Deep brain stimulation of the anterior nucleus of the thalamus in drug-resistant epilepsy in the MORE multicenter patient registry

This is an open access article distributed under the terms of the Creative Commons Attribution License 4.0 (CC BY), which permits unrestricted use, distribution, and reproduction in any medium, provided the original work is properly cited.Background and objectives: The efficacy of deep brain stimulation of the anterior nucleus of the thalamus (ANT DBS) in patients with drug-resistant epilepsy (DRE) was demonstrated in the double-blind Stimulation of the Anterior Nucleus of the Thalamus for Epilepsy randomized controlled trial. The Medtronic Registry for Epilepsy (MORE) aims to understand the safety and longer-term effectiveness of ANT DBS therapy in routine clinical practice.
Methods: MORE is an observational registry collecting prospective and retrospective clinical data. Participants were at least 18 years old, with focal DRE recruited across 25 centers from 13 countries. They were followed for at least 2 years in terms of seizure frequency (SF), responder rate (RR), health-related quality of life (Quality of Life in Epilepsy Inventory 31), depression, and safety outcomes.
Results: Of the 191 patients recruited, 170 (mean [SD] age of 35.6 [10.7] years, 43% female) were implanted with DBS therapy and met all eligibility criteria. At baseline, 38% of patients reported cognitive impairment. The median monthly SF decreased by 33.1% from 15.8 at baseline to 8.8 at 2 years (p 10 implantations) had 42.8% reduction in median monthly SF by 2 years in comparison with 25.8% in low-volume center. In patients with cognitive impairment, the reduction in median monthly SF was 26.0% by 2 years compared with 36.1% in patients without cognitive impairment. The most frequently reported adverse events were changes (e.g., increased frequency/severity) in seizure (16%), memory impairment (patient-reported complaint, 15%), depressive mood (patient-reported complaint, 13%), and epilepsy (12%). One definite sudden unexpected death in epilepsy case was reported.
Discussion: The MORE registry supports the effectiveness and safety of ANT DBS therapy in a real-world setting in the 2 years following implantation.

COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study
Background:
The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms.
Methods:
International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms.
Results:
‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country.
Interpretation:
This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

From senses to politics : towards a new approach of sonic environment
Cette communication tente de montrer l’importance d’une étude de la perception ordinaire pour la compréhension des pratiques et des représentations qui organisent l’environnement vécu, en s’attachant plus particulièrement à sa dimension auditive. A partir d’une réflexion sur la notion de bruit, une série d’approches de l’environnement sonore aux fondements épistémologiques contrastés seront identifiées. Une interrogation sur les rapports entre perception et politique sera menée à partir de la présentation critique d’un ensemble de recherches issues de la sociologie du quotidien. Des conclusions méthodologiques et épistémologiques pour le prolongement du travail de recherche sur cette question seront finalement dégagées
Écouter le bruit, faire entendre la gêne
Noise issues and ordinary listening have long been addressed separately in the social sciences. This paper argues in favor of the usefulness of an integrated approach to both, from the standpoint of an ethnographic investigation on the perception and management of the sonic environment which was carried out in French-speaking Belgium. This heuristic approach - combining listening and noise - makes it possible to describe precisely the process of when the feeling of annoyance comes about and the ability of people to express it.Les questions du bruit et de l'écoute ordinaire ont longtemps fait l'objet de traitements séparés en sciences sociales. Cet article défend l'intérêt heuristique de les articuler, à travers une enquête ethnographique sur la perception et la gestion de l'environnement sonore en Belgique francophone. Le croisement des perspectives de l'écoute et de la nuisance permet de décrire la manière dont se développe le sentiment de gêne chez les individus confrontés à une nuisance sonore. Qualitative soundscape assessment methodology : experiment in 3 belgian localities
La communication présente l'expérimentation d'une méthodologie d’évaluation qualitative de l’environnement sonore dans 3 localités belges. Elle vise à améliorer la gestion des nuisances sonores environnementales, en intégrant dans celle-ci le ressenti et le vécu de l'environnement sonore. Après un rapide exposé de la mise en place du projet, je présenterai ses postulats théoriques et méthodologiques, les difficultés rencontrées dans sa mise en œuvre et les perspectives qu’il ouvre.The paper presents a methodology of qualitative souyndscape assessment in three communities in Belgium. It aims to improve the management of environmental noise, taking into account the feeling and experience of the sonic environment. Ce chapitre rend compte de la tentative de mettre sur pied un protocole d’évaluation de l’environnement sonore qui soit basé sur l’expérience auditive des habitants plutôt que sur des mesures acoustiques. Je m’intéresserai essentiellement ici à la résistance opposée par les habitants eux-mêmes au projet. Being in tune with one's own listening : noise annoyance and reflexivity
Dans cet article, nous nous interrogerons sur l’expérience de la dimension
sonore de l’environnement humain, à partir d’éléments issus d’une enquête
ethnographique en cours en Belgique francophone. Après avoir examiné rapidement
les propositions de conceptualisation de cette expérience en sciences sociales, nous
insisterons sur les difficultés inhérentes à son observation. Nous tâcherons alors de
montrer en quoi l’approche de cette expérience par le biais des nuisances sonores
peut permettre de contourner en partie cette difficulté, précisément en raison de
la posture réflexive que ces phénomènes suscitent chez les personnes qui y sont
confrontées. Grâce à cette posture, l’individu prend conscience, non sans difficultés,
des manières d’investir son milieu sonore qu’il adopte d’ordinaire sans y prendre
garde. Cette réflexivité particulière des acteurs est toutefois à double tranchant : en
même temps qu’elle fournit à l’individu une prise nouvelle sur son milieu sonore,
elle le rend vulnérable à ses effets.In this paper, I examine the experience of listening to one's environment from an ethnographic standpoint. After a brief review of the conceptualizations of everyday listening in the literature, I focus on the difficulties of its observation. I then propose to take noise annoyance as a mean to investigate everyday listening.
