Synchronisation of sedimentary records using tephra : a postglacial tephrochronological model for the Chilean Lake District
Well-characterised tephra horizons deposited in various sedimentary environments provide a means of synchronising sedimentary archives. The use of tephra as a chronological tool is however still widely underutilised in southern Chile and Argentina. In this study we develop a postglacial tephrochronological model for the Chilean Lake District (ca. 38 to 42 degrees S) by integrating terrestrial and lacustrine records. Tephra deposits preserved in lake sediments record discrete events even if they do not correspond to primary fallout. By combining terrestrial with lacustrine records we obtain the most complete tephrostratigraphic record for the area to date. We present glass geochemical and chronological data for key marker horizons that may be used to synchronise sedimentary archives used for palaeoenvironmental, palaeoclimatological and palaeoseismological purposes. Most volcanoes in the studied segment of the Southern Volcanic Zone, between Llaima and Calbuco, have produced at least one regional marker deposit resulting from a large explosive eruption (magnitude >= 4), some of which now have a significantly improved age estimate (e.g., the 10.5 ka Llaima Pumice eruption from Llaima volcano). Others, including several units from Puyehue-Cordon Caulle, are newly described here. We also find tephra related to the Cha1 eruption from Chaiten volcano in lake sediments up to 400 km north from source. Several clear marker horizons are now identified that should help refine age model reconstructions for various sedimentary archives. Our chronological model suggests three distinct phases of eruptive activity impacting the area, with an early-to-mid-Holocene period of relative quiescence. Extending our tephrochronological framework further south into Patagonia will allow a more detailed evaluation of the controls on the occurrence and magnitude of explosive eruptions throughout the postglacial
THINGS about MOND
We present the analysis of 12 high-resolution galactic rotation curves from
The HI Nearby Galaxy Survey (THINGS) in the context of modified Newtonian
dynamics (MOND). These rotation curves were selected to be the most reliable
for mass modelling, and they are the highest quality rotation curves currently
available for a sample of galaxies spanning a wide range of luminosities. We
fit the rotation curves with the "simple" and "standard" interpolating
functions of MOND, and we find that the "simple" function yields better
results. We also redetermine the value of a0, and find a median value very
close to the one determined in previous studies, a0 = (1.22 +- 0.33) x 10^{-8}
cm/s^2. Leaving the distance as a free parameter within the uncertainty of its
best independently determined value leads to excellent quality fits for 75% of
the sample. Among the three exceptions, two are also known to give relatively
poor fits also in Newtonian dynamics plus dark matter. The remaining case (NGC
3198), presents some tension between the observations and the MOND fit, which
might however be explained by the presence of non-circular motions, by a small
distance, or by a value of a0 at the lower end of our best-fit interval, 0.9 x
10^{-8} cm/s^2. The best-fit stellar M/L ratios are generally in remarkable
agreement with the predictions of stellar population synthesis models. We also
show that the narrow range of gravitational accelerations found to be generated
by dark matter in galaxies is consistent with the narrow range of additional

Delivering precision antimicrobial therapy through closed-loop control systems
Sub-optimal exposure to antimicrobial therapy is associated with poor patient outcomes and the development of antimicrobial resistance. Mechanisms for optimizing the concentration of a drug within the individual patient are under development. However, several barriers remain in realizing true individualization of therapy. These include problems with plasma drug sampling, availability of appropriate assays, and current mechanisms for dose adjustment. Biosensor technology offers a means of providing real-time monitoring of antimicrobials in a minimally invasive fashion. We report the potential for using microneedle biosensor technology as part of closed-loop control systems for the optimization of antimicrobial therapy in individual patients
Antibiotic prescribing in general medical and surgical specialties: a prospective cohort study.
