    Use of cognitive mapping in the diagnosis of tolerance failure

    The management of construction tolerances is a necessary and routine part of the construction activity and is normally brought to our attention only when failures are reported. In a study of tolerance management, the authors found widespread evidence of the same failures and the reasons for them. There seems to be no shortage of experience of the effects of failures in tolerances or of knowledge about how to avoid them. The situation is frustrating for all involved, especially the owner, end-users, designers and operatives. In questioning practitioner experts in this field, the authors identified a misalignment in the perception of ‘problem, cause and effect’. In workshops involving experts from various construction backgrounds, the issue of tolerance management and, in particular, failures and their causes were examined. The experts were introduced to the concept of fault diagnosis using backwards-chaining ‘cause and effect’ analysis. Experts were then asked to undertake several analyses of their own of preselected failures using a cognitive mapping tool. The purpose of the study is to see how useful the method is among the experts and later be able to identify the root causes to the issues of tolerance management. The preliminary results showed that the experts were initially reluctant to break with discussing the effects and what they saw as the solutions, but gradually began to trace the causes backwards until they believed they had identified the root causes. The results show a possibility to reach beyond the obvious problems and therefore as a consequence be able to find a new approach in the following steps of the research process. This is a proven working method for research problems where the interaction with partners in the industry is of great importance

    COMPARISON OF CONSTRUCTION RELATED REWORK IN UGANDA AND MOZAMBIQUE

    The realisation of the expected minimum client quality requirements in both the products and processes in the construction industry remains challenging. The effects have been increased wastage and value loss through rework in both public and private sector initiatives providing social services in developing countries. The purpose of the research was to compare the impact of construction related rework on project budgets and schedule in public building construction in both Uganda and Mozambique, being examples of developing countries. The objective of such comparison is to document where focus should probably be placed in ensuring that quality requirements in construction are achieved. Case study approach was adopted and separate case study protocols prepared. The scope of study in the Ugandan project involved construction of classroom blocks, health centres and staff housing between 2008 and 2011. The scope of study in the Mozambique project involved the construction of 209 public housing units in Mozambique. The units of study were rework-related project budget and schedule overruns with further comparison of rework-related impacts on both project budgets and schedules under study. In Uganda, the mean percentage rework range was 12.45% - 15.58% of the construction contract scope. It was determined that the mean percentage of rework-related impact on project budget and schedule was 4.53% and 8.42% respectively. In Mozambique, the mean percentage rework range was 3.35% - 4.40% of the construction contract scope. It was determined that the mean percentage of rework-related impact on project budget and schedule was 0.56% and12.0% respectively. Acknowledging the differing cultural contexts of both countries where the study was conducted, these findings could improve governance and strengthen the regulatory framework on quality management in both Uganda andMozambique.&nbsp

    Predictors of vitamin D status and its association with parathyroid hormone in young New Zealand children.

    BACKGROUND: Despite increased awareness of the adverse health effects of low vitamin D status, few studies have evaluated 25-hydroxyvitamin D [25(OH)D] status in young children. OBJECTIVES: We aimed to assess vitamin D status on the basis of 25(OH)D and its relation with parathyroid hormone (PTH) and to identify possible predictors of 25(OH)D status in young children living in a country with minimal vitamin D fortification. DESIGN: Serum 25(OH)D and PTH concentrations were measured in a cross-sectional sample of children aged 12-22 mo [n = 193 for 25(OH)D, n = 144 for PTH] living in Dunedin, New Zealand (latitude: 45 degrees S). Anthropometric, dietary, and sociodemographic data were collected. RESULTS: The majority of children sampled in the summer (94%; 47 of 50) had 25(OH)D >50 nmol/L; however, nearly 80% of children sampled in the winter (43 of 55) had serum concentrations 60-65 nmol/L, a plateau in PTH was evident. CONCLUSIONS: Seasonal variation in 25(OH)D concentration implies that postsummer vitamin D stores were insufficient to maintain status >50 nmol/L year-round. Examination of the predictors of 25(OH)D in our model shows few modifiable risk factors, and thus effective dietary strategies may be required if future research determines that children with 25(OH)D concentrations <50 nmol/L are at significant health risk.     The response of perennial and temporary headwater stream invertebrate communities to hydrological extremes

