Short view of leukemia diagnosis and treatment in Iran
Background: Early diagnosis and treatment of leukemia patients remains a fundamental aim in clinical oncology, especially in developing country. Present study highlights the basic requirements of these patients in Iran. Better understanding of these issues may lead to improve the healthcare standards toward leukemia diagnosis and treatment. Methods: This descriptive study included 101 specialists in hematology-oncology and pathology serving in oncology centers. The participants were then asked to fill out a standard questionnaire on the issues around diagnosis and treatment of blood malignancies. Results: According to specialists, unfair distribution of facilities across the country, delayed diagnosis of disease, absence of psychological support for patients, and insufficient financial support were the main reasons of inappropriate diagnosis and treatment in leukemia patients. Conclusions: Our results show that making an amendment to health policies by preparing well-equipped medical centers in all provinces, improving the morale of patients through consultation during the process of treatment, and above all, subsiding leukemia patients' financial problems will promote the health standard regarding the leukemia diagnosis and treatment in Iran. © 2015, Tehran University of Medical Sciences (TUMS). The characteristics of adults with upper gastrointestinal bleeding admitted to Tripoli Medical Center: a retrospective case-series analysis
Background: Acute upper gastrointestinal bleeding (UGIB) is a common reason for hospital admissions worldwide. Aetiological causes of UGIB vary according to geographic region and socioeconomic status. However, despite the implementation of early endoscopy as the standard method for the diagnosis and treatment of UGIB, data on the characteristics of patients with UGIB in Libya are still minimal. In this study, we describe patient demographics, aetiological causes for UGIB, and possible risk factors for upper gastrointestinal bleeding in patients admitted to the Gastroenterology Department at Tripoli Medical Center from January 2001 through June 2006. Method: This is a retrospective case-series analysis of all adult patients with upper gastrointestinal bleeding admitted to the Gastroenterology Department at TMC. Patients’ medical records were individually reviewed and relevant data abstracted. Results: A total of 928 cases with diagnoses of UGIB were admitted to Tripoli Medical Center during the study period. Of these cases, 60.3% were males and 39.7% females (3:2) and males were significantly younger than females (49.6 years vs. 53.9 years, p=0.001). The most common cause of UGIB was peptic ulcer (37.1%) of which duodenal ulcer was the most common (30.7% of all UGIB), especially amongst male patients (36.4%). The second most common cause was bleeding due to varices (29.8%), especially amongst females (35.1%). Additionally, smoking and NSAIDs use were reported by 18.6% and 9.7% of cases and both were significantly associated with bleeding due to peptic ulcers. Conclusion: This study has investigated the characteristics of adults with UGIB at a tertiary referral center in Libya. The high frequency of bleeding due to varices amongst females mandates further investigations into the possible underlying hepatic causes and their management, and the potential impact on patient outcome and prognosis.Keywords: bleeding; UGIB; gastroenterology; Liby
Avocado (Persea americana) pulp improves cardiovascular and autonomic recovery following submaximal running: a crossover, randomized, double-blind and placebo-controlled trial
Previous studies have demonstrated that regular avocado consumption presents advantageous effects on cardiovascular system. However, little attention has been paid to the use of avocado as a dietary supplement, in particular, for individuals involved in physical exercise training. Therefore, this study aims to evaluate the effect of acute avocado pulp intake on cardiovascular and autonomic recovery subsequent to moderate exercise. Using a crossover, randomized, double-blind and placebo-controlled trial design, 16 healthy female adults underwent two protocols: Avocado pulp (600 mg in capsule) and placebo (600 mg starch in capsule). After the ingestion of Avocado pulp or placebo, the subjects were seated for 60 min at rest, followed by running on a treadmill at a submaximal level and then remained seated for 60 min during recovery from the exercise. Heart rate (HR), heart rate variability (HRV) [rMSSD, SD1, HF (ms2)] and skin conductance were evaluated before and during exercise, as well as during recovery. HR, systolic blood pressure, HRV and skin conductance recovered faster when subjects were given avocado pulp prior to exercise. In conclusion, avocado pulp improved cardiovascular and autonomic recovery after exercise, suggesting a reduced risk of cardiovascular events after exertion. The current results support the beneficial effects of ingestion of avocado prior to submaximal treadmill running
Occult Hepatitis B Infection in Patients With Cryptogenic Liver Cirrhosis in Southwest of Iran
Background: Chronic hepatitis B virus (HBV) infection has a broad spectrum of manifestation, ranging from silent carrier state to
advanced cirrhosis and hepatocellular carcinoma. The persistence of HBV DNA in serum and hepatocytes of the cirrhotic patient could be
detected by molecular techniques in spite of negative HBV serologic markers.
