Obstetric care models in the Southern Region of Brazil and associated factors
The study sought to identify obstetric care models for low-risk pregnancies in the Southern Region of Brazil and to estimate factors associated with these models and maternal and neonatal outcomes. This is a cross-sectional, hospital-based study using data from the Birth in Brazil survey regarding puerperae and newborns. We identified 2,668 low-risk pregnant women. We carried out an exploratory analysis using the proportion of practices per hospital, among them inducing labor, presence of a companion, cesarean section and skin-to-skin contact, in order to obtain the care models we called Best Practice, Interventionist I and Interventionist II. We then carried out an inferential analysis of the associated characteristics. Results show that access to public or private funding, cultural factors and actions taken by health professional are associated with the care models. Public care had different contexts, one based on public policies and evidence-based practices; and another, that suggests the intentionality of vaginal delivery without considering humanization principles. Private care, on the other hand, is standardized and centered on the medical professional, with higher intervention levels. We conclude there is a predominance of interventionist obstetric care models in the Southern Region of Brazil, a type of care that goes against the best evidence, and that women who receive care in public hospitals have greater chances of benefiting from good practices
Sprinting with an amputation: Some race-based lower-limb step observations.
BACKGROUND: T44 sprinting with an amputation is still in a state of relative infancy. Future scope for athletic training and prosthetic limb development may be assisted with a better understanding of information derived from T44 athletes when under race-based conditions. OBJECTIVES: To investigate the behaviour of step count and step frequency when under competitive conditions. STUDY DESIGN: The study comprises two elements: (1) a video-based analysis of race-based limb-to-limb symmetry and (2) a video-based analysis of race-based step count. METHODS: Video analysis of several major events from 1996-2012 are assessed for step count and step limb-to-limb symmetry characteristics. RESULTS: The video analysis highlights limb-to-limb imbalances greater than those indicated in the previous literature. A low step count is determined to be desirable for success in the 100-m event. CONCLUSION: Future analysis of athletes with a lower-limb amputation would be worthwhile when placed under race-based conditions as the limb-to-limb behaviour is more exaggerated than those seen in typical studies held within a laboratory setting. The within-event behaviour of step counts requires further investigation to establish where these take place or whether it is a cumulative step length issue. CLINICAL RELEVANCE: This article increases the understanding of the race-based behaviour of amputee athletes and provides more information to contribute to any discussions on the performance of lower-limb prostheses
Energy Spectra of Elemental Groups of Cosmic Rays: Update on the KASCADE Unfolding Analysis
The KASCADE experiment measures extensive air showers induced by cosmic rays
in the energy range around the so-called knee. The data of KASCADE have been
used in a composition analysis showing the knee at 3-5 PeV to be caused by a
steepening in the light-element spectra. Since the applied unfolding analysis
depends crucially on simulations of air showers, different high energy hadronic
interaction models (QGSJet and SIBYLL) were used. The results have shown a
strong dependence of the relative abundance of the individual mass groups on
the underlying model. In this update of the analysis we apply the unfolding
method with a different low energy interaction model (FLUKA instead of GHEISHA)
in the simulations. While the resulting individual mass group spectra do not
change significantly, the overall description of the measured data improves by
using the FLUKA model. In addition data in a larger range of zenith angle are
analysed. The new results are completely consistent, i.e. there is no hint to
any severe problem in applying the unfolding analysis method to KASCADE data.Comment: accepted for publication in Astroparticle Physic
Stretching the spines of gymnasts: a review
Gymnastics is noted for involving highly specialized strength, power, agility and flexibility. Flexibility is perhaps the single greatest discriminator of gymnastics from other sports. The extreme ranges of motion achieved by gymnasts require long periods of training, often occupying more than a decade. Gymnasts also start training at an early age (particularly female gymnasts), and the effect of gymnastics training on these young athletes is poorly understood. One of the concerns of many gymnastics professionals is the training of the spine in hyperextension-the ubiquitous 'arch' seen in many gymnastics positions and movements. Training in spine hyperextension usually begins in early childhood through performance of a skill known as a back-bend. Does practising a back-bend and other hyperextension exercises harm young gymnasts? Current information on spine stretching among gymnasts indicates that, within reason, spine stretching does not appear to be an unusual threat to gymnasts' health. However, the paucity of information demands that further study be undertaken
Identification of a new European rabbit IgA with a serine-rich hinge region
<div><p>In mammals, the most striking IgA system belongs to Lagomorpha. Indeed, 14 IgA subclasses have been identified in European rabbits, 11 of which are expressed. In contrast, most other mammals have only one IgA, or in the case of hominoids, two IgA subclasses. Characteristic features of the mammalian IgA subclasses are the length and amino acid sequence of their hinge regions, which are often rich in Pro, Ser and Thr residues and may also carry Cys residues. Here, we describe a new IgA that was expressed in New Zealand White domestic rabbits of <i>IGHV</i>a1 allotype. This IgA has an extended hinge region containing an intriguing stretch of nine consecutive Ser residues and no Pro or Thr residues, a motif exclusive to this new rabbit IgA. Considering the amino acid properties, this hinge motif may present some advantage over the common IgA hinge by affording novel functional capabilities. We also sequenced for the first time the IgA14 CH2 and CH3 domains and showed that IgA14 and IgA3 are expressed.</p></div
Biogenesis and functions of bacterial S-layers.
