    Role of the first WHO mutation catalogue in the diagnosis of antibiotic resistance in Mycobacterium tuberculosis in the Valencia Region, Spain: a retrospective genomic analysis

    9 pĂĄginas, 2 figuras, 1 tablaBackground: In June, 2021, WHO published the most complete catalogue to date of resistance-conferring mutations in Mycobacterium tuberculosis. Here, we aimed to assess the performance of genome-based antimicrobial resistance prediction using the catalogue and its potential for improving diagnostics in a real low-burden setting. Methods: In this retrospective population-based genomic study M tuberculosis isolates were collected from 25 clinical laboratories in the low-burden setting of the Valencia Region, Spain. Culture-positive tuberculosis cases reported by regional public health authorities between Jan 1, 2014, and Dec 31, 2016, were included. The drug resistance profiles of these isolates were predicted by the genomic identification, via whole-genome sequencing (WGS), of the high-confidence resistance-causing variants included in the catalogue and compared with the phenotype. We determined the minimum inhibitory concentration (MIC) of the isolates with discordant resistance profiles using the resazurin microtitre assay. Findings: WGS was performed on 785 M tuberculosis complex culture-positive isolates, and the WGS resistance prediction sensitivities were: 85·4% (95% CI 70·8–94·4) for isoniazid, 73·3% (44·9–92·2) for rifampicin, 50·0% (21·1–78·9) for ethambutol, and 57·1% (34·0–78·2) for pyrazinamide; all specificities were more than 99·6%. Sensitivity values were lower than previously reported, but the overall pan-susceptibility accuracy was 96·4%. Genotypic analysis revealed that four phenotypically susceptible isolates carried mutations (rpoB Leu430Pro and rpoB Ile491Phe for rifampicin and fabG1 Leu203Leu for isoniazid) known to give borderline resistance in standard phenotypic tests. Additionally, we identified three putative resistance-associated mutations (inhA Ser94Ala, katG Leu48Pro, and katG Gly273Arg for isoniazid) in samples with substantially higher MICs than those of susceptible isolates. Combining both genomic and phenotypic data, in accordance with the WHO diagnostic guidelines, we could detect two new multidrug-resistant cases. Additionally, we detected 11 (1·6%) of 706 isolates to be monoresistant to fluoroquinolone, which had been previously undetected. Interpretation: We showed that the WHO catalogue enables the detection of resistant cases missed in phenotypic testing in a low-burden region, thus allowing for better patient-tailored treatment. We also identified mutations not included in the catalogue, relevant at the local level. Evidence from this study, together with future updates of the catalogue, will probably lead in the future to the partial replacement of culture testing with WGS-based drug susceptibility testing in our setting.     Diseñando para la comunidad de costureras de La Mezquitera

    El objetivo del PAP Diseñando para la comunidad de costureras de la Mezquitera es crear empresarios de la industria textil y de la moda con responsabilidad social. Se trabajó con un equipo multidisciplinario de alumnos, así como con la comunidad de costureras de la Mezquitera. Una comunidad organizada años atrås por un PAP del Iteso. Una vez entendido el contexto y entorno actual que rodea a la comunidad se procedió a dar solución a la problemåtica. Se propusieron 9 marcas de diseño, de prendas de vestir y accesorios para hogar, hechas de materiales textiles. Se hicieron estudios de factibilidad de acuerdo con las tendencias de consumo actuales, se diseñaron prendas con elementos diferenciadores para reducir al måximo la posibilidad de fracaso y se sacaron producciones pequeñas para evitar la sobreproducción de prendas y el fast fashion. Para llevar a cabo dichas producciones se pasó por la etapa de prototipado, optimizando recursos y minimizando riesgos, y se pasó a producción en volumen. Se procedió a hacer la identidad de cada marca, de acuerdo con el nicho de mercado de cada una, redes sociales individuales para publicidad y venta y fotografía de producto. También se hicieron redes comunitarias para mostrar el trabajo del PAP e impulsar las marcas, ademås de un sitio web comunitario del colectivo. Se logró un equilibrio entre diseño, emprendimiento y generación de empleos con trabajo ético y salario justo, cuidando la optimización de recursos y la reducción de desperdicios.ITESO, A.C

    Spread of a SARS-CoV-2 variant through Europe in the summer of 2020.

