    Scientific Objectives of Einstein Telescope

    The advanced interferometer network will herald a new era in observational astronomy. There is a very strong science case to go beyond the advanced detector network and build detectors that operate in a frequency range from 1 Hz-10 kHz, with sensitivity a factor ten better in amplitude. Such detectors will be able to probe a range of topics in nuclear physics, astronomy, cosmology and fundamental physics, providing insights into many unsolved problems in these areas.Comment: 18 pages, 4 figures, Plenary talk given at Amaldi Meeting, July 201

    Three-row versus two-row circular staplers for left-sided colorectal anastomosis: a propensity score-matched analysis of the iCral 2 and 3 prospective cohorts

    Background: Since most anastomoses after left-sided colorectal resections are performed with a circular stapler, any technological change in stapling devices may influence the incidence of anastomotic adverse events. The aim of the present study was to analyze the effect of a three-row circular stapler on anastomotic leakage and related morbidity after left-sided colorectal resections. Materials and methods: A circular stapled anastomosis was performed in 4255 (50.9%) out of 8359 patients enrolled in two prospective multicenter studies in Italy, and, after exclusion criteria to reduce heterogeneity, 2799 (65.8%) cases were retrospectively analyzed through a 1:1 propensity score-matching model including 20 covariates relative to patient characteristics, to surgery and to perioperative management. Two well-balanced groups of 425 patients each were obtained: group (A) – true population of interest, anastomosis performed with a three-row circular stapler; group (B) – control population, anastomosis performed with a two-row circular stapler. The target of inferences was the average treatment effect in the treated (ATT). The primary endpoints were overall and major anastomotic leakage and overall anastomotic bleeding; the secondary endpoints were overall and major morbidity and mortality rates. The results of multiple logistic regression analyses for the outcomes, including the 20 covariates selected for matching, were presented as odds ratios (OR) and 95% confidence intervals (95% CI). Results: Group A versus group B showed a significantly lower risk of overall anastomotic leakage (2.1 vs. 6.1%; OR 0.33; 95% CI 0.15–0.73; P = 0.006), major anastomotic leakage (2.1 vs. 5.2%; OR 0.39; 95% CI 0.17–0.87; P = 0.022), and major morbidity (3.5 vs. 6.6% events; OR 0.47; 95% CI 0.24–0.91; P = 0.026). Conclusion: The use of three-row circular staplers independently reduced the risk of anastomotic leakage and related morbidity after left-sided colorectal resection.     Mother-to-child transmission of human immunodeficiency virus in Italy : temporal trends and determinants of infection

    In order to analyse temporal trends in vertical transmission rates of human immunodeficiency virus (HIV) and determinant of congenital HIV infection in Italy, we have considered data from a network of hospitals co-operating in the Italian Collaborative Study on HIV infection in pregnancy, conducted between 1988 and 1995. A total of 1040 women entered the study. The HIV-1 status of the babies was known in 848 cases (81.5%). Transmission rates were highest in the period 1988\u20131991, then tended to decrease and in 1995 the rate was 9.7 per 100 children (this finding, however, was based on only six infected children and the trend was not statistically significant). Considering the overall series, the risk of vertical HIV transmission was higher in women with low CD4 count in pregnancy [odds ratio (OR) <400 versus \u2a7e400 1.8, 95% confidence interval (CI) 1.1\u20132.9]. In comparison with vaginal delivery the risk of transmission was 0.3 (95% CI 0.1\u20130.5) and 0.6 (95% CI 0.3\u20131.2) respectively for elective and emergency delivery. In comparison with women who delivered at term (\u2a7e37 gestation weeks) the OR of HIV infection of the babies for the whole series was 2.2 (95% CI 1.3\u20133.6) in women who delivered preterm. Similar findings emerged when the analysis was conducted considering, separately, subjects observed in the period 1988\u20131991 and 1992\u20131995. The frequency of Caesarean section increased from 26.5% of deliveries in 1988\u20131991 to 36.2% in 1992\u20131995. Consequently, most temporal differences disappeared after standardization for mode of delivery, but the rate in 1995 was still lower than in 1988\u20131994

    A MODEST review

    We present an account of the state of the art in the fields explored by the research community invested in 'Modeling and Observing DEnse STellar systems'. For this purpose, we take as a basis the activities of the MODEST-17 conference, which was held at Charles University, Prague, in September 2017. Reviewed topics include recent advances in fundamental stellar dynamics, numerical methods for the solution of the gravitational N-body problem, formation and evolution of young and old star clusters and galactic nuclei, their elusive stellar populations, planetary systems, and exotic compact objects, with timely attention to black holes of different classes of mass and their role as sources of gravitational waves. Such a breadth of topics reflects the growing role played by collisional stellar dynamics in numerous areas of modern astrophysics. Indeed, in the next decade, many revolutionary instruments will enable the derivation of positions and velocities of individual stars in the Milky Way and its satellites and will detect signals from a range of astrophysical sources in different portions of the electromagnetic and gravitational spectrum, with an unprecedented sensitivity. On the one hand, this wealth of data will allow us to address a number of long-standing open questions in star cluster studies; on the other hand, many unexpected properties of these systems will come to light, stimulating further progress of our understanding of their formation and evolution.Comment: 42 pages; accepted for publication in 'Computational Astrophysics and Cosmology'.     Evaluation of appendicitis risk prediction models in adults with suspected appendicitis

