A new manual wheelchair propulsion system with self-locking capability on ramps

A wheelchair user faces many difficulties in their everyday attempts to use ramps, especially those of some length. The present work describes the design and build of a propulsion system for manual wheelchairs for use in ascending or descending long ramps. The design is characterized by a self-locking mechanism that activates automatically to brake the chair when the user stops pushing. The system consists of a planetary transmission with a self-locking capacity coupled to a push rim with which the user moves the system. Different transmission ratios are proposed, adapted to the slope and to the user’s physical capacity (measured as the power the user can apply over ample time periods). The design is shown to be viable in terms of resistance, and approximate dimensions are established for the height and width of the propulsion system. Also, a prototype was built in order to test the self-locking system on ramps

El proyecto es un de recomendación y guía turística, funciona con un mapa completo de la República Mexicana ahí tiene registrados todos los estados con una barra para cada categoría de lo que se está estudiando que son: salud, educación, seguridad y empleo. Al entrar a la página te pediría permiso para localizar tu ubicación para saber en qué estado vives, después usted podrá darle calificación a ese mismo estado u otro que usted conozca. Usaremos una barra con la que el usuario podrá elegir cuanta calificación le da a cada estado de un promedio de -100 a 100 con esto toda la gente podría ver cuál es la ciudad más segura, o con mejor educación, o con mayor cantidad de empleos, y en cual la salud es muy buena, con la calificación que cada uno le dará, también en base a la api de Twitter sacaremos los Tweets que hablen de el estado sobre las categorías sin tomar en cuenta campañas publicitarias o por cuestiones políticas. La página también tendrá una pequeña guía turística para cada uno de los estados, esto con la intención de que la gente de otros países que vienen a visitar algún estado, o si alguien de México quiera viajar a un estado de la república por diversión o vacaciones. La intención de esta guía es mostrarles un poco de historia del estado a visitar, los lugares más relevantes o las recomendaciones que le asemos que debería visitar si viaja a algunos de estos estados, también contiene un poco de información sobre la cantidad de población de cada estado y en donde se localiza el mismo estado

Association between neuroserpin and molecular markers of brain damage in patients with acute ischemic stroke.

BACKGROUND: Neuroserpin has shown neuroprotective effects in animal models of cerebral ischemia and has been associated with functional outcome after ischemic stroke. Our aim was to study whether neuroserpin serum levels could be associated to biomarkers of excitotoxicity, inflammation and blood brain barrier disruption. METHODS: We prospectively included 129 patients with ischemic stroke (58.1% male; mean age, 72.4 ± 9.6 years) not treated with tPA within 12 hours (h) of symptoms onset (mean time, 4.7 ± 2.1 h). Poor functional outcome at 3 months was considered as a modified Rankin scale score >2. Serum levels of neuroserpin, Interleukin 6 (IL-6), Intercellular adhesion molecule-1 (ICAM-1), active Matrix metalloproteinase 9 (MMP-9), and cellular fibronectin (cFn) (determined by ELISA) and glutamate (determined by HPLC) were measured on admission, 24 and 72 h. The main variable was considered the decrease of neuroserpin levels within the first 24 h. ROC analysis was used to select the best predictive value for neuroserpin to predict poor functional outcome due to a lack of linearity. RESULTS: The decrease of neuroserpin levels within the first 24 h was negatively correlated with serum levels at 24 hours of glutamate (r = -0.642), IL-6 (r = -0.678), ICAM-1 (r = -0.345), MMP-9 (r = -0.554) and cFn (r = -0.703) (all P < 0.0001). In the multivariate analysis, serum levels of glutamate (OR, 1.04; CI95%, 1.01-1.06, p = 0.001); IL-6 (OR, 1.4; CI95%, 1.1-1.7, p = 0.001); and cFn (OR, 1.3; CI95%, 1.1-1.6, p = 0.002) were independently associated with a decrease of neuroserpin levels <70 ng/mL at 24 h after adjusting for confounding factors. CONCLUSIONS: These findings suggest that neuroprotective properties of neuroserpin may be related to the inhibition of excitotoxicity, inflammation, as well as blood brain barrier disruption that occur after acute ischemic stroke