    Following its emergence in late 2019, the spread of SARS-CoV-21,2 has been tracked by phylogenetic analysis of viral genome sequences in unprecedented detail3–5. Although the virus spread globally in early 2020 before borders closed, intercontinental travel has since been greatly reduced. However, travel within Europe resumed in the summer of 2020. Here we report on a SARS-CoV-2 variant, 20E (EU1), that was identified in Spain in early summer 2020 and subsequently spread across Europe. We find no evidence that this variant has increased transmissibility, but instead demonstrate how rising incidence in Spain, resumption of travel, and lack of effective screening and containment may explain the variant’s success. Despite travel restrictions, we estimate that 20E (EU1) was introduced hundreds of times to European countries by summertime travellers, which is likely to have undermined local efforts to minimize infection with SARS-CoV-2. Our results illustrate how a variant can rapidly become dominant even in the absence of a substantial transmission advantage in favourable epidemiological settings. Genomic surveillance is critical for understanding how travel can affect transmission of SARS-CoV-2, and thus for informing future containment strategies as travel resumes. © 2021, The Author(s), under exclusive licence to Springer Nature Limited

    Population-based sequencing of Mycobacterium tuberculosis reveals how current population dynamics are shaped by past epidemics

    23 pĂĄginas, 4 figuras, 1 tabla.Transmission is a driver of tuberculosis (TB) epidemics in high-burden regions, with assumed negligible impact in low-burden areas. However, we still lack a full characterization of transmission dynamics in settings with similar and different burdens. Genomic epidemiology can greatly help to quantify transmission, but the lack of whole genome sequencing population-based studies has hampered its application. Here, we generate a population-based dataset from Valencia region and compare it with available datasets from different TB-burden settings to reveal transmission dynamics heterogeneity and its public health implications. We sequenced the whole genome of 785 Mycobacterium tuberculosis strains and linked genomes to patient epidemiological data. We use a pairwise distance clustering approach and phylodynamic methods to characterize transmission events over the last 150 years, in different TB-burden regions. Our results underscore significant differences in transmission between low-burden TB settings, i.e., clustering in Valencia region is higher (47.4%) than in Oxfordshire (27%), and similar to a high-burden area as Malawi (49.8%). By modeling times of the transmission links, we observed that settings with high transmission rate are associated with decades of uninterrupted transmission, irrespective of burden.     Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Evaluation of Normalized Difference Vegetation Index by Remote Sensing with Landsat Satellites in the Tayacaja Valley in the Central Andes of Peru

    The research sought to evaluate the normalized difference vegetation index (NDVI) of the study area located in the province of Tayacaja, which includes the districts of Acraquia, Ahuaycha, Pampas and Daniel HernĂĄndez, which is part of the central Andes of Peru. The data were collected in low water seasons with a longitudinal cut of 30 years with one sample per year, starting in 1993 until 2022; these samples corresponded to the month of August of each year. The images were extracted from maps from Landsat satellite databases, which were filtered for low cloud cover to avoid interference with the images. Maps from 1993 to 2012 were obtained from Landsat 5 satellite, while from 2013 to 2022 data were obtained from Landsat 8 satellite. The normalized difference vegetation index was determined using Quantum GIS based on the red and near infrared maps; being the minimum NDVI value obtained -0.18, which corresponds to the aquatic body of the Upamayu River that crosses from west to east the study area; while the highest NDVI obtained was 0.79 indicating a greater vegetation cover constituted by mainly eucalyptus plants. The mean NDVI of the 30 years is close to 0.21; this is an indicator that the vegetation is scarce and that it is decreasing mainly due to population growth