    The headwaters of karst rivers experience considerable hydrological variability, including spates and streambed drying. Extreme summer flooding on the River Lathkill (Derbyshire, UK) provided the opportunity to examine the invertebrate community response to unseasonal spate flows, flow recession and, at temporary sites, streambed drying. Invertebrates were sampled at sites with differing flow permanence regimes during and after the spates. Following streambed drying at temporary sites, dewatered surface sediments were investigated as a refugium for aquatic invertebrates. Experimental rehydration of these dewatered sediments was conducted to promote development of desiccation-tolerant life stages. At perennial sites, spate flows reduced invertebrate abundance and diversity, whilst at temporary sites, flow reactivation facilitated rapid colonisation of the surface channel by a limited number of invertebrate taxa. Following streambed drying, 38 taxa were recorded from the dewatered and rehydrated sediments, with Oligochaeta being the most abundant taxon and Chironomidae (Diptera) the most diverse. Experimental rehydration of dewatered sediments revealed the presence of additional taxa, including Stenophylax sp. (Trichoptera: Limnephilidae) and Nemoura sp. (Plecoptera: Nemouridae).     Oncoplastic Breast Consortium consensus conference on nipple-sparing mastectomy

    Purpose Indications for nipple-sparing mastectomy (NSM) have broadened to include the risk reducing setting and locally advanced tumors, which resulted in a dramatic increase in the use of NSM. The Oncoplastic Breast Consortium consensus conference on NSM and immediate reconstruction was held to address a variety of questions in clinical practice and research based on published evidence and expert panel opinion. Methods The panel consisted of 44 breast surgeons from 14 countries across four continents with a background in gynecology, general or reconstructive surgery and a practice dedicated to breast cancer, as well as a patient advocate. Panelists presented evidence summaries relating to each topic for debate during the in-person consensus conference. The iterative process in question development, voting, and wording of the recommendations followed the modified Delphi methodology. Results Consensus recommendations were reached in 35, majority recommendations in 24, and no recommendations in the remaining 12 questions. The panel acknowledged the need for standardization of various aspects of NSM and immediate reconstruction. It endorsed several oncological contraindications to the preservation of the skin and nipple. Furthermore, it recommended inclusion of patients in prospective registries and routine assessment of patient-reported outcomes. Considerable heterogeneity in breast reconstruction practice became obvious during the conference. Conclusions In case of conflicting or missing evidence to guide treatment, the consensus conference revealed substantial disagreement in expert panel opinion, which, among others, supports the need for a randomized trial to evaluate the safest and most efficacious reconstruction techniques

    Association between convalescent plasma treatment and mortality in COVID-19: a collaborative systematic review and meta-analysis of randomized clinical trials.

    Funder: laura and john arnold foundationBACKGROUND: Convalescent plasma has been widely used to treat COVID-19 and is under investigation in numerous randomized clinical trials, but results are publicly available only for a small number of trials. The objective of this study was to assess the benefits of convalescent plasma treatment compared to placebo or no treatment and all-cause mortality in patients with COVID-19, using data from all available randomized clinical trials, including unpublished and ongoing trials (Open Science Framework, https://doi.org/10.17605/OSF.IO/GEHFX ). METHODS: In this collaborative systematic review and meta-analysis, clinical trial registries (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform), the Cochrane COVID-19 register, the LOVE database, and PubMed were searched until April 8, 2021. Investigators of trials registered by March 1, 2021, without published results were contacted via email. Eligible were ongoing, discontinued and completed randomized clinical trials that compared convalescent plasma with placebo or no treatment in COVID-19 patients, regardless of setting or treatment schedule. Aggregated mortality data were extracted from publications or provided by investigators of unpublished trials and combined using the Hartung-Knapp-Sidik-Jonkman random effects model. We investigated the contribution of unpublished trials to the overall evidence. RESULTS: A total of 16,477 patients were included in 33 trials (20 unpublished with 3190 patients, 13 published with 13,287 patients). 32 trials enrolled only hospitalized patients (including 3 with only intensive care unit patients). Risk of bias was low for 29/33 trials. Of 8495 patients who received convalescent plasma, 1997 died (23%), and of 7982 control patients, 1952 died (24%). The combined risk ratio for all-cause mortality was 0.97 (95% confidence interval: 0.92; 1.02) with between-study heterogeneity not beyond chance (I2 = 0%). The RECOVERY trial had 69.8% and the unpublished evidence 25.3% of the weight in the meta-analysis. CONCLUSIONS: Convalescent plasma treatment of patients with COVID-19 did not reduce all-cause mortality. These results provide strong evidence that convalescent plasma treatment for patients with COVID-19 should not be used outside of randomized trials. Evidence synthesis from collaborations among trial investigators can inform both evidence generation and evidence application in patient care
