    Centimeter to decimeter hollow concretions and voids in Gale Crater sediments, Mars

    Voids and hollow spheroids between ∼1 and 23 cm in diameter occur at several locations along the traverse of the Curiosity rover in Gale crater, Mars. These hollow spherical features are significantly different from anything observed in previous landed missions. The voids appear in dark-toned, rough-textured outcrops, most notably at Point Lake (sols 302-305) and Twin Cairns Island (sol 343). Point Lake displays both voids and cemented spheroids in close proximity; other locations show one or the other form. The spheroids have 1-4 mm thick walls and appear relatively dark-toned in all cases, some with a reddish hue. Only one hollow spheroid (Winnipesaukee, sol 653) was analyzed for composition, appearing mafic (Fe-rich), in contrast to the relatively felsic host rock. The interior surface of the spheroid appears to have a similar composition to the exterior with the possible exceptions of being more hydrated and slightly depleted in Fe and K. Origins of the spheroids as Martian tektites or volcanic bombs appear unlikely due to their hollow and relatively fragile nature and the absence of in-place clearly igneous rocks. A more likely explanation to both the voids and the hollow spheroids is reaction of reduced iron with oxidizing groundwater followed by some re-precipitation as cemented rind concretions at a chemical reaction front. Although some terrestrial concretion analogs are produced from a precursor siderite or pyrite, diagenetic minerals could also be direct precipitates for other terrestrial concretions. The Gale sediments differ from terrestrial sandstones in their high initial iron content, perhaps facilitating a higher occurrence of such diagenetic reactions

    RepeatsDB in 2021: Improved data and extended classification for protein tandem repeat structures

    The RepeatsDB database (URL: https://repeatsdb.org/) provides annotations and classification for protein tandem repeat structures from the Protein Data Bank (PDB). Protein tandem repeats are ubiquitous in all branches of the tree of life. The accumulation of solved repeat structures provides new possibilities for classification and detection, but also increasing the need for annotation. Here we present RepeatsDB 3.0, which addresses these challenges and presents an extended classification scheme. The major conceptual change compared to the previous version is the hierarchical classification combining top levels based solely on structural similarity (Class > Topology > Fold) with two new levels (Clan > Family) requiring sequence similarity and describing repeat motifs in collaboration with Pfam. Data growth has been addressed with improved mechanisms for browsing the classification hierarchy. A new UniProt-centric view unifies the increasingly frequent annotation of structures from identical or similar sequences. This update of RepeatsDB aligns with our commitment to develop a resource that extracts, organizes and distributes specialized information on tandem repeat protein structures.Fil: Paladin, Lisanna.     Peri-operative red blood cell transfusion in neonates and infants: NEonate and Children audiT of Anaesthesia pRactice IN Europe: A prospective European multicentre observational study

    BACKGROUND: Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12 g dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (≥week 3) onwards. OBJECTIVE: To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN: A multicentre observational study. SETTING: The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS: The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES: The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS: Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g dl-1 in week 2 and 8.0 [7.3 to 9.0] g dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS: Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies.     COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Calidad de vida y daño cerebral sobrevenido infantil secundario a cirugía cardíaca extracorpórea

