    Survival benefit with adjuvant chemotherapy in stage III microsatellite-high/deficient mismatch repair colon cancer: a systematic review and meta-analysis

    Clinical observations have demonstrated that microsatellite instability-high (MSI-H) and/or deficient MMR (dMMR) status are associated with favorable prognosis and no benefit from 5-Fluorouracil (5-FU)-based adjuvant chemotherapy in patients with resected stage II colorectal cancer (CRC). This study represents a systematic review and meta-analysis exploring the predictive role of MSI-H status in stage III CRC undergoing or not adjuvant chemotherapy. Published articles that evaluated the role of adjuvant chemotherapy in resected stage III CRC from inception to September 2020 were identified by searching the PubMed, EMBASE, and Cochrane Library databases. The random-effects model was conducted to estimate the pooled effect size of OS and DFS. The primary outcome of interest was OS. 21,590 patients with MSI-H/dMMR stage III CRC, from n = 17 retrospective studies, were analyzed. Overall, OS was improved with any adjuvant chemotherapy vs. any control arm (single-agent 5-FU or surgery alone): HR 0.42, 95% CI 0.26-0.66; P < 0.01. Conversely, DFS was not significantly improved (HR 0.7, 95% CI 0.45-1.09; P = 0.11). In patients with stage III MSI-H/dMMR CRC, adjuvant chemotherapy is associated with a significant OS improvement. Thus, MSI-H/dMMR status does represent a predictive factor for postoperative chemotherapy benefit in stage III CRC beyond its prognostic role

    Molecular Investigation on a Triple Negative Breast Cancer Xenograft Model Exposed to Proton Beams

    Specific breast cancer (BC) subtypes are associated with bad prognoses due to the absence of successful treatment plans. The triple-negative breast cancer (TNBC) subtype, with estrogen (ER), progesterone (PR) and human epidermal growth factor-2 (HER2) negative receptor status, is a clinical challenge for oncologists, because of its aggressiveness and the absence of effective therapies. In addition, proton therapy (PT) represents an effective treatment against both inaccessible area located or conventional radiotherapy (RT)-resistant cancers, becoming a promising therapeutic choice for TNBC. Our study aimed to analyze the in vivo molecular response to PT and its efficacy in a MDA-MB-231 TNBC xenograft model. TNBC xenograft models were irradiated with 2, 6 and 9 Gy of PT. Gene expression profile (GEP) analyses and immunohistochemical assay (IHC) were performed to highlight specific pathways and key molecules involved in cell response to the radiation. GEP analysis revealed in depth the molecular response to PT, showing a considerable immune response, cell cycle and stem cell process regulation. Only the dose of 9 Gy shifted the balance toward pro-death signaling as a dose escalation which can be easily performed using proton beams, which permit targeting tumors while avoiding damage to the surrounding healthy tissue

    ELIMED: MEDICAL APPLICATION AT ELI-BEAMLINES. STATUS OF THE COLLABORATION AND FIRST RESULTS

    ELI-Beamlines is one of the four pillars of the ELI (Extreme Light Infrastructure) pan-European project. It will be an ultrahigh-intensity, high repetition-rate, femtosecond laser facility whose main goal is to generate and apply high-brightness X-ray sources and accelerated charged particles. In particular, medical applications are treated by the ELIMED task force, which has been launched by collaboration between ELI and INFN researchers. ELIMED aims to demonstrate the clinical applicability of laser accelerated ions. In this article, the state of the ELIMED project and the first scientific results are reported. The design and realisation of a preliminary beam handling system and of an advanced spectrometer for diagnostics of high energy (multi-MeV) laser-accelerated ion beams will also be briefly presented

    Feeling anxious? The mechanisms of vocal deception in tufted capuchin monkeys

    An ability to deceive conspecifics is thought to have favoured the evolution of large brains in social animals, but evidence that such behaviours require cognitive complexity is lacking. Tufted capuchin monkeys (Sapajus spp.) have been documented to use false alarm calls during feeding in a manner that functions to deceive competitors. However, comparative evidence suggests that the production of vocalizations by nonhuman primates is largely underpinned by emotional mechanisms, calling into question more cognitive interpretations of this behaviour. To determine whether emotional states are plausibly necessary and sufficient to proximately explain deceptive alarm call production, we examined the association between self-directed behaviours (SDBs), as a proxy for anxiety, and the production of spontaneous false alarm calls among tufted capuchins. Specifically, we predicted that if anxiety is necessary for the production of false alarms, then individuals that produce spontaneous false alarms should exhibit more SDBs in those contexts in which they call. If anxiety is also sufficient to explain the false alarm call production, then we predicted that individuals that call more in a given context would show higher rates of SDBs in that context, and that high rates of calling would be temporally associated with high rates of SDBs. Our results support the contention that states of anxiety are necessary for an individual to spontaneously produce false alarms, but that such states are not sufficient to explain patterns of calling. The link between anxiety and deceptive calling thus appears complex, and cognitively based decision-making processes may play some role in call production

