    Influencia de la luz y distancia entre pórticos de acero en la huella de CO2 de la estructura

    Tras décadas de proceder sin las necesarias preocupaciones en nuestro modo de construir, se hace imprescindible una nueva conciencia. Surge la necesidad de cuantificar de alguna forma cómo de contaminantes y negativas son nuestras acciones. Es entonces cuando se empiezan a escuchar conceptos como sostenibilidad y eficiencia en nuestro día a día. Se buscan soluciones que nos ofrezcan una huella humana menos invasiva sobre el medioambiente. Para ello se propone este trabajo, en el que se realiza un estudio comparativo sobre la influencia de la geometría de las estructuras de acero en la huella de carbono. Para ello, se elige una estructura de tipo industrial, de pórticos acartelados y correas apoyadas sobre los pórticos. Se analizan distintos modelos, con luces y separaciones entre pórticos diferentes. Se determina un sector industrial tipo, y se delimita el estudio a naves industriales comprendidas entre 500 m2 y 1000 m2 de superficie, considerando la mayor representatividad posible del mercado actual. Finalmente, tras el dimensionado de los módulos estructurales, se procede a la obtención del peso total y las emisiones de CO2 en toneladas de cada una de las naves industriales objeto de estudio, con el objetivo de especificar que tipo de luces y separaciones entre pórticos presentan soluciones óptimas para cada situación respecto al medioambiente.After decades of proceeding without the necessary concerns in our way of building, a new conscience becomes imperative. The need to somehow quantify how pollutants and negatives are our actions. That is when we begin to hear concepts such as sustainability and efficiency in our day to day. Solutions are sought that offer us a less invasive human footprint on the environment. For that, this work is proposed, in which a comparative study is carried out on the influence of the geometry of steel structures on the carbon footprint. For this, an industrial type structure is chosen, with haunched frames and belts supported on the frames. Different models are analyzed, with lights and separations between different frames. A typical industrial sector is determined, and the study is delimited to industrial buildings with areas between 500 m2 and 1000 m2. Considering the greater representativeness of the current market. Finally, after the calculation of the structural modules, the total weight and the CO2 emissions in tons of each of the industrial buildings studied. Un pueblo caracterizado por la cantidad de restaurantes que alberga y por un turismo masivo de sólo unas horas al día. Con esta intervención se pretende aumentar la actividad profesional y residencial de dicha zona. Para ello, se realizan pequeñas intervenciones urbanísticas a lo largo de todo el pueblo. Se potencia el eje izquierdo como el eje público, disponiendo arbolado y acondicionando terrazas de restaurantes. Por último, se colmata el pueblo con un conjunto edificado que alberga una escuela de hostelería.     Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio

    A facility and community-based assessment of scabies in rural Malawi.

    Background Scabies is a neglected tropical disease of the skin, causing severe itching, stigmatizing skin lesions and systemic complications. Since 2015, the DerMalawi project provide an integrated skin diseases clinics and Tele-dermatology care in Malawi. Clinic based data suggested a progressive increase in scabies cases observed. To better identify and treat individuals with scabies in the region, we shifted from a clinic-based model to a community based outreach programme. Methodology/principal findings From May 2015, DerMalawi project provide integrated skin diseases and Tele-dermatological care in the Nkhotakota and Salima health districts in Malawi. Demographic and clinical data of all patients personally attended are recorded. Due to a progressive increase in the number of cases of scabies the project shifted to a community-based outreach programme. For the community outreach activities, we conducted three visits between 2018 to 2019 and undertook screening in schools and villages of Alinafe Hospital catchment area. Treatment was offered for all the cases and school or household contacts. Scabies increased from 2.9% to 39.2% of all cases seen by the DerMalawi project at clinics between 2015 to 2018. During the community-based activities approximately 50% of the population was assessed in each of three visits. The prevalence of scabies was similar in the first two rounds, 15.4% (2392) at the first visit and 17.2% at the second visit. The prevalence of scabies appeared to be lower (2.4%) at the third visit. The prevalence of impetigo appeared unchanged and was 6.7% at the first visit and 5.2% at the final visit. Conclusions/significance Prevalence of scabies in our setting was very high suggesting that scabies is a major public health problem in parts of Malawi. Further work is required to more accurately assess the burden of disease and develop appropriate public health strategies for its control

La literatura ha mostrado ampliamente que la inteligencia emocional (IE) tiene un rol favorecedor de la convivencia en el contexto escolar. Por ello, se analiza la evidencia que existe sobre el papel de la IE en el acoso escolar. Hay un mayor nivel de percepción emocional en el alumnado implicado en el acoso (tanto agresor como víctima) y menor en comprensión y regulación en comparación con el alumnado que no está implicado en estas conductas. Además, se observaron diferencias en el grado de IE de los estudiantes implicados en acoso según el género. Se indica la importancia que tiene diseñar e implementar programas de desarrollo de la IE en el contexto educativo como prevención y actuación en el acoso escolar.ES

    Sex-Specific Association between Fasting Plasma Glucose and Serum Selenium Levels in Adults from Southern Mexico

    Selenium (Se) is an essential trace element that by its antioxidant properties has been studied to elucidate its participation in the development of obesity and type 2 diabetes. We evaluated the association between cardiometabolic traits and serum Se levels in a sample of adults from southern Mexico. In 96 nondiabetic individuals, anthropometric data and clinical biochemistry measurements were analyzed. Serum total Se levels were measured with inductively coupled plasma mass spectrometry (ICP-MS). Serum Se level in the whole sample was 10.309 &plusmn; 3.031 &mu;g mL&minus;1 and no difference between the women and men was observed (p = 0.09). Additionally, fasting plasma glucose (FPG) was significantly associated with serum Se level (&beta; = &minus;0.07 &plusmn; 0.03, p = 0.02, analysis adjusted for age, sex and BMI). Furthermore, sex shows significant interaction with FPG on the serum Se levels (p = 0.01). A follow-up analysis revealed the particular association between FPG and Se levels in women (&beta; = &minus;0.10 &plusmn; 0.04, p = 0.01). In conclusion, our data evidenced a women-specific association between FPG and serum Se levels in a sample of adults from southern Mexico
