    GIFTed Demons: deformable image registration with local structure-preserving regularization using supervoxels for liver applications.

    Deformable image registration, a key component of motion correction in medical imaging, needs to be efficient and provides plausible spatial transformations that reliably approximate biological aspects of complex human organ motion. Standard approaches, such as Demons registration, mostly use Gaussian regularization for organ motion, which, though computationally efficient, rule out their application to intrinsically more complex organ motions, such as sliding interfaces. We propose regularization of motion based on supervoxels, which provides an integrated discontinuity preserving prior for motions, such as sliding. More precisely, we replace Gaussian smoothing by fast, structure-preserving, guided filtering to provide efficient, locally adaptive regularization of the estimated displacement field. We illustrate the approach by applying it to estimate sliding motions at lung and liver interfaces on challenging four-dimensional computed tomography (CT) and dynamic contrast-enhanced magnetic resonance imaging datasets. The results show that guided filter-based regularization improves the accuracy of lung and liver motion correction as compared to Gaussian smoothing. Furthermore, our framework achieves state-of-the-art results on a publicly available CT liver dataset

    Registration and analysis of dynamic magnetic resonance image series

    Cystic fibrosis (CF) is an autosomal-recessive inherited metabolic disorder that affects all organs in the human body. Patients affected with CF suffer particularly from chronic inflammation and obstruction of the airways. Through early detection, continuous monitoring methods, and new treatments, the life expectancy of patients with CF has been increased drastically in the last decades. However, continuous monitoring of the disease progression is essential for a successful treatment. The current state-of-the-art method for lung disease detection and monitoring is computed tomography (CT) or X-ray. These techniques are ill-suited for the monitoring of disease progressions because of the ionizing radiation the patient is exposed during the examination. Through the development of new magnetic resonance imaging (MRI) sequences and evaluation methods, MRI is able to measure physiological changes in the lungs. The process to create physiological maps, i.e. ventilation and perfusion maps, of the lungs using MRI can be split up into three parts: MR-acquisition, image registration, and image analysis. In this work, we present different methods for the image registration part and the image analysis part. We developed a graph-based registration method for 2D dynamic MR image series of the lungs in order to overcome the problem of sliding motion at organ boundaries. Furthermore, we developed a human-inspired learning-based registration method. Here, the registration is defined as a sequence of local transformations. The sequence-based approach combines the advantage of dense transformation models, i.e. large space of transformations, and the advantage of interpolating transformation models, i.e. smooth local transformations. We also developed a general registration framework called Autograd Image Registration Laboratory (AIRLab), which performs automatic calculation of the gradients for the registration process. This allows rapid prototyping and an easy implementation of existing registration algorithms. For the image analysis part, we developed a deep-learning approach based on gated recurrent units that are able to calculate ventilation maps with less than a third of the number of images of the current method. Automatic defect detection in the estimated MRI ventilation and perfusion maps is essential for the clinical routine to automatically evaluate the treatment progression. We developed a weakly supervised method that is able to infer a pixel-wise defect segmentation by using only a continuous global label during training. In this case, we directly use the lung clearance index (LCI) as a global weak label, without any further manual annotations. The LCI is a global measure to describe ventilation inhomogeneities of the lungs and is obtained by a multiple breath washout test

    Sliding at first order: Higher-order momentum distributions for discontinuous image registration

    In this paper, we propose a new approach to deformable image registration that captures sliding motions. The large deformation diffeomorphic metric mapping (LDDMM) registration method faces challenges in representing sliding motion since it per construction generates smooth warps. To address this issue, we extend LDDMM by incorporating both zeroth- and first-order momenta with a non-differentiable kernel. This allows to represent both discontinuous deformation at switching boundaries and diffeomorphic deformation in homogeneous regions. We provide a mathematical analysis of the proposed deformation model from the viewpoint of discontinuous systems. To evaluate our approach, we conduct experiments on both artificial images and the publicly available DIR-Lab 4DCT dataset. Results show the effectiveness of our approach in capturing plausible sliding motion

    Computerized Analysis of Magnetic Resonance Images to Study Cerebral Anatomy in Developing Neonates

