    The World Press Photo exhibition brings together, every year during the last eleven, the more representative set of photojournalism of the previous year. The collection of the winners in each edition represents, if one so wishes, an opportunity to reflect on the impact of the images in the public observer, beyond the aesthetic judgment in a particular time and space. This article aims to leave an open window to reflection. An approach on how snapshots shown are supported by three points: the photographer, the spectator and the surrounding social context. A triangle that inevitably possesses a constant ethical conflict: the duty between informing or helping, being anesthetized through the “compassion fatigue” or go beyond the initial discomfort when observing the pain of others and react against it. Human frailty observed as a driver or a deterrent of a broader social action; beyond the photographer, beyond the visit to an exhibition.ext of the abstract.  La exposición del World Press Photo reúne, cada año desde hace once, al conjunto de fotoperiodismo más representativo del año anterior. Este artículo pretende dejar una ventana abierta hacía dicha reflexión.     Change and predictors of quality of life in institutionalized older adults with dementia

    PURPOSE: This study aims to assess the change in and predictive factors of the quality of life (QoL) of institutionalized older adults with dementia over a 20-month period. METHODS: Information was used from a follow-up study conducted over an average period of 19.61 ± 1.93 months on a sample of 274 institutionalized older adults aged 60 or over, diagnosed with dementia. Two linear regression models were built to predict change in the EQ-5D index and the quality of life in Alzheimer's disease (QOL-AD) scale, taking as independent variables: sociodemographic characteristics and measures of functional ability (Barthel Index), depression in dementia (Cornell Scale), number of chronic health problems, cognitive level (MEC, the Spanish Mini-Mental State Examination) and severity of dementia (Clinical Dementia Rating) at baseline. RESULTS: The majority of the participants were women (81.75 %) with an average age of 84.70 ± 6.51 years, single (78.15 %), with severe dementia and moderate functional dependence. There was a significant decrease on the EQ-5D, EQ-VAS and QOL-AD between baseline and follow-up scores. The main predictors of QoL of the institutionalized older adults with dementia were the number of chronic problems and baseline scores of the QoL measures. CONCLUSIONS: A significant decrease in the QoL of institutionalized older adults was observed over a 20-month period. Results suggest that interventions aimed at reducing the number of chronic medical conditions may have a beneficial effect on older adults' QoL.This study was funded by the CIEN Foundation, Carlos III Institute of Health (Ref.     Magnetosomes could be protective shields against metal stress in magnetotactic bacteria

    Magnetotactic bacteria are aquatic microorganisms with the ability to biomineralise membrane-enclosed magnetic nanoparticles, called magnetosomes. These magnetosomes are arranged into a chain that behaves as a magnetic compass, allowing the bacteria to align in and navigate along the Earth's magnetic field lines. According to the magneto-aerotactic hypothesis, the purpose of producing magnetosomes is to provide the bacteria with a more efficient movement within the stratified water column, in search of the optimal positions that satisfy their nutritional requirements. However, magnetosomes could have other physiological roles, as proposed in this work. Here we analyse the role of magnetosomes in the tolerance of Magnetospirillum gryphiswaldense MSR-1 to transition metals (Co, Mn, Ni, Zn, Cu). By exposing bacterial populations with and without magnetosomes to increasing concentrations of metals in the growth medium, we observe that the tolerance is significantly higher when bacteria have magnetosomes. The resistance mechanisms triggered in magnetosome-bearing bacteria under metal stress have been investigated by means of x-ray absorption near edge spectroscopy (XANES). XANES experiments were performed both on magnetosomes isolated from the bacteria and on the whole bacteria, aimed to assess whether bacteria use magnetosomes as metal storages, or whether they incorporate the excess metal in other cell compartments. Our findings reveal that the tolerance mechanisms are metal-specific: Mn, Zn and Cu are incorporated in both the magnetosomes and other cell compartments; Co is only incorporated in the magnetosomes, and Ni is incorporated in other cell compartments.     The impact of ABCDE bundle implementation on patient outcomes: a nationwide cohort study

    Background: The ABCDE bundle is a set of evidence-based practices to systematically reduce the risks of sedation, delirium, and immobility in intensive care patients. Implementing the bundle improves clinical outcome. Aims and Objectives: To investigate the association between patient outcomes and compliance with bundle components ABC (analgosedation algorithms), D (delirium protocol), and E (early mobilization protocol). Design: A Spanish multicentre cohort study of adult patients receiving invasive mechanical ventilation (IMV) for ≥48 h until extubation. Methods: The primary outcome was pain level, cooperation to permit Medical Research Council Scale administration, patient days of delirium, and mobility. The secondary outcome was cumulative drug dosing by IMV days. Tertiary outcomes (ICU days, IMV days, bed rest days, ICU mortality, ICUAW) and independent variables (analgosedation, delirium, early mobilization protocols) were also studied. Results: Data were collected from 605 patients in 80 ICUs and 5214 patient days with IMV. Two-thirds of the ICUs studied applied no protocols. Pain was not assessed on 83.6% of patient days. Patient cooperation made scale administration feasible on 20.7% of days. Delirium and immobility were found on 4.2% and 69.9% of days, respectively. Patients had shorter stays in ICUs with bundle protocols and fewer days of IMV in ICUs with delirium and mobilization bundle components (P = 0.006 and P = 0.03, respectively). Analgosedation protocols were associated with more opioid dosing (P = 0.02), and delirium and early mobilization protocols with more propofol (P = 0.001), dexmedetomidine (P = 0.001), and lower benzodiazepine dosing (P = 0.008). Conclusions: The implementation rate of ABCDE bundle components was very low in our Spanish setting, but when implemented, patients had a shorter ICU stay, more analgesia dosing, and lighter sedation. Relevance to clinical practice: Applying some but not all the bundle components, there is increased analgesia and light sedation drug use, decreased benzodiazepines, and increased patient cooperation and mobility, resulting in a shorter ICU stay and fewer days of IMV