    Background Appendicitis is the most common general surgical emergency worldwide, but its diagnosis remains challenging. The aim of this study was to determine whether existing risk prediction models can reliably identify patients presenting to hospital in the UK with acute right iliac fossa (RIF) pain who are at low risk of appendicitis. Methods A systematic search was completed to identify all existing appendicitis risk prediction models. Models were validated using UK data from an international prospective cohort study that captured consecutive patients aged 16–45 years presenting to hospital with acute RIF in March to June 2017. The main outcome was best achievable model specificity (proportion of patients who did not have appendicitis correctly classified as low risk) whilst maintaining a failure rate below 5 per cent (proportion of patients identified as low risk who actually had appendicitis). Results Some 5345 patients across 154 UK hospitals were identified, of which two‐thirds (3613 of 5345, 67·6 per cent) were women. Women were more than twice as likely to undergo surgery with removal of a histologically normal appendix (272 of 964, 28·2 per cent) than men (120 of 993, 12·1 per cent) (relative risk 2·33, 95 per cent c.i. 1·92 to 2·84; P < 0·001). Of 15 validated risk prediction models, the Adult Appendicitis Score performed best (cut‐off score 8 or less, specificity 63·1 per cent, failure rate 3·7 per cent). The Appendicitis Inflammatory Response Score performed best for men (cut‐off score 2 or less, specificity 24·7 per cent, failure rate 2·4 per cent). Conclusion Women in the UK had a disproportionate risk of admission without surgical intervention and had high rates of normal appendicectomy.     Dynamics of intermediate mass black holes in globular clusters: Wander radius and anisotropy profiles

    Context. We recently introduced a new method for simulating collisional gravitational N-body systems with approximately linear time scaling with N. Our method is based on the multi-particle collision (MPC) scheme, previously applied in fluid dynamics and plasma physics. We were able to simulate globular clusters with a realistic number of stellar particles (at least up to several times 106) on a standard workstation. Aims. We simulated clusters hosting an intermediate mass black hole (IMBH), probing a broad range of BH-cluster and BH-average-star mass ratios, unrestricted by the computational constraints that affect direct N-body codes. Methods. We set up a grid of hybrid particle-in-cell-MPC N-body simulations using our implementation of the MPC method, MPCDSS. We used either single mass models or models with a Salpeter mass function (a single power law with an exponent of -2.35), with the IMBH initially sitting at the centre. The force exerted by and on the IMBH was evaluated with a direct sum scheme with or without softening. For all simulations we measured the evolution of the Lagrangian radii and core density and velocity dispersion over time. In addition, we also measured the evolution of the velocity anisotropy profiles. Results. We find that models with an IMBH undergo core collapse at earlier times, the larger the IMBH mass the shallower they are, with an approximately constant central density at core collapse. The presence of an IMBH tends to lower the central velocity dispersion. These results hold independently of the mass function of the model. For the models with Salpeter MF, we observed that equipartition of kinetic energies is never achieved, even long after core collapse. Orbital anisotropy at large radii appears to be driven by energetic escapers on radial orbits, triggered by strong collisions with the IMBH in the core. We measured the wander radius, that is the distance of the IMBH from the centre of mass of the parent system over time, finding that its distribution has positive kurtosis. Conclusions. Among the results we obtained, which mostly confirm or extend previously known trends that had been established over the range of parameters accessible to direct N-body simulations, we underline that the leptokurtic nature of the IMBH wander radius distribution might lead to IMBHs presenting as off-centre more frequently than expected, with implications on observational IMBH detection

    Relaxation of N-body systems with additive r <sup>-α</sup> interparticle forces

    In Newtonian gravity the final states of cold dissipationless collapses are characterized by several structural and dynamical properties remarkably similar to those of observed elliptical galaxies. Are these properties a peculiarity of the Newtonian force or a more general feature of long-range forces? We study this problem by means of N-body simulations of dissipationless collapse of systems of particles interacting via additive r-\u3b1 forces. We find that most of the results holding in Newtonian gravity are also valid for \u3b1 60 2. In particular, the end-products are triaxial and never flatter than an E7 system, their surface density profiles are well described by the S\ue9rsic law, the global density slope-anisotropy inequality is obeyed, the differential energy distribution is an exponential over a large range of energies (for \u3b1 65 1), and the pseudo-phase-space density is a power law of radius. In addition, we show that the process of virialization takes longer (in units of the system's dynamical time) for decreasing values of \u3b1, and becomes infinite for \u3b1 = -1 (the harmonic oscillator). This is in agreement with the results of deep Modified Newtonian Dynamics collapses (qualitatively corresponding to \u3b1 = 1) and it is due to the fact that the force becomes more and more similar to the \u3b1 = -1 case, where, as well known, no relaxation can happen and the system oscillates forever