    Las cardiopatías congénitas (CC) suponen la malformación congénita más frecuente, con una incidencia aproximada de 1 por cada 600 recién nacidos vivos. La mejora tanto en las técnicas quirúrgicas y de la sedo/anestesia, como en el diagnóstico precoz y manejo de las complicaciones postquirúrgicas han contribuido a mejorar la supervivencia en los enfermos con CC complejas. Sin embargo, se estima que la presencia de daño cerebral adquirido está presente en torno a un tercio de los niños intervenidos mediante cirugía cardíaca. Los objetivos de este estudio son determinar las secuelas neuropsicológicas a largo plazo, estudiar la calidad de vida de los pacientes diagnosticados de CC y sometidos a cirugía extracorpórea, describir los factores sociodemográficos, clínicos, quirúrgicos, medioambientales, secuelas a largo plazo evidenciadas, e intentar establecer factores de riesgo para desarrollar estas secuelas. Hemos estudiado un total de 27 pacientes y 54 controles, hemos recogido variables relacionadas a la cardiopatía (diagnóstico, edad del mismo, duración cirugía extracorpórea y número realizado, complicaciones postquirúrgicas), variables sociodemográficas de casos y controles (edad, sexo, nivel de renta mensual familiar, nivel de estudios y situación laboral de los padres, percepción de salud), medioambientales de casos y controles (percepción de contaminación, ambiental, convivencia con fumadores, ejercicio físico), calidad de vida percibida por progenitores y pacientes de casos y controles, nivel de estrés parental, calidad de vida de los padres y secuelas secundarias, valoración cognitiva, integración visuoespacial y conductual. Tras la realización del estudio concluimos, que los pacientes afectos de CC presentan un retraso en el inicio de lenguaje y dificultades en el aprendizaje mayores que la población control. Se han identificado mayores alteraciones conductuales que en población general. El nivel socio económico de las familias es más bajo que en los controles y su exposición a agentes medioambientales nocivos mayor. En las CC más complejas identificamos con más frecuencia la presencia de DCS. La calidad de vida se ve afectada por la actividad física, el nivel de estudios de los padres y la gravedad de la cardiopatía. En relación a población control, la esfera más afectad es la escolar.Congenital heart disease (CHD) is the most common congenital malformation, with an approximate incidence of 1 in every 600 live newborns. The improvement in both surgical and sedation / anesthesia techniques, as well as in the early diagnosis and management of post-surgical complications have contributed to improving survival in patients with complex CHD. However, it is estimated that the presence of acquired brain damage is present in around one third of children who undergo cardiac surgery. The objectives of this study are to determine the long-term neuropsychological sequelae, study the quality of life of patients diagnosed with CHD and undergoing extracorporeal surgery, describe the sociodemographic, clinical, surgical, environmental factors, evidenced long-term sequelae, and try establish risk factors for developing these sequelae. We have studied a total of 27 patients and 54 controls, we have collected variables related to heart disease (diagnosis, age of the same, duration of extracorporeal surgery and number performed, post-surgical complications), sociodemographic variables of cases and controls (age, sex, income level monthly family, educational level and employment situation of parents, perception of health), environmental of cases and controls (perception of pollution, environmental, living with smokers, physical exercise), quality of life perceived by parents and patients of cases and controls , parental stress level, parental quality of life and secondary sequelae, cognitive assessment, visuospatial and behavioral integration. After conducting the study, we concluded that patients with CHD have a delay in the onset of language and learning difficulties greater than the control population. Greater behavioral disturbances have been identified than in the general population. The socio-economic level of the families is lower than in the controls and their exposure to harmful environmental agents is higher. In more complex CC, we more frequently identify the presence of DCS. The quality of life is affected by physical activity, the educational level of the parents and the severity of the heart disease.     Evolutionary trajectories are often biased by developmental and historical factors. However, environmental factors can also impose constraints on the evolutionary trajectories of organisms leading to convergence of morphology in similar ecological contexts. The physical properties of water impose strong constraints on aquatic feeding animals by generating pressure waves that can alert prey and potentially push them away from the mouth. These hydrodynamic constraints have resulted in the independent evolution of suction feeding in most groups of secondarily aquatic tetrapods. Despite the fact that snakes cannot use suction they have invaded the aquatic milieu many times independently. Here we test whether the aquatic environment has constrained head shape evolution in snakes and whether shape converges on that predicted by biomechanical models. To do so, we used 3D geometric morphometrics and comparative, phylogenetically informed analyses on a large sample of aquatic snake species. Our results show that aquatic snakes partially conform to our predictions and have a narrower anterior part of the head and dorsally positioned eyes and nostrils. This morphology is observed irrespective of the phylogenetic relationships among species suggesting that the aquatic environment does indeed drive the evolution of head shape in snakes, thus biasing the evolutionary trajectory of this group of animals