Background: Qualitative work has described the differences in prescribing practice across medical and surgical specialties. This study aimed to understand if specialty impacts quantitative measures of prescribing practice. Methods: We prospectively analysed the antibiotic prescribing across general medical and surgical teams for acutely admitted patients. Over a 12-month period (June 2016 - May 2017) 659 patients (362 medical, 297 surgical) were followed for the duration of their hospital stay. Antibiotic prescribing across these cohorts was assessed using Chi-squared or Wilcoxon rank-sum, depending on normality of data. The t-test was used to compare age and length of stay. A logistic regression model was used to predict escalation of antibiotic therapy. Results: Surgical patients were younger (p < 0.001) with lower Charlson Comorbidity Index scores (p < 0.001). Antibiotics were prescribed for 45% (162/362) medical and 55% (164/297) surgical patients. Microbiological results were available for 26% (42/164) medical and 29% (48/162) surgical patients, of which 55% (23/42) and 48% (23/48) were positive respectively. There was no difference in the spectrum of antibiotics prescribed between surgery and medicine (p = 0.507). In surgery antibiotics were 1) prescribed more frequently (p = 0.001); 2) for longer (p = 0.016); 3) more likely to be escalated (p = 0.004); 4) less likely to be compliant with local policy (p < 0.001) than medicine. Conclusions: Across both specialties, microbiology investigation results are not adequately used to diagnose infections and optimise their management. There is significant variation in antibiotic decision-making (including escalation patterns) between general surgical and medical teams. Antibiotic stewardship interventions targeting surgical specialties need to go beyond surgical prophylaxis. It is critical to focus on of review the patients initiated on therapeutic antibiotics in surgical specialties to ensure that escalation and continuation of therapy is justified
The "Ram Effect": A "Non-Classical" Mechanism for Inducing LH Surges in Sheep
During spring sheep do not normally ovulate but exposure to a ram can induce ovulation. In some ewes an LH surge is induced immediately after exposure to a ram thus raising questions about the control of this precocious LH surge. Our first aim was to determine the plasma concentrations of oestradiol (E2) E2 in anoestrous ewes before and after the "ram effect" in ewes that had a "precocious" LH surge (starting within 6 hours), a "normal" surge (between 6 and 28h) and "late» surge (not detected by 56h). In another experiment we tested if a small increase in circulating E2 could induce an LH surge in anoestrus ewes. The concentration of E2 significantly was not different at the time of ram introduction among ewes with the three types of LH surge. "Precocious" LH surges were not preceded by a large increase in E2 unlike "normal" surges and small elevations of circulating E2 alone were unable to induce LH surges. These results show that the "precocious" LH surge was not the result of E2 positive feedback. Our second aim was to test if noradrenaline (NA) is involved in the LH response to the "ram effect". Using double labelling for Fos and tyrosine hydroxylase (TH) we showed that exposure of anoestrous ewes to a ram induced a higher density of cells positive for both in the A1 nucleus and the Locus Coeruleus complex compared to unstimulated controls. Finally, the administration by retrodialysis into the preoptic area, of NA increased the proportion of ewes with an LH response to ram odor whereas treatment with the α1 antagonist Prazosin decreased the LH pulse frequency and amplitude induced by a sexually active ram. Collectively these results suggest that in anoestrous ewes NA is involved in ram-induced LH secretion as observed in other induced ovulators

Analysis of pre-weaning feeding policies and other risk factors influencing growth rates in calves on 11 commercial dairy farms
Growth rates in pre-weaned calves influence their health, age at first calving and lifetime productivity. Many farms restrict milk rations to encourage solid feed intake and facilitate early weaning, but this can compromise growth. This study determined the milk feeding policies and associated growth rates on 11 commercial dairy farms in South East England, each following their normal management regime. Between 26 and 54 heifers were recruited per farm, providing a final cohort of 492, of which 71% were pure Holstein. Information on calf rearing practices (feeding, weaning, housing) and health was collected via questionnaires and weekly observations. Estimates of actual milk fed (kg solids) between 1 and 63 days were calculated for individual calves. Morphometric data (weight, height, length) were taken at weeks 1, 5 and 9 and at a median age of 7.5 months and growth rates were calculated. Most calves were fed milk replacer via automated feeders (four farms), teat feeder (one) or buckets (four) whereas two farms provided drums of acidified waste milk. Farms fed between 4 and 6 l/day of milk at mixing rates of 10% to 15%, providing 400 to 900 g/day of milk solids. Both skeletal growth rates and average daily weight gain (ADG) increased in the second month of life compared with the first: height growth from 0.17±0.14 to 0.25±0.16 cm/day and ADG from 0.48±0.25 to 0.71±0.28 kg/day. Post-weaning heifers up to 7.5 months had height increases of 0.16±0.035 cm/day and ADG of 0.83±0.16 kg/day. From 1 to 63 days 70% of calves had growth rates <0.7 kg/day and of these 19.6% gained <0.5 kg/day. Mean ADG before 9 weeks varied between farms from 0.52±0.30 to 0.75±0.20 kg/day. This was related to the amount of milk fed at both a farm and individual calf level. Increasing the total milk solids fed between 1 and 63 days from 20.4 to 46.3 kg (the 10th to 90th percentile observed) was associated with an increase of 0.11 kg/day ADG. All farms had a wide variation in growth rates despite single feeding policies. Higher circulating immunoglobulin G and IGF1 concentrations were associated with better growth, whereas low temperatures in month of birth, high scores for diarrhoea, respiratory and umbilical disease and large birth size reduced growth. Many commercially grown dairy heifers therefore experienced growth restriction in the pre-weaned period, potentially reducing their health, welfare and productivity

Mapping the decision pathways of acute infection management in secondary care among UK medical physicians: a qualitative study.