    Tradução e adaptação transcultural para a língua portuguesa da Cocaine Selective Severity Assessment

    The objective of this study was to translate, to adapt culturally and to verify the semantic equivalence from english to portuguese of the Co­caine Selective Severity Assessment (CSSA), which evaluates the wi­thdrawal for cocaine. The CSSA was translated from English to Portu­guese, administered to 10 persons, and then submitted to brainstorming in a group of 3 psychiatrics for individual and verbal reproduction, item by item. Backtranslation was executed based on first translation and from brainstorming to the origin language. Then, it was translated again into Portuguese. The entire process was analyzed by a committee of psychiatrists with expertise in addiction, which emitted opinions and the pertinent comments. The scale in its general scope was well understood by both patients and professionals. Few changes were done from the first translation to final version in items 3, 4, 5, 10, 11 e 12. To the others items, the original translation was kept. Statements were added to the end of all items. Considering the opinions of the experts and a final pilot study, was constructed the final Brazilian version of the CSSA. Results indicated a satisfactory semantic equivalence between the two versions and so the CSSA may be helpful instrument in treating withdrawal of cocaine / crack.O objetivo deste estudo foi traduzir, adaptar culturalmente e verificar a equivalência semântica do inglês para o português da escala Cocaine Selective Severity Assessment (CSSA), a qual avalia a abstinência de co­caína. A CSSA foi traduzida do inglês para o português, aplicada em 10 pacientes e submetida ao brainstorming em um grupo de 3 psiquiatras para reprodução individual e verbal, item a item. Foi realizada a retrotradução (backtrans­lation) da primeira tradução e do brainstorming, sendo realizada nova tradução para o português. Todo o processo foi analisado por um comitê de psiquiatras com experiência em toxicomania, os quais emitiram pareceres com as observações pertinentes. A escala, em seu âmbito geral, foi bem compreendida pelos pacientes e profissionais. Pou­cas mudanças foram feitas da primeira tradução até a versão final nos itens 3, 4, 5, 10, 11 e 12. Para os demais itens, a tradução original foi mantida. Sentenças foram adicionadas ao final de todos os itens. Considerando os pareceres dos especialistas e um último estudo piloto, construiu-se a versão final brasileira da CSSA. Os resultados indicaram uma equivalência semântica satisfatória entre as versões, podendo a CSSA ser um instrumento útil no tratamento de abstinência de cocaína/crack

    Genome Wide Binding Site Analysis Reveals Transcriptional Coactivation of Cytokinin-Responsive Genes by DELLA Proteins

    [EN] The ability of plants to provide a plastic response to environmental cues relies on the connectivity between signaling pathways. DELLA proteins act as hubs that relay environmental information to the multiple transcriptional circuits that control growth and development through physical interaction with transcription factors from different families. We have analyzed the presence of one DELLA protein at the Arabidopsis genome by chromatin immunoprecipitation coupled to large-scale sequencing and we find that it binds at the promoters of multiple genes. Enrichment analysis shows a strong preference for cis elements recognized by specific transcription factor families. In particular, we demonstrate that DELLA proteins are recruited by type-B ARABIDOPSIS RESPONSE REGULATORS (ARR) to the promoters of cytokinin-regulated genes, where they act as transcriptional co-activators.     Multiscale precipitation variability and extremes over South America: analysis of future changes from a set of CORDEX regional climate model simulations

    The precipitation behaviour at different timescales (interannual, intraseasonal and synoptic) and extreme events in a warmer scenario over South America is analysed. This study is based on a set of Regional Climate Models (RCM) from the CORDEX database and their driving Global Climate Models (GCM) from the CMIP5 Project. Historical simulations for 1979–2005 and from the RCP4.5 scenario for 2071–2100 are used. The projected changes in the precipitation behaviour are evaluated in terms of the consistency between the pairs RCM-GCM aiming to explore the added value of RCMs. The agreement between projected changes from RCMs is also evaluated as a measure of confidence of the regional climate change signal. The analysis is carried out for two extended seasons (April to September and October to March). The projected change in the mean precipitation over subtropical latitudes is associated with both changes in the frequency of rainy days and in the intensity of extreme events, while for the tropics the changes are mainly associated with changes in the wet day frequency. The increase of extreme precipitation events over the subtropics arises as a robust signal among models, while for tropical latitudes the dispersion among models is high, which reduces the confidence of the projections. In general, the consistency of the projections in the pairs RCM-GCM and the agreement among models are higher for the low frequency variability patterns during winter, while for summertime a better agreement in the projected changes of the precipitation behaviour at different timescales among RCMs is found.Fil: Blazquez, Josefina.
