    Perspectives and Best Practices for Artificial Intelligence and Continuously Learning Systems in Healthcare

    Goals of this paper Healthcare is often a late adopter when it comes to new techniques and technologies; this works to our advantage in the development of this paper as we relied on lessons learned from CLS in other industries to help guide the content of this paper. Appendix V includes a number of example use cases of AI in Healthcare and other industries. This paper focuses on identifying unique attributes, constraints and potential best practices towards what might represent “good” development for Continuously Learning Systems (CLS) AI systems with applications ranging from pharmaceutical applications for new drug development and research to AI enabled smart medical devices. It should be noted that although the emphasis of this paper is on CLS, some of these issues are common to all AI products in healthcare. Additionally, there are certain topics that should be considered when developing CLS for healthcare, but they are outside of the scope of this paper. These topics will be briefly touched upon, but will not be explored in depth. Some examples include: Human Factors – this is a concern in the development of any product – what are the unique usability challenges that arise when collecting data and presenting the results? Previous efforts at generating automated alerts have often created problems (e.g. alert fatigue.) CyberSecurity and Privacy – holding a massive amount of patient data is an attractive target for hackers, what steps should be taken to protect data from misuse? How does the European Union’s General Data Protection Regulation (GDPR) impact the use of patient data? Legal liability – if a CLS system recommends action that is then reviewed and approved by a doctor, where does the liability lie if the patient is negatively affected? Regulatory considerations – medical devices are subject to regulatory oversight around the world; in fact, if a product is considered a medical device depends on what country you are in. AI provides an interesting challenge to traditional regulatory models. Additionally, some organizations like the FTC regulate non-medical devices. This paper is not intended to be a standard, nor is this paper trying to advocate for one and only one method of developing, verifying, and validating CLS systems – this paper highlights best practices from other industries and suggests adaptation of those processes for healthcare. This paper is also not intended to evaluate existing or developing regulatory, legal, ethical, or social consequences of CLS systems. This is a rapidly evolving subject with many companies, and now some countries, establishing their own AI Principles or Code of Conduct which emphasize legal and ethical considerations including goals and principles of fairness, reliability and safety, transparency around how the results of these learning systems are explained to the people using those systems5 . The intended audience of this paper are Developers, Researchers, Quality Assurance and Validation personnel, Business Managers and Regulators across both Medical Device and Pharmaceutical industries that would like to learn more about CLS best practices, and CLS practitioners wanting to learn more about medical device software development

    The association of health literacy with adherence in older 2 adults, and its role in interventions: a systematic meta-review

    Background: Low health literacy is a common problem among older adults. It is often suggested to be associated with poor adherence. This suggested association implies a need for effective adherence interventions in low health literate people. However, previous reviews show mixed results on the association between low health literacy and poor adherence. A systematic meta-review of systematic reviews was conducted to study the association between health literacy and adherence in adults above the age of 50. Evidence for the effectiveness of adherence interventions among adults in this older age group with low health literacy was also explored. Methods: Eight electronic databases (MEDLINE, ERIC, EMBASE, PsycINFO, CINAHL, DARE, the Cochrane Library, and Web of Knowledge) were searched using a variety of keywords regarding health literacy and adherence. Additionally, references of identified articles were checked. Systematic reviews were included if they assessed the association between health literacy and adherence or evaluated the effectiveness of interventions to improve adherence in adults with low health literacy. The AMSTAR tool was used to assess the quality of the included reviews. The selection procedure, data-extraction, and quality assessment were performed by two independent reviewers. Seventeen reviews were selected for inclusion. Results: Reviews varied widely in quality. Both reviews of high and low quality found only weak or mixed associations between health literacy and adherence among older adults. Reviews report on seven studies that assess the effectiveness of adherence interventions among low health literate older adults. The results suggest that some adherence interventions are effective for this group. The interventions described in the reviews focused mainly on education and on lowering the health literacy demands of adherence instructions. No conclusions could be drawn about which type of intervention could be most beneficial for this population. Conclusions: Evidence on the association between health literacy and adherence in older adults is relatively weak. Adherence interventions are potentially effective for the vulnerable population of older adults with low levels of health literacy, but the evidence on this topic is limited. Further research is needed on the association between health literacy and general health behavior, and on the effectiveness of interventions

