    Perioperative hazards in myotonic dystrophy

    A 32-year-old man with myotonic dystrophy underwent a thoracotomy for removal of a mediastinal thymoma. Pre-operative examination revealed features of myotonic dystrophy, the only other abnormality was mild restrictive pulmonary disease. Anaesthesia was induced with thiopentone and maintained with enflurane, nitrous oxide, oxygen and curare. Following surgery, the patient was mechanically ventilated for several hours and remained intubated for 9 hours. The anaesthesia and surgery were tolerated well; however, postoperative complications included refractory dysrhythmias, hypoxia, pneumoccocal pneumonia and pulmonary emboli. More intensive preoperative pulmonary evaluation and physiotherapy coupled with more aggressive postoperative pulmonary care might have resulted in a smoother recovery phase.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/75595/1/j.1365-2044.1980.tb03827.x.pd

    Predicting intention and behaviour following participation in a theory-based intervention to improve gluten free diet adherence in coeliac disease

Objective: To determine whether changes in theory of planned behaviour (TPB) constructs could predict intention and gluten-free diet (GFD) adherence following participation in an online theory-based intervention designed to improve adherence in coeliac disease. Design: Theory-based process evaluation of the mechanisms of change over the course of a six-week online intervention. Measures of GFD adherence and TPB variables were administered at baseline and follow-up (immediate post-intervention: n = 74; three-month: n = 68; six-month: n = 65). Hierarchical regression analyses using residualised change scores were conducted at each time point (dependent variables: intention and adherence). Results: Baseline intention and GFD adherence were the strongest predictors of follow-up intention and adherence, respectively. Change in attitude accounted for significant variance in intention. Change in intention accounted for significant variance in GFD adherence immediately post-intervention; by the six-month follow-up change in perceived behavioural control was the stronger predictor. Conclusions: Partial support for the hypotheses suggests that, for certain behaviours, the TPB may be relevant in explaining the mechanism of action responsible for changes in intention and behaviour following participation in a behaviour change intervention. Additional predictive pathways are also likely to exist and, in the area of GFD adherence, may include habit strength and actual behavioural control

    Unraveling the complex polymorphic crystallization behavior of the alternating copolymer DMDS-alt-DVE

    [Abstract]: A complex crystallization behavior was observe the alternating copolymer DMDS-alt-step-growth polymerization. Understanding the underlying complex crystallization processes of such innovative polythioethers is critical for their application, for example, in polymer coating technologies. These alternating copolymers have polymorphic traits, resulting in different phases that may display distinct crystalline structures. The copolymer DMDS-alt-DVE was studied in an earlier work, where only two crystalline phases were reported: a low melting, L − Tm, and high melting, H − Tm phase. Remarkably, the H − Tm form was only achieved by the previous formation and melting of the L − Tm form. We applied calorimetric techniques encompassing seven orders of magnitude in scanning rates to further explore this complex polymorphic behavior. Most importantly, by rapidly quenching the sample to temperatures well below room temperature, we detected an additional polymorphic form (characterized by a very low melting phase, denoted VL − Tm). Moreover, through tailored thermal protocols, we successfully produced samples containing only one, two, or all three polymorphs, providing insights into their interrelationships.     The association between the inflammatory response to surgery and postoperative complications in older patients with cancer; a prospective prognostic factor study

    BACKGROUND: Accurate prognostic biomarkers would substantially improve surgical planning and decisions making yet no studies have been reported exploring the inflammatory response in surgically treated older patients with cancer. The aim of this study was to explore inflammatory biomarkers as potential prognostic factors for postoperative complications within 30 days in older patients with cancer. METHOD: Patients 65 years and older undergoing surgery for removal of a solid malignant tumour were included in an observational cohort study. All complications occurring up to 30 days postoperatively were documented prospectively. Inflammatory markers were measured in plasma samples pre- and postoperatively: C-reactive protein (CRP), Interleukin-1 beta (IL-1ÎČ), IL-6, IL-10, IL-12, and Tumour necrosis factor-alpha (TNF-α). Associations between inflammatory markers and postoperative complications were explored using logistic regression analysis. RESULTS: Between July 2010 and April 2014, plasma samples of 224 patients were collected. Median age was 72 (65-89) years and 116 (51.8%) patients were female. Approximately half of the patients developed postoperative complications (49.6%) of whom 62 patients (55.9%) developed >1 complication. An independent prognostic effect was observed for the inflammatory biomarkers IL-6 and IL-10 for the occurrence of postoperative complications. CONCLUSION: The perioperative inflammatory response is associated with complications, independently from patient and surgical factors which are also associated with outcome. Research is warranted towards further exploration of the perioperative inflammatory response with the aim to improve perioperative care and outcome, and might help to improve surgical planning and decision making for older patients with cancer

    Answering Twitter Questions: a Model for Recommending Answerers through Social Collaboration