Synthesis, Photochemical, and Redox Properties of Gold(I) and Gold(III) Pincer Complexes Incorporating a 2,2′:6′,2″-Terpyridine Ligand Framework

Reaction of [Au(C6F5)(tht)] (tht = tetrahydrothiophene) with 2,2′:6′,2″-terpyridine (terpy) leads to complex [Au(C6F5)(η1-terpy)] (1). The chemical oxidation of complex (1) with 2 equiv of [N(C6H4Br-4)3](PF6) or using electrosynthetic techniques affords the Au(III) complex [Au(C6F5)(η3-terpy)](PF6)2 (2). The X-ray diffraction study of complex 2 reveals that the terpyridine acts as tridentate chelate ligand, which leads to a slightly distorted square-planar geometry. Complex 1 displays fluorescence in the solid state at 77 K due to a metal (gold) to ligand (terpy) charge transfer transition, whereas complex 2 displays fluorescence in acetonitrile due to excimer or exciplex formation. Time-dependent density functional theory calculations match the experimental absorption spectra of the synthesized complexes. In order to further probe the frontier orbitals of both complexes and study their redox behavior, each compound was separately characterized using cyclic voltammetry. Regulatory T cells modulate inflammation and reduce infarct volume in experimental brain ischaemia

Brain ischaemia (stroke) triggers an intense inflammatory response predominately mediated by the accumulation of inflammatory cells and mediators in the ischaemic brain. In this context, regulatory T (Treg) cells, a subpopulation of CD4 + T cells with immunosuppressive and anti-inflammatory properties, are activated in the late stages of the disease. To date, the potential therapeutic usefulness of Treg cells has not been tested. In this study, we aimed to investigate whether Treg cells exert protection/repair following stroke. Both the adoptive transfer of Treg cells into ischaemic rats and the stimulation of endogenous T-cell proliferation using a CD28 superagonist reduced the infarct size at 3-28 days following the ischaemic insult. Moreover, T cell-treated animals had higher levels of FoxP3 and lower levels of IL-1β, CD11b+ and CD68+ cells in the infarcted hemisphere when compared with control animals. However, T-cell treatment did not alter the rate of proliferation of NeuN-, NCAM- or CD31-positive cells, thereby ruling out neurogenesis and angiogenesis in protection. These results suggest that adoptive transfer of T cells is a promising therapeutic strategy against the neurological consequences of stroke

Failure analysis of Co–Cr hip resurfacing prosthesis during solidification

In this study a failure originated during solidification process into the femoral stem component of Hip Resurfacing prosthesis was investigated. Visual inspection, optical microscopy, scanning electron microscopy (SEM), energy dispersive spectroscopy (EDS) and a commercial software simulation ProCAST were carried out in order to determine the cause and solution of this failure. The results exhibited hot tearing, shrinkage porosity and metal oxide films due to inadequate heat dissipation during solidification process, as a consequence of poor investment casting ceramic mold configuration. Diabetes mellitus y obesidad: la importancia de realizar un abordaje integral de ambas entidades

We present a 63-year-old woman diagnosed with type 2 diabetes mellitus in 2007 and grade III obesity, in addition to gonarthrosis and sequelae of a traffic accident. She was unable to perform physical exercise due to gonarthrosis and the aftermath of the traffic accident. Progressive treatment with oral antidiabetic agents did not achieve optimal glycemic control after several years of follow-up. Neither there was an improvement in obesity despite following several diets prescribed by specialists. We offered her therapy with insulin but the patient rejected it systematically. Finally, the use of combined treatment with agonists of the GLP-1 receptor and SGLT-2 inhibitors allowed to achieve an adequate glycemic control for the first time. In the subsequent controls, HbA1c values of less than 7% were reached, in addition to a significant reduction in body weight, with good tolerance and without adverse effects.Se presenta el caso de una mujer de 63 años diagnosticada de diabetes mellitus tipo 2 y obesidad grado III, muy limitada para la realización de ejercicio físico. Durante años no se consiguió un control glucémico óptimo con antidiabéticos orales ni una reducción del peso a pesar de seguir varias dietas pautadas por especialistas. Además rechazó sistemáticamente iniciar tratamiento insulínico. Ambos objetivos se lograron finalmente con un tratamiento combinado formado por agonistas del receptor GLP-1 e inhibidores SGLT-2, todo ello con buena tolerancia y sin aparición de efectos adversos reseñables