    Self-control enhancement in children:Ethical and conceptual aspects

    Childhood self-control is currently receiving great scientific and public attention because it could predict much of adult’s life success and well-being. Specialized interventions based on findings in social psychology and neuroscience potentially enhance children’s capacity to exercise self-control. This perspective triggers hopes that self-control enhancement allows us to say good-bye for good to potentially unsafe psychopharmacological agents and electronic brain stimulants. This chapter provides an in-depth ethical analysis of pediatric self-control enhancement and points toward a series of serious conceptual and ethical concerns. First, it gives an overview of current psychological as well as neuroscientific research on self-control, and it presents longitudinal studies that emphasize the importance of childhood self-control for adult life success. Second, it critically discusses the concept of self-control presupposed in these approaches and points to crucial limitations. Going beyond an understanding of self-control as a sophisticated means of goal-achievement, i will argue for a comprehensive understanding that takes the inherent normativity of self-controlled behavior seriously. In that context, self-control enhancement appears as not necessarily desirable and occasionally even detrimental. Finally, this chapter questions the notion of childhood implicit in current research and how values typically put on this phase of life could get affected by self-control enhancement. I finish with an exploration of the conditions under which pediatric self-control enhancement is either impermissible, permissible, or maybe obligatory

    The relationship between gambling event frequency, motor response inhibition, arousal, and dissociative experience

    Speed of play has been identified as a key structural characteristic in gambling behaviour, where games involving higher playing speeds enhance the experience of gambling. Of interest in the present study is the consistent finding that games with higher event frequencies are preferred by problem gamblers and are associated with more negative gambling outcomes, such as difficulty quitting the game and increased monetary loss. The present study investigated the impact of gambling speed of play on executive control functioning, focusing on how increased speeds of play impact motor response inhibition, and the potential mediating role arousal and dissociative experience play in this relationship. Fifty regular non-problem gamblers took part in a repeated-measures experiment where they gambled with real money on a simulated slot machine across five speed of play conditions. Response inhibition was measured using an embedded Go/No-Go task, where participants had to withhold motor responses, rather than operating the spin button on the slot machine when a specific colour cue was present. Results indicated that response inhibition performance was significantly worse during faster speeds of play, and that the role of arousal in this relationship was independent of any motor priming affect. The implications of these findings for gambling legislation and gambling harm-minimisation approaches are discussed

    Whole-body diffusion-weighted imaging for staging malignant lymphoma in children

    CT is currently the mainstay in staging malignant lymphoma in children, but the risk of second neoplasms due to ionizing radiation associated with CT is not negligible. Whole-body MRI techniques and whole-body diffusion-weighted imaging (DWI) in particular, may be a good radiation-free alternative to CT. DWI is characterized by high sensitivity for the detection of lesions and allows quantitative assessment of diffusion that may aid in the evaluation of malignant lymphomas. This article will review whole-body MRI techniques for staging malignant lymphoma with emphasis on whole-body DWI. Furthermore, future considerations and challenges in whole-body DWI will be discussed

    Multidifferential cross section measurements of νμ -argon quasielasticlike reactions with the MicroBooNE detector

    We report on a flux-integrated multidifferential measurement of charged-current muon neutrino scattering on argon with one muon and one proton in the final state using the Booster Neutrino Beam and MicroBooNE detector at Fermi National Accelerator Laboratory. The data are studied as a function of various kinematic imbalance variables and of a neutrino energy estimator, and are compared to a number of event generator predictions. We find that the measured cross sections in different phase-space regions are sensitive to nuclear effects. Our results provide precision data to test and improve the neutrino-nucleus interaction models needed to perform high-accuracy oscillation analyses. Specific regions of phase space are identified where further model refinements are most needed

    Data Mining the Brain to Decode the Mind

    In recent years, neuroscience has begun to transform itself into a “big data” enterprise with the importation of computational and statistical techniques from machine learning and informatics. In addition to their translational applications such as brain-computer interfaces and early diagnosis of neuropathology, these tools promise to advance new solutions to longstanding theoretical quandaries. Here I critically assess whether these promises will pay off, focusing on the application of multivariate pattern analysis (MVPA) to the problem of reverse inference. I argue that MVPA does not inherently provide a new answer to classical worries about reverse inference, and that the method faces pervasive interpretive problems of its own. Further, the epistemic setting of MVPA and other decoding methods contributes to a potentially worrisome shift towards prediction and away from explanation in fundamental neuroscience

    Search for electron antineutrino appearance in a long-baseline muon antineutrino beam

    Electron antineutrino appearance is measured by the T2K experiment in an accelerator-produced antineutrino beam, using additional neutrino beam operation to constrain parameters of the Pontecorvo-Maki-Nakagawa-Sakata (PMNS) mixing matrix. T2K observes 15 candidate electron antineutrino events with a background expectation of 9.3 events. Including information from the kinematic distribution of observed events, the hypothesis of no electron antineutrino appearance is disfavored with a significance of 2.40σ and no discrepancy between data and PMNS predictions is found. A complementary analysis that introduces an additional free parameter which allows non-PMNS values of electron neutrino and antineutrino appearance also finds no discrepancy between data and PMNS predictions
