysregulation support diagnosis and outcome prediction in AD and MCI. Further replication is needed before clinical translation