Objectives: This case-control study was designed to evaluate the prevalence of occult HBV infection (OBI) in patients with cryptogenic liver
cirrhosis in comparison with healthy subjects.
Patients and Methods: Of 165 patients with liver cirrhosis, 50 consecutive patients with cryptogenic cirrhosis and 80 healthy individual
without any risk factors as a control group were enrolled in this study. Their sera were tested for HBV DNA using nested PCR method.
Results: Of 50 patients with cryptogenic cirrhotic, 36 (72%) were male. The mean age of patients was 53.34 ± 14.73 years; 80 healthy subjects
were selected as control group with mean age of 32.65 ± 8.51 years; 7 (14%) of the patients with cryptogenic cirrhosis showed positive HBV
DNA by PCR, while HBV DNA was negative for the control group (P = 0.0001); 4 (57%) cases with positive HBV shown by PCR were negative for
anti-HBc and anti-HBs tests. The mean level of transaminases was significantly higher in patients with cirrhosis. There were no significant
differences in demographic parameters, transaminases level and degree of hepatic failure among cirrhotic patients with and without OBI.
Conclusions: The prevalence of OBI was relatively high in patients with cryptogenic cirrhosis. OBI was found among the patients above 40
years old. Prospective cohort studies are needed to evaluate the clinical significance of OBI
AzTEC Millimetre Survey of the COSMOS Field - II. Source Count Overdensity and Correlations with Large-Scale Structure
We report an over-density of bright sub-millimetre galaxies (SMGs) in the
0.15 sq. deg. AzTEC/COSMOS survey and a spatial correlation between the SMGs
and the optical-IR galaxy density at z <~ 1.1. This portion of the COSMOS field
shows a ~ 3-sigma over-density of robust SMG detections when compared to a
background, or "blankfield", population model that is consistent with SMG
surveys of fields with no extragalactic bias. The SMG over-density is most
significant in the number of very bright detections (14 sources with measured
fluxes S(1.1mm) > 6 mJy), which is entirely incompatible with sample variance
within our adopted blank-field number densities and infers an over-density
significance of >> 4. We find that the over-density and spatial correlation to
optical-IR galaxy density are most consistent with lensing of a background SMG
population by foreground mass structures along the line of sight, rather than
physical association of the SMGs with the z <~ 1.1 galaxies/clusters. The SMG
positions are only weakly correlated with weak-lensing maps, suggesting that
the dominant sources of correlation are individual galaxies and the more
tenuous structures in the region and not the massive and compact clusters.
These results highlight the important roles cosmic variance and large-scale

A Fibreoptic Endoscopic Study of Upper Gastrointestinal Bleeding at Bugando Medical Centre in Northwestern Tanzania: a Retrospective Review of 240 Cases.
