    Plasma lipid profile, atherogenic and coronary risk indices in some residents of Abeokuta in south-western Nigeria

    The incidence of chronic degenerative diseases like stroke and myocardial infarction in African subpopulations is reported to be increasing. In view of the association between dyslipidemia and these chronic degenerative diseases, we investigated some well-established cardiovascular risk factors (plasma cholesterol and its fractions as high-density lipoprotein (HDL), low-density lipoprotein (LDL), triglyceride, adiposity and blood pressure) in 92 subjects (43 males and 49 females) in Abeokuta, Nigeria. LDL cholesterol was significantly higher in the women compared with men (128.58mg/dl vs 108.73mg/dl; p = 0.028). Total cholesterol, although slightly higher in women, was not significantly different from that of men (155.71mg/dl vs 147.26mg/dl). HDL cholesterol and triglyceride were not significantly different between the two sexes, although women tended to have lower HDL cholesterol when compared with men. LDL/HDL cholesterol and total cholesterol/HDL cholesterol were significantly higher in women compared with men (4.20 vs 2.97; p = 0.004; 5.03 vs 4.06; p = 0.024, respectively). Systolic blood pressure was higher in men compared with women (117.58 vs 104.84; p = 0.002). Results indicate that the association between cholesterol and chronic degenerative diseases is continuous with no single cholesterol level separating those who are at high risk from those who are not. Rather in defining cardiovascular risks in African populations, the ratios LDL/HDL cholesterol and total cholesterol/HDL cholesterol should be considered. It might be appropriate at this time in Nigeria to consider physical activity and pharmacological interventions in lowering blood lipids

    Plasma lipid profiles and risk of cardiovascular disease in occupational lead exposure in Abeokuta, Nigeria

    In order to investigate the effects of lead exposure on risk of cardiovascular disease during occupational exposure to this metal, plasma cholesterol and its fractions as high-density liporotein (HDL), low-density liporotein (LDL) and triglyceride were determined in various artisans in Abeokuta, Nigeria who have been shown to be occupationally exposed to lead and these were related to blood lead levels. Increased risk of cardiovascular disease was observed in the artisans. Total cholesterol in the artisans was between 1.5 and 2.0 times higher in the artisans than that present in controls while LDL cholesterol was between 1.6 and 2.4 times higher in the artisans when compared with control subjects [p < 0.001]. HDL cholesterol and triglyceride levels were not affected [p > 0.05]. A significant positive correlation was observed between blood lead and total cholesterol on one hand [r = 0.372; p = 3.0 × 10(-5)] and blood lead and LDL cholesterol on the other hand [r = 0.283; p = 0.001]. LDL/HDL cholesterol ratio was also higher in the artisans when compared with control. Blood pressure (systolic and diastolic) and other anthropometric parameters were not significantly different between the artisans and the control subjects [p > 0.05]. Results suggest that lead exposure increases cholesterol synthesis and transport to peripheral tissues whereas reverse cholesterol transport to the liver is not affected

    Effects of Fibre-Enriched Diets on Tissue Lipid Profiles of MSG Obese Rats

    In order to investigate the influence of some fibre-enriched diets on tissue lipids in an animal model of obesity induced by the administration of monosodium glutamate (MSG), obese rats were fed diets containing 30% of Acha, Cassava, Maize and Plantain for five weeks and weight gain, feed intake and lee index were recorded. The lipid profiles of plasma, erythrocytes, kidney, heart and liver as well as hepatic 3- hydroxyl-3-methylglutaryl-CoA (HMG-CoA) reductase activity were measured. The diets significantly (p < 0.05) reduced weight gain and lee index in the obese rats. Obesity-induced increase in plasma and erythrocytes lipid levels was significantly (p < 0.05) reduced by these diets. MSG-induced obesity also resulted in a significant increase (p < 0.05) in hepatic cholesterol level which was reduced by the diets. MSG-obesity was characterised by a significant (p < 0.05) increase in cholesterol, triacylglycerol and phospholipids in kidney and this was reversed by the diets except Maize which did not reverse the increased cholesterol level. Only Acha reversed the obesity-induced increase in heart cholesterol and phospholipids. The increased activity of hepatic HMG-CoA reductase associated with obesity was also significantly (p < 0.05) reduced by the diets. In conclusion, dyslipidemia associated with MSG-induced obesity could be attenuated by consumption of fibre-enriched diet

