    Anesthetic blocks to treat cancer pain

    BACKGROUND AND OBJECTIVES: Anesthesiologists have traditionally developed and mastered regional anesthetic techniques over the years. To help treating chronic pain, they need to transfer acquired know-how in addition to surgical indications, recommending procedures at the right time in a multidisciplinary approach. This research aimed at showing anesthesiologists that for anesthetic blocks to be useful and effective in treating chronic cancer pain, they have to be indicated in a timely manner, as part of a systemic approach where they need to play an active role. CONTENTS: WHO guidelines provide adequate pain control to most patients with advanced cancer disease. Its first three steps confirm the efficacy of this approach, with a low incidence of complications. There are however patients who experience undesirable side effects of systemic opioids, and the presence of bone metastases and neuropathies may determine noxious effects on those patients. Invasive techniques are seldom indicated, but they may provide analgesia in the treatment of opioid-resistant pain when adequately indicated. CONCLUSIONS: Neurolytic agent blocks, which require anesthesiologist skills, are the last treatment options when systemic drugs are unable to maintain desired effects or produce untreatable side effects. Anesthetic blocks and spinal opioids play an important role in the treatment of cancer pain and should be considered in specific situations as part of a multidisciplinary approach.JUSTIFICATIVA Y OBJETIVOS: Tradicionalmente los anestesiologistas desarrollaron las técnicas regionales de anestesia que hoy dominan. Para participar del tratamiento de los pacientes con dolores crónicos, necesitan transferir los conocimientos adquiridos, además de las indicaciones quirúrgicas, indicándolos en el momento correcto, haciéndolos útiles y eficaces, a un atendimiento que debe ser multidisciplinar. El objetivo de este trabajo es mostrar a los anestesiologistas que los bloqueos anestésicos, en el tratamiento del dolor crónico de origen cancerosa, para que sean útiles y eficaces, deben ser indicados en el momento correcto, introduciéndolos como parte de un tratamiento sistémico, del cual se debe participar activamente. CONTENIDO: El uso correcto del protocolo de la Organización Mundial de la Salud proporciona orientación para el control del dolor en la mayoría de los pacientes con molestia cancerosa avanzada. La eficacia de este abordaje es demostrada en sus tres primeros pasos, con pequeño número de complicaciones. Con todo eso, hay pacientes que experimentan efectos colaterales indeseables a los medicamentos opioides sistémicos y a la presencia de metástases óseas y las neuropatias pueden determinar efectos deletéricos en esos pacientes. Técnicas invasivas son raramente indicadas, más pueden resultar en analgesia en el tratamiento de los dolores resistentes a los opioides, cuando usados adecuadamente. CONCLUSIONES: Bloqueos con agentes neurolíticos que exigían alto grado de pericia de los anestesiologistas son las últimas opciones de tratamiento cuando las drogas sistémicas no mantienen el efecto deseado o producen efectos colaterales de difícil tratamiento. Bloqueos anestésicos e inyección de drogas opioides por vía subaracnóidea tienen lugar de destaque en el tratamiento del dolor de origen canceroso y deben ser considerados en situaciones específicas como parte de un tratamiento multidisciplinar.JUSTIFICATIVA E OBJETIVOS: Tradicionalmente os anestesiologistas desenvolveram as técnicas de anestesia regional que hoje dominam. Para participarem do tratamento dos pacientes com dores crônicas, necessitam transferir os conhecimentos adquiridos, além das indicações cirúrgicas, indicando-os no momento correto, tornando-os úteis e eficazes, num atendimento que deve ser multidisciplinar. O objetivo deste trabalho é mostrar aos anestesiologistas que os bloqueios anestésicos, no tratamento da dor crônica de origem cancerosa, para serem úteis e eficazes, devem ser indicados no momento correto inserindo-os como parte de um tratamento sistêmico, do qual deve-se participar ativamente. CONTEÚDO: O uso correto do protocolo da Organização Mundial da Saúde proporciona diretriz para o controle da dor na maioria dos pacientes com moléstia cancerosa avançada. A eficácia desta abordagem é demonstrada nos seus três primeiros passos, com pequeno número de complicações. Contudo, há pacientes que experimentam efeitos colaterais indesejáveis aos medicamentos opióides sistêmicos e a presença de metástases ósseas e as neuropatias podem determinar efeitos deletéricos nesses pacientes. Técnicas invasivas são raramente indicadas, mas podem resultar em analgesia no tratamento das dores resistentes aos opióides quando usados adequadamente. CONCLUSÕES: Bloqueios com agentes neurolíticos que exigiam alto grau de perícia dos anestesiologistas são as últimas opções de tratamento quando as drogas sistêmicas não mantêm o efeito desejado ou produzem efeitos colaterais de difícil tratamento.     Leishmaniose cutânea no nordeste do Brasil: uma avaliação crítica dos estudos realizados no Estado de Pernambuco