    The study of cerebral anatomy in developing neonates is of great importance for the understanding of brain development during the early period of life. This dissertation therefore focuses on three challenges in the modelling of cerebral anatomy in neonates during brain development. The methods that have been developed all use Magnetic Resonance Images (MRI) as source data. To facilitate study of vascular development in the neonatal period, a set of image analysis algorithms are developed to automatically extract and model cerebral vessel trees. The whole process consists of cerebral vessel tracking from automatically placed seed points, vessel tree generation, and vasculature registration and matching. These algorithms have been tested on clinical Time-of- Flight (TOF) MR angiographic datasets. To facilitate study of the neonatal cortex a complete cerebral cortex segmentation and reconstruction pipeline has been developed. Segmentation of the neonatal cortex is not effectively done by existing algorithms designed for the adult brain because the contrast between grey and white matter is reversed. This causes pixels containing tissue mixtures to be incorrectly labelled by conventional methods. The neonatal cortical segmentation method that has been developed is based on a novel expectation-maximization (EM) method with explicit correction for mislabelled partial volume voxels. Based on the resulting cortical segmentation, an implicit surface evolution technique is adopted for the reconstruction of the cortex in neonates. The performance of the method is investigated by performing a detailed landmark study. To facilitate study of cortical development, a cortical surface registration algorithm for aligning the cortical surface is developed. The method first inflates extracted cortical surfaces and then performs a non-rigid surface registration using free-form deformations (FFDs) to remove residual alignment. Validation experiments using data labelled by an expert observer demonstrate that the method can capture local changes and follow the growth of specific sulcus

    Enhancing Registration for Image-Guided Neurosurgery

    Pharmacologically refractive temporal lobe epilepsy and malignant glioma brain tumours are examples of pathologies that are clinically managed through neurosurgical intervention. The aims of neurosurgery are, where possible, to perform a resection of the surgical target while minimising morbidity to critical structures in the vicinity of the resected brain area. Image-guidance technology aims to assist this task by displaying a model of brain anatomy to the surgical team, which may include an overlay of surgical planning information derived from preoperative scanning such as the segmented resection target and nearby critical brain structures. Accurate neuronavigation is hindered by brain shift, the complex and non-rigid deformation of the brain that arises during surgery, which invalidates assumed rigid geometric correspondence between the neuronavigation model and the true shifted positions of relevant brain areas. Imaging using an interventional MRI (iMRI) scanner in a next-generation operating room can serve as a reference for intraoperative updates of the neuronavigation. An established clinical image processing workflow for iMRI-based guidance involves the correction of relevant imaging artefacts and the estimation of deformation due to brain shift based on non-rigid registration. The present thesis introduces two refinements aimed at enhancing the accuracy and reliability of iMRI-based guidance. A method is presented for the correction of magnetic susceptibility artefacts, which affect diffusion and functional MRI datasets, based on simulating magnetic field variation in the head from structural iMRI scans. Next, a method is presented for estimating brain shift using discrete non-rigid registration and a novel local similarity measure equipped with an edge-preserving property which is shown to improve the accuracy of the estimated deformation in the vicinity of the resected area for a number of cases of surgery performed for the management of temporal lobe epilepsy and glioma

    Optical techniques for 3D surface reconstruction in computer-assisted laparoscopic surgery

    One of the main challenges for computer-assisted surgery (CAS) is to determine the intra-opera- tive morphology and motion of soft-tissues. This information is prerequisite to the registration of multi-modal patient-specific data for enhancing the surgeon’s navigation capabilites by observ- ing beyond exposed tissue surfaces and for providing intelligent control of robotic-assisted in- struments. In minimally invasive surgery (MIS), optical techniques are an increasingly attractive approach for in vivo 3D reconstruction of the soft-tissue surface geometry. This paper reviews the state-of-the-art methods for optical intra-operative 3D reconstruction in laparoscopic surgery and discusses the technical challenges and future perspectives towards clinical translation. With the recent paradigm shift of surgical practice towards MIS and new developments in 3D opti- cal imaging, this is a timely discussion about technologies that could facilitate complex CAS procedures in dynamic and deformable anatomical regions

    Realistic tool-tissue interaction models for surgical simulation and planning

    Surgical simulators present a safe and potentially effective method for surgical training, and can also be used in pre- and intra-operative surgical planning. Realistic modeling of medical interventions involving tool-tissue interactions has been considered to be a key requirement in the development of high-fidelity simulators and planners. The soft-tissue constitutive laws, organ geometry and boundary conditions imposed by the connective tissues surrounding the organ, and the shape of the surgical tool interacting with the organ are some of the factors that govern the accuracy of medical intervention planning.\ud \ud This thesis is divided into three parts. First, we compare the accuracy of linear and nonlinear constitutive laws for tissue. An important consequence of nonlinear models is the Poynting effect, in which shearing of tissue results in normal force; this effect is not seen in a linear elastic model. The magnitude of the normal force for myocardial tissue is shown to be larger than the human contact force discrimination threshold. Further, in order to investigate and quantify the role of the Poynting effect on material discrimination, we perform a multidimensional scaling study. Second, we consider the effects of organ geometry and boundary constraints in needle path planning. Using medical images and tissue mechanical properties, we develop a model of the prostate and surrounding organs. We show that, for needle procedures such as biopsy or brachytherapy, organ geometry and boundary constraints have more impact on target motion than tissue material parameters. Finally, we investigate the effects surgical tool shape on the accuracy of medical intervention planning. We consider the specific case of robotic needle steering, in which asymmetry of a bevel-tip needle results in the needle naturally bending when it is inserted into soft tissue. We present an analytical and finite element (FE) model for the loads developed at the bevel tip during needle-tissue interaction. The analytical model explains trends observed in the experiments. We incorporated physical parameters (rupture toughness and nonlinear material elasticity) into the FE model that included both contact and cohesive zone models to simulate tissue cleavage. The model shows that the tip forces are sensitive to the rupture toughness. In order to model the mechanics of deflection of the needle, we use an energy-based formulation that incorporates tissue-specific parameters such as rupture toughness, nonlinear material elasticity, and interaction stiffness, and needle geometric and material properties. Simulation results follow similar trends (deflection and radius of curvature) to those observed in macroscopic experimental studies of a robot-driven needle interacting with gels