Ni tipo 1 ni tipo 2: una diabetes de perfil infrecuente con base autoinmunitaria

We present a 66-year-old patient with history of Graves-Basedow disease, who was admitted in charge of Internal Medicine with suspected debut of diabetes mellitus, being initially catalogued as type 2. The poor response to treatment with oral antidiabetics, requiring insulin therapy to achieve a proper glycemic control, along with the underlying pathology, raised the suspicion of latent autoimmune diabetes in adults. This was confirmed by the discovery of specific positive autoantibodies. The association between these two pathologies also makes it possible to classify the patient within a group of autoimmune endocrine disorders known as type 3 polyglandular syndrome.Paciente de 66 años con antecedente de enfermedad de Graves-Basedow que ingresó a cargo de Medicina Interna por diabetes mellitus de debut, siendo inicialmente catalogada como tipo 2. La mala respuesta al tratamiento con antidiabéticos orales, precisando insulinoterapia para conseguir un correcto control glucémico, junto a su patología de base, plantearon la sospecha de diabetes autoinmune latente del adulto. Esta se confirmó con el hallazgo de autoanticuerpos específicos positivos. La asociación de estas dos patologías permite además clasificar a la paciente dentro de un grupo de trastornos endocrinos autoinmunes conocidos como síndromes poliglandulares, en este caso tipo 3

Rol de la persona profesional en Orientación en la promoción del involucramiento del estudiantado de secundaria en su proceso educativo

Enfoque cualitativoEn esta investigación se estudian los significados que las personas profesionales en Orientación atribuyen a las acciones que realizan para la promoción del involucramiento del estudiantado, en instituciones educativas públicas de secundaria. Además, se ofrecen recomendaciones a las personas profesionales participantes para la promoción del mismo, esto a partir de los aportes brindados. Como parte de los antecedentes se encontraron estudios a nivel internacional y nacional referentes al involucramiento estudiantil, la permanencia, el compromiso e implicación de esta población en su trayectoria académica. Se visualiza la necesidad de abordar la participación de las personas profesionales en Orientación en este tema inherente al proceso educativo. El estudio se sustenta teóricamente con la comprensión del fenómeno del involucramiento, el cual tiene tres componentes: cognitivo, afectivo y conductual. También se constituye un fundamento en lo relativo al quehacer de la persona profesional en Orientación en la promoción del involucramiento, en instituciones educativas de secundaria. La investigación se basa en un paradigma naturalista, con enfoque cualitativo y método fenomenológico, como guía de los procedimientos a seguir, pues se busca describir e interpretar lo expresado por las personas participantes. Una de las poblaciones participantes fueron profesionales en Orientación en ejercicio, a quienes se aplicaron entrevistas semiestructuradas; la segunda correspondió a estudiantes de secundaria de sétimo a undécimo año, que aportaron información mediante la técnica de grupo focal. Se utilizó la herramienta Atlas.ti 8, como programa de análisis cualitativo para manejar la información, donde se favoreció la construcción de tres dimensiones que responden a los objetivos de la investigación. Se presenta en primera instancia las acciones de la persona profesional en Orientación para la promoción del involucramiento del estudiantado, después, el rol de la persona profesional en Orientación en la promoción del involucramiento del estudiantado y por último, la tercera dimensión relativa a las recomendaciones para la promoción del involucramiento del estudiantado. El análisis permite concluir, en cuanto al involucramiento; que las personas profesionales en Orientación le brindan significado a las acciones que realizan, tal como: la importancia de que cada estudiante se adueñe de su proceso y que se sienta cómoda o cómodo en este, por lo que creen esencial tomar en cuenta el contexto y las necesidades, considerando además el hecho de contribuir en el aspecto personal y académico, además visualizan la importancia en alcanzar un equilibrio entre el apoyo de la persona profesional y la responsabilidad del estudiantado, valoran el cuidar el accionar profesional, siendo congruente con lo que se dice y se hace. De esta manera las personas profesionales llevan a cabo acciones que promueven habilidades para el desarrollo integral de la persona estudiante y su vivencia como agente protagónico; además de realizar un trabajo en conjunto con docentes, familias.