    American cutaneous leishmaniasis (ACL) is a complex disease with clinical and epidemiological features that may vary from region to region. In fact, at least seven different Leishmania species, including Leishmania (Viannia) braziliensis, Leishmania (Viannia) guyanensis, Leishmania (Viannia) lainsoni, Leishmania (Viannia) naiffi, Leishmania (Viannia) shawi, Leishmania (Viannia) lindenbergi, and Leishmania (Leishmania) amazonensis, have been implicated in the etiology of ACL in Brazil, and numerous phlebotomine sandfly species of the genus Lutzomyia have been regarded as putative or proven vectors. Because ACL is a focal disease, understanding the disease dynamics at the local level is essential for the implementation of more effective control measures. The present paper is a narrative review about the ACL epidemiology in Pernambuco, northeastern Brazil. Furthermore, the need for more effective diagnosis, treatment, control and prevention strategies for the affected populations is highlighted. This paper will provide researchers with a critical appraisal of ACL in Pernambuco. Hopefully, it will also be helpful for public health authorities to improve current control strategies against ACL at the state and country levels

    Impact of Wearing Palatal Expanders on the Quality of Life of Children Aged 8 to 10 Years

    Objective: To assess the impact of wearing palatal expanders on the oral health-related quality of life (OHRQoL) of children aged 8 to 10 years. Material and Methods: The sample consisted of 35 children aged 8 to 10 years treated at the orthodontics and pediatric dentistry outpatient clinics affiliated with the Dental School. Children were divided in two groups: 17 were submitted to palatal expansion treatment (Group 1), while 18 just received coronal polishing and topical fluoride application (Group 2). The Brazilian version of the Child Perceptions Questionnaire (CPQ8-10) was used to assess children’s OHRQoL. The questionnaire was administered before the expander was placed, at the end of its activation, and before its removal. Intervals between the interviews were similar in the two groups. Descriptive statistics and ANOVA were used for data analyses. Results: The overall CPQ8-10 score was not significantly different between the groups. The results show statistically significant differences between the two groups only for the functional limitations domain after activation of the appliance (p=0.001). Conclusion: Palatal expanders may negatively affect children's functions only during the initial wearing period

    Aortic Thromboembolism in a Cat

    Background: The aortic thromboembolism in cats is usually associated with cardiomyopathy, when a thrombus or a clot is formed in the heart, transported through the bloodstream and fixed at somewhere. According to Virchow’s triad, changes in the endocardial surface or in the blood flow/composition can result in thrombus formation. The most common clinical signs are: hind limb paralysis, lack of femoral pulse, cold and cyanotic extremities. The treatment should be performed as soon as possible and it is based on antiplatelet agents, anticoagulants, thrombolytics agents or surgical procedures. It is reported the case of a cat presenting aortic thromboembolism.Case: Macroscopically it was observed that the hind limb extremities were with a dark red color and with a bad odor on cut. There were in the subcutaneous tissue of the hind limbs a severe and diffuse accumulation of a reddish material, translucent, shiny, gelatinous (intense diffuse edema) and the skeletal muscles of the hind limbs had extensive pale and friable areas. Inside the medial saphenous vein lumen there was a greyish-white and soft material which adhered to the vessel wall. The lungs were not fully collapsed, it had a smooth and shiny surface and an extensive dark red area in the right middle lobe (moderate extensive bleeding). The spleen was with slightly bulging edges and on cut flowed moderate amount of blood (moderate congestive splenomegaly). In the kidneys there was numerous whitish and slightly depressed areas measuring approximately 0.5 cm in diameter, which was deep and wedge-shaped on cut (multifocal infarction). Microscopically it was observed mild diffuse congestion in the spleen; mild diffuse congestion and moderate focal bleeding in the lungs; intense multifocal neutrophilic infiltrate associated with coagulation necrosis in the cortical and medullary with multifocal cysts in moderate amounts in the kidneys; abdominal aorta artery with intense accumulation of fibrin, red blood cells and cellular debris organized and stuck in the tunica intima featuring a multifocal thrombosis area. The saphenous artery contained fibrin organization in the lumen, which was compatible with embolus.Discussion: The patient presented paralysis of hind limbs, absence of superficial and deep pain, cyanotic and cold hind limb extremities, loss of proprioception of the limbs and absence of detectable pulse in the vascular Doppler, which is characteristic of thromboembolic disease, but no specific sign of the underlying cause. Thromboembolic episodes are compatible with thrombus in the trifurcation of the abdominal aorta artery and the embolus in the saphenous artery. The focal lung bleeding can be the clinical sign of pulmonary thromboembolism. The unilateral renal infarction may have been induced after surgical excision of the thrombus, as the patient had no change in renal biochemical parameters prior to the surgical procedure. This is a complication, because small thrombus fragments can break off and cause thromboembolism elsewhere. The surgical procedure is generally non indicated because most domestic cats that have arterial thrombus have significant heart disease and more than half have congestive heart failure, making them poor candidates for surgery, which did not occur in the clinical case described. The repetitions of the events are common, and significant embolization of the kidney, intestines or other organs worsen the prognosis.Keywords: aortic thromboembolism, feline