    Computational methods for the analysis of functional 4D-CT chest images.

    Medical imaging is an important emerging technology that has been intensively used in the last few decades for disease diagnosis and monitoring as well as for the assessment of treatment effectiveness. Medical images provide a very large amount of valuable information that is too huge to be exploited by radiologists and physicians. Therefore, the design of computer-aided diagnostic (CAD) system, which can be used as an assistive tool for the medical community, is of a great importance. This dissertation deals with the development of a complete CAD system for lung cancer patients, which remains the leading cause of cancer-related death in the USA. In 2014, there were approximately 224,210 new cases of lung cancer and 159,260 related deaths. The process begins with the detection of lung cancer which is detected through the diagnosis of lung nodules (a manifestation of lung cancer). These nodules are approximately spherical regions of primarily high density tissue that are visible in computed tomography (CT) images of the lung. The treatment of these lung cancer nodules is complex, nearly 70% of lung cancer patients require radiation therapy as part of their treatment. Radiation-induced lung injury is a limiting toxicity that may decrease cure rates and increase morbidity and mortality treatment. By finding ways to accurately detect, at early stage, and hence prevent lung injury, it will have significant positive consequences for lung cancer patients. The ultimate goal of this dissertation is to develop a clinically usable CAD system that can improve the sensitivity and specificity of early detection of radiation-induced lung injury based on the hypotheses that radiated lung tissues may get affected and suffer decrease of their functionality as a side effect of radiation therapy treatment. These hypotheses have been validated by demonstrating that automatic segmentation of the lung regions and registration of consecutive respiratory phases to estimate their elasticity, ventilation, and texture features to provide discriminatory descriptors that can be used for early detection of radiation-induced lung injury. The proposed methodologies will lead to novel indexes for distinguishing normal/healthy and injured lung tissues in clinical decision-making. To achieve this goal, a CAD system for accurate detection of radiation-induced lung injury that requires three basic components has been developed. These components are the lung fields segmentation, lung registration, and features extraction and tissue classification. This dissertation starts with an exploration of the available medical imaging modalities to present the importance of medical imaging in today’s clinical applications. Secondly, the methodologies, challenges, and limitations of recent CAD systems for lung cancer detection are covered. This is followed by introducing an accurate segmentation methodology of the lung parenchyma with the focus of pathological lungs to extract the volume of interest (VOI) to be analyzed for potential existence of lung injuries stemmed from the radiation therapy. After the segmentation of the VOI, a lung registration framework is introduced to perform a crucial and important step that ensures the co-alignment of the intra-patient scans. This step eliminates the effects of orientation differences, motion, breathing, heart beats, and differences in scanning parameters to be able to accurately extract the functionality features for the lung fields. The developed registration framework also helps in the evaluation and gated control of the radiotherapy through the motion estimation analysis before and after the therapy dose. Finally, the radiation-induced lung injury is introduced, which combines the previous two medical image processing and analysis steps with the features estimation and classification step. This framework estimates and combines both texture and functional features. The texture features are modeled using the novel 7th-order Markov Gibbs random field (MGRF) model that has the ability to accurately models the texture of healthy and injured lung tissues through simultaneously accounting for both vertical and horizontal relative dependencies between voxel-wise signals. While the functionality features calculations are based on the calculated deformation fields, obtained from the 4D-CT lung registration, that maps lung voxels between successive CT scans in the respiratory cycle. These functionality features describe the ventilation, the air flow rate, of the lung tissues using the Jacobian of the deformation field and the tissues’ elasticity using the strain components calculated from the gradient of the deformation field. Finally, these features are combined in the classification model to detect the injured parts of the lung at an early stage and enables an earlier intervention