Upper gastrointestinal (GI) bleeding is recognized as a common and potentially life-threatening abdominal emergency that needs a prompt assessment and aggressive emergency treatment. A retrospective study was undertaken at Bugando Medical Centre in northwestern Tanzania between March 2010 and September 2011 to describe our own experiences with fibreoptic upper GI endoscopy in the management of patients with upper gastrointestinal bleeding in our setting and compare our results with those from other centers in the world. A total of 240 patients representing 18.7% of all patients (i.e. 1292) who had fibreoptic upper GI endoscopy during the study period were studied. Males outnumbered female by a ratio of 2.1:1. Their median age was 37 years and most of patients (60.0%) were aged 40 years and below. The vast majority of the patients (80.4%) presented with haematemesis alone followed by malaena alone in 9.2% of cases. The use of non-steroidal anti-inflammatory drugs, alcohol and smoking prior to the onset of bleeding was recorded in 7.9%, 51.7% and 38.3% of cases respectively. Previous history of peptic ulcer disease was reported in 22(9.2%) patients. Nine (3.8%) patients were HIV positive. The source of bleeding was accurately identified in 97.7% of patients. Diagnostic accuracy was greater within the first 24 h of the bleeding onset, and in the presence of haematemesis. Oesophageal varices were the most frequent cause of upper GI bleeding (51.3%) followed by peptic ulcers in 25.0% of cases. The majority of patients (60.8%) were treated conservatively. Endoscopic and surgical treatments were performed in 30.8% and 5.8% of cases respectively. 140 (58.3%) patients received blood transfusion. The median length of hospitalization was 8 days and it was significantly longer in patients who underwent surgical treatment and those with higher Rockall scores (P < 0.001). Rebleeding was reported in 3.3% of the patients. The overall mortality rate of 11.7% was significantly higher in patients with variceal bleeding, shock, hepatic decompensation, HIV infection, comorbidities, malignancy, age > 60 years and in patients with higher Rockall scores and those who underwent surgery (P < 0.001). Oesophageal varices are the commonest cause of upper gastrointestinal bleeding in our environment and it is associated with high morbidity and mortality. The diagnostic accuracy of fibreoptic endoscopy was related to the time interval between the onset of bleeding and endoscopy. Therefore, it is recommended that early endoscopy should be performed within 24 h of the onset of bleeding
D-brane potentials in the warped resolved conifold and natural inflation
In this paper we obtain a model of Natural Inflation from string theory with
a Planckian decay constant. We investigate D-brane dynamics in the background
of the warped resolved conifold (WRC) throat approximation of Type IIB string
compactifications on Calabi-Yau manifolds. When we glue the throat to a compact
bulk Calabi-Yau, we generate a D-brane potential which is a solution to the
Laplace equation on the resolved conifold. We can exactly solve this equation,
including dependence on the angular coordinates. The solutions are valid down
to the tip of the resolved conifold, which is not the case for the more
commonly used deformed conifold. This allows us to exploit the effect of the
warping, which is strongest at the tip. We inflate near the tip using an
angular coordinate of a D5-brane in the WRC which has a discrete shift
symmetry, and feels a cosine potential, giving us a model of Natural Inflation,
from which it is possible to get a Planckian decay constant whilst maintaining
control over the backreaction. This is because the decay constant for a wrapped
brane contains powers of the warp factor, and so can be made large, while the
wrapping parameter can be kept small enough so that backreaction is under

The physical scale of the far-infrared emission in the most luminous submillimetre galaxies II: evidence for merger-driven star formation
We present high-resolution 345 GHz interferometric observations of two
extreme luminous (L_{IR}>10^{13} L_sun), submillimetre-selected galaxies (SMGs)
in the COSMOS field with the Submillimeter Array (SMA). Both targets were
previously detected as unresolved point-sources by the SMA in its compact
configuration, also at 345 GHz. These new data, which provide a factor of ~3
improvement in resolution, allow us to measure the physical scale of the
far-infrared in the submillimetre directly. The visibility functions of both
targets show significant evidence for structure on 0.5-1 arcsec scales, which
at z=1.5 translates into a physical scale of 5-8 kpc. Our results are
consistent with the angular and physical scales of two comparably luminous
objects with high-resolution SMA followup, as well as radio continuum and CO
sizes. These relatively compact sizes (<5-10 kpc) argue strongly for
merger-driven starbursts, rather than extended gas-rich disks, as the preferred
channel for forming SMGs. For the most luminous objects, the derived sizes may
also have important physical consequences; under a series of simplifying
assumptions, we find that these two objects in particular are forming stars
close to or at the Eddington limit for a starburst.Comment: 9 pages, 3 Figures, submitted to MNRA
Observational tests of the galaxy formation process
The mutual feedback between star formation and nuclear activity in large
spheroidal galaxies may be a key ingredient to overcome several difficulties
plaguing current semi-analytic models for galaxy formation. We discuss some
observational implications of the model by Granato et al. (2003) for the
co-evolution of galaxies and active nuclei at their centers and stress the
potential of the forthcoming surveys of the Sunyaev-Zeldovich effect on
arcminute scales, down to K levels, to investigate the early galaxy