    Socioeconomic status and family functioning influence oral health literacy among adolescents

    OBJECTIVE: Evaluate socio-demographic, family and behavioral factors associated with oral health literacy (OHL) in adolescents. METHODS: Cross-sectional study conducted with adolescents aged 15 to 19 years in Campina Grande, Brazil. Parents/guardians answered a questionnaire addressing socio-demographic data. The adolescents answered validated instruments on family cohesion and adaptability (family adaptability and cohesion evaluation scale), drug use (alcohol, smoking and substance involvement screening test), type of dental service used for last appointment and OHL (Brazilian version of the Rapid Estimate of Oral Health Literacy in Dentistry). Two dentists were trained to evaluate OHL (K = 0.87–0.88). Descriptive analysis was performed, followed by Poisson regression analysis (α = 5%). A directed acyclic graph was used to select independent variables in the study. RESULTS: The following variables remained associated with better OHL: high mother’s schooling level (RR = 1.07; 95%CI: 1.03–1.12), high income (RR = 1.04; 95%CI: 1.01–1.09), white ethnicity/skin color (RR = 1.05; 95%CI: 1.01–1.10), married parents (RR = 1.04; 95%CI: 1.01–1.09), “enmeshed” family cohesion (RR = 1.21; 95%CI: 1.12–1.30), “structured” (RR = 1.06; 95%CI: 1.01–1.12) or “rigid” (RR = 1.11; 95%CI: 1.04–1.19) family adaptability, having more than five residents in the home (RR = 1.07; 95%CI: 1.01–1.14) and having used a private dental service during the last appointment (RR = 1.08; 95%CI: 1.03–1.13). CONCLUSION: Family functioning and socio-demographic factors influence the level of oral health literacy among adolescents

    Revisiting hydroxychloroquine and chloroquine for patients with chronic immunity-mediated inflammatory rheumatic diseases

    Hydroxychloroquine and chloroquine, also known as antimalarial drugs, are widely used in the treatment of rheumatic diseases and have recently become the focus of attention because of the ongoing COVID-19 pandemic. Rheumatologists have been using antimalarials to manage patients with chronic immune-mediated inflammatory rheumatic diseases for decades. It is an appropriate time to review their immunomodulatory and anti-inflammatory mechanisms impact on disease activity and survival of systemic lupus erythematosus patient, including antiplatelet effect, metabolic and lipid benefits. We also discuss possible adverse effects, adding a practical and comprehensive approach to monitoring rheumatic patients during treatment with these drugs

    Impactos negativos da administração de hidroxicloroquina e anticoagulante em pacientes com infecção por SARS-COV-2: um ensaio clínico randomizado