The outer surface of many archaea and bacteria is coated with a proteinaceous surface layer (known as an S-layer), which is formed by the self-assembly of monomeric proteins into a regularly spaced, two-dimensional array. Bacteria possess dedicated pathways for the secretion and anchoring of the S-layer to the cell wall, and some Gram-positive species have large S-layer-associated gene families. S-layers have important roles in growth and survival, and their many functions include the maintenance of cell integrity, enzyme display and, in pathogens and commensals, interaction with the host and its immune system. In this Review, we discuss our current knowledge of S-layer and related proteins, including their structures, mechanisms of secretion and anchoring and their diverse functions
DNA barcoding of reef brittle stars (Ophiuroidea, Echinodermata) from the southwestern Indian Ocean evolutionary hot spot of biodiversity
In anticipation of the current biodiversity crisis, it has become critical to rapidly and
accurately assess biodiversity. DNA barcoding has proved efficient in facilitating the
discovery and description of thousands of species and also provides insight into the
dynamics of biodiversity. Here, we sequenced a portion of the mitochondrial
cytochrome
c oxidase subunit I (COI) gene from all morphospecies of reef brittle stars
collected during a large-scale
biodiversity survey in the southwestern Indian Ocean
(SWIO). Three methods of species delineation (Automatic Barcode Gap Discovery,
Generalized Mixed Yule Coalescent model, and Bayesian Poisson Tree Processes)
showed concordant results and revealed 51 shallow reef species in the region. Mean
intraspecific genetic distances (0.005–0.064) and mean interspecific genetic distances
within genera (0.056–0.316) were concordant with previous echinoderm studies. This
study revealed that brittle-star
biodiversity is underestimated by 20% within SWIO
and by >40% when including specimens from the Pacific Ocean. Results are discussed
in terms of endemism, diversification processes, and conservation implications for the
Indo-West
Pacific marine biodiversity. We emphasize the need to further our knowledge

Management of cerebral azole-resistant Aspergillus fumigatus infection : a role for intraventricular liposomal-amphotericin B
Objectives: In the pre-azole era, central nervous system (CNS) infections with Aspergillus had a dismal outcome. Survival improved with voriconazole but CNS infections caused by azole-resistant Aspergillus fumigatus preclude its use. Intravenous liposomal-amphotericin B (L-AmB) is the preferred treatment option for azole-resistant CNS infections but has suboptimal brain concentrations. Methods: We describe three patients with biopsy-proven CNS aspergillosis where intraventricular L-AmB was added to systemic therapy. Two patients with azole-resistant aspergillosis and one patient with azole-susceptible CNS aspergillosis were treated with intraventricular L-AmB at a dose of 1 mg weekly. Results: We describe three patients successfully treated with a combination of intravenous and intraventricular L-AmB. All three patients survived but one patient developed serious headaches, most likely not related to this treatment. Conclusions: Intraventricular L-AmB may have a role in the treatment of therapy-refractory CNS aspergillosis when added to systemic therapy. (C) 2020 The Author(s). The Opportunistic Pathogen Propionibacterium acnes: Insights into Typing, Human Disease, Clonal Diversification and CAMP Factor Evolution
We previously described a Multilocus Sequence Typing (MLST) scheme based on eight genes that facilitates population
genetic and evolutionary analysis of P. acnes. While MLST is a portable method for unambiguous typing of bacteria, it is
expensive and labour intensive. Against this background, we now describe a refined version of this scheme based on two
housekeeping (aroE; guaA) and two putative virulence (tly; camp2) genes (MLST4) that correctly predicted the phylogroup
(IA1, IA2, IB, IC, II, III), clonal complex (CC) and sequence type (ST) (novel or described) status for 91% isolates (n = 372) via
cross-referencing of the four gene allelic profiles to the full eight gene versions available in the MLST database (http://
pubmlst.org/pacnes/). Even in the small number of cases where specific STs were not completely resolved, the MLST4
method still correctly determined phylogroup and CC membership. Examination of nucleotide changes within all the MLST
loci provides evidence that point mutations generate new alleles approximately 1.5 times as frequently as recombination;
although the latter still plays an important role in the bacterium’s evolution. The secreted/cell-associated ‘virulence’ factors
tly and camp2 show no clear evidence of episodic or pervasive positive selection and have diversified at a rate similar to
housekeeping loci. The co-evolution of these genes with the core genome might also indicate a role in commensal/normal
existence constraining their diversity and preventing their loss from the P. acnes population. The possibility that members of
the expanded CAMP factor protein family, including camp2, may have been lost from other propionibacteria, but not P.
acnes, would further argue for a possible role in niche/host adaption leading to their retention within the genome. These
evolutionary insights may prove important for discussions surrounding camp2 as an immunotherapy target for acne, and
the effect such treatments may have on commensal lineages