    Beneficial effects of omega-3 fatty acid on dyslipidemia in organs of alloxan-induced diabetic rats

    Diabetes Mellitus is one of the heterogeneous metabolic disorders associated with dyslipidemia, a major risk factor contributing to cardiovascular disease. This metabolic abnormality affects virtually all organs. Over the years, antidiabetic drugs which majorly aim at the hyperglycemic aspect of the disease have been used. Therefore, to address this dyslipidemia, omega-3 fatty acid (O3FA) supplement was employed. Its effects on lipid metabolism in the organs (heart, liver, kidney, pancreas, spleen and brain) of alloxan-induced (150mg/kg body weight, intraperitoneally) diabetic male rats were investigated. O3FA (0.4g/kg b.wt/day) was administered as pre- and posttreatment for 2 weeks. The lipid levels were significantly increased (p<0.05) in diabetic rats. O3FA administration significantly reduced (p<0.05) the levels of cholesterol, phospholipids, triacylglycerol by varying extents, in the examined organs without affecting hyperglycemia in the diabetic rats. The ratio of HMG CoA/mevalonate decreased in the liver of the diabetic rats by 28% indicating increased activity of HMG-CoA reductase. This diabetes-induced dyslipidemia was accompanied by a 28% increase in the activity of hepatic HMG-CoA reductase. Administration of O3FA to the diabetic rats however resulted in 10% and 17% decrease in the activity of this enzyme in the pre- and post-treated groups respectively. Also, lipid peroxidation was significantly reduced (p<0.05) by O3FA suggesting that it has protective effect against oxidative damage. This study reveals that O3FAsupplement has beneficial effects in attenuating dyslipidemia observed in diabetes mellitus and could be beneficial as an adjunct in the management of diabetes mellitus

    OPTIMA prelim: a randomised feasibility study of personalised care in the treatment of women with early breast cancer