    International audienceIn this paper, we specifically consider the challenging task of solving a question posted on Twitter. The latter generally remains unanswered and most of the replies, if any, are only from members of the questioner's neighborhood. As outlined in previous work related to community Q&A, we believe that question-answering is a collaborative process and that the relevant answer to a question post is an aggregation of answer nuggets posted by a group of relevant users. Thus, the problem of identifying the relevant answer turns into the problem of identifying the right group of users who would provide useful answers and would possibly be willing to collaborate together in the long-term. Accordingly, we present a novel method, called CRAQ, that is built on the collaboration paradigm and formulated as a group entropy optimization problem. To optimize the quality of the group, an information gain measure is used to select the most likely " informative " users according to topical and collaboration likelihood predictive features. Crowd-based experiments performed on two crisis-related Twitter datasets demonstrate the effectiveness of our collaborative-based answering approach

    The impact of interactions, bars, bulges, and AGN on star formation efficiency in local massive galaxies

    Using observations from the GASS and COLD GASS surveys and complementary data from SDSS and GALEX, we investigate the nature of variations in gas depletion time observed across the local massive galaxy population. The large and unbiased COLD GASS sample allows us to assess the relative importance of galaxy interactions, bar instabilities, morphologies and the presence of AGN in regulating star formation efficiency. Both the H2 mass fraction and depletion time vary as a function of the distance of a galaxy from the main sequence in the SFR-M* plane. The longest gas depletion times are found in below-main sequence bulge-dominated galaxies that are either gas-poor, or else on average less efficient than disk-dominated galaxy at converting into stars any cold gas they may have. We find no link between AGN and these long depletion times. The galaxies undergoing mergers or showing signs of morphological disruptions have the shortest molecular gas depletion times, while those hosting strong stellar bars have only marginally higher global star formation efficiencies as compared to matched control samples. Our interpretation is that depletion time variations are caused by changes in the ratio between the gas mass traced by the CO(1-0) observations, and the gas mass in high density star-forming cores, with interactions, mergers and bar instabilities able to locally increase pressure and raise the ratio of efficiently star-forming gas to CO-detected gas. Building a sample representative of the local massive galaxy population, we derive a global Kennicutt-Schmidt relation of slope 1.18+/-0.24, and observe structure within the scatter around this relation, with galaxies having low (high) stellar mass surface densities lying systematically above (below) the mean relation, suggesting that gas surface density is not the only parameter driving the global star formation ability of a galaxy.Comment: 19 pages, 12 figures, accepted for publication in Ap

    Initial Evaluation of the Effects of Aerosolized Florida Red Tide Toxins (Brevetoxins) in Persons with Asthma

    Florida red tides annually occur in the Gulf of Mexico, resulting from blooms of the marine dinoflagellate Karenia brevis. K. brevis produces highly potent natural polyether toxins, known as brevetoxins, that activate voltage-sensitive sodium channels. In experimental animals, brevetoxins cause significant bronchoconstriction. A study of persons who visited the beach recreationally found a significant increase in self-reported respiratory symptoms after exposure to aerosolized Florida red tides. Anecdotal reports indicate that persons with underlying respiratory diseases may be particularly susceptible to adverse health effects from these aerosolized toxins. Fifty-nine persons with physician-diagnosed asthma were evaluated for 1 hr before and after going to the beach on days with and without Florida red tide. Study participants were evaluated with a brief symptom questionnaire, nose and throat swabs, and spirometry approved by the National Institute for Occupational Safety and Health. Environmental monitoring, water and air sampling (i.e., K. brevis, brevetoxins, and particulate size distribution), and personal monitoring (for toxins) were performed. Brevetoxin concentrations were measured by liquid chromatography mass spectrometry, high-performance liquid chromatography, and a newly developed brevetoxin enzyme-linked immunosorbent assay. Participants were significantly more likely to report respiratory symptoms after Florida red tide exposure. Participants demonstrated small but statistically significant decreases in forced expiratory volume in 1 sec, forced expiratory flow between 25 and 75%, and peak expiratory flow after exposure, particularly those regularly using asthma medications. Similar evaluation during nonexposure periods did not significantly differ. This is the first study to show objectively measurable adverse health effects from exposure to aerosolized Florida red tide toxins in persons with asthma. Future studies will examine the possible chronic effects of these toxins among persons with asthma and other chronic respiratory impairment

    Returning Individual Research Results from Digital Phenotyping in Psychiatry

    Psychiatry is rapidly adopting digital phenotyping and artificial intelligence/machine learning tools to study mental illness based on tracking participants’ locations, online activity, phone and text message usage, heart rate, sleep, physical activity, and more. Existing ethical frameworks for return of individual research results (IRRs) are inadequate to guide researchers for when, if, and how to return this unprecedented number of potentially sensitive results about each participant’s real-world behavior. To address this gap, we convened an interdisciplinary expert working group, supported by a National Institute of Mental Health grant. Building on established guidelines and the emerging norm of returning results in participant-centered research, we present a novel framework specific to the ethical, legal, and social implications of returning IRRs in digital phenotyping research. Our framework offers researchers, clinicians, and Institutional Review Boards (IRBs) urgently needed guidance, and the principles developed here in the context of psychiatry will be readily adaptable to other therapeutic areas
