    Enacting communication cycles after prediabetes: Mothers’ stories of diabetes risk and seeking support

    Background: Diabetes is a family systems experience of multiple intergenerational relationships. Communicating about diabetes as a familial risk helps individuals perceive greater control over preventing diabetes. Aim: The present study investigates how mothers disclose their prediabetes to their family and the precipitating family communication, with the goal to interrupt the multigenerational legacy of diabetes. Method: Data were collected from a Family Medicine outpatient clinic. Semi-structured interviews were conducted to illicit information about mother-child communication about diabetes. Sort and Sift, Think and Shift analysis provided rich descriptions of mothers’ experiences. Results: The sample consisted of 9 women with prediabetes, aged 42-70, who had at least one child. Eight participants described at least 2 generations of family members with a diabetes-related diagnosis (prediabetes, type 2 diabetes mellitus, and gestational diabetes). First, we present two contextual factors that preexisted the prediabetes diagnosis, which influenced the disclosure decision: general health communication approach and personal evaluation of diabetes. Second, we present the communication cycles that supported self-management. Discussion: Continual communication cycles with family and clinicians can support mothers with prediabetes to enact self-management behaviors and delay the onset of type 2 diabetes mellitus. Conclusion: Mothers need family members to respond favorably to complete a positive communication cycle that can generate continuous emotional, social, and instrumental support

    The Effects of Therapeutic Riding on Social Skills and Sensory Functioning in Children with Autism

    Purpose and Significance: Therapeutic riding (TR) is equine-assisted riding lessons that are adapted for people with a wide variety of impairments and is often used as an adjunct to traditional therapies. Previous research has shown that TR helped to improve sensory functioning, cognition and social skills in children with Autism Spectrum Disorder (ASD) but sessions have typically been for 10-12 weeks. The purpose of this study was to investigate if social skills and sensory functioning improved after only 8 weeks of TR. Subjects: Children were recruited from a local TR center. Participants included seven children (5M, 2F) with a diagnosis of ASD. The mean age of participants was 10.6 years (6-19 years). Methods: Participants were recruited via letters and emails distributed through a local TR Center. The Social Responsiveness Scale (SRS) and Sensory Profile Survey (SPS) were administered and completed by the parents of the participants. Surveys were completed prior to the start and at the end of the 8-week TR session. Each weekly TR session lasted for 45 minutes and was led by certified TR instructors. The sessions involved mounting/dismounting the horse, riding skills, and individual or group games. Descriptive statistics were calculated for all measures. Pre- and post-measures were analyzed using a Wilcoxon signed-rank test. Results: At a significance level of alpha \u3c0.05, a significant difference was found between pre and post total scores on the SRS (p=0.028). No significant differences were found between pre and post total scores on the SPS or pre and post scores of the selected subcategories of the SPS and SRS. Conclusion: Eight weeks of TR may help to improve social skills in children with ASD. However, more research is needed with a larger sample size and long-term follow-up in order to develop stronger evidence. Clinical Merit: This study indicates that TR may benefit children with ASD, by improving social skills, and may be used as an additional therapeutic intervention

    How do self‐advocates use community development to change attitudes to disability?

    Background Negative attitudes remain a major barrier to the equality of people with disability, especially when coupled with the lack of autonomy imposed on many people. This paper analyses how disability self‐advocacy groups seek to change community attitudes and work towards systemic change by mobilising knowledge from their lived experience. Methods The paper applies a cycle of praxis community development approach (a cycle of experience, learning and reflection, synthesis and planning, and implementation and review) to conceptualise and analyse their activities. The methods were a desktop document search, focus groups and reflective analysis with members of two self‐advocacy groups. Findings A synthesised data analysis found that applying the four‐part community development framework was useful to understand the practice and the purpose of work by self‐advocacy groups to change attitudes. The analysis also demonstrates the benefits for advocates and codesigned activities to intentionally apply the cycle of praxis model to guide their future efforts to change attitudes. Conclusions The research provides evidence that self‐advocacy groups achieve sustained impacts on attitudes in the community, beyond the direct benefit to their members. Government investment in self‐advocacy has potential to leverage wider system change in attitudes to achieve policy goals for the rights of people with disability. Methodologically, the research also has implications for the benefit of inclusive roles in reflective analysis to understand the lived experience of how practices contribute to system change. The design is an opportunity for inclusive researchers to intentionally incorporate reflective analysis into research processes

