The utility of cardiac sonography and capnography in predicting outcome in cardiac arrest
Emergency physicians and intensivists are increasingly utilizing capnography and bedside echocardiography during medical resuscitations. These techniques have shown promise in predicting outcomes in cardiac arrest, and no cases of return of spontaneous circulation in the setting of sonographic cardiac standstill and low end-tidal carbon dioxide have been reported. This case report illustrates an example of such an occurrence. Our aims are to report a case of return of spontaneous circulation in a patient with sonographic cardiac standstill, electrocardiographic pulseless electrical activity, and low end-tidal carbon dioxide tensions and to place the case in the context of previous literature on this topic. Case report and brief review of the literature. In 254 cases reported, no patient has survived in the setting of sonographic cardiac standstill and low end-tidal carbon dioxide tension, making the reported case unique. This case should serve to illustrate the utility and limitations of combined cardiac sonography and end-tidal carbon dioxide measurement in determining prognosis during cardiac arrest
Viral and cellular mRNA-specific activators harness PABP and eIF4G to promote translation initiation downstream of cap binding
Regulation of mRNA translation is a major control point for gene expression and is critical for life. Of central importance is the complex between cap-bound eukaryotic initiation factor 4E (eIF4E), eIF4G, and poly(A) tail-binding protein (PABP) that circularizes mRNAs, promoting translation and stability. This complex is often targeted to regulate overall translation rates, and also by mRNA-specific translational repressors. However, the mechanisms of mRNA-specific translational activation by RNA-binding proteins remain poorly understood. Here, we address this deficit, focusing on a herpes simplex virus-1 protein, ICP27. We reveal a direct interaction with PABP that is sufficient to promote PABP recruitment and necessary for ICP27-mediated activation. PABP binds several translation factors but is primarily considered to activate translation initiation as part of the PABP-eIF4G-eIF4E complex that stimulates the initial cap-binding step. Importantly, we find that ICP27-PABP forms a complex with, and requires the activity of, eIF4G. Surprisingly, ICP27-PABP-eIF4G complexes act independently of the effects of PABP-eIF4G on cap binding to promote small ribosomal subunit recruitment. Moreover, we find that a cellular mRNA-specific regulator, Deleted in Azoospermia-like (Dazl), also employs the PABP-eIF4G interaction in a similar manner. We propose a mechanism whereby diverse RNA-binding proteins directly recruit PABP, in a non-poly(A) tail-dependent manner, to stimulate the small subunit recruitment step. This strategy may be particularly relevant to biological conditions associated with hypoadenylated mRNAs (e.g., germ cells/neurons) and/or limiting cytoplasmic PABP (e.g., viral infection, cell stress). This mechanism adds significant insight into our knowledge of mRNA-specific translational activation and the function of the PABP-eIF4G complex in translation initiation
Intercomparison of MODIS Albedo Retrievals and In Situ Measurements Across the Global FLUXNET Network
Surface albedo is a key parameter in the Earth's energy balance since it affects the amount of solar radiation directly absorbed at the planet surface. Its variability in time and space can be globally retrieved through the use of remote sensing products. To evaluate and improve the quality of satellite retrievals, careful intercomparisons with in situ measurements of surface albedo are crucial. For this purpose we compared MODIS albedo retrievals with surface measurements taken at 53 FLUXNET sites that met strict conditions of land cover homogeneity. A good agreement between mean yearly values of satellite retrievals and in situ measurements was found (R(exp 2)= 0.82). The mismatch is correlated to the spatial heterogeneity of surface albedo, stressing the relevance of land cover homogeneity when comparing point to pixel data. When the seasonal patterns of MODIS albedo is considered for different plant functional types, the match with surface observation is extremely good at all forest sites. On the contrary, in non-forest sites satellite retrievals underestimate in situ measurements across the seasonal cycle. The mismatch observed at grasslands and croplands sites is likely due to the extreme fragmentation of these landscapes, as confirmed by geostatistical attributes derived from high resolution scenes
Internet access and use by COPD patients in the National Emphysema/COPD Association Survey
Technology offers opportunities to improve healthcare, but little is known about Internet use by COPD patients. We tested two hypotheses: Internet access is associated with socio-demographic disparities and frequency of use is related to perceived needs. We analyzed data from a 2007–2008 national convenience sample survey of COPD patients to determine the relationship between Internet access and frequency of use with demographics, socio-economic status, COPD severity, and satisfaction with healthcare. Among survey respondents (response rate 7.2%; n = 914, 59.1% women, mean age 71.2 years), 34.2% reported lack of Internet access, and an additional 49% had access but used the Internet less than weekly. Multivariate models showed association between lack of access and older age (OR 1.10, 95% CI 1.07, 1.13), lower income (income below $30,000 OR 2.47, 95% CI 1.63, 3.73), less education (high school highest attainment OR 2.30, 95% CI 1.54, 3.45), comorbid arthritis or mobility-related disease (OR 1.56, 95% CI 1.05, 2.34). More frequent use (at least weekly) was associated with younger age (OR 0.95, 95% CI 0.93, 0.98), absence of cardiovascular disease (OR 0.48, 95% CI 0.29, 0.78), but with perception of needs insufficiently met by the healthcare system, including diagnostic delay (OR 1.72, 95% CI 1.06, 2.78), feeling treated poorly (OR 2.46, 95% CI 1.15, 5.24), insufficient physician time (OR 2.29, 95% CI 1.02, 5.13), and feeling their physician did not listen (OR 3.14, 95% CI 1.42, 6.95). An analysis of the characteristics associated with Internet access and use among COPD patients identified two different patient populations. Lack of Internet access was a marker of socioeconomic disparity and mobility-associated diseases, while frequent Internet use was associated with less somatic disease but dissatisfaction with care.https://doi.org/10.1186/1471-2466-14-6
Chronic psychosocial and financial burden accelerates 5-year telomere shortening: findings from the Coronary Artery Risk Development in Young Adults Study.