    Identification of genetic variants associated with Huntington's disease progression: a genome-wide association study

    Background Huntington's disease is caused by a CAG repeat expansion in the huntingtin gene, HTT. Age at onset has been used as a quantitative phenotype in genetic analysis looking for Huntington's disease modifiers, but is hard to define and not always available. Therefore, we aimed to generate a novel measure of disease progression and to identify genetic markers associated with this progression measure. Methods We generated a progression score on the basis of principal component analysis of prospectively acquired longitudinal changes in motor, cognitive, and imaging measures in the 218 indivduals in the TRACK-HD cohort of Huntington's disease gene mutation carriers (data collected 2008–11). We generated a parallel progression score using data from 1773 previously genotyped participants from the European Huntington's Disease Network REGISTRY study of Huntington's disease mutation carriers (data collected 2003–13). We did a genome-wide association analyses in terms of progression for 216 TRACK-HD participants and 1773 REGISTRY participants, then a meta-analysis of these results was undertaken. Findings Longitudinal motor, cognitive, and imaging scores were correlated with each other in TRACK-HD participants, justifying use of a single, cross-domain measure of disease progression in both studies. The TRACK-HD and REGISTRY progression measures were correlated with each other (r=0·674), and with age at onset (TRACK-HD, r=0·315; REGISTRY, r=0·234). The meta-analysis of progression in TRACK-HD and REGISTRY gave a genome-wide significant signal (p=1·12 × 10−10) on chromosome 5 spanning three genes: MSH3, DHFR, and MTRNR2L2. The genes in this locus were associated with progression in TRACK-HD (MSH3 p=2·94 × 10−8 DHFR p=8·37 × 10−7 MTRNR2L2 p=2·15 × 10−9) and to a lesser extent in REGISTRY (MSH3 p=9·36 × 10−4 DHFR p=8·45 × 10−4 MTRNR2L2 p=1·20 × 10−3). The lead single nucleotide polymorphism (SNP) in TRACK-HD (rs557874766) was genome-wide significant in the meta-analysis (p=1·58 × 10−8), and encodes an aminoacid change (Pro67Ala) in MSH3. In TRACK-HD, each copy of the minor allele at this SNP was associated with a 0·4 units per year (95% CI 0·16–0·66) reduction in the rate of change of the Unified Huntington's Disease Rating Scale (UHDRS) Total Motor Score, and a reduction of 0·12 units per year (95% CI 0·06–0·18) in the rate of change of UHDRS Total Functional Capacity score. These associations remained significant after adjusting for age of onset. Interpretation The multidomain progression measure in TRACK-HD was associated with a functional variant that was genome-wide significant in our meta-analysis. The association in only 216 participants implies that the progression measure is a sensitive reflection of disease burden, that the effect size at this locus is large, or both. Knockout of Msh3 reduces somatic expansion in Huntington's disease mouse models, suggesting this mechanism as an area for future therapeutic investigation

    Exemplar-learning and schematization in a usage-based account of syntactic acquisition

    The early phases of syntactic acquisition are characterized by many input frequency and item effects, which argue against theories assuming innate access to classical syntactic categories. In formulating an alternative view, we consider both prototype and exemplar-learning models of categorization. We argue for a 'hybrid' usage-based view in which acquisition depends on exemplar learning and retention, out of which permanent abstract schemas gradually emerge and are immanent across the summed similarity of exemplar collections. These schemas are graded in strength depending on the number of exemplars and the degree to which semantic similarity is reinforced by phonological, lexical, and distributional similarity

The authors present Bodei’s historical and philosophical reconstruction of the intertwined relationships between structures of social and natural dominance, human work and machines. Baccelli focuses on the philosophical variations of the Aristotelian justification of natural slavery in the Spanish debate of the Sixteenth Century, during the modern history of military and spiritual conquest of the American continent. Looking at Leonardo’s scientia machinale, Henry insists on the symbolic mediation between materiality and immateriality played by socio-technological tools throughout human history, even before the cybernetic revolution. Tomasello sheds light on the main challenges and opportunity inaugurated by the digital revolution with respect to the modern revolution of industrial technologies