    Objetivo: Avaliar antimalárico com ou sem tratamento anticoagulante, em pacientes com infecção recente por SARS-COV-2. Métodos: Estudo clínico realizado no Hospital das Clínicas Samuel Libânio da Universidade do Vale do Sapucaí, Pouso Alegre-MG. Aprovado pelo Comitê de Ética (4.034.077) e registrado nos Ensaios Clínicos (NCT04788355). Pacientes suspeitos de COVID-19 foram incluídos na sala de emergência. Os grupos foram: C (controle) com 6 pacientes, A (anticoagulante apixabana) com 9 pacientes, H (hidroxicloroquina) com 5 pacientes e HA (hidroxicloroquina e anticoagulante apixabana) com 8 pacientes. Resultados: não houve diferenças significativas entre os grupos. O grupo HA, no qual houve intervenção com dois medicamentos, apresentou maior número de dias com sintomas (p = 0,037) e piores resultados, quando comparado ao controle: os sintomas mais relevantes foram: tosse (p = 0,001), e anosmia/ageusia (p = 0,011) cefaléia (p = 0,001). Conclusão: O presente estudo teve início quando havia dúvidas sobre o uso de medicamentos como hidroxicloroquina (HCQ) e apixabana (APX). O “n” reduzido foi definido por meio de questões burocráticas e polêmicas independentes das ações dos autores. Nenhum benefício clínico foi associado com HCQ e APX. Houve um aumento no número de dias sintomáticos quando HCQ e APX foram administrados. Apesar das limitações, não houve indicação terapêutica dos medicamentos avaliados.Purpose: To evaluate antimalarial with or without anticoagulant treatment, in patients with recent SARS-COV-2 infection. The groups were: C (control) with 6 patients, A (anticoagulant apixaban) with 9 patients, H (hydroxychloroquine) with 5 patients and HA (hydroxychloroquine and anticoagulant apixaban) with 8 patients. Results: there were no significant differences between groups. The HA group, in which there was an intervention with two drugs, presented a greater number of days with symptoms (p = 0.037) and worse results, when compared to the control: most relevant symptoms, were: cough (p = 0.001), and anosmia / ageusia (p = 0.011) headache (p = 0.001). Conclusion: The present study began when there were doubts about the use of drugs such as Hydroxychloroquine (HCQ) and apixaban (APX). The reduced “n” was defined through bureaucratic and polemic issues independent of the authors’ actions. No clinical benefit was associated with HCQ and APX. There was an increase in the number of symptomatic days when HCQ and APX were administered. Despite the limitations, there was no therapeutic indication of the evaluated drugs

    High levels of immunosuppression are related to unfavourable outcomes in hospitalised patients with rheumatic diseases and COVID-19 : first results of ReumaCoV Brasil registry

    Objectives To evaluate risk factors associated with unfavourable outcomes: emergency care, hospitalisation, admission to intensive care unit (ICU), mechanical ventilation and death in patients with immune-mediated rheumatic disease (IMRD) and COVID-19. Methods Analysis of the first 8 weeks of observational multicentre prospective cohort study (ReumaCoV Brasil register). Patients with IMRD and COVID-19 according to the Ministry of Health criteria were classified as eligible for the study. Results 334 participants were enrolled, a majority of them women, with a median age of 45 years; systemic lupus erythematosus (32.9%) was the most frequent IMRD. Emergency care was required in 160 patients, 33.0% were hospitalised, 15.0% were admitted to the ICU and 10.5% underwent mechanical ventilation; 28 patients (8.4%) died. In the multivariate adjustment model for emergency care, diabetes (prevalence ratio, PR 1.38; 95% CI 1.11 to 1.73; p=0.004), kidney disease (PR 1.36; 95% CI 1.05 to 1.77; p=0.020), oral glucocorticoids (GC) (PR 1.49; 95% CI 1.21 to 1.85; p50 years (PR 1.89; 95% CI 1.26 to 2.85; p=0.002), no use of tumour necrosis factor inhibitor (TNFi) (PR 2.51;95% CI 1.16 to 5.45; p=0.004) and methylprednisolone pulse therapy (PR 2.50; 95% CI 1.59 to 3.92; p<0.001); for ICU admission, oral GC (PR 2.24; 95% CI 1.36 to 3.71; p<0.001) and pulse therapy with methylprednisolone (PR 1.65; 95% CI 1.00 to 2.68; p<0.043); the two variables associated with death were pulse therapy with methylprednisolone or cyclophosphamide (PR 2.86; 95% CI 1.59 to 5.14; p<0.018). Conclusions Age >50 years and immunosuppression with GC and cyclophosphamide were associated with unfavourable outcomes of COVID-19. Treatment with TNFi may have been protective, perhaps leading to the COVID-19 inflammatory process