Leukocyte telomere length, a marker of immune system function, is sensitive to exposures such as psychosocial stressors and health-maintaining behaviors. Past research has determined that stress experienced in adulthood is associated with shorter telomere length, but is limited to mostly cross-sectional reports. We test whether repeated reports of chronic psychosocial and financial burden is associated with telomere length change over a 5-year period (years 15 and 20) from 969 participants in the Coronary Artery Risk Development in Young Adults (CARDIA) Study, a longitudinal, population-based cohort, ages 18-30 at time of recruitment in 1985. We further examine whether multisystem resiliency, comprised of social connections, health-maintaining behaviors, and psychological resources, mitigates the effects of repeated burden on telomere attrition over 5 years. Our results indicate that adults with high chronic burden do not show decreased telomere length over the 5-year period. However, these effects do vary by level of resiliency, as regression results revealed a significant interaction between chronic burden and multisystem resiliency. For individuals with high repeated chronic burden and low multisystem resiliency (1 SD below the mean), there was a significant 5-year shortening in telomere length, whereas no significant relationships between chronic burden and attrition were evident for those at moderate and higher levels of resiliency. These effects apply similarly across the three components of resiliency. Results imply that interventions should focus on establishing strong social connections, psychological resources, and health-maintaining behaviors when attempting to ameliorate stress-related decline in telomere length among at-risk individuals

Early Life History and Fisheries Oceanography: New Questions in a Changing World
In the past 100 years since the birth of fisheries oceanography, research on the early life history of fishes, particularly the larval stage, has been extensive, and much progress has been made in identifying the mechanisms by which factors such as feeding success, predation, or dispersal can influence larval survival. However, in recent years, the study of fish early life history has undergone a major and, arguably, necessary shift, resulting in a growing body of research aimed at understanding the consequences of climate change and other anthropogenically induced stressors. Here, we review these efforts, focusing on the ways in which fish early life stages are directly and indirectly affected by increasing temperature; increasing CO₂ concentrations, and ocean acidification; spatial, temporal, and magnitude changes in secondary production and spawning; and the synergistic effects of fishing and climate change. We highlight how these and other factors affect not only larval survivorship, but also the dispersal of planktonic eggs and larvae, and thus the connectivity and replenishment of fish subpopulations. While much of this work is in its infancy and many consequences are speculative or entirely unknown, new modeling approaches are proving to be insightful by predicting how early life stage survival may change in the future and how such changes will impact economically and ecologically important fish populations
The use of complementary and alternative medicines among patients with locally advanced breast cancer – a descriptive study
BACKGROUND: Complementary and alternative medicine (CAM) use is common among cancer patients. This paper reviews the use of CAM in a series of patients with locally advanced breast cancer (LABC). METHODS: Women with LABC attending a specialist clinic at a single Canadian cancer centre were identified and approached. Participants completed a self-administered survey regarding CAM usage, beliefs associated with CAM usage, views of their risks of developing recurrent cancer and of dying of breast cancer. Responses were scored and compared between CAM users and non-users. RESULTS: Thirty-six patients were approached, 32 completed the questionnaire (response rate 89%). Forty-seven percent of LABC patients were identified as CAM users. CAM users were more likely to be younger, married, in a higher socioeconomic class and of Asian ethnicity than non-users. CAM users were likely to use multiple modalities simultaneously (median 4) with vitamins being the most popular (60%). Motivation for CAM therapy was described as, "assisting their body to heal" (75%), to 'boost the immune system' (56%) and to "give a feeling of control with respect to their treatment" (56%). CAM therapy was used concurrently with conventional treatment in 88% of cases, however, 12% of patients felt that CAM could replace their conventional therapy. Psychological evaluation suggests CAM users perceived their risk of dying of breast cancer was similar to that of the non-Cam group (33% vs. 35%), however the CAM group had less severe anxiety and depression. CONCLUSION: The motivation, objectives and benefits of CAM therapy in a selected population of women with LABC are similar to those reported for women diagnosed with early stage breast cancer. CAM users display less anxiety and depression and are less likely to believe they will die of their breast cancer. However the actual benefit to overall and disease free survival has yet to be demonstrated, as well as the possible interactions with conventional therapy. Consequently more research is needed in this ever-growing field
The effects of hypoxia on zooplankton population estimates and migration in lakes
Many zooplankton species typically exhibit diel vertical migration (DVM), where
zooplankton migrate from the hypolimnion to the epilimnion of lakes at night.
Zooplankton exhibit this behavior to avoid visual predators and UV radiation by
remaining in the bottom waters during the day and ascending to the surface waters to feed
on phytoplankton at night. However, hypoxic conditions in the hypolimnion of lakes mayinterfere with DVM and force zooplankton to increase diel horizontal migration (DHM)
to find predation refuge in littoral zones. Climate change and eutrophication are expected
to increase the prevalence and severity of hypoxic conditions worldwide and thereby
possibly alter zooplankton migration patterns. We hypothesize that hypoxia will force
zooplankton to shift their migration patterns from predominantly DVM to DHM to avoid
oxygen-depleted bottom waters. To test our hypothesis, we are conducting a standardized
global sampling program to test whether pelagic, full water column estimates of
zooplankton are greater at night versus the day under hypolimnetic hypoxic versus oxic
conditions. Participants are aiming to sample at least one lake with an oxic hypolimnion
and one lake with a hypoxic hypolimnion during the thermally-stratified period at midday
and midnight. With our global dataset (currently expecting about 60 lakes in 22
countries), our goal is to improve our understanding of how global change may alter
