    Sand deposits reveal great earthquakes and tsunamis at Mexican Pacific Coast

    Globally, instrumentally based assessments of tsunamigenic potential of subduction zones have underestimated the magnitude and frequency of great events because of their short time record. Historical and sediment records of large earthquakes and tsunamis have expanded the temporal data and estimated size of these events. Instrumental records suggests that the Mexican Subduction earthquakes produce relatively small tsunamis, however historical records and now geologic evidence suggest that great earthquakes and tsunamis have whipped the Pacific coast of Mexico in the past. The sediment marks of centuries old-tsunamis validate historical records and indicate that large tsunamigenic earthquakes have shaken the Guerrero-Oaxaca region in southern Mexico and had an impact on a bigger stretch of the coast than previously suspected. We present the first geologic evidence of great tsunamis near the trench of a subduction zone previously underestimated as potential source for great earthquakes and tsunamis. Two sandy tsunami deposits extend over 1.5 km inland of the coast. The youngest tsunami deposit is associated with the 1787 great earthquake, M 8.6, producing a giant tsunami that poured over the coast flooding 500 km alongshore the Mexican Pacific coast and up to 6 km inland. The oldest event from a less historically documented event occurred in 1537. The 1787 earthquake, and tsunami and a probable predecessor in 1537, suggest a plausible recurrence interval of 250 years. We prove that the common believe that great tsunamis do not occur on the Mexican Pacific coast cannot be sustained

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    Recommendations for treatment with recombinant human growth hormone in pediatric patients in Colombia

    En Colombia, actualmente no existen parámetros claros para el diagnóstico de pacientes con talla baja, ni sobre el tratamiento de esta población con hormona de crecimiento recombinante humana (somatropina), lo cual se ve favorecido por la diversidad de programas de formación de profesionales en endocrinología pediátrica. En respuesta a esta problemática se realizó el primer acuerdo colombiano de expertos en talla baja liderado por la Asociación Colegio Colombiana de Endocrinología Pediátrica (ACCEP); este trabajo contó con la participación y el aval de expertos clínicos de importantes instituciones de salud públicas y privadas del país, además de expertos metodológicos del instituto Keralty, quienes garantizaron la estandarización del uso de la somatropina. Después de realizar una minuciosa revisión de la literatura, se propone la unificación de definiciones, un algoritmo diagnóstico, los parámetros de referencia de las pruebas bioquímicas y dinámicas, una descripción de las consideraciones de uso de la somatropina para el tratamiento de las patologías con aprobación por la entidad regulatoria de medicamentos y alimentos en Colombia y, por último, un formato de consentimiento informado y de ficha técnica del medicamento.In Colombia there are no guidelines for diagnosis and management of patients with short stature and for the use of recombinanthuman growth hormone, mainly caused by the diversity of training centers in pediatric endocrinology.     El informe ha sido elaborado por la Sección Juvenil de la Asociación Española de Derecho del Trabajo y Seguridad SocialEste Informe deja constancia de los cambios normativos más relevantes y de las tendencias judiciales más paradigmáticas del ordenamiento laboral en 2015. En él se observa el imparable dinamismo del Derecho del Trabajo y de la Seguridad Social en España.     First report of fossilized cysts produced by the benthic Bysmatrum subsalsum (Dinophyceae) from a shallow Mexican lagoon in the Gulf of Mexico

    Cysts belonging to the benthic dinoflagellate Bysmatrum subsalsum were recovered from palynologically treated sediments collected in the Alvarado Lagoon (southwestern Gulf of Mexico). The cysts are proximate, reflecting the features of the parent thecal stage, and their autofluorescence implies a dinosporin composition similar to the cyst walls of phototrophic species. This finding is important for our understanding of B. subsalsum life cycle transitions and ecology. Encystment may play an important role in the bloom dynamics of this species as it can enable the formation of a sediment cyst bank that allows reinoculation of the water column when conditions become favorable. This is the first report of a fossilized cyst produced by a benthic dinoflagellate recovered from sub-recent sediments

    One century of sedimentation and Hg pollution at the mouth of the Sagua la Grande River (Cuba)

    The Sagua la Grande River is the largest of northern Cuba. The socioeconomic development in this region during the last 100 years has caused changes in its natural conditions, such as an increase in Hg levels. In 1981, a chlor-alkali plant with mercury-cell technology was built on the river bank and has released several contaminants to the environment. Sedimentation along the river and the nearby coastal zone was reconstructed using the radionuclides 210Pb, 226Ra, 137Cs, and 239,240Pu in sediment cores. The changes in sediment accumulation were related to important changes in the river basin after 1950, such as the channeling of the river in 1955 and the construction of the Alacranes Dam in 1972. Maximum sediment accumulation rates were attributed to large floods in 1933, 1948, and 1985 (Hurricane Kate). The 210Pb chronology allowed the reconstruction of Hg pollution during the last four decades. In the locations with greatest fluvial influence, Hg concentrations increased since the early 1980s exceeding the Effects Range-Median (0.71 mg kg–1). The accumulated Hg inventories released from the plant were one order of magnitude larger than those due to other sources. These results show the impact of Hg releases from this industry on the estuary, especially in the coastal zone close to the river mouth.