BACKGROUND: The inappropriate use of antimicrobials drives antimicrobial resistance. We conducted a study to map physician decision-making processes for acute infection management in secondary care to identify potential targets for quality improvement interventions.
METHODS: Physicians newly qualified to consultant level participated in semi-structured interviews. Interviews were audio recorded and transcribed verbatim for analysis using NVIVO11.0 software. Grounded theory methodology was applied. Analytical categories were created using constant comparison approach to the data and participants were recruited to the study until thematic saturation was reached.
RESULTS: Twenty physicians were interviewed. The decision pathway for the management of acute infections follows a Bayesian-like step-wise approach, with information processed and systematically added to prior assumptions to guide management. The main emerging themes identified as determinants of the decision-making of individual physicians were (1) perceptions of providing 'optimal' care for the patient with infection by providing rapid and often intravenous therapy; (2) perceptions that stopping/de-escalating therapy was a senior doctor decision with junior trainees not expected to contribute; and (3) expectation of interactions with local guidelines and microbiology service advice. Feedback on review of junior doctor prescribing decisions was often lacking, causing frustration and confusion on appropriate practice within this cohort.
CONCLUSION: Interventions to improve infection management must incorporate mechanisms to promote distribution of responsibility for decisions made. The disparity between expectations of prescribers to start but not review/stop therapy must be urgently addressed with mechanisms to improve communication and feedback to junior prescribers to facilitate their continued development as prudent antimicrobial prescribers
Lactate Dehydrogenase and Glutamate Pyruvate Transaminase biosensing strategies for lactate detection on screen-printed sensors. Catalysis efficiency and interference analysis in complex matrices: from cell cultures to sport medicine
Lactate detection with electrochemical biosensors is a fundamental part of the sensing and biosensing research field. Together with glucose detection, this topic attracted scientist attention since the 60s. Lactate is a metabolite involved in many processes among several fields of application, from food degradation, to sport medicine, cell cultures analysis, and clinical monitoring. However, few electrochemical lactate biosensors are currently present in the market.In this work, we analyzed and compared the standard Lactate Dehydrogenase catalysis mechanism with a two-enzymes commercial method that proved to dramatically boost the catalysis efficiency from 0.6% to 72.5%. Both the mechanisms result in the production of reduced Nicotinamide Adenine Dinucleotide (NADH). Therefore, we analyzed its electrochemical detection performance using commercial screen-printed carbon electrodes (SPCE), along with the possible interferences of common mediums and buffers. We found that these sensors can be used for NADH detection down to a 10 μM concentration with a sensitivity of 33.14 nA/μM at a 456 mV overpotential. Moreover, we analytically defined the interferences effects of Dulbecco's Modified Eagle Medium on the sensors by Electrochemical Impedance Spectroscopy measurements and equivalent electrical circuits modeling.Finally, we tested the two-enzyme method in two case studies, i.e., cell cultures lactate monitoring as a valuable tool to evaluate their growth, and lactate quantification in human sweat to monitor physical exercise intensity. We proved the feasibility of the combination of commercial two-enzymes kit and SPCE as a low-cost, accurate, and sensitive lactate biosensor in different applications, regardless of sample interferences.