    Why Don't We Ask? A Complementary Method for Assessing the Status of Great Apes

    Species conservation is difficult. Threats to species are typically high and immediate. Effective solutions for counteracting these threats, however, require synthesis of high quality evidence, appropriately targeted activities, typically costly implementation, and rapid re-evaluation and adaptation. Conservation management can be ineffective if there is insufficient understanding of the complex ecological, political, socio-cultural, and economic factors that underlie conservation threats. When information about these factors is incomplete, conservation managers may be unaware of the most urgent threats or unable to envision all consequences of potential management strategies. Conservation research aims to address the gap between what is known and what knowledge is needed for effective conservation. Such research, however, generally addresses a subset of the factors that underlie conservation threats, producing a limited, simplistic, and often biased view of complex, real world situations. A combination of approaches is required to provide the complete picture necessary to engage in effective conservation. Orangutan conservation (Pongo spp.) offers an example: standard conservation assessments employ survey methods that focus on ecological variables, but do not usually address the socio-cultural factors that underlie threats. Here, we evaluate a complementary survey method based on interviews of nearly 7,000 people in 687 villages in Kalimantan, Indonesia. We address areas of potential methodological weakness in such surveys, including sampling and questionnaire design, respondent biases, statistical analyses, and sensitivity of resultant inferences. We show that interview-based surveys can provide cost-effective and statistically robust methods to better understand poorly known populations of species that are relatively easily identified by local people. Such surveys provide reasonably reliable estimates of relative presence and relative encounter rates of such species, as well as quantifying the main factors that threaten them. We recommend more extensive use of carefully designed and implemented interview surveys, in conjunction with more traditional field methods

    Restrained Th17 response and myeloid cell infiltration into the central nervous system by human decidua-derived mesenchymal stem cells during experimental autoimmune encephalomyelitis

    Background: Multiple sclerosis is a widespread inflammatory demyelinating disease. Several immunomodulatory therapies are available, including interferon-ÎČ, glatiramer acetate, natalizumab, fingolimod, and mitoxantrone. Although useful to delay disease progression, they do not provide a definitive cure and are associated with some undesirable side-effects. Accordingly, the search for new therapeutic methods constitutes an active investigation field. The use of mesenchymal stem cells (MSCs) to modify the disease course is currently the subject of intense interest. Decidua-derived MSCs (DMSCs) are a cell population obtained from human placental extraembryonic membranes able to differentiate into the three germ layers. This study explores the therapeutic potential of DMSCs. Methods: We used the experimental autoimmune encephalomyelitis (EAE) animal model to evaluate the effect of DMSCs on clinical signs of the disease and on the presence of inflammatory infiltrates in the central nervous system. We also compared the inflammatory profile of spleen T cells from DMSC-treated mice with that of EAE control animals, and the influence of DMSCs on the in vitro definition of the Th17 phenotype. Furthermore, we analyzed the effects on the presence of some critical cell types in central nervous system infiltrates. Results: Preventive intraperitoneal injection of DMSCs resulted in a significant delay of external signs of EAE. In addition, treatment of animals already presenting with moderate symptoms resulted in mild EAE with reduced disease scores. Besides decreased inflammatory infiltration, diminished percentages of CD4+IL17+, CD11b+Ly6G+ and CD11b+Ly6C+ cells were found in infiltrates of treated animals. Early immune response was mitigated, with spleen cells of DMSC-treated mice displaying low proliferative response to antigen, decreased production of interleukin (IL)-17, and increased production of the anti-inflammatory cytokines IL-4 and IL-10. Moreover, lower RORÎłT and higher GATA-3 expression levels were detected in DMSC-treated mice. DMSCs also showed a detrimental influence on the in vitro definition of the Th17 phenotype. Conclusions: DMSCs modulated the clinical course of EAE, modified the frequency and cell composition of the central nervous system infiltrates during the disease, and mediated an impairment of Th17 phenotype establishment in favor of the Th2 subtype.     As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

    Multimessenger Search for Sources of Gravitational Waves and High-Energy Neutrinos: Results for Initial LIGO-Virgo and IceCube

    We report the results of a multimessenger search for coincident signals from the LIGO and Virgo gravitational-wave observatories and the partially completed IceCube high-energy neutrino detector, including periods of joint operation between 2007-2010. These include parts of the 2005-2007 run and the 2009-2010 run for LIGO-Virgo, and IceCube's observation periods with 22, 59 and 79 strings. We find no significant coincident events, and use the search results to derive upper limits on the rate of joint sources for a range of source emission parameters. For the optimistic assumption of gravitational-wave emission energy of 10−210^{-2}\,M⊙_\odotc2^2 at ∌150\sim 150\,Hz with ∌60\sim 60\,ms duration, and high-energy neutrino emission of 105110^{51}\,erg comparable to the isotropic gamma-ray energy of gamma-ray bursts, we limit the source rate below 1.6×10−21.6 \times 10^{-2}\,Mpc−3^{-3}yr−1^{-1}. We also examine how combining information from gravitational waves and neutrinos will aid discovery in the advanced gravitational-wave detector era