    Abstract Background There is uncertainty about the chemotherapy sensitivity of some oestrogen receptor (ER)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancers. Multiparameter assays that measure the expression of several tumour genes simultaneously have been developed to guide the use of adjuvant chemotherapy for this breast cancer subtype. The assays provide prognostic information and have been claimed to predict chemotherapy sensitivity. There is a dearth of prospective validation studies. The Optimal Personalised Treatment of early breast cancer usIng Multiparameter Analysis preliminary study (OPTIMA prelim) is the feasibility phase of a randomised controlled trial (RCT) designed to validate the use of multiparameter assay directed chemotherapy decisions in the NHS. Objectives OPTIMA prelim was designed to establish the acceptability to patients and clinicians of randomisation to test-driven treatment assignment compared with usual care and to select an assay for study in the main RCT. Design Partially blinded RCT with adaptive design. Setting Thirty-five UK hospitals. Participants Patients aged ≥ 40 years with surgically treated ER-positive HER2-negative primary breast cancer and with 1–9 involved axillary nodes, or, if node negative, a tumour at least 30 mm in diameter. Interventions Randomisation between two treatment options. Option 1 was standard care consisting of chemotherapy followed by endocrine therapy. In option 2, an Oncotype DX® test (Genomic Health Inc., Redwood City, CA, USA) performed on the resected tumour was used to assign patients either to standard care [if ‘recurrence score’ (RS) was > 25] or to endocrine therapy alone (if RS was ≤ 25). Patients allocated chemotherapy were blind to their randomisation. Main outcome measures The pre-specified success criteria were recruitment of 300 patients in no longer than 2 years and, for the final 150 patients, (1) an acceptance rate of at least 40%; (2) recruitment taking no longer than 6 months; and (3) chemotherapy starting within 6 weeks of consent in at least 85% of patients. Results Between September 2012 and 3 June 2014, 350 patients consented to join OPTIMA prelim and 313 were randomised; the final 150 patients were recruited in 6 months, of whom 92% assigned chemotherapy started treatment within 6 weeks. The acceptance rate for the 750 patients invited to participate was 47%. Twelve out of the 325 patients with data (3.7%, 95% confidence interval 1.7% to 5.8%) were deemed ineligible on central review of receptor status. Interviews with researchers and recordings of potential participant consultations made as part of the integral qualitative recruitment study provided insights into recruitment barriers and led to interventions designed to improve recruitment. Patient information was changed as the result of feedback from three patient focus groups. Additional multiparameter analysis was performed on 302 tumour samples. Although Oncotype DX, MammaPrint®/BluePrint® (Agendia Inc., Irvine, CA, USA), Prosigna® (NanoString Technologies Inc., Seattle, WA, USA), IHC4, IHC4 automated quantitative immunofluorescence (AQUA®) [NexCourse BreastTM (Genoptix Inc. Carlsbad, CA, USA)] and MammaTyper® (BioNTech Diagnostics GmbH, Mainz, Germany) categorised comparable numbers of tumours into low- or high-risk groups and/or equivalent molecular subtypes, there was only moderate agreement between tests at an individual tumour level (kappa ranges 0.33–0.60 and 0.39–0.55 for tests providing risks and subtypes, respectively). Health economics modelling showed the value of information to the NHS from further research into multiparameter testing is high irrespective of the test evaluated. Prosigna is currently the highest priority for further study. Conclusions OPTIMA prelim has achieved its aims of demonstrating that a large UK clinical trial of multiparameter assay-based selection of chemotherapy in hormone-sensitive early breast cancer is feasible. The economic analysis shows that a trial would be economically worthwhile for the NHS. Based on the outcome of the OPTIMA prelim, a large-scale RCT to evaluate the clinical effectiveness and cost-effectiveness of multiparameter assay-directed chemotherapy decisions in hormone-sensitive HER2-negative early breast would be appropriate to take place in the NHS

    Oxidative Stress in Extrahepatic Tissues of Rats Co-Exposed to Aflatoxin B1 and Low Protein Diet

    Early life exposure to aflatoxin B1 (AFB1) and low protein diet through complementary foods during weaning is common in parts of Africa and Asia. This study evaluated the effect of co-exposure to AFB1 and low protein diet on the extrahepatic tissues of rats. Twenty-four three-week old weanling male albino rats were used for this study and were randomly assigned into four groups: group 1 served as control and was fed normal protein diet (20% protein), group 2 was fed low protein diet (5% protein), group 3 was fed normal protein diet + 40 ppb AFB1 while group 4 received low protein diet + 40 ppb AFB1, all for eight weeks. Afterward, biomarkers of anemia (packed cell volume (PCV), hemoglobin) and kidney function (urea, uric acid, and creatinine) were determined in the blood while biomarkers of oxidative stress were determined in the tissues spectrophotometrically. Co-exposure to AFB1 and low protein diet significantly (p < 0.05) decreased body weight gain and PCV, increased biomarkers of kidney functions and induced oxidative stress in the tissues studied. There was significant (p < 0.05) reduction in glutathione concentration while TBARS was significantly increased in the tissues. Co-exposure to AFB1 and low protein diet had additive effects on decreasing the weight gain and potentiation effect of kidney dysfunction in the rats. The co-exposure also decreased antioxidant enzymes and increased oxidant status in the tissues. Our results demonstrate that this co-exposure has deleterious health effects on extrahepatic tissues and should be a public health concern especially in developing countries where AFB1 contamination is common

    Neonatal neuroblastoma with adrenal primary and metastasis to the liver: A case report and a review of literature

    We report the case of a 23-day-old neonate with neuroblastoma (NBL) in the right adrenal gland and widespread metastases to the liver. This raises the possibility of foetal NBL, which was missed during periodic ultrasonography done during the mother's pregnancy. We hope that this report would increase the awareness of physicians about foetal, congenital and neonatal NBL; and of sonographers about space-occupying lesions in the foetus. The clinicopathologic features and the management of neonatal NBL are discussed
